Overview

LSC’s senior services in North Carolina include eleven senior living communities, an adult day services program, home care services, affordable housing, and several PACE programs (Program of All-inclusive Care for the Elderly).

HistoryLutheran Services

Carolinas (LSC) is the affiliation of two Lutheran social ministries whose roots extend over more than a century. Both are sponsored by the North and South Carolina Synods of the Evangelical Lutheran Church in America (ELCA).

Lutheran Services for the Aging (LSA) was founded in 1960 and opened its first nursing home in 1962. Lutheran Family Services in the Carolinas (LFS) was founded in 1976 and incorporated in 1980. Each organization has its own bylaws and board of trustees, but the membership of those boards is identical. Each affiliate corporation has 501(c)(3) non-profit status.

LSC’s corporate culture is informed and enriched by Christian faith and values. At the same time, the organization embraces all people who share its commitment to service,

regardless of faith. LSC expresses its ethics through a set of six core values and a customer service program called The LSC Way. (See page 13.)

LSC’s continuing care retirement communities include Trinity Elms in Clemmons, Trinity Landing in Wilmington, Trinity Oaks in Salisbury and – coming soon – Trinity Village in Hickory. Trinity View in Arden offers independent and assisted living; and LSC’s assisted living and skilled nursing and rehab communities

include Trinity Glen in Winston-Salem, Trinity Grove in Wilmington, Trinity Place in Albemarle, Trinity Ridge and Trinity Village in Hickory, Aston Park in Asheville, and GlenFlora in Lumberton. Learn more on pages 4 and 5.

LSC has grown into a $250 million ministry serving both North and South Carolina. The organization directly serves more than 10 thousand people per year, employs over 2,600 teammates, and touches the lives of thousands more. LSC’s programs, services, and communities welcome everyone regardless of age, race, sex, religion, or national origin.

As part of its commitment to quality at every level, LSC also operates two low-income housing communities designed to meet the needs of seniors and individuals with disabilities and dementia: Dallas High School Apartments in Dallas (owned by LSC) and Lutherhaus in Hickory (a partnership with St. Andrew’s and Bethany Lutheran Churches).

Trinity Living Center in Salisbury provides safe and meaningful daytime activities for seniors and adults who need a safe place during the day while their loved ones work or care for themselves. Trinity at Home, also in Salisbury, provides a variety of services to help seniors age in place. For more information, see page 7.

LSC partners with several other regional nonprofits to offer PACE programs in Charlotte, Greensboro, Hickory, Gastonia, and Shelby. See page 8 for details.

Affordable independent living through LSC

LSC is committed to providing quality at every level – and that includes seniors on a budget. Trinity Elms independent living in Clemmons, Trinity View independent living in Arden, and Trinity Village independent living in Hickory (opening soon) offer attractive, affordable monthly rental apartments with no entrance fee.

Economies of scale: LSA Pharmacy

An innovative solution for LSC’s senior living communities

As one of the largest nonprofit senior care providers in North Carolina, with senior living communities scattered statewide, how does LSC achieve efficient, high-quality service while keeping costs in check? Through LSA (Lutheran Services for the Aging) Pharmacy. Established in 1998, this one-stop pharmacy in Salisbury provides prescription, proprietary, and over-the-counter medicines to the 1,100-plus residents of LSC's assisted living, skilled nursing, rehab, and memory care communities. It also offers discounted over-the-counter medications to LSC teammates.

LSC strives to provide abundant life for everyone it serves. That means providing care wherever seniors are in need.

“We have about 30 teammates working at LSA Pharmacy, including five pharmacists,” says Pharmacist Manager David Beam.

“Our teammates are so dedicated to their jobs that we hardly ever have open positions,” adds Business Office Manager Sherron Ellis. “They take pride in their work and are always available to fill orders. I can count on this team every time.”

Trinity Living Center – LSC’s adult day program in Salisbury, North Carolina provides social and cognitive enrichment, nutritious meals, and medical supervision to adults who need assistance with daily life activities. Many are seniors living with dementia; some are adults with intellectual and developmental disabilities. Trinity Living Center is open Monday through Friday and can accommodate up to 60 participants per day. It accepts reimbursement from county, state, and federal sources as well as private pay.

