KNH staff and the public came out in large numbers to get screened and vaccinated for Hepatitis B. Our dedicated team of specialists availed themselves to participate in the drive as various guest speakers emphasized on the need for implementation of the screening, diagnosis, management and vaccination activities that took place.

It is worth noting that every 30 seconds, someone dies from a viral hepatitis related illness - even in the current COVID-19 crisis. However, with the existing prevention, testing and treatment services that are available, every hepatitis related death is preventable.

Rediscovering joy at work

“J

oy at work” sounds more like an oxymoron, a subject of the past or future. Most employees hope they find joy at their current workplace or the next.

The Covid-19 pandemic has taken a toll on the workforce, and employee well-being has suffered.

The sobering statistics of the pandemic and its significant disruption have obliged organizations to rethink the workplace and find ways to infuse joy to create more fulfilling employee experiences at work and their well-being.

Kenyatta National Hospital views its employees as valued contributors to its success and posterity. In addition, the patient is the Hospital’s priority. Positive employee experience consequently leads to positive patient experience and better clinical outcomes.

In September 2021, the Hospital adopted a program; “Joy at work”, spearheaded by the KNH Health Care Quality team in collaboration with The Africa Consortium for Quality Improvement Research in Frontline Healthcare (ACQUIRE) and governed by the Institute for Healthcare Improvement (IHI).

The quality improvement program focuses on building employee wellbeing by embarking on a journey of discovery to see what frontline staff need. It tracks measures to assess burnout, identifies innovations, and tests changes to see what will work to ensure employees derive pleasure and happiness at work.

How the ACQUIRE-IHI Joy at work program works Dr. Rhoda Kalondu, HoU Patient Safety, and Joy at Work champion, unraveled to Newsline how the program works: “When we started the program, we aimed at having both the clinical and corporate service departments part of the program. We engaged the pharmacy division, human resource division, mental health department, and Newborn unit. The pharmacy division has proactively participated

in the program, and we have walked the journey with them. We plan to revisit the other three areas this year and pick up from where we left off early this year”.

“We begin by carrying out a “What Matters to You” conversation with the staff. We seek answers to this question by encouraging staff to share with us what would make them have the best day at work, what would make work joyless and hard and what changes they would like to see in their workstations,” she said.

“Staff members are encouraged to disclose what made them happy or sad daily. In the pharmacy division, the quality improvement team is currently measuring good and bad days. We have provided color-coded pebbles that represent ‘good days’ and ‘bad days,’ which are put in two separate envelopes for the department’s quality improvement champion to tally the feedback daily and record it on the template provided. They needed to be respected, appreciated, involved in meetings and decision making, have enough tea, breakout rooms, start the day with prayer, background music at work, and better contract terms,” noted Dr. Kalondu”

where the team presents reasons for good or bad days. The Joy at Work team then converts the comments to an electronic form to reflect on the Joy at Work online tool periodically for review with the department and devise ways to implement the proposed change ideas,” added Dr. Kalondu.

The change ideas are then tested to determine whether they are improving the employee’s joy at work and consequently assisting in better service delivery and a positive patient experience.

The aim of the program KNH Health Care Quality team realized the importance of focusing on the employees that deliver the service for the patient to have a better experience while at the Hospital.

In September 2021, the Joy at Work team conducted an online survey on burnout and what makes work joyless. According to IHI, burnout leads to lower levels of staff engagement, patient

experience, productivity, and an increased risk of workplace accidents. Lower levels of staff engagement are linked with lowerquality patient care, including safety, and burnout limits providers’ empathy which is a crucial component of effective and personcentered care.

“The findings were shocking, to say the least. Employees said they just needed basic things to be happy, which we did not expect.

They needed to be respected, appreciated, involved in meetings and decision making, have enough tea, breakout rooms, start the day with prayer, background music at work, and better contract terms,” noted Dr. Kalondu.

The quality improvement program aims to ensure that employees find joy at work and transfer the same to clients as they offer quality health care services to them. It enlightens employees to navigate the challenges they experience

at work and go the extra mile to ensure they do not project them to the patients.

It also informs the Health Care Quality Division about what affects patient care from the employees’ point of view.

The response to the program since its inception The program received senior management buy-in and active teams’ involvement. The Senior Director of Clinical Services (SDCS), Senior Director of Corporate Services (SDCoS), and the Director of Human Resource have backed the program and its benefits. The sensitized teams have also found great joy in disclosing their joys and frustrations at work, as this has helped them work better as teams.

It has also provided a safe space for staff to open up to each other.

The future of the ACQUIRE-IHI Joy at Work Program

“It is important to note that this quality improvement initiative does not work in isolation. Other contributing factors and processes are based on the Strategic Plan and work hand-in-hand with the program to ensure its success. The vision of this quality improvement program is to see all employees happy at work. We aim to have employees find creative ways of working with what they have as they wait for hospital-wide system changes rather than being disgruntled,” said Dr. Kalondu.

