Medical debt erased for thousands of Oakland County residents
Over 14,000 Oakland County residents opened their mailboxes last December to find letters notifying them that their medical debt had been canceled. The total equated to $9.1 million. With the help of Undue Medical Debt, a nonprofit organization that partners with health systems, individual hospitals, and physicians’ groups to identify those who are eligible for medical debt relief, Oakland County plans to eliminate approximately another $190.9 million for thousands of other residents. Near the time of publication in March, an Oakland County official told Hour Detroit there were tentative plans to send out the next batch of letters in coming months. “This initiative highlights our commitment to building a healthy and thriving community, lowering costs for working families, and helping people achieve economic freedom,” Board of Commissioners Chair David T. Woodward, D-Royal Oak, says.
Corewell Health urges patients to know before you go
To address the long waits in emergency rooms that occurred over the winter in metro Detroit, in February, Corewell Health urged people to properly match their symptoms with where they should be treated. In other words, stop using the ER for primary care. In a handy chart found on its website, you can see where to go (or not go — virtual urgent care is an option) for different maladies. For example, moderate abdominal pain calls for a visit to primary care or urgent care, while severe abdominal pain warrants a trip to the ER. Minor burns? Go to virtual urgent care, primary care, or urgent care; for severe burns, go to the ER. Gov. Whitmer signs bills focused on reproductive freedom
As reproductive freedoms continue to be revoked across the country in states like Louisiana, Florida, and Texas, Gov. Gretchen Whitmer signed 16 bills on Jan. 21 expanding those rights in Michigan. These bills give women easier access to health care by allowing pharmacists to directly prescribe birth control and requiring insurers to cover it; improve someone’s ability to identify potential risks in their health by requiring health insurance coverage of blood pressure monitors for pregnant and postpartum women; and more. “Whether you’re getting ready to start a family or figuring out which contraceptive option is best for you, I’ll fight to make sure you have the freedom to make that choice,” Whitmer said.
Detroit schools’ chronic absenteeism linked to asthma
Not only can an asthma diagnosis hold children back from playing certain sports and enjoying playtime, but in Detroit, it seems that it’s also impacting their ability to attend school on a regular basis. “We know for sure that [asthma] can be one of the main reasons why kids miss school,” Dr. Dee Poowuttikul, chief of the Children’s Hospital of Michigan’s allergy center in Detroit, told Chalkbeat Detroit. “We can see it firsthand when school starts here in September, October — that’s usually the highest time when we have asthma hospitalizations.”
About 1 in 10 Michigan students who have asthma miss at least six school days yearly, and a state report in 2021 revealed that 14.6% of children in Detroit had asthma, almost double the percentage statewide. Officials from the Asthma and Allergy Foundation of America point to many of the schools’ proximity to highways, mold and mildew, and a lack of air conditioning systems in the schools as potential contributing factors that may be impacting air quality and exacerbating asthma in children. They are working with partners to install air quality monitors throughout the entire city.
Renovations at C.S. Mott Children’s
Hospital to address increase in NICU patients starting this year
After noting an increase in high-risk births and pediatric heart surgeries over the past few years, the University of Michigan Health C.S. Mott Children’s Hospital had to convert its beds in the Reese Partlow Pediatric Cardiothoracic Intensive Care Unit (or PCTU) to care for the growing number of babies being sent to the Brandon Neonatal Intensive Care Unit (or NICU), as well as renovate multiple rooms to accommodate twins. To address this issue, C.S. Mott Children’s Hospital was provided a $10 million grant from the Charles Stewart Mott Foundation, which will be used to renovate the NICU’s eighth floor so that it can hold the 13 beds that are currently on the 10th floor of the PCTU, allowing this space to return to its original 30-bed capacity. Renovations are expected to start in the first half of 2025 and be completed by the end of 2026. During this time, the NICU and PCTU will continue to operate at full capacity.
Detroit barbers are helping their clients fight health disparities
Making It Count, a nonprofit organization dedicated to promoting healthy lifestyles and resources to Detroiters, is fighting chronic diseases within the African American community with the help of local barbers through its Tight4Life program. “Tight4Life leverages partnerships with hospitals, universities, and our state health department to train barbers and beauticians as wellness ambassadors,” MIC founder and CEO Minou Jones told Rolling Out, a Blackowned magazine based in Atlanta. Barbers are able to provide referrals, help schedule appointments, give advice on disease prevention, and educate the importance of regular checkups and screenings.
New Henry Ford Health hospital focuses on mental health treatment and training
To address the growing need for inpatient mental health care treatment in the region, Henry Ford Health opened the Henry Ford Behavioral Health Hospital in West Bloomfield Township in January. The new hospital — located within the existing Henry Ford West Bloomfield Hospital campus — not only provides a full range of behavioral health therapy for adults, adolescents, and seniors but will also serve as an academic site for future behavioral health care providers.
Services at the 192-bed, state-of-theart hospital include treatment for mood disorders, thought disorders, and dualdiagnosis/substance use disorders. The hospital also offers individual, group, family, and activity therapy, which involves art, music, physical fitness, and cognitive and dialectical behavioral therapy.
In a joint venture between Henry Ford Health and Acadia Healthcare, the hospital will train approximately 120 nursing students from Michigan State University, Wayne State University, and the University of Detroit Mercy; 12 psychiatric residents from MSU and WSU; and five social work interns from the University of Michigan and WSU.

