Hierarchy of Controls Definitions

Elimination
Substitution
This control measure involves eliminating or removing the risk in its entirety
This form of control involves substituting a safer process or material for the hazardous process/material found.
Isolation
Engineering
This control involves separating the hazard or hazardous work practice from employee’s other work areas. This may involve sectioning off the area by erecting barriers or by moving either the hazardous work practice or the „other‟ employees and their work practices.
This method of control involves designing and/or adding physical safety features to plant or equipment.
Admin
PPE
This type of control is most effective when used in conjunction with measures mentioned above or as an interim control whilst more effective control measures are developed and implemented.
PPE is not a particularly effective control method and should only be used:
● When all other control measures are impractical; or
● In conjunction with other more effective, control measures.
Workers

Plant operation
● Unsafe plant.
● Unsafe operation
● Plant roll over causing crushing.

● Plant MUST only be operated by a person who has been deemed competent in the use of that plant.
● Ensure that the applicable Plant Inspection Checklist is completed prior to work commencing.

● Inspect daily all tie-down points, ramps, chains, and tie-down mechanisms are in good order.
● Do not use plant with any defects and ensure that the Log books are completed.
● Seat belts MUST be worn where fitted.
● Only use plant with Roll Over Protective Structure (ROPS).
● Lock quick hitches change buckets and/or attachments away from work area and workers.
● Use warning signs and engage flashing lights and reversing signals where appropriate (audible reversing alarm).
Workers3
Carrying out work near overhead power lines or underground services
● Working near power lines, underground services.
● Electrocution, hitting asset.

Loading / Unloading of Trucks

● Use an “Authorised Person” (observer / spotter) to assist the plant operator(s) by maintaining sight of the service(s).
Overhead:
● At all times whilst working within the vicinity of high voltage a spotter will be provided by Talbots for the plant operator.
● Operator to ensure no part of the plant is to come within the following distances.

● 132,000 volts 3 meters
● >132,000 volts 6 meters
● 330,000 volts and over 8 meters
● Loading zones will be pre-identified.
● Tiger tails will be used if needed to provide useful visual indication to workers completing works in the vicinity of the overhead electric lines.
Underground:
● All workers to be made aware of the content of the DBYD and service location documents.
● Spotter to be in place at all times while excavation being undertaken by operator.
Workers
● Plant roll over
● Plant fall into excavation
● Pedestrians and motorists in close proximity to moving plant.
L3 C2 HR LR
● Operator is to check the workplace for rough and uneven surface.
● Roller weighing over 2.7 tonne is to have roll over protection (ropes) in place prior to commencing work.
● Establish designated area for placement.
● Operator must ensure to observe distance from edge of excavation.
● Use of smaller compaction plant, petrol whackers and walk behind rollers should be used closer to the edge of excavation.
● Exclusion zone to be set up around the works area prior to commencement for the safety of both motorists and pedestrians.