She added, “We hope to see the Joy at Work program adopted as a culture change initiative at the Hospital. We also hope the program will ensure better client feedback on the departmental and hospitalwide customer satisfaction surveys.”

Quality health care starts with the employees. A joyful employee ensures a delighted client.

A show of warmth as KNH receives 10 beds from Ushuru Sacco Society

By Effie Sande & Maureen Oloo Kenyatta National Hospital’s adopt a bed campaign has received a shot in the arm following a donation of ten (10) beds from Ushuru Sacco Society Limited.

The beds were handed over to Board Chairman Mr. George Ooko by his counterpart Mr. Isaac Kiprop on Tuesday, July 19, 2022.

“A single bed will benefit three (3) patients in a month and hundreds of thousands in the next few years.

The new bed designs make patient management easier for the health care workers, thus contributing to their comfort while at the Hospital,” said Mr. Ooko.

The ABC of neurological rehabilitation

By Verah Mugambi In a snap, life can change. An injury, surgery, disease, illness, or decline in functioning because of age might make one in need of rehabilitation. This can happen to children and adults; it may affect the ability to even perform routine activities.

Neurological Rehabilitation (rehab) is a doctor-supervised program designed for people with diseases, injuries, or nervous system disorders. Neurological Rehab can often improve function, reduce symptoms, and improve the well-being of the patient.

The World Health Organization’s definition of rehabilitation states: “A set of measures that assist individuals, who experience or are likely to experience disability, to achieve and maintain optimum functioning in interaction with their environments” (WHO, 2011).

At Kenyatta National Hospital, Neurological Rehabilitation is at the Occupational Therapy (OT) department which is located on the ground floor; tower block, Opposite the Blood Transfusion Unit (B.T.U).

The clinic runs on Mondays & Thursdays morning; from 8.00 AM to 1 PM. Initially, it used to be Mondays only but after cases started rising, the department saw the need to schedule the rehab on Thursdays too.

Newsline spoke with Mr. Japheth Asungu, KNH’s Assistant Chief Occupational Therapy department. “Occupational therapy is about promoting health, promoting functions after the injury, disease, etc.,” he explains.

According to Asungu, a Neurological Rehab program is designed to meet your individual needs, depending on your specific problem or disease.

Active involvement of the patient

and their family is vital to the success of the program.

“The caregivers are very important when it comes to neurological rehabilitation. These are the husband, wife, sister, and friend; the patient’s support system. Remember this is a person whose life has changed and has to relearn everything.

The caregiver initially is always as sick as the patient since they also don’t know how to take care of the patient’s new condition. So, you have to involve them from the start,” Asungu emphasized.

improving your overall quality of life — physically, emotionally, and socially.

“Neurological conditions mostly affect the brain, the spinal cord, and the nerves…i.e., stroke. “Stroke is the highest… almost 50% of our neurological patients are suffering from a stroke. Others include a wide range of disorders, such as epilepsy, learning disabilities, neuromuscular disorders, autism, ADD, brain tumors, and cerebral palsy, just to name a few.” He added.

What conditions can benefit from neurological rehab?

Vascular disorders, such as ischemic strokes (caused by blood clots), hemorrhagic strokes (caused by bleeding in the brain), subdural hematoma, and transient ischemic attacks (TIAs), Infections, such as meningitis, encephalitis, polio, and brain abscesses, Trauma, such as brain and spinal cord injury, Structural or neuromuscular disorders, such as Bell palsy, myasthenia gravis, and Guillain-Barre syndrome.

Functional disorders, such as headache, seizure disorder, dizziness, and neuralgia, and Degenerative disorders, such as Parkinson’s disease, multiple sclerosis, amyotrophic lateral sclerosis (ALS), Alzheimer’s disease, and Huntington’s chorea.

“Neurological rehab is a journey, not a one-day hospital visit. We can work with patients for weeks, months, or even years.

“Patients with neurological conditions will present symptoms like, physical challenges, nerve psychosocial, cognitive deficit, visual… when you put all these factors together, you will realize their daily activities are affected. For example, they are not able to feed well or dress well by themselves, and even their leisure activities are affected.

Where do you get the patients?

We have a referral system for our patients we get them from; first Clinic no.17 the neurological clinic on Mondays & Thursdays morning hours only. The team will refer the patient to the outpatient here in O.T. Also, we get referrals from other clinics within our hospitals for patients who present neurological deficits like orthopedics, diabetic clinics, or even self-referrals from other private hospitals.

the accident/disease and need to relearn/ redefine how to do it. Once a patient suffers a condition, the patient needs to relearn how to do things to live day-to-day living.

Outcomes.

“When the patient tells you I wasn’t able to bathe, but I now can, I wasn’t able to lift, speak, and use the phone, etc. that’s when you feel you have done your job right.