How two women’s weight loss journeys inspired them to found a keto bakery

BUSINESS PARTNERS Lisa Mislevy and Anna Sherman have a friendship that’s lasted about 30 years — since the two met in seventh grade. As adults, the two moms enjoyed cooking together, regularly hosting each other’s families for dinner.
However, as time went on, both women noticed a change. “With each pregnancy, we just gained more and more weight,” Mislevy says. By the time each had given birth to her second child, both women had reached obesity. Mislevy was also diagnosed with Hashimoto’s disease, an autoimmune disorder in which the immune system attacks the thyroid, which can lead to weight gain.
So, as with many milestones they had shared before, the two friends supported one another’s weight loss journeys. Through diet and exercise, Mislevy dropped 75
pounds, while Sherman lost 150.
“Losing weight is complicated and involves emotional, behavioral, and nutritional work,” Mislevy says. “That was definitely the beginning of my understanding of how impactful nutrition is.”
Mislevy returned to school and obtained a master’s in nutrition and dietetics from Eastern Michigan University in 2015. She began working in Beaumont Hospital’s Integrative Medicine department, until she was furloughed during the pandemic. Around then, Sherman, who had a culinary certificate, was also furloughed.
With extra time on their hands, the two continued one of their favorite pastimes
— preparing food together. That inspired bigger ideas: What if their passion became a business venture?
They settled on a name, Keto Katz, and in 2022, they opened their first brick-andmortar in Clinton Township on Garfield Road. Their niche concept — a keto bakery — quickly attracted a niche audience.
“People have come in, in tears, saying, ‘My kid is an epileptic who manages seizures with the ketogenic diet, and now they can have a cupcake,’” Mislevy says. “Or, ‘My kid has celiac disease, and now they can bring cookies to school to share with their friends.’”

The ketogenic diet, or keto, causes a person to enter a state of ketosis by consuming a low amount of carbohydrates and higher amount of fats. Ketosis occurs when the body burns fat to create energy instead of glucose, the latter of which we obtain by consuming carbohydrates. Doctors prescribe it to control seizures for some people with epilepsy, and it has been reported to help manage diabetes.
Studies have also shown the keto diet to provide additional benefits to some people in the short term, including weight loss and improved blood sugar, blood pressure, and cholesterol levels, according to the Harvard T.H. Chan School of Public Health.
However, studies have painted a limited picture so far — most were short-term, relied on small groups, and didn’t include a control group.
“I’m not a cookie-cutter dietician where I think, ‘I love keto, so everyone needs to be keto,’” Mislevy says. “I do think certainly no one needs large amounts of sugar like the American diet typically provides.
“I think people get really caught up in ‘Oh, my parents are overweight,’ or, ‘Diabetes runs in my family,’” Mislevy continues. “But our message is that you have control.”