Trinity at Home – LSC offers at-home care and other services for seniors within a 90mile radius of Salisbury, North Carolina. Care ranges from temporary respite to roundthe-clock, daily assistance. Trinity at Home also offers companionship services, memory care support groups, educational programs, and a well-stocked library of enrichment tools to help seniors age safely and successfully in place. The program serves about 100 clients per year.

Dallas High School Apartments – LSC manages and owns a low-income housing complex in Dallas, North Carolina for those 62 and older, or 55 and up if disabled. Formerly a historic school, it has been extensively renovated to create 33 comfortable apartments with common areas. Learn more about LSC’s commitment to affordable senior housing on pages 6 and 8.

PACE Programs - Program for All-Inclusive Care of the Elderly (PACE) provides comprehensive longterm services and support for seniors with Medicaid and Medicare. Participants typically live at home and receive care from an interdisciplinary team of health professionals at the PACE site during the day. LSC is a partner in four PACE programs across five locations:

• PACE@Home in Hickory, North Carolina is a partnership with Palliative Care and Hospice of Catawba County. LSC is a 50 percent owner.

• PACE of the Southern Piedmont in Charlotte, North Carolina is a partnership with Kintegra Health. LSC is a 50 percent owner.

• Senior Total Life Care – now in Gastonia and Shelby, North Carolina – is a partnership with Kintegra Health and CaroMont Health. LSC is a 25 percent owner.

Trinity Rehab – Co-founded by LSC in 2013, this nonprofit provider offers speech, occupational, and physical therapy services to LSC senior services operations and other nonprofit partners. In 2023, LSC sold a portion of its 50 percent ownership through its affiliate, LSA Therapy, to EveryAge and Presbyterian Homes. LSC now owns 30 percent of Trinity Rehab; these profits are available to further LSC's mission.

• PACE of the Triad in Greensboro, North Carolina is a partnership with Cone Health, Well-Spring Retirement Community, and Hospice and Palliative Care Charlotte Region. LSC is a 15 percent owner.

Inspiritus – In 2019, LSC began providing backoffice financial/accounting support services for this Georgia-based Lutheran social services nonprofit. Inspiritus offers services similar to Lutheran Family Services in the Carolinas’ (LFS), such as disability, foster care, refugee and disaster services.

Tsali Care Center – LSC collaborates with The Cherokee Indian Hospital Authority to enhance services for members of the Eastern Band of Cherokee Indians. Through a partnership agreement, LSC manages Tsali Care Center, a senior living community in Cherokee, North Carolina. A new, state-of-the-art facility will open in fall 2024.

Foster Care

Searstone Retirement Community – LSC is a sponsor of this nonprofit senior living community in Cary, North Carolina. LSC assists Searstone with grantmaking in which a portion of the community’s proceeds benefit programs for low-income seniors in Wake County.

LSC provides a broad continuum of foster care programs to ensure each child receives the care they need. Highly qualified LSC teammates offer information, training and licensure, 24/7 crisis support, and access to respite care.

• Family Foster Care serves children who must live apart from their parents because of abuse, neglect, or other adverse circumstances.

• Therapeutic Foster Care is for children who need a higher level of care due to their emotional, behavioral, and/or medical needs.

• Medically Fragile Foster Care helps children with significant medical conditions avoid

In each state, LSC also offers specialized foster care programs. See pages 10 and 11.

Disaster Response

LSC is a local partner of Lutheran Disaster Response (LDR) in North and South Carolina. After local emergency agencies have responded, LSC helps survivors return to safe, sanitary, secure housing – providing case management, connecting survivors to financial resources, and coordinating construction and repair resources. LSC also connects local partners with distribution centers, transportation services, volunteers, and more.

Refugee and Immigrant Services

LSC provides a safe haven for legal refugees and immigrants in North and South Carolina, including unaccompanied children and survivors of human trafficking.

New Americans Program – LSC works with the U.S. Office of Refugee Resettlement and a nonprofit partner, Global Refuge, to resettle refugees in local communities including Raleigh, Salisbury, and Asheville, North Carolina and Columbia, Greenville, Myrtle Beach, and Charleston, South Carolina. LSC’s New Americans Program helps clients find housing and employment, apply for the benefits for which they legally qualify, and become accustomed to American culture.