“Neurological rehab is not a one-man show, but a multidisciplinary approach to get better results. We work closely with neurologists since they are the ones who make the diagnosis and the referrals, the physiotherapists, orthopedics tech no 46, and also speech therapists”, Asungu concluded.

Demystifying Osteogenesis Imperfecta (Brittle Bone Disease)

By Luke Kung’u A spot check by the Newsline team gave a rather busy-than-normal day at the Orthopedic Technology Department. Curiosity drove the team to the Head of Department Mr. John Ondiege who expounded more on the day’s activities.

It was a clinic day for Osteogenesis Imperfecta patients, and the staff were busy issuing special wheelchairs. This is what Mr.Ondiege had to say about the disorder. Read on…

What is Osteogenesis Imperfecta?

Osteogenesis Imperfecta (OI) is a congenital and inheritable disorder that is caused by deficiencies in the synthesis of type 1 collagen in which bones are abnormally soft and brittle.

It is also known as ‘Brittle Bone Disease’ or ‘Type 1 Collagen disease’.

Although OI has prominent skeletal manifestations, other anatomical structures rich in Type 1 Collagen, such as joints, eyes, ears, skin, and teeth are affected as well.

OI is usually transmitted as an autosomal dominant, but in severe variants of the disease, the parents are normal and a fresh gene mutation or autosomal recessive inheritance is postulated. The hallmark feature is bone fragility, with a tendency to fracture from minimal trauma.

Kindly elaborate on the clinical features that it presents Four major sub-types have been recognized: Type I: Most common or mildest type; patient exhibits less collagen than normal, with little bone deformity, although the bones are nevertheless fragile. There may also be a bluish discoloration of the sclera of the eyes.

Type II: Most severe form, with improperly formed collagen. A baby has very short arms and legs, a small chest, and a soft skull. They may be born with fractured bones or may also have a low birth weight and lungs that are not well developed.

Fractures are common; many babies

usually die within weeks of birth.

Type III: Severe bone deformities, where infants are often born with fractures, however, discoloration of the sclera may not occur. It is compatible with long life, generally of below-average height. OI may have skeletal with or without respiratory problems and brittle teeth.

Type IV: Bones are fragile and sclera may be of normal color.

The bone abnormalities are mild to moderate in severity, adults are shorter than average and may have brittle teeth.

In worst cases, the child is born with multiple fractures and does not survive. In less severe cases, fractures occur after birth, often from trivial violence. However, fractures unite readily, but in more severe cases marked deformities like curving of long bones, scoliosis, and short stature often develop.

What are the possible symptoms of OI? The symptoms of OI vary greatly within and between types, they include: • Easily broken bones • Bone deformities, such as bowing of the legs • Discoloration of the white of the eye (sclera), may be blue or grey • A barrel-shaped chest • A curved spine • A triangle-shaped face • Loose joints • Muscle weakness • Skin that easily bruises • Hearing loss in early adulthood • Soft, discolored teeth

Dual Energy X-ray Absorptiometry scan (DXA or DEXA scan): A scan of the bones to check for softening.

Bone biopsy: A sample of the hipbone is checked. This test is performed under general anesthesia.

Are there possible treatments for OI? The main goal of treatment is to prevent deformities and fractures.

How is OI diagnosed?

Diagnostic tests: X-rays: These may show many changes such as weak or deformed bones and fractures.

Lab tests: Blood, saliva, and skin may be checked.

The tests may include gene testing.

Bisphosphonate medicines: These are medicines that help to strengthen bones, reduced bone pain, prevented new vertebral fractures, reshaped previously fractured vertebral bodies, and reduced the number of long-bone fractures. They may be administered orally or intravenously.

Fracture management: The lightest possible materials are used to cast and immobilize fractured bones.

To prevent further problems, it is recommended that a child begin mobilizing the affected area as soon as possible.

Orthotic management: This may include bracing and splinting the affected extremities and spine.

Orthopedic treatment: e.g. Rodding.

Metal rods are surgically inserted in the long bones to improve strength, hold them in place (stabilize) and prevent deformities of long bones.

Dental procedures: Treatments, including capping teeth, braces, and surgery may be needed.

Physical and occupational therapy: To address fine motor skills, such as buttoning your shirt or tying your shoes, increase muscle power, flexibility, and joint range of motion.

Mobility devices: Wheelchairs, walkers, canes, or crutches can be prescribed to enhance mobility.

Thanks to the driver and the vehicles that paved way. The baby is now out of hospital after 42 days in KNH. David M Matayo: I remember that day I took my son to KNH from Nyahururu.

My son was admitted for over two months; precisely 36 days at New Born HDU. Thanks to KNH because today he is a big boy.

How has your learning experience been?

I have gained lots of field experience, especially about marketing KNH services. I have also gained soft skills that will enhance my working experience.