It goes well with rice and can be easily supplemented with your protein or cheese of choice, depending on your dietary preferences.
SWEET POTATO, SQUASH, AND BRAISED GREENS BOWL
Yield: 2-4 servings
Ingredients
1 yellow squash, sliced into strips
1 zucchini, sliced into strips
1 sweet potato, diced into 1-inch cubes
½ pound kale, chopped
½ red onion, sliced
1 red bell pepper, sliced
½ pound collard greens, chopped
1 tablespoon vegetable stock
3 cloves garlic, minced
Olive oil
Honey or agave syrup
To taste: Salt
Black pepper
Paprika
Garlic powder
Directions
Preheat the oven to 350 F.
Braised Greens
1. In a deep pan or dutch oven, drizzle a little olive oil over medium-high heat.
2. Once the pan is hot, place the onions and bell peppers in the pan and cook down for 5 minutes until the onions break down. During the last 30 seconds, add the garlic.
3. Add in the greens. Toss the greens and pour 1 cup of water into the pan with 1 tablespoon of vegetable stock.
4. Season with salt, black pepper, paprika, and garlic powder, to taste.
5. Cover your pan and reduce the heat to medium low and let the greens cook down.
6. Once the greens have cooked down, add a small spoonful of honey or agave to help make the greens less bitter.
7. Continue cooking, tossing and seasoning until your preferred taste has been achieved.
Roasted Sweet Potato and Squash
1. In a medium bowl, add the squash, zucchini, and sweet potatoes.
2. Toss the vegetables in olive oil and season with salt and pepper.
3. Lay the vegetables on an aluminum-lined baking sheet and cook them in the oven for 5 minutes.
4. After 5 minutes, remove the squash and zucchini, and let the potatoes continue cooking for another 5 minutes.
Once everything is complete, grab a bowl and place the braised greens on the base and add the roasted vegetables on top.

The Kirk Gibson Foundation for Parkinson’s is bringing a comprehensive and free Parkinson’s wellness facility to Farmington Hills
KIRK GIBSON has a history as a local hero, both as an All-American wide receiver for Michigan State and as an All-Star for the Detroit Tigers. Today, his efforts are being spent as a champion for those with Parkinson’s disease.
This fall, the Kirk Gibson Center for Parkinson’s Wellness will open in Farmington Hills, providing a variety of programs and activities specifically chosen to help and benefit the Parkinson’s community.

The 30,000-square-foot facility — designed by Gensler, a Detroit-based architecture firm — is being built by funds raised by the Kirk Gibson Foundation for Parkinson’s, which was founded by Kirk and JoAnn Gibson after the former Tiger was diagnosed with the degenerative disease. Their purpose in establishing the foundation was to help others understand the power of exercise and movement in confronting the disease.
“Kirk’s vision is the wellness center will be open and free of charge to the Parkinson’s community,” says Steve Annear, the CEO of the Kirk Gibson Foundation for Parkinson’s. Preregistration for free membership began in April, during World Parkinson’s Awareness Month.

Programs range from physical activities such as boxing, active stretching, yoga, pilates, tai chi, and dance to cognitive improvement exercises like speech therapy, music therapy, and art programs. The facility will also hold educational offerings such as fall safety, kitchen safety, and nutrition.
Farmington Hills — in the former Compuware gymnasium complex on the corner of Middlebelt and Northwestern Highway — was chosen as the center’s location due to the area’s large Parkinson’s community.
“There’s something like 8,000 people with Parkinson’s disease within a 30-minute drive of this location,” Annear says, adding that
“the center will be open to anybody that wants to come, no matter where they live. If they’re somewhere else in Michigan and they want to come down and spend a couple of days, it’s available.” Annear and Gibson anticipate 60,000 visits annually.
In the past, such facilities in the United States have been derailed due to funding, as the type of group classes offered are not covered by insurance.
“The gap in care is enormous,” Annear says. “So we feel that, for the people in Michigan, we can step up and provide a facility like this that doesn’t really exist right now and provides that kind of programming.”

its $15 million goal, and Phase 2 of the capital campaign was underway. “Our $15 million goal will fund the center’s first five years of operations until 2030, including salaries for the staff and registered trainers, educational programs, and other resources,” Annear says.
“This is an expeditious project by any measure which is only possible because of the foundation’s staff and our incredible volunteers who are working around the clock to open the doors on schedule,” he continues.
“We are especially grateful to Farmington Hills Mayor Theresa Rich and City Manager Gary Mekjian for the warm welcome to their community … and to the Michael and Marian Ilitch Foundation and the late Peter Secchia and his wife, Joan. They each made an early commitment to the campaign to encourage others to bring this one-of-a-kind facility to the Parkinson’s community in Michigan.”
“Every step forward, every breakthrough we make together, reflects the strength we find in each other,” Gibson said at the fall press conference. “This journey is personal, but it’s also for all those who need to know they’re not alone.”

