LEADING RELIABLY TO Zero HARM LEADING RELIABLY TO Zero HARM

LearningBoards LIFE Inspira LIFE holds their Daily Huddles virtually at the beginning of each day, Monday through Friday. Information shared at the daily huddle is then reported on the Inspira LIFE Daily Safety Briefing each morning. The team begins by reviewing the daily message in Leading Reliably, completes a look back and look forward on network and campus specific updates, provides a Daily Safety Message, and completes a round table discussion with all staff present at the huddle. Megan James, manager of Social Services and Behavior Wellness shared, “During a Social Work Departmental Huddle, several staff members reported they were no longer receiving messages via text/email in a timely manner, if at all, and calls were dropping. This made it very difficult to work effectively especially while the staff are essentially working remotely, and this is the method in which they communicate with our participants and colleagues. The issue was added to the Red (new) section of the Learning Board to help clarify if the current work assigned cell phones were fully upgraded. All staff were asked to provide their assigned number along with specific issues they were having. This information was provided to the IS department for further review. It was determined that seven phones were due for upgrades. Since only three were experiencing problems at the time, it was determined we would ask for those three to be upgraded and then report out whether this was a successful intervention.

The LIFE team holds their daily huddle virtually through Microsoft Teams.

The upgraded phones were ordered, and delivered to the LIFE Center. Another staff member reported massive amounts of dropped calls, missed communications and calls and voicemails that do not populate - causing the voicemail box to be full despite nothing listed. This staff member was added to the list of staff to receive an upgrade. The new phones arrived and IS was able to transfer information onto new phones and return old phones.” The Social Work Team now has the tools needed to successfully meet the needs of participants and their department. Staff celebrated this at huddle and shared,

“It is great to see reported concerns move from the Red (new) section to the Solved (green) section of the Learning Board.”

Briefing. Once the safety concerns are captured, they are added to the EMS/Ground Transport virtual Learning Board for all of the staff to see. Some items are closed out immediately on the Daily Safety Briefing, but there are some safety concerns that take longer to solve.

A recent safety concern that was reported on the EMS/ Ground Transport Daily Safety Briefing was a radio that was not functioning properly on truck 316, causing a safety concern for communication between the staff. Kasey shared, “I took the safety concern from the call and placed it in the New (red) section of the virtual Learning Board, assigning it to the radio repair team. The radio team then looked at the truck and documented what was needed for repair. As the team worked on it, the item was moved into the Working (yellow) section, and once the radio was functional again it was moved into the Solved (green) section.” Amy Cattell, field supervisor, Ground Transport shared, “Ground Transport is a large department, spread throughout different locations in southern New Jersey. The Daily Safety Briefing provides an opportunity for all our crews to come together as one large group, to share safety and patients’ rights concerns with our management team. Issues reported on these calls are taken seriously and assigned to the appropriate person for follow-up. We rely on our employees to recognize and report issues as they occur - it is essential to our operation.”

Reliability

Speak Up for Safety Great Catch of the Month Jessica Kendig, HIM specialist, LIFE

“Jessica Kendig, HIM specialist at Inspira LIFE knows the importance of the HRO Tool Validate and Verify. Jessica applied the HRO Tool as she was validating and verifying information when utilizing the medical record system. During a Medical Administration Record (MAR) retrieval project, Jessica could not locate a participant’s chart in a contracted facility’s electronic medical record. The MAR is used to complete medication reconciliation. If this task is not completed in a timely manner, a medication error can occur, which can potentially cause harm to a participant. Jessica used the Validate and Verify HRO Tool to confirm the patient was admitted to that specific facility. After confirmation, Jessica utilized ARCC to notify the facility, who confirmed that the participant’s insurance was entered erroneously upon admission, which caused the error. The insurance information was updated, and the MAR was retrieved. It is important that everyone makes a habit of using our HRO Tools, such as Validate and Verify, to ensure information is accurate at all levels. By taking the time to not only verify the patient’s information, but also alert the facility and correct the error, Jessica prevented possible patient harm. Thank you, Jessica for utilizing effective validation and verification methods to maintain safe and reliable patient care.”

DAISY Award Winners Joscelyn Parrella, R.N., 4th Stepdown, Mullica Hill

Ryan Ruiz, R.N., 3 West Observation, Mullica Hill

My mother was a patient at Inspira Mullica Hill on the 4th floor for a long, hard month. The nursing staff was wonderful. Joscelyn joined my mother’s team a couple days into her stay, and never really left. She went above and beyond in everything she did. She spent hours in my mother’s room, answering every single question or concern. She even took the time to gently lotion her legs, doing all she could to help with the pain. Most of all, she was a much-needed friend to my mother in her time of need. Joscelyn was an advocate who gave us the answers we needed. If the doctors could not give us a straight answer, Joscelyn did her best to explain. If we needed a shoulder of support, Joscelyn was there holding our hand. After a month in the hospital, the decision was made for her to come home on hospice. Even though my mother was no longer her patient, Joscelyn went out of her way after her shift to come see us. I want to personally thank Joscelyn for the time she spent with my mother, and for the profound impact she made on her life during such a difficult time.” – Patient’s Daughter

“My nurse Ryan has been excellent. He has showed great compassion. I accidentally sat on my glasses and broke the arm off. When he came in, I asked for tape to so I could fix them, but he fixed them for me instead. It took a few minutes to do, but he did it. I had tears in my eyes - anyone else would have just given me the tape. He also did a bunch of little things to make me more comfortable like talking to the doctor about changing my diet and making sure I could shower. I've been here a few times, and there was only one other nurse I felt this way about. They are both great nurses and will go far in their career. As for Ryan, he deserves this award for all he does. I see him running around doing all he can for his patients including me. I feel he deserves this award for being such a kind, compassionate, caring, and hard-working nurse.” - Patient

Tallie Ross, R.N., 2 South, Elmer “Tallie was very helpful in getting me to open up about my depression and the thoughts I was having about hurting myself. I never listen to anyone when it has to do with my depression, but I did this time. Thank you Tallie – you made me see that my family is more important, and I need to be here for them. I will miss you when I am discharged. Thank you for saving me.” - Patient

Helen Bennett, R.N., Medical Acute, Vineland “I came to the hospital with abdominal pain and was admitted on June 14, 2021. Everyone was wonderful but in the short time I was there, I met one of your special nurses, Helen Bennett. She has a beautiful soul and heart – she put an afraid 63-year-old woman at ease. I am sure this is not the first letter you have received about Helen – we are all blessed to have her work for Inspira. There should be more Helen Bennetts in this world.” - Patient