Home

Study/Post Release

Services – Children under age 18, often from Latin America, flee unsafe situations and arrive in the United States without a parent or guardian. Many hope to reunite with family members who are already here. Without LSC’s help, they may be forced to wait in detention facilities or shelters.

LSC provides case management before, during, and after reunification – ensuring unaccompanied children go to safe environments, and connecting them and their families or sponsors to services for good longterm outcomes. LSC guides foster families through the licensing process and trains and supports them for the duration of the child’s stay.

Survivors Assistance Program – LSC helps survivors of human trafficking achieve stability and live independently. Survivors receive emergency assistance, safety planning, and housing, health, and legal services – as well as financial assistance to support basic needs. LSC follows up to ensure continued access to services.

Host Homes for Adults with Disabilities

LSC recruits, trains, and supports "host home" caregivers who open their homes to adults with intellectual and/or developmental disabilities and/or traumatic brain injury (TBI). Clients are encouraged to participate in family and community life, and caregivers have access to 24/7 on-call support.

In North Carolina

Foster Care and Adoption

In addition to the programs listed on page 9, LSC offers two specialized foster care programs in North Carolina:

• Intensive Alternative Family Treatment (IAFT) provides intensive treatment in a family setting for foster children who need an elevated level of support, intervention, and supervision.

• LSC’s Transitional Living Program helps to prepare foster youth ages 15 to 21 in Buncombe and Haywood Counties for adult life as they age out of the foster care system. Clients are placed with qualified foster families and receive employment services and personalized coaching toward independence.

In North Carolina, LSC also provides comprehensive services for families who wish to adopt from foster care (the Special Needs Adoption Program, or SNAP). LSC conducts home studies, serves as a liaison to the local Department of Social Services, assists with adoption finalization services, and provides post-adoption support.

Adult Disability & Mental Health Services

LSC provides a comprehensive array of community, residential, and support services to North Carolina adults with intellectual or developmental disabilities, TBI, and/ or severe and persistent mental illness. LSC aims to help clients live as fully and independently as possible.

In addition to host homes (see above), LSC operates several supportive living residences in North Carolina. (See the table on opposite page.) LSC also offers coordinated care services to help its supportive living residents achieve an integrated experience among their various health care providers.

For veterans – LSC provides “wraparound” care for combat wounded veterans with TBI, augmenting the traditional therapy and case management they receive from its partner, Neuro Community Care. LSC teammates plan a robust calendar of events to keep veterans engaged, help them explore personal interests and goals, and encourage greater independence.

Supervised independent living with 24/7 support and mental health services. Operated by LSC in partnership with Alliance Behavioral Health and The Arc of North Carolina.

Supporting recovery and building resilience

Community-based behavioral health treatment – In Orange, Person, and Chatham Counties, LSC meets adults with severe and persistent mental illness where they are – whether at home, at work, or in the community. They receive assistance with basic daily living needs, medications, and socialization; have access to 24/7 on-call support; and more.

In South Carolina Foster Care

In addition to the programs listed on page 9, LSC offers two programs designed to meet urgent needs.

LSC is helping to ensure that South Carolinians in the Upstate and Midlands regions have the support they need to address substance use disorder and work toward healing.

Day Services for Youth – In the Midlands and Upstate, LSC provides a safe place from 7 a.m. to 7 p.m. for foster youth who do not have a placement. They receive meals and snacks, educational support, and enrichment with help from teachers, clinicians, and case managers. LSC also provides overnight services for foster youth in the Midlands without a placement.

Transitional Foster Care for Unaccompanied Children – LSC secures loving foster homes for unaccompanied children in the Columbia area while they await reunification. Other services include culturally competent bilingual education, personalized case management, and transportation to medical and behavioral health appointments.

In Greenwood, Lexington, Newberry, and Richland counties, LSC’s Trinity Recovery program trains church congregations on stages of readiness for recovery, local recovery resources, and how to administer naloxone to reverse an opioid overdose. In these counties plus Calhoun, Kershaw, Orangeburg, and Sumter counties, LSC distributes harm reduction kits in highneed areas and offers peer support to individuals.

in South Carolina: Kinard Manor for women in Greenwood, and WeCo Cottage for college and university students in West Columbia.