Intriguing findings from Michigan universities

Air pollution increases risks of Type 2 diabetes
OF ALL THE IMPACTS AIR POLLUTION could have on the body, many people likely wouldn’t predict an increased risk of Type 2 diabetes to be one of them. However, a paper by Wayne State University researchers published in the journal Diabetes from the American Diabetes Association established a connection between exposure to benzene, an airborne volatile organic compound found in crude oil and gasoline, and insulin resistance across all ages. To test the correlation, researchers exposed mice to benzene and found that “within seven days of exposure, they developed high blood glucose insulin levels,” says Marianna Sadagurski, associate professor at Wayne State University’s Institute of Environmental Health Sciences.
The dangers of a bored teenager
Although you may know an adult who’s using nicotine vapes to curb their cigarette use, that doesn’t appear to be a common reason that teenagers vape. A study by Megan Patrick and others from U-M’s Institute for Social Research published in Pediatrics found that feelings of curiosity, boredom, and stress are what most often lead teens to use vaping products. The study examined data from in-school surveys of U.S. students in grades 8, 10, and 12, finding that a need to relax was cited as the most common reason to vape among all age groups, with boredom landing in the top three. “Our findings illustrate a shift over the past decade in the reasons adolescents vape, moving from experimentation to stress relief and relaxation, highlighting key areas for intervention,” Patrick says.
Shh! Don’t celebrate so loud
The Super Bowl, America’s mostwatched broadcast, is bound to be filled with loud cheers. To understand how exposure to loud noise levels can impact hearing, researchers from the Apple Hearing Study, a collaboration between the University of Michigan and Apple, analyzed data from the Noise app on Apple Watch devices of more than 115,000 participants during the Super Bowl in 2021, 2022, 2023, and 2024. They found that on Super Bowl Sundays, noise levels were twice as high as those recorded on following Sundays. “Small changes in decibel levels represent a big change in how much noise our ears are exposed to, and those small changes are barely noticeable to us,” said Richard Neitzel, a professor in U-M’s School of Public Health, in the study. “It’s important to remember that even small changes in decibel levels that are hardly noticeable can have a big effect on hearing.”
How soil responds to adversity
Scientists from Michigan State University have uncovered insights into how soil recovers after a humancaused disaster. The team, led by professor Ashley Shade, analyzed soil microbes near a Pennsylvania mine fire that has been burning since 1962. From 2015 to 2021, they sampled soil at various sites before, during, and after the fire crossed them. They then determined what bacteria were present and which were active versus dormant. While the soil microbiome recovered after the fire passed, the species of bacteria that were active or dormant changed. Since only active microbes help the ecosystem function, these findings could help scientists devise ways to restore ecosystems impacted by ecological disturbances. “If we can understand what wakes up the dormant microbes, we can try to manage the microbiome, for example, to wake up when we need it to,” Shade says.
A treat(ment) for the eyes
INFECTION WITH ZIKA VIRUS
during pregnancy is well known to cause fetal neurological disorders — but also ocular abnormalities such as retinal lesions, hemorrhagic retinopathy, and optic neuritis. Despite this, no vaccines or antiviral treatments are currently available. However, researchers at Wayne State University have discovered avenues for treatment of the eyerelated effects of the virus. The team analyzed Zika virus-infected retinal pigment epithelial cells and discovered there were significant changes in the cholesterol pathway. They studied two important regulators of cholesterol metabolism, which revealed that increased activity of one, called ABCG1, and inhibition of the other, SREBP-2, reduced viral replication. When tested in mice, treatments based on these findings mitigated ocular complications. The mechanisms discovered could potentially be applied to other viruses, including West Nile virus.
Keeping potatoes healthy
THE POTATO INDUSTRY is no small potatoes in Michigan: Our state leads the U.S. in producing spuds for chips. Potatoes must be stored in the offseason to meet demand yearround; however, preserving potatoes in cold temperatures results in a process called coldinduced sweetening — or CIS — in which starches convert to sugars. When those potatoes are processed, a carcinogenic compound called acrylamide forms and the chips are discolored. Fortunately, scientists from Michigan State University have identified the regulator that switches on the CIS-causing gene. The results of experiments implementing a technique to stop CIS through gene editing were promising. “This discovery represents a significant advancement in our understanding of potato development and its implications for food quality and health,” says MSU professor Jiming Jiang. “It has the potential to affect every single bag of potato chips around the world.”
A toast to a longer life
Couples who drink together tend to live longer, according to a study co-authored by Kira Birditt, a research professor at the U-M Institute for Social Research’s Survey Research Center, and published in The Gerontologist, a journal of the Gerontological Society of America. “We found, interestingly, that couples in which both indicated drinking alcohol in the last three months lived longer than the other couples that either both indicated not drinking or had discordant drinking patterns in which one drank and the other did not,” Birditt says. However, although this study may seem like the green light to drink alcohol on a regular basis, Birditt says it’s “not necessarily good for health.”
Listen to your internal clock when setting your bedtime
Annoyed by your one friend whose 3 a.m. bedtime doesn’t negatively impact their mood?
A study from the University of Michigan shows it’s lining up your sleep schedule with your circadian rhythm, also known as your internal clock, that’s most important. “It’s not just, ‘If you go to bed earlier, you will be happier,’” says Minki Lee, an undergraduate researcher and lead author of the Intern Health Study, in which medical interns completed mood surveys and wore Fitbits to monitor their heart rate, activity, and sleeping habits. “To some degree, that will be true, but it will be because your sleep schedule is aligning with your internal rhythms.” In the study, the team — led by U-M professor Daniel Forger — analyzed both the central circadian clock and the peripheral clock in the heart and found that having a sleep cycle out of sync with either had a negative effect on mood, though the central clock had a larger impact.

