Hierarchy of Controls Definitions

Elimination

Substitution

This control measure involves eliminating or removing the risk in its entirety

This form of control involves substituting a safer process or material for the hazardous process/material found.

Isolation

Engineering

This control involves separating the hazard or hazardous work practice from employee’s other work areas. This may involve sectioning off the area by erecting barriers or by moving either the hazardous work practice or the „other‟ employees and their work practices.

This method of control involves designing and/or adding physical safety features to plant or equipment.

Admin

PPE

This type of control is most effective when used in conjunction with measures mentioned above or as an interim control whilst more effective control measures are developed and implemented.

PPE is not a particularly effective control method and should only be used:

• When all other control measures are impractical; or

• In conjunction with other more effective, control measures.

Once all electricity has been turned off, complete a pre-work risk assessment of the restricted space by looking around the space to identify hazards that may pose risks. These may include hazard identified in “Identify Hazards” column (left).

Establish rescue procedure

• Exposure to potential injury

L3 C2 HR LR

• If hazards assessed are deemed low risk or not evident, ensure someone is aware of where you are and contact with them is maintained until work is complete.

• If hazards assessed are deemed medium to high risk, ensure a standby person is present at the worksite.

• Identify method of communication with entrant/s whilst entry takes place.

• Establish method of entrant retrieval will be employed in the event of an emergency.

• Be prepared to remove large sections of roof to gain access for rescue or emergency services if working in ceiling space.

• Identify first aid processes to be employed

• Establish fire prevention & fighting practices to be employed

• STANDBY PERSON IS NOT TO ENTER SPACE IN EVENT OF EMERGENCY

• If entry because of an emergency purposes it must only be undertaken once hazard has been eliminated or controlled

Make safe entry

• High temperatures.

• Evidence of vermin

• Sharp objects

• Asbestos

• Loose fill Asbestos

• Mould

• Type of lighting

• Type of insulation material

• Accessibility to the work area (e.g., Cramped, and awkward positions)

• Isolate any energy source (example: Power in roof or subfloor) by turning off main switch, solar, battery and generating equipment and locking out.

• Being aware that heat and humidity may cause heat stress, so make sure fluid intake is sufficient to ensure you do not become dehydrated

• Taking additional lighting (e.g. torch) with you.

• Taking care accessing and traversing the work area, avoiding tripping over debris, material and ceiling trusses

• Step carefully on ceiling joists or other beams not the ceiling material (i.e. Gyprock sheeting) to avoid risk of falling or injury

• Using/providing appropriate tools - preferably manual or batteryoperated tools

• Being aware of the location of electrical cables, fittings and equipment and avoiding contact with them

3

• Location of electrical wiring and water or gas piping. L2 C2 HR LR

• Ensuring that, if fixing points are required (e.g. saddling TV aerial cable in place), fixings are well clear of all electrical cables and equipment

• Making sure you do not damage any electrical cables or electrical equipment. Please note: if any electrical cable or equipment is damaged, consult with the owner and engage a licensed electrical contractor to inspect the installation

• Wearing appropriate, well maintained and correctly fitted personal protective equipment when working in dusty ceiling spaces, including:

• a half-face (class P1 or P2) disposable particulate respirator, in accordance with AS/NZS 1715;

• a head covering and goggles, to avoid eye irritation.

• long-sleeved, loose-fitting clothing and gloves, to minimise skin contact with insulation material; and

• wearing appropriate footwear.

• Keeping your work areas clean and clear of fibres and dust and place waste in plastic bags capable of containing the dust.

Exit/ Finish job

• Exposure to potential injury

L3 C2 HR LR

• Exit using established safe exit process.

• replace any insulation material that may have been disturbed or moved for access to the work area, ensuring that it is not covering any electrical fittings or equipment, especially down lights

• Secure all entry and exit points to ensure no one can re-enter

• Ensure all energy sources that were isolated are re-energised and lock out tag outs removed.
