Hierarchy of Controls Definitions

Elimination
Substitution
This control measure involves eliminating or removing the risk in its entirety
This form of control involves substituting a safer process or material for the hazardous process/material found.
Isolation
Engineering
This control involves separating the hazard or hazardous work practice from employee’s other work areas. This may involve sectioning off the area by erecting barriers or by moving either the hazardous work practice or the „other‟ employees and their work practices.
This method of control involves designing and/or adding physical safety features to plant or equipment.
Admin
PPE
This type of control is most effective when used in conjunction with measures mentioned above or as an interim control whilst more effective control measures are developed and implemented.
PPE is not a particularly effective control method and should only be used:
• When all other control measures are impractical; or
• In conjunction with other more effective, control measures.

• No person is to ever work alone on an excavation job if the job is ranked as a medium or high risk.
• Machine Operator to conduct daily visual prestart inspection of machine and report any issues.
• Plant is to be maintained in accordance with manufacturer's specification and all records kept for verification if requested.
• Only Operators that hold a verification of competency can excavate.
• Workers to be made aware of overhead clearance heights and requirements.
• Tiger tails and a spotter to be in place when work activity is being undertaken in close proximity to overhead powerlines.
• Trucks parked in areas where no cables are above whilst machines are being unloaded.
• Ensure ramps are placed in correct positions and locking pins correctly used.
• Traffic Controllers to control vehicles movements in the area, especially when loading or unloading machines.
• Pre-determined unloading / loading zone
• Pre-start toolbox talk/risk assessment completed covering site evacuation, excavation emergency response and any other site-specific hazards. Be aware of prevailing weather conditions. Implement measures to prevent polluted water runoff entering watercourses.

• Conduct ground search before breaking any ground. Use ground search contractor or Dial 1100 for dial before you dig.
• Site familiarisation. Know nearest live points at all times. Where necessary, either place barricades between personnel and live terminals or cover with appropriate plastic sheeting.
• Precautions are to be taken by operators when moving plant to avoid contact with any overhead conductors. Tiger Tails and a spotter to be in place when work activity is being undertaken in close proximity to overhead power lines.
• Work area to be clearly defined using barriers and signs.
• Access to work area only via specified route.
• Spotter to be used where necessary to ensure Limits of Approach is maintained.
• If asbestos is found, work is to cease and the asbestos management plan (AMP) must be followed. All personnel working in excavation are trained in identifying asbestos prior to work.

Fall Prevention
• Falls L2 C2 HR LR
• All excavations are to be protected in such a manner so as to prevent any person falling into the excavation.
• In addition to the primary 2-metre-high site fence where the potential to fall greater than 1.5 metres exists, in this case Para webbing / Bunting or similar should be positioned at least 2 metres from the edge of the hazard and be adequately supported. Hazard tape is not acceptable.
• Provide alternate pathway for public to access their route safely.

Back Filling Excavation/Trench

• Plant operator to approach excavation / trench at 90 to the side.
• Plant operator to back fill in layers so as shoring can be gradually lifted if applicable.