Gen Alpha has become the top purchasers of beauty products. However, some recommendations from social media influencers are sending them to the dermatologist.

SKIN DOESN’T typically begin to show signs of aging until your 20s or 30s, depending on different factors like hormones and genetics. So why are groups of elementary school students, some as young as 8 years old, flooding beauty stores to purchase expensive skin care products, including those with antiaging ingredients? Known as the “Sephora kids,” Gen Alpha (those born from 2010 to 2024) spent $4.7 billion on beauty products in 2023, more than any other age group.
According to a study conducted by Revlon and Aytm, which surveyed 1,000 parents of girls between 9 and 15 years old, 66% of them stated that their child’s interest in skin care products stemmed from content creators, citing TikTok and YouTube as their most-used platforms. With hundreds of thousands of beauty influencers in the U.S. alone, and surveys finding that 40% of children ages 8 to 12 use social media, this should come as no surprise.
And with many viral skin care trends online, like the glass-skin look — the attempt to achieve a flawless complexion that resembles glass — products containing active ingredients like retinol, salicylic acid, and hydroxy acids, which are meant to encourage turnover and fade wrinkles and dark spots to reveal smoother skin, are flying off the shelves.
Dr. Amanda Hill, a dermatologist at Clarkston Dermatology, says that some of her patients, ages 8 to 12 years old, are coming to her office after experiencing opposite effects.
“Most of the time, they’ve gone to Sephora, they’ve dragged their mom in there, … and the 10-year-old’s essentially pulling everything off the shelf based on what
they’ve seen on social media,” Hill says. As a result, her young patients are coming in with various side effects, like acne, eczema, rosacea, periorificial dermatitis (a variant of rosacea), and contact dermatitis.
Because a child’s skin is not fully developed, Hill says, it is more susceptible to irritation and too delicate and sensitive to handle products with active ingredients that encourage the skin to shed. And depending on the side effect, it could take one to six weeks of treatment to resolve the issue.
However, there are also long-term side effects children could face.

“More things penetrate the skin in children, and they’re being sensitized to potential allergens, too, that they could have issues with later in life and longerterm,” Hill says. “Because when your skin barrier is disrupted by a rash, you’re more susceptible to developing an allergy to something.”
Instead, Hill says that if a child wants to develop a skin care routine that encourages healthy habits at a younger age, they can start with gentle cleansers and moisturizers, as well as a mineral sunscreen, while also minimizing exposure to fragrances and different chemicals. Some
brands she suggests, particularly their gentle skin lines, include Aveeno, Cetaphil, and La Roche-Posay.
And if your child wants to use the same skin care products as their friends?
Rather than outright saying no, Hill, who has a 10-year-old daughter, says it’s best to come from a place of understanding, educate them on why it’s not the best option long-term, and help them find a reasonable alternative.
Triple Threat

