#SCOTHIF has become a key meeting place for representatives from Scottish National Government, NHS Scotland, Health Boards, IJBs and Social Care Organisations to develop their strategic approach to integration of health and social care services across Scotland. The 2020 Vision provides the strategic narrative and context for taking forward the implementation of the Quality Strategy, and the required actions to improve efficiency and achieve financial sustainability. The Scottish Government’s 2020 Vision is that by 2020 everyone is able to live longer healthier lives at home, or in a homely setting and, that we will have a healthcare system where there is a focus on prevention, anticipation and supported self-management. Working in partnership, the UK Health Informatics Forum (UKHIF) and the Digital Health & Care Institute (DHI) have created a platform to engage senior health and social care stakeholders from across Scotland, to understand how latest, innovative technology will enable an accelerated move towards patient centric integrated health and social care. Attendance at the forum provides a key insight into the role effective technology implementation plays in achieving integrated services and ultimately improved services to the public. Industry experts will have the chance to share examples of best practice, giving a 360 degree view of health and social care across Scotland, reducing time replicating what is already being implemented effectively by other healthcare providers, harnessing existing experience. Following the ever popular #UKHIF interactive format, the agenda is designed to raise the latest challenges being faced by health and social care providers responsible for creating a fully integrated care journey for patients and applying appropriate technology solutions to overcome these issues. In the build up to the forum, delegates will have the chance to provide specific areas they need advice and guidance on, this will form the agenda ensuring resolutions are reached using the expertise of health and social care providers and the technology suppliers in attendance. The success of these meetings are based on the number of healthcare providers requirements answered on the day, this forum is about turning words into action, to identify opportunities to improve patient services and apply specific technologies to implement that change. Established by NHS24, University of Edinburgh and Glasgow School of Art; the Digital Health & Care Institute (DHI) will co-create sustainable economic growth through new products, services and systems developed together with businesses, academics, healthcare specialists and citizens. Our aim is to generate high value health and social care solutions to the benefit of the people of Scotland and wider. Scotland provides a rich platform for academic excellence, with nineteen Universities, some of whom lead the way in informatics, life sciences and medicine. This is built upon by progressive Health & Care services, willing and able interact with other sectors in pursuit of innovation. The Digital Health & Care Institute (DHI) harness’s these capabilities, delivering value for academic, business and civic partners through its dynamic innovation model. The model is made up of the three settings below, which will overlap and sequence dynamically to meet the needs of the opportunity in question. The Digital Health Exploratory engages a wide range of national and international organisations to develop priorities for action informed by the most current, diverse and relevant research, innovation and knowledge sources. This provides a crucial horizon scanning capability, allowing Scotland to design and innovate for the future. The Exploratory runs activities to stimulate the development of ideas that, in the fullness of time, will deliver opportunities developed both inside and outside the DHI. The Experience Laboratories provide a safe environment where users (service users, carers, clinicians, practitioners and third sector), businesses and researchers can collaborate and rapidly prototype in response to health and care challenges. Experience labs provide a creative and innovative environment (replicating real life practice) using design to carry out rapid cycles of experience trialling of new ideas including new technology, new services and roles and behaviours (often the biggest barrier to successful innovation implementation). Ideas gaining evidence of success in Experience Laboratories may become candidates for further research, development and exploitation. The Digital Health Factory supports our members in progressing a known solution much closer to market and deployment. The Factory offers a facilitation and resource platform, including access to expertise and test environments, business mentoring support and facilitation to source funding. The Factory works with industry, health and care partners, third sector and university partners to develop commercialisable solutions to real life situational challenges that can make a difference in clinical, professional or user based settings.

AGENDA ARD O B Y R O IS V AD Our agenda board members have come together from different backgrounds and have bought together expertise from a number of sectors. The issues they have highlighted reach across both the public and private sectors with the adoption of new and already available technology as a central theme. The combined expertise of all our agenda board members highlights issues within areas such as the utilisation and management of data within the health service, the Integration of health and social care systems as well as the barriers to technology implementation for end users. All of our agenda board have endorsed the format of our events as a platform for co-operation between end users and private companies which provides genuine progression. HOW THE DAY WORKS

Influencing change in key areas: Existing organisational cultures - changing the mindset of health and social care providers along with the public to understand the benefits of integration. Identifying a shared language - between patients / care providers, commissioners / providers and commissioners / providers / private sector. The fear of data sharing - providers and patients need to be educated as to the major benefits this can have on their services and reassured of the security that is in place to keep this data secure. Interoperability of technology - reducing individual effort is key to successful participation and engagement, technology that talks to each other is vital for swift uptake and providers need to see this in action. Interactive Panel Sessions: The panel sessions make up the core of the forum and are used to create an interactive, animated discussion between delegates. Each panel will feature six delegates from across Scotland responsible for health and social care integration. The agenda for the session will be formulated and developed throughout the build up to the forum, all registered delegates will have a chance to contribute themes and topics for discussion which ensures the very latest subjects will be addressed. Audience delegates will be given the option to submit questions prior to the forum that will be delivered in a “Question Time� style throughout the panel sessions. Innovation in Action Showcase: Creating a platform for innovation and new approaches to integrated care, the showcase will allow up to six delegates to provide short, 15 minute presentations that highlight the ways in which innovative techniques have been used to improve services. Interaction Spaces: To progress the discussions as far as possible on the day, we create an area that features selected NHS Trusts and technology vendors. Delegates will be on hand to highlight project requirements and to demonstrate the best way to overcome specific challenges discussed during the panel and networking sessions.

DISCUSSION THEMES

Forum objective: The challenge for delegates is to create a strategy for technology implementation that supports healthcare providers to achieve a fully integrated, patient led health and social care system focusing on the overall care journey of patients with a personalised, preventative approach.

Topics for discussion: The role of technology in enabling a truly integrated health and social care system in Scotland, how are healthcare providers adapting their exisiting culture to embrace this new approach to working? Are the levels of uptake from frontline staff as high as expected? What examples of best practice can be shared and applied by delegates that can really enhance the quality of the patients care journey? Do delegates have specific case studies that could be shared? What level of collaboration between NHS Scotland, Health Boards, Social Care Providers and Councils needs to be achieved to deliver integrated care? Should the Emergency Services be involved to create a truely integrated service for the public? What technology/infrastructure needs to be in place to enable healthcare providers to integrate health and social care? How easy is it for suppliers to provide case studies of clinical and cost effectiveness of their latest solutions? Are there any opportunities for disinvestment of existing technologies that would enable uptake of more resilient solutions that can be upgraded overtime? How are experts using data to reduce the number of patients visiting hospitals? Can these examples be replicated across Scotland? Techniques and best practices for capturing and reporting on health outcomes in different clinical settings, are we measuring the right things? Once the data is collected and in the system, are healthcare providers using it to improve patient care?
