This brochure is designed to help you understand the benefits available to you, and to utilize them to the fullest. Please contact us if you have any questions or we can support you in any way.

Eligibility

Eligible Employees

You may enroll in the Embassy of the Netherlands Employee Benefits Program if you are an active employee working a minimum of 17.5 hours per week.

Eligible Dependents

If you are eligible for our medical benefits, then your dependents are too. In general, eligible dependents for medical and vision include your spouse, domestic partner and children up to age 26. Spouses are eligible for health coverage under the Embassy’s plans if the spouse’s employment income is less than the employee’s. If your child is mentally or physically disabled, coverage may continue beyond age 26 once proof of the ongoing disability is provided. Children may include natural, adopted, step-children and children obtained through court-appointed legal guardianship, as well as dependents of a domestic partner.

Eligible dependent children under the age of 24, as long as they are full time students, can receive dental insurance coverage via the employee.

Enrolling in Coverage

Your benefits are effective on your date of hire. Please note you must elect benefits within your first 30 days of employment. The benefits you elect will remain in place for the full plan year, which renews annually on July 1, unless you have a qualifying family status change.

Family Status Change

A change in family status is a change in your personal life that may impact your eligibility or dependent’s eligibility for benefits.

Examples of family status changes include:

„ Change of legal marital status (i.e., marriage, divorce, death of spouse)

„ Change in number of dependents (i.e., birth, adoption, death of dependent, ineligibility due to age)

„ Change in employment or job status (e.g., spouse gains or loses access to benefits through his/her employer)

If such a change occurs, you must make the changes to your benefits within 30 days of the event date. Documentation may be required to verify your change of status. Failure to request a change of status within 30 days of the event may result in your having to wait until the next open enrollment period to make your change. Please contact HR to make these changes.

The Embassy of the Netherlands offers the choice between three medical plans administered by UnitedHealthcare (UHC). All three plans provide excellent, comprehensive coverage, and provide access to a vast national network of providers. To find a participating provider, go to uhc.com and search under ”Employer Plans”. The network is noted in the benefit information below.

Medical and prescription drug plan summary

A choice of three platinum plans

The medical plans offered by the Embassy are all designed to ensure you get the care you need when you need it. Consequently, all three plans have a lot in common!

„ All three plans cover the same services, such as physical and mental health, prescription drugs, and infertility treatments. Your copay for the service will vary based on the plan selected. For example, a Tier 2 medication will cost $30 on the High and Choice plans and $35 on the Base plan, but it will be covered as a Tier 2 medication on all three plans.

„ All plans use the same national Choice network. The high plan network is called “Choice Plus” because it also provides some out-of-network benefits, whereas the Choice and Base plans only pay benefits for in-network care (except for emergencies, which are covered in or out of network). UHC’s Choice network has over 1.5 million providers across the US so you will always have access to the care you need regardless of the plan you select.

„ Make sure to use Designated Diagnostic Providers - not just “in network” providers - when getting labs and imaging done. If you do not use a DDP provider, your costs will be higher. DDP providers are clearly indicated on UHC’s website.

„ You do not need a referral to seek specialty care. Some services, such as MRIs or surgeries, require prior authorization by UHC before care will be covered. Your in-network provider will obtain this authorization for you.

„ In-network preventive care, including your annual physical and routine vaccinations, is covered at no cost to you on all three plans.

„ Virtual Visits are covered at no cost to you on all three plans.

„ Emergency care is covered at the in-network level anywhere in the world.

Important note regarding retiree medical:

Please note that as you approach age 65, you must contact the Centers for Medicare & Medicaid Services (CMS) at 800-633-4227 to determine if you are eligible for Medicare benefits and to enroll if so. If you, your spouse, or your dependents are Medicare eligible, if you fail to enroll in Medicare when first eligible, you may be subject to Medicare late enrollment penalties, which can cause a delay in coverage and result in higher Medicare premiums when you do enroll. In addition, any benefits paid by the Embassy’s plan may be reduced, even if you have not enrolled in Medicare. It is your responsibility to confirm your Medicare eligibility with CMS and with UHC.

Medicare Supplement Plan

Retirees who are Medicare eligible may enroll in our Medicare Supplement plan. The plan design is the same as the High plan; however, it will pay secondary to Medicare.

Retirees who are ineligible for Medicare may enroll in the High, Choice, or Base plans.

Telemedicine

Life is demanding. It’s hard to find time to take care of yourself and your family members as it is, never mind when one of you isn’t feeling well. That’s why your health plan through UHC includes access to virtual care. Whether it’s late at night and your doctor isn’t available, you’re traveling, or you just don’t have the time or energy to leave the house, you can:

„ Access care from anywhere via video or phone.

„ Get minor medical virtual care 24/7/365 – even on weekends and holidays. Common reasons to call include cold and flu symptoms, fever, rashes, urinary tract infections, stomachaches, and eye infections.

„ Save money – on all three plans, virtual visits are at NO COST TO YOU!

„ Have a prescription sent directly to your local pharmacy, if appropriate.

UHC Health Support Services and Wellness Benefits

UnitedHealthcare Rewards - Earn up to $300!

Included in our medical plan is a program called UHC Rewards. With UHC Rewards, a variety of actions — including many things you may already be doing — lead to rewards. The activities you go for are up to you — same goes for ways to spend your earnings.