WE’VE ALL been there before. Something is not right, and we seek help from a specialist, only to find that it’s not the right specialist. So, we go to another, and another, and perhaps never find the answer.
The Kado family has a remedy for that, at least when it comes to immunology. Sisters and Wayne State University alumni Dr. Jessica and Dr. Rachel Kado and their sisterin-law, Dr. Ruba Kado, who graduated from the University of Iowa, opened Kado Clinic last September, where patients who have needs and symptoms that overlap between their specialties have quick and effective
access to all three doctors “under one roof.”
Their practice — located in Bloomfield Hills — covers a range of “ologies,” including dermatology, rheumatology, and immunology, as well as allergies. And it’s the first practice of its kind in the United States.
The Kados have created a unique system where they’re able to quickly identify the patient’s cause of concern. For example, a patient with a rash may schedule an appointment with Jessica, the dermatologist, but upon further examination, end up being cared for by Rachel, the allergist and immunologist. “That’s care that really nobody
else can even compare to,” Jessica says.
“All of our fields deal, in a bigger picture, with immunology,” Rachel says. “I deal with an underactive immune system, Dr. Ruba [the rheumatologist] deals with an overactive immune system, and Dr. Jessica deals with a mutated immune system.” So, rather than having patients jump from one specialist to another, which can take months and, at many times, create more confusion for the patient and potentially lead to their experiencing a medical error, their patients, who are often shared anyway, are managed and cared for from start to end.
“It’s shocking for our patients,” Rachel says, adding that “they cannot believe” they’re able to see more than one doctor within an hour and develop a treatment plan within a week.
SKIN CANCER SCREENINGS
SOMETIMES the Kados go to where they’re needed the most. Last May, during Skin Cancer Awareness Month, they provided free skin cancer screenings to firefighters — an occupation that has a 21% higher risk of melanoma compared with the general public — at the Bloomfield Fire Department.
“It’s all of the materials in modern-day buildings that they will be exposed to,” Jessica says. “Skin is your largest organ, so it absorbs into their skin, and it just transforms into malignancies.”
“I’ve called the fire department. Have you ever called the fire department?” she continues. “I was so happy to see them; they’re the people that give you the biggest relief. … So, it’s like, ‘What can I do to help them?’”

A Rare Condition Affecting Young Women
Heart attack survivor preaches awareness about SCAD heart attacks

MORGAN DRUTCHAS still remembers her mom crying while saying goodbye to her in the cardiac ICU in February 2019 after she had been given a 10% chance to live through the night.
Drutchas had just su ered a second heart attack in a three-day span as a result of spontaneous coronary artery dissection, or SCAD, a condition that has recently become increasingly recognized in younger patients, especially women.
Dr. Akshay Khandelwal, now the chief of cardiovascular medicine at Allegheny Health Network in western Pennsylvania, was the cardiologist who cared for Drutchas that night at Henry Ford Hospital in Detroit. He recalls thinking about escalating her into consideration for a full heart transplant because her condition was so severe.
“I think we were surprised by the extent to which she had SCAD involving her left main coronary artery, which ends up supplying about 75% of the blood flow in most people’s hearts,” Khandelwal says.
“We need greater awareness among doctors and patients on existing gender-based guidelines and how women experience heart attacks.”

Fortunately, Khandelwal and his team were successful in saving Drutchas’s life, thanks to no fewer than six stents acting virtually as replacement arteries in her heart. She spent three weeks in the ICU following her procedure before being able to return home.
Drutchas was 35, was otherwise completely healthy, and had no family history of heart disease when she had her first heart attack, three days prior to almost losing
her life. She went to another hospital’s ER with chest pains, was admitted, and was discharged the next day. Although she was diagnosed with a heart attack, Drutchas says the doctors never used those words. She wouldn’t find out until after her second emergency, which nearly killed her, that SCAD triggered both heart attacks.
“We need greater awareness among doctors and patients on existing genderbased guidelines and how women experience heart attacks,” she says. “We need to change the perception of who is a heart patient.”
Studies included in the National Library of Medicine and referenced by the American College of Cardiology support her sentiment that cardiovascular disease can be perceived as occurring mostly in men. This leads to misdiagnoses, men being more likely to be referred to a cardiologist when experiencing chest pain, and women not receiving secondary prevention medications after experiencing heart problems.
Khandelwal highlights how SCAD recognition in particular is only more of a recent development and says that it’s
important to “maintain a high index of suspicion” when making diagnoses and deciding on treatment.
“While we are recognizing it, I think SCAD is easy to miss,” he says. “It can be di cult to diagnose. I’ve had cases that I look back on in the past 15 years and go, ‘Oh no, that was actually SCAD.’ Not everyone has a dramatic presentation like Morgan did with her ECG. When you take a step back and look at the patient, just because they don’t seem like the type, what you don’t want to do is make an error in judgment.”
Drutchas says it’s never too early to start taking your heart health seriously and make good choices, even if you have no risk factors or family history.
“You just never know what might happen,” she says. “It’s hard enough for me to make heart-healthy choices every day, and my life literally depends on it. You don’t want to leave your family and friends too soon.”