What have you learned about KNH? It is the largest and the oldest referral hospital in the country. A lot of teamwork goes into ensuring the institution runs smoothly.

What challenges have you faced so far?

Getting a grasp of the whole hospital was a challenge at first, due to its size but with great support from my members of the department, I got used to the place and I now understand it fully.

Your career plan?

I want to further my studies and become one of the leading marketing and communication officers in the country.

Change of guard: KPCC ushers in a new Finance and Administration Manager

By Petterson Njogu Kenyatta Prime Care Centre witnessed a successful handover as the new Manager, Finance and Administration. Ms. Hamina Kathambi took the reigns from the outgoing Mr. David Chibeka.

Mr. Chibeka will now be tasked to lead the Finance docket at the newly opened Mama Margaret Uhuru Hospital- Korogocho.

Those who attended the event hailed Mr. Chibeka as an excellent worker, saying it was under his stewardship that KPCC developed into a profit-making entity.

“We feel orphaned by your departure,” said Dr. Ngigi, Senior Director at KPCC. “David did not only have a commendable work ethic but he also ensured that we meet our financial contractual targets every year and that is not a mean feat,” he added.

Many referred to him as a mentor, highly professional, a steady reasoning and fair gentleman, a problem solver who always rose above pressure for the greater good of KPCC. During his speech, Mr. Chibeka thanked everyone, terming every milestone as a team effort and also issuing a rallying call to support the incoming manager.

“I thank the whole team I worked together with all along, it was such an honour and privilege. I am not going with KPCC; the strategies

that we have put in place must continue regardless. Ms. Kathambi brings on board loads of experience, so let us accord her every bit of support she may need and ensure KPCC soars to greater heights,” concluded Mr. Chibeka.

Mr. Chibeka’s tenure has seen KPCC make profits of Kshs. 1.7B up from a mere Kshs. 645m five years ago- a true testament of exceptional stewardship.

Additionally, he recently oversaw the rollout and execution of the Queue Management System (QMS) at Corporate Outpatient Centre (COC), among other accomplishments.

The KPCC team appreciates Mr. Chibeka’s input to the facility and wishes him happiness and fulfilment in his new role.

Mary Atamba Ilavo: The multi-talented, one-time colleague

Retirement is a term not so many people often look up to after years of service.

For Ms. Mary, the case was different when Newsline had a sit down with her. After about 35 years of service at KNH, she is more than excited to be finally taking the ‘final break from work’. She is retiring and cannot help but wait to see what the ‘outside world’ has in store for her.

True to her promise, Ms. Mary who by the time of the interview was on terminal leave, took her time to share what life at KNH was like.

Kindly give us a brief introduction of yourself and where you come from My name is Mary Atamba Ilavo. I have been working as the Senior Sterile Service Assistant (SSSA) at the CSSD for the last 33 years. I was born and bred in Kakamega town, the former Western Province. I started school in 1969 at Mukumu Girls. I attended both Primary and

Secondary school in the same province.

Growing up, what was your career aspiration?

I had a dream to become a nurse.

However, since I did not make it to become one, it was God’s plan I ended up working in a hospital.

Tell us about your family I am a wife and mother of 3 children. I am also a proud grandmother to 2 grandchildren.

Where did you work before you joined KNH? Immediately I completed secondary school, I worked at Mater Hospital for 6 years. On 9th January, 1987, I joined KNH with my first posting being at the Paediatric burns ward as the General Support Staff after successfully applying and getting the job. I was later on transferred to the CSSD as the Sterile Service Assistant (SSA) where I advanced to the position of SSSA till my retirement.

What has working at CSSD been like?

I am happy to have worked at the CSSD which is a very important department in the hospital for sterilization of all theatre sets. I worked with organized and proactive colleagues who made workflow easier.

I pride myself to have been the longest-serving staff in the department and was happy to nurture my fellow colleagues.

What co-curriculum activities did you like?

As a young girl, I developed my talent for signing from the teachers who gave us flutes for music.

The KNH choir started in 1987 where I have been singing the alto voice since. During my time, the choir performed at various hospital events and even got the opportunity to travel to National Presidential functions and music festivals where we competed against other choirs and emerged the best in major competitions. I remember the time we went to

Hungary in 2006 for a competition where we emerged number 1.

I will continue singing with my church choir since it is very therapeutic to me.

What else do you pride yourself in?

I was the founder of KNH Netball team which started in 1999. We have gone to both Uganda and Dar es Salaam in Tanzania where we were among the top teams. I have mentored new leaders and I would like to see them prosper to greater levels. I will be pointing at them and taking pride by saying I took part in bringing up the team.

In 2008, I was the best worker in our Department and got accolades for being the best employee. I received several certificates by participating in choir, games, and my department at large.

How do you feel about retirement?