Employee Assistance Program

Get guidance in navigating mental health, financial, or legal concerns with our free, confidential EAP. The UHC EAP includes 3 free counseling sessions per incident, per year. Call the EAP 24/7 at 888887-4114.

Support from UHC Advocates

Whether you have questions about a new claim, need to find a doctor or just want to better understand your benefits, UHC advocates are here to help. Call the number on your health plan ID card or sign in to myuhc.com and click on “Call” or “Chat”.

Dental

The Embassy of the Netherlands offers Dental coverage also through UHC. Under this plan, you may visit any dentist you choose. However, your benefits will go farther if you visit an in-network provider. Not only will the provider charge a discounted fee, but the plan will pay a higher percentage of that fee.

To find a participating provider, visit myuhc.com and search for the Options PPO 30 network.

Preventive care is 100% covered by the plan – no cost to you for your twice-yearly dental checkups and cleanings! We encourage you to get regular dental care to help detect and deter dental issues.

Annual Dental Rollover

This is a summary of your coverage only. Please refer to your summary plan description for the full scope of coverage.

Employee Dental Payroll Contributions (monthly)

If your claims for the year are less than 50% of the annual maximum benefit (ex. $1,250), you will have $600 rollover to your following year’s annual maximum benefit. Additionally, if all of your claims throughout the plan year are submitted with in-network providers, you’ll be eligible to earn an additional $100 towards the following year’s annual maximum benefit.

Vision

The Embassy of the Netherlands offers a vision plan through UHC. The Vision plan provides coverage for routine eye exams and also provides benefits for the cost of glasses or contact lenses if you need them. You can see in- or out-of-network providers, however, your benefits will go farther if you visit an in-network provider.

401(k) Plan

„ An employee may elect to become a Participant in the Plan the first day of the month after completion of three months of service

„ Maximum contribution: 15% of salary

„ The Embassy will contribute 5% of pensionable pay. Employer contributions are immediately vested and are not contingent on any employee contribution.

„ The Embassy will also match up to a maximum of 4-4.8% of pensionable after-tax pay (depending on nationality)

„ Once you are eligible to participate, you will automatically be enrolled into the 401(k) with a 2% after-tax employee contribution, unless and until you select an alternate contribution percentage

Life and Disability Protection

The Embassy of the Netherlands provides the following protections to you should you become unable to work. The costs of these programs are fully borne by the Embassy.

Life Benefits

The Embassy provides a surviving dependents’ pension. In addition, an additional payment may be made if you suffer disability or death during the course of your official duties.

Short-Term Disability (STD)

If you are unable to work due to illness, injury, or maternity, the Embassy provides the following benefits, beginning on the date you are unable to work.

„ First six months: 100% of earnings

„ Next six months: 70% of earnings

„ Job security will be maintained for 52 weeks

Long-Term Disability (LTD)

„ After 52 weeks, disability benefits will be 60% of earnings

Specific details of the life and long-term disability benefits are based upon your service history. Please contact HR for details.

Paid Time Off

Vacation Days

Each year, you will be eligible for 150 hours, or 18.75 days, of vacation (prorated based on hire date). As your years of service increase, you will be eligible for additional vacation time as follows:

Holidays and Leaves

In addition to vacation time, you will also receive paid time off for the following events as applicable:

„ 13 paid holidays per year (days will be set every year upon advice upon the Mission Council)

„ 16 weeks paid maternity leave

„ Special leave

z Marriage of employee – 3 days

z Marriage of parent or child – 1 day

z When your partner gives birth, when adopting or fostering a child – 10 weeks within 6 months after birth or after the day on which the child joins the family

z Death of partner, child, parent, or family member in the 2nd degree – 7 days

z Death of family member in the 3rd degree – 2 days

„ Jury duty

Employees may also be eligible for additional time off under the Family and Medical Leave Act to care for a newborn, adopted, or foster child; or to care for a family member with a serious health condition.

Please note:

Glossary

BRAND NAME DRUGS: Drugs that have trade names and are protected by patents. Brand name drugs are generally the most costly choice.

COINSURANCE: The percentage of a covered charge paid by the plan.

COPAYMENT (COPAY): A flat dollar amount you pay for medical or prescription drug services regardless of the actual amount charged by your doctor or health care provider.

DEDUCTIBLE: The annual amount you must pay each year before the plan pays benefits. In general, the deductible does not apply to services with copays, such as office visits and prescription drugs.

GENERIC DRUGS: Generic drugs are less expensive versions of brand name drugs that have the same intended use, dosage, effects, risks, safety and strength. The strength and purity of generic medications are strictly regulated by the Federal Food and Drug Administration.

IN-NETWORK: Use of a health care provider that participates in the plan’s network. When you use providers in the network, you lower your out-ofpocket expenses because the plan pays a higher percentage of covered expenses.

OUT-OF-NETWORK: Use of a health care provider that does not participate in a plan’s network.

INPATIENT: Services provided to an individual during an overnight hospital stay.

OUTPATIENT: Services provided to an individual at a hospital facility without an overnight hospital stay.

OUT-OF-POCKET MAXIMUM: The maximum amount you and your family must pay for eligible expenses each plan year. Once your expenses reach the out-of-pocket maximum, the plan pays 100% of eligible expenses for the remainder of the year.

PRIMARY CARE PHYSICIAN (PCP): physician (generally a family practitioner, internist or pediatrician) who provides ongoing medical care.

SPECIALIST: A physician who has specialized training in a particular branch of medicine (e.g., a surgeon, gastroenterologist or neurologist).