RELATIONSHIPS ARE TOUGH, but when you get them right, they mean going through life married to your best friend. Just ask Alexis and Brian Cole.
These metro Detroit relationship influencers have been together for 14 years — married for 10 — share two children, and coauthored the book The Willingness to Change: A Couples Guide to Communication and Intimacy, which candidly shares their experiences as an interethnic couple, including communication, struggles with infertility, cultural di erences, and other challenges, while taking readers on a “28-day journey that ultimately helps them create a roadmap for their relationship.”
To help you build your relationship road map, we sat down with Alexis and Brian to find out more about their book and to get some of their tips on building and maintaining healthy romantic relationships.
What inspired you to write The Willingness to Change?
Brian: Our book was actually inspired by our journey together being an interracial couple.
Over the course of our dating experience together, we were very intentional about having the di cult conversations about race, subconscious bias, stereotypes, and cultural di erences that most people are either very uncomfortable having or unwilling to have. We knew that in order for our relationship, and ultimately our marriage, to work, we would have to work through these challenges now and work to get on the same page. …
Being “willing to change” is the first step towards identifying unhealthy mentalities and behaviors that have not been serving you all these years. Even if it’s what you are comfortable with.
What can people expect from your book?
Brian: People can expect to hear a very vulnerable and transparent story from us each day about things we have learned in our lives that have taught us valuable lessons regarding intimacy and communication. People will then spend time answering
questions as they become introspective and dig into their own relationships.
There are also a few activities for couples to do together and many tips we o er along the way.
As an interethnic couple, what are some of the di cult conversations you have had to have?
Alexis: There were a lot of challenges and conversations — how we viewed the world, learning how to have empathy, how do I feel safe around family members. It was a lot of heavy conversations. I love Brian for being willing to not only have those conversations but learn how to make me feel safe, seen, and heard in those conversations.
How did you overcome these topics as a couple?
Brian: By being able to step into the conversation without already having your argument set and your data ready. Instead, just going into the conversation wanting the other person to be seen and heard, and the same goes for Lexi — we try (really hard) to come into these conversations wanting to listen first! It’s not always easy, but it’s essential to having a healthy relationship where both people feel appreciated and heard.
What do you think is one of the biggest challenges in relationships?
Brian: Communication. I think what we want from our spouses in their communication changes, and learning how to adjust can be hard. We as individuals are evolving constantly, and it’s not always easy for a spouse to understand the di erent versions
of ourselves. You are not the same person from 10 years ago, so it is up to each person to continue to relearn one another.
How do you tackle communication in your own relationship?
Alexis: By being proactive instead of reactive. We do quarterly check-ins, [and] we also do proactive therapy. We stay in the loop with one another, so it makes these life changes — [things like] being a mom or just getting older — not so drastic. That doesn’t mean they aren’t hard, but keeping a pulse on each other and having accountability helps a lot!
What advice do you have for a newer couple trying to build a healthy relationship?
Alexis: Don’t stop being intentional! Brian and I still do date nights; we care about our connection after almost 10 years of marriage! We have worked hard to keep our friendship in our relationship. Outside of God, to us, those things have been essential in what has kept our marriage alive.
Also, people will try to tell you what to expect in marriage (in a negative way) and say things like, “Oh, just wait until you’re done with the honeymoon stage.” Our advice is to not let anyone else dictate the quality of your relationship. You have the ability (with intentionality) to make your relationship incredible and to set a higher standard than what society typically sets.
What advice would you give to an established couple who may be having issues within their relationship?
Alexis: It may seem simple, but it never fails: Get help! Find people who you can really build a community with, people who want what is best for you and will support you through di cult times. Life is not meant to be lived alone, and if you are trying to be married long-term, which is not an easy thing to do at all, then you will need to surround yourself with like-minded people who can hold you accountable and help build you up.
Brian: Also, as mentioned previously, reach out to a therapist, pastor, mentor … somebody! Seeking help consistently, and not waiting for something to be wrong, is the best way to build healthy habits in your relationship.