I am happy and I thank God for giving me life to have worked at KNH. I am going home a strong and active person due to the nature of physical activities I have been engaging in. I can still do 100 metres sprint which I did when I was young at the Nyayo National Stadium.

My family is happy and looking forward to spending time with me.

Parting shot?

I thank the KNH management for the support they gave me as an employee; both at work and socially.

The games they support help staff a lot.

To my colleagues, I pray that you reach greater heights.

To my fellow choir and netball team members, I would like you to continue with your commitment by playing and singing outstandingly as this will uplift the hospital’s name.

For the many years I have worked at KNH, I will not forget our hospital; I will call it my hospital even after retirement because of the years I have spent here.

Physiotherapy: Helping you regain your physical health

As an institution that promotes research, KNH is dedicated to providing quality health care services, to aid individuals in achieving their highest level of functionality.

The department assists patients in achieving their rehabilitation to optimize patient experience through innovative, evidencebased specialized health care, facilitate training and research, and participating in national health policy formulation through accurate diagnosis, clinically proven treatment, intervention, and patient education.

KNH offers a full spectrum of services on orthopedic injuries, pediatrics, musculoskeletal injuries, neurological conditions, geriatrics, foot pain, back pain, and pain management (just to name a few).

The Physiotherapy department is a frequently visited clinic and they attend to all patients with different conditions daily.

They offer individualized treatment and also offer an affordable wellness fitness program on Mondays, Wednesdays, and Fridays between 12:45 pm to 2 pm.

Lamaze classes are also offered and Ergonomics through highly trained physiotherapists, consultants, and collaborations with the University of Nairobi.

“In every treatment process, a physiotherapist is vital,” said Mr. Kennedy O. Mogire a Principal physiotherapist and Head of the Unit physiotherapy department tells Newsline in an interview.

“Physiotherapy works to prevent the loss of mobility before it occurs, or recover from serious medical conditions and disabilities without the use of drugs. Physiotherapy provides services that address existing ailments, maintain and restore maximum movement to people throughout their lifespan including infants, children, adults, and geriatrics,” explained Mr. Mogire.

Common conditions that require physiotherapy attention are due to aging; that is degenerative conditions such as Arthritis, conditions due to injuries, which contribute to disorders that are brought about by musculoskeletal changes, backaches, muscles, joints, and anything that threatens an individual’s movement.

KNH physiotherapy department frequently receives patients with problems on; backache and neck are classified as musculoskeletal like leg, Arthritis, stroke and its complications, diabetes and its complications, neurological conditions. like nerve, paralysis, and children born with congenital conditions. This they collaborate with different clinics and different specialists e.g newborn unit and critical care units.

Mr. Mogire admits that KNH has a bigger burden in society for handling non-communicable diseases like cancer, cardiovascular diseases,

hypertension, diabetes, road traffic accidents and the worst of it is obesity; brought about by lack of exercise and lifestyle changes.

“We, however, accept the challenge and KNH is up to the task to help everyone at the moment, KNH has been able to set aside specialized wards for such patients where physiotherapists and other multidisciplinary teams conduct ward rounds, they address the problems and advise the best way forward. The physiotherapist can refer the patients accordingly and conduct a trend analysis for the patients and this enhances research and training and helps in addressing the gaps.”

“There is also a big burden in orthopedic, medicine and pediatrics which has the most patients with pneumonia and reproductive health issues and because KNH is a specialized hospital, the physiotherapy department allocates a physiotherapist in all sections (pediatrics and adults sections) and this makes our work easier,” adds Mr. Mogire.

Mr. Mogire concludes: “KNH should be supported to offer further training with more funding and adequate staffing, taking into account the rapid expansion of services and also providing modern equipment.

At the moment, the Physiotherapy department is supporting the annexes (Mama Margaret Uhuru Hospital and the KNH Othaya branch).

about every condition. On the contrary, there is a high degree of specialization within the industry, and it’s important to find

physiotherapists that have the training and experience to help you with your specific injury or condition.

Specialized ENT, Otology, and Filter Clinic services at KNH

By Emmanuel Wambua Imagine not being able to listen to your favorite music or smell the sweet aroma of your favorite meal or the strangely satisfying smell of the earth after a spring rain, or even worse, not being able to speak coherently because of a persistent and annoying sore throat?

It could also be your child’s swimmer’s ear that is making them uncomfortable or a clogged nose that is making it hard for him to breathe. These are but some of the conditions that ENT doctors treat.

A specialist who diagnoses and treats ears, nose, sinus, throat, head, and neck conditions is known as an Otolaryngologist (simply spelled Otologist).

Otology is a branch of medicine that studies anatomy and physiology of the ear as well as the diseases, diagnosis, and treatment that relate to it. Surgeons at the ENT clinic treat many medical conditions such as; ear conditions like an infection, hearing loss or trouble with balance, nose and nasal issues like allergies, sinuses (Endoscopic Sinus surgery) and growths, throat ailments such as tonsillitis,

difficulty swallowing, and voice issues, sleep troubles like snoring and obstructive sleep apnea (which means an individual’s airway is narrow or blocked and can cause trouble breathing while they are asleep).

