There are two survey options: 1) Citizen and Visitor Survey, and 2) Business Survey. The citizen and visitor survey is intended for those who live and regularly visit Lake Worth. The business survey is intended for those who either own a business in Lake Worth or who work in Lake Worth. The surveys take about 15-20 minutes to complete and can be reached by scanning the QR codes (above) with your mobile phone.