Mary started her career with LSC in 1996, covering for a maternity leave at Trinity Oaks health and rehab. She returned to LSC the following year and has been here ever since. Mary served as business office manager at Trinity Oaks health and rehab and as software support and systems analyst before becoming director of information technology. She was named CIO in 2023. Mary holds a bachelor’s degree in business administration from Catawba College and an MBA from the University of North Carolina at Pembroke.

Chief Administrative Officer

Christina joined LSC in 2007 and previously served as senior accountant, executive director of Trinity Living Center, director of community-based services for Trinity at Home/Trinity Living Center, and director of administration. In 2023, she assumed her current role leading organization-wide administrative efforts and research projects. Christina holds a bachelor’s degree in business management with a concentration in finance from North Carolina State University.

Ted has spent his entire career in health and human services. He worked on the front lines of care as a certified nursing assistant before becoming a licensed nursing home administrator. He was the administrator of Trinity Village for over 10 years and spent three years in administration and development with LSC before being named president in 2000. Ted is active in a number of state and national organizations. He holds an Honorary Doctorate of Humane Letters from his alma mater, Lenoir-Rhyne University, and a Master of Science degree from Pfeiffer University.

Mary Beth Lemly

Vice President for Teammate Services

Mary Beth joined LSC in 2014 as executive director of the Trinity Elms campus. In 2021 she moved to the LSC Administrative Office, serving as operations coordinator and director of strategic systems before assuming her current position. Mary Beth is a licensed nursing home and assisted living administrator with 30 years' experience in long-term care management. She is also a North Carolina-qualified activity director and a licensed real estate broker.

Compassion: We believe compassion is the foundation of LSC’s ministry. To be compassionate is to recognize the hurt or unhappiness of another and act to change it. LSC teammates work in a compassionate manner to improve the lives of all they serve.

Faith: Many LSC teammates choose to work here because their faith calls them to love and to serve. All LSC teammates strive to be content and joyful in their work and to create an atmosphere of support and forgiveness.

Integrity: LSC teammates work responsibly, honestly, and in a transparent manner. We expect honesty at all levels of the organization. LSC teammates are encouraged to voice concerns, seek help, and admit failures.

Excellence: We believe any job worth doing is worth doing well. LSC teammates strive for excellence and encourage it in others. We seek out opportunities for personal growth and development and encourage others to do so as well.

Justice: At LSC, we strive for a right relationship with God and with each other. LSC teammates work to create a just culture that is free of bias and discrimination.

Kirby has been LSC’s chief financial officer and treasurer since 2012. Previously, he served as CFO for three other healthcare organizations in Florida, North Carolina, and Virginia. Kirby holds an MBA from Florida State University as well as a nursing home administrator’s license.

Myra has been with LSC since 2003 and has served in multiple roles, including vice president for quality and risk management and chief operating officer for child and family services. She holds a bachelor’s degree in psychology from the University of North Carolina at Charlotte and a Master of Social Work degree from the University of South Carolina.

Courtney Johnson

Vice President, Senior Services

Courtney joined LSC in 2013, serving as a nursing home administrator and director of business development before assuming oversight of LSC's senior services. Trained as a speechlanguage pathologist, she holds bachelor’s and master’s degrees in communication sciences and disorders from the University of New Hampshire.

Heidi joined LSC as director of development in 2016 and became chief development officer in 2022. She is also president of The Foundation for Abundant Living, LSC's avenue for giving. A Certified Fundraising Executive (CFRE), Heidi previously held leadership roles with Bancroft, a leading nonprofit service provider in New Jersey. Heidi is a graduate of Delaware Valley University and holds a master's degree in nonprofit management from Eastern University.

Kesha L. Smith

Chief Operating Officer

Kesha joined LSC in 2003 and served in several capacities including chief administrative officer before being named chief operating officer in 2013. She provides management and oversight for all of LSC’s operations, including construction. Kesha holds a bachelor’s degree in accounting from the University of North Carolina at Charlotte and is a licensed nursing home administrator and real estate broker.