The Newsline team interviewed the ENT Clinic in charge, Ms. Jacqueline Andal (Senior Nursing Officer), who has been in the department for the last 21 years.

On a normal day, Ms. Andal begins by briefing patients on the procedures to be followed, then works on the first files of the patients seeking ENT services. She assigns the different files to the Filter clinic where triaging is done.

At the Filter clinic, the doctors or clinicians obtain information about the patient’s condition by carrying out various tests to diagnose their illness. After the triaging process, the emergencies are reported and the patients are taken to the clinic’s satellite theatre, where minor surgeries are done.

Some of the minor surgeries that are carried out are; children swallowing foreign objects such as coins, foreign objects inserted in the ears or nose, over-swollen tonsils, or

any other accidents related to ENT. Different clinics are booked for different days within the ENT Clinic from Monday to Friday for the less urgent cases. The appointments are attended to by the consultants or registrars who recommend and book major surgeries in the main theatre where necessary.

Ms. Andal further informed us that the department has various specialized machines such as the BERA machine (Brainstem Evoked Response Audiometry) which is used to test any hearing impairment or difficulty in hearing, and there is also an ear surgery machine.

Other services offered are neck and throat surgeries such as voice box or larynx cancer which is mostly caused by cigarette smoking; majorly affecting men, and vocal cord conditions which mostly affect musicians, preachers, and politicians among others, when they strain their voices for long periods.

“There is also the Operation Eardrop Project, which is a partnership between KNH and Netherlands that does projects twice a year (March and November) for ear surgeries. The project is sponsored

by the Netherlands and they provide manpower, train the registrars and donate requisite equipment,” added Ms. Andal.

The ENT department also works hand in hand with the occupational therapy department to help autistic individuals with speech therapy.

Ms. Andal has had many success stories at the ENT clinic, where she has had a hand in saving lives and avoiding a potential disaster.

Other than that, she hopes that one day, NHIF will be able to cater to outpatient services for ENT patients that lack sufficient funds to pay for the various procedures done.

Overall, the ENT department at KNH has had more compliments from our patients than complaints. It is still putting in more effort to provide the best services to our patients within the East Africa region.

Her final word to the general public

is that one should at least visit an ENT doctor once a year for check-ups since most ENT problems initially

produce symptoms that could easily be confused with other minor health issues.

KNH marks World Hepatitis Day 2022

By Yvonne Gichuru Hepatitis Day, one of the World Health Organization’s seven officially mandated global public health days is observed every year on 28th July to raise awareness and promote global action on viral hepatitis.

This year’s theme; ‘Bringing hepatitis care closer to you’ , aimed to raise awareness about the need to simplify and bring hepatitis care to primary health facilities, communitybased venues and locations beyond hospital sites, so that care is closer to communities and people wherever they are.

‘I can’t wait’ was the new campaign theme to launch World Hepatitis Day 2022. It highlighted the need to accelerate the fight against viral hepatitis and the importance of testing and treatment for people who need it. The campaign also called for immediate action and the end of stigma and discrimination.

In this regard, KNH in conjunction with the Ministry of Health (MOH), National Aids and STIs Control Programme (NASCOP), MSF – Doctors without Borders, President’s Emergency Plan for AIDS Relief (PEPFAR) – Centers for Health and Education Programmes (C-HEP);

University of Maryland, Nairobi Metropolitan Services (NMS), Kenya Medical Research Institute (KEMRI) and MegaLife Sciences Kenya came together and set up an event to honor and celebrate the day.

The day kicked off with a health walk from the Ministry of Health – Afya house to the KNH Administration Block led by a marching band from the National Police Service together with patient representatives and beneficiaries from the Medicationassisted treatment (MAT) programme and other health care workers to commemorate the day and raise awareness on Hepatitis.

The event saw various invited guests such as representatives from the partners aforementioned in attendance who all participated in the implementation of Hepatitis screening, diagnosis, management and vaccination activities that took place.

Dr. Rose Nyabanda – Director, Diagnostic and Health Information Services gave the welcoming remarks on behalf KNH CEO, Dr. Evanson Kamuri, EBS during the ceremony.

“As KNH our main objective is to always ensure better access to treatment and care for all our

assisted therapy to people who use drugs and treatment for viral Hepatitis in Kiambu County called upon the government to ensure tests’ availability for the vulnerable population.

“Hepatitis is amongst us and we have a responsibility to collectively change our attitude for the opportunity for prevention, testing and treatment to prevent the spread, ” she said.

“Hepatitis is responsible for 125 deaths every year in Africa despite the availability of treatment. We recognize as WHO that there are more than 90million people living with Hepatitis in Africa. Kenya carries a high burden of chronic Hepatitis B of about 1 million people in need of care and treatment. We call upon countries to advance towards decentralized care and integrate efforts to the primary care level in order to scale up service delivery of hepatitis B and C services that is in line with the Universal Health Coverage principles” said World Health Organization representative Mr. Otieno Cosmas on behalf of WHO Country Director – Kenya, Dr. Abdourahmane Diallo during the ceremony.

Mr. Richard Wanjohi – a beneficiary from Mathare National Teaching & Referral Hospital – Medically Assisted Programme also gave his experience on his journey with hepatitis.

“I started smoking cigarettes then graduated to self-injection drugs.

I used to share injections with my colleagues then later got infected with Hepatitis C,” he said.

After his diagnosis, he joined Support for Addictions Prevention and Treatment in Africa (SAPTA) and after 3 months of treatment, the

infection was virally suppressed and he was free from hepatitis.

“Hepatitis is a common infection within both the hospital and the community. We have active programmes in the renal unit and maternity unit in KNH where patients get screened against Hepatitis due to the nature of their conditions.

We have advanced diagnostic capabilities to test for this disease

and drug treatments are also available in KNH.” said Dr. Juliana Mwikali Muiva – Head, Pediatrics Department - KNH. During his keynote address on behalf of Dr. Patrick Amoth - Ag. Director General, MOH, Dr. Simon Kibias - Head, Directorate of Health Standards, Quality Assurance and Regulation reiterated on the need for public health preventative measures in the management of Hepatitis.

“Let us unite and create awareness as we celebrate the progress we have made and mitigate the challenges.

Together we shall win this war against Hepatitis. We can’t wait!” He concluded.

The free testing and vaccination of Hepatitis B was offered to everyone including KNH staff, patients and visitors. Folks turned up in large numbers to get the services and the medical team managed to test 265 people and vaccinate 258 people against Hepatitis B.

KNH participates in an inter-hospital Mass Casualty Incident drill

By Edel Q. Mwende The Hospital participated in an inter-hospital Mass Casualty Incident drill on Tuesday, July 26, 2022.

The drill involved four (4) private hospitals; Aga Khan University Hospital, Gertrude’s Children’s Hospital, MP Shah Hospital and Avenue Hospital.

The Kenyatta National Hospital Disaster Management team played a mentorship role to the four private hospitals in the preparation for the MCI drill.

Three representatives from the KNH Disaster Management Committee; Ms. Poline Njue (Nursing, Emergency Ward 42), Mr. Victor Waweru (Facilities & Services) and Ms. Edel Mwende (Marketing and Communication), were sent to Aga Khan, Gertrude’s, and Avenue Hospital to play observer roles and provide feedback on corrective measures for effective disaster preparedness and response.

The main objective of the drill was to test the MOU that exists between the four institutions as pertains to disaster response during mass casualty incidents. The drill also tested the preparedness of the private hospitals to respond to and manage disasters.

The drill scenario was a stampede that occurred after a bomb scare at Aga Khan Academy involving teachers and students with fractures and poly traumas.

The staging was done at Aga Khan Academy on 1st Parklands Avenue. There were 70 players who were tagged in 3 different tags; 18 red, 29 yellow and 23 green. The players were distributed among the four (4) hospitals as per their MOU. Aga Khan University Hospital has carried out several MCI drills before. They received 19 casualties from the incident with fractures and burns. The hospital was well prepared for disaster response and it offered a benchmarking opportunity for all participants.

Gertrude’s Hospital has carried out various drills to test its disaster response and preparedness. They received 17 casualties from the incident with fractures, burns, and abdominal injuries and were able to handle them effectively. Some gaps were identified and suggestions were shared with the team to enhance disaster preparedness.

It was the first time that Avenue Hospital carried out an MCI drill.

They received 20 casualties from the MCI with various injuries including fractures and burns. The drill was managed effectively and

feedback for improvement was shared.

The rest of the 14 casualties were received by MP Shah.

Evacuation from the scene was done in under 1 hour.

The drill ended with a face-toface and zoom debrief from the emergency response teams and drill participants. Drills are an important exercise for the health care sector as they ensure that the teams are always ready for disaster response and this collaboration offered a benchmarking opportunity for all participants.

Adolescent Sexual Reproductive Health at KNH Youth Centre

By Daniel Nthumo Adolescent Sexual Reproductive Health refers to the physical and emotional well-being of these individuals.

This includes their ability to remain free from unwanted pregnancy, unsafe abortion, Sexually Transmitted Infections (STIs) including HIV/AIDS, and all forms of sexual violence and coercion.

Adolescents need to understand the risks, responsibilities, and consequences of sexual behaviors.

Sexual Reproductive Health (SRH) among the youth in low and middle-income countries is still a public challenge.

Early school-based education programs and sexual health information sharing between teachers, parents, guardians, and youth have been inadequate to protect them against the sexual health risks to which they are exposed.

To know more about sexual reproductive health, Newsline visited the Kenyatta National Hospital Youth Centre where we engaged with Ms. Caroline Thuranira, Assistant Chief Nurse (ACN), in charge of the Youth Centre.

Synopsis of KNH Youth Centre “The KNH Youth Centre was established in 1999 as a project proposal to provide mental health services and safe abortion. The Centre at its formation was run by University nurses.

The project was finalized and officially handed over to KNH between the years 1999 to 2000. The department underwent a series of changes of name. Initially, it was known as a high-risk clinic that dealt with delicate pregnancies. It was later changed to Adolescent Clinic whereby the name was rejected since the youths felt like they were treated like patients. The name was then changed to what it is currently; the Youth Centre which the youth appreciate being

associated with.

In 2005, the Centre introduced a policy incorporating standards of youth-friendly services, which helps to handle the youth. This concept was taught to other health workers who directly serve the youth and it involves being non-judgmental, exercising confidentiality on youth matters, and also giving youth a safe space where their ideas are respected. The concept was affirmed by the Government of Kenya through the Ministry of Health and the Policy was produced and ran for 10 years and was later reviewed in 2016.

The concept of giving youthfriendly services is categorized into two: Stand-alone which is a clinical model that includes all the youth.

The second one is the integrated model where they teach youthfriendly services to people working at the antenatal, post-natal, labor ward, clinic 66 for them to deal with the youth in a friendly manner,” said Ms. Thuranira.

Youth Centre Services The key counseling services that are offered about reproductive health include; teenage pregnancies, post-abortion care, Gender Based Violence counseling (rape and traumatic experiences), STIs management, Family Planning, Preventive services which include Reproductive Health information, Cancer screening (both cervical and prostate cancer) and HIV testing.

“KNH is one of the pilot youth centers that is very unique and offers more comprehensive services that go beyond reproductive health services.

When the youth come and have other needs aside from mental and reproductive issues, they are still able to be referred to the right place,” she explained.

‘Currently, we offer both reproductive and mental health services, such as Psychiatric services (depression support or mental illness support) and psycho-social support (includes

more counseling) where we work with social workers, occupational therapy for adolescents because there is need to protect them as they remain to be vulnerable to the community. She further explained that they get referrals from schools as they also collaborate with the wards and young oncology patients where they help them manage the adolescent period.

“We have helped students write appeal letters to return to their institutions to continue with their studies,” she added.

The services at KNH Youth Centre are free for counseling and consultation. However, drug testing and reports letters are charged at Sh1200 and Sh600 respectively.

Achievements of the Youth Centre KNH Youth Centre participated in the development of the National Adolescence sexual reproductive health policies; the latest being the 2016 policy.

It is also the Centre of excellence for bench-marking for other youth centres across Kenya.

KNH hosts nurses’ and pharmacists’ oncology training

By Samira Khisa Yusuf Training oncologists is the first step toward reducing the burden of cancer and equipping medical workers with the skills and knowledge to further their practice.

According to research done by the World Health Organisation (2021), cancer is the second leading cause of death in the country.

Kenyatta National Hospital hosted the Oncology training of nurses and pharmacists from various parts of the country.

The event that kicked off on Tuesday 19th July 2022 to Friday 22nd July 2022 at the KNH Diagnostic and Reporting center brought together some of the key personnel in the health sector including Dr. Martin Mwangi, Head of the National Cancer Institute of Kenya (NCI-K), Dr. Joy Frida Chepchumba - Program officer in charge of treatment, Palliative Care and Survivorship at the National Cancer Control Programme (NCCP), KNH Director, Nursing Services - Ms. Raheli Mukhwana and Paediatric and Nursing Oncologists from KNH among other facilitators.

The event was organized by the department of Non-communicable Diseases under the Ministry of Health and aimed at training the medics on Chemotherapy safe handling.

The trainees were drawn from a total of 12 regional centers across the country including Meru, Nyeri, Embu, Kisumu, Mombasa, Bomet, Kakamega, Makueni, Garissa, Nakuru, Vihiga, and Machakos.

Other participants in the training program included Kenyatta University Teaching and Referral Hospital, Moi Teaching and Referral Hospital as well as Kenyatta National Hospital- Othaya.

The extensive training covered fundamental areas including but not limited to policies and perspectives on Chemosafe handling, Cancer Treatment modalities, Chemotherapy routes of administration, Standards of practice for handling

“The drugs are very dangerous, especially to our nurses and pharmacists who are directly

involved in the mixing and administration of the drugs to the patients. Gears that include specific kinds of masks and gloves should be used in chemotherapy sessions. All processes of mixing chemotherapy drugs should be done in a biosafety cabinet to avoid exposure.