These local doctors are helping to address the distinct needs of underserved populations

The medical director of a cancer rehabilitation center and one of his patients shed light on discriminatory practices in care for people with disabilities
When Susan Odgers, columnist for the Traverse City Record-Eagle, has to “break in” a new health care provider, she starts by listing the conditions for which she’s being treated: “‘I’m a paraplegic; I’ve got osteomyelitis; I’m being treated for breast cancer; I’ve got a suprapubic catheter tube’ — [the list],” she says, “goes on and on. They look at me like, ‘You’re a medical tsunami.’”
Odgers was 18 when she had a spinal stroke; she’d been using a wheelchair for about four decades when she was diagnosed with breast cancer in February 2021. Not long after, she became a patient of Dr. Sean Smith, medical director of Michigan Medicine’s Cancer Rehabilitation program.
According to Smith, who co-authored a paper titled “Anti-Ableist Oncology Care” in November 2023, people with disabilities have less access to cancer screenings and medical equipment; are, in theory, more likely to be diagnosed with cancer at later stages in the disease; and often face discrimination from their health care providers.
Both Smith and Odgers say that patients with physical disabilities often have trouble scheduling basic tests and screenings because it can be difficult to

find accessible medical equipment, especially in rural areas.
Standard mammogram machines, for example, require patients to stand up. To find an accessible machine, patients who use wheelchairs may have to travel long distances — a daunting and potentially expensive prospect for those already contending with serious illness.
Odgers, who has long been an advocate for people with disabilities, says that because of the many difficulties involved, many simply “stop getting screened. You stop getting things done. It’s so much to go through.”
At hospitals and clinics that don’t have sling lifts to help ease Odgers from her chair up onto an exam table, “my husband is constantly looked at like it’s his job [to lift me],” Odgers says. “We’ve gotten to the point now that I just won’t go somewhere where they expect that of him — because he could throw his back out.”
Odgers also mentioned the “crazy things” people with physical disabilities are sometimes forced to do if their regular clinic doesn’t have a scale accessible to wheelchairs — like visiting a vet clinic or even a zoo to be weighed.
Equipment aside, there’s also the question of bias. Smith’s paper cites a 2021 survey of American physicians with disturbing findings: Only 56.5% of physicians “strongly agreed that they welcome people with

disability into their practice”; a mere 40.7% felt “very confident providing equitable care to people with disabilities.”
Asked about the specific dangers introduced into health care by providers with either an explicit or implicit bias, Smith says there are “a million things.”
“There’s certainly a risk that oncologists who aren’t aware of the unique needs and abilities of people with disabilities may, with their bias, undertreat patients,” he says.
That appears to arise from an ableist assumption that people with disabilities are more fragile than they actually are.
When these happen to be patients who were also diagnosed at a later stage, “they might need more aggressive treatment for their cancer,” Smith adds.
Still, because of provider bias, patients with disabilities “might not receive chemo or might receive lower doses than the guidelines say they should” — and lower than the patient could likely handle.
“That’s really scary,” says Odgers, who’s quick to add, “It’s not your place to say whether my life is worth saving or [worth] living.”
She says, “People say to me, ‘Susan, are you about ready to call it quits? You have so much to deal with.’ And I’m thinking, ‘Not helpful.’”
Surgery is another consideration.
According to Smith, for “a wheelchair user who relies on her arms and has breast cancer, sometimes the standard is to do less extensive surgery” — a lumpectomy, say, rather than a mastectomy, where the entire breast is removed, a
longer recovery is involved, and the patient’s “arm is just not as functional during that recovery.”
Smith says he’s seen cases “where very good, very well-intentioned surgeons” might have performed “a less invasive surgery on a wheelchair user. It may have been the right call, but it also might not have been — we don’t have a good way to know.”
Rather than assuming their patients can’t handle aggressive treatment, Smith says, oncologists should be asking, “How can we adapt [the treatment] to make this work for the patient?”
“Start with education,” Smith says, “meaning the next generation of doctors [and] nurses.”
Although the U.S. Surgeon General, WHO, and the American Association of Medical Colleges have stressed the importance of increased disability training for physicians, only about half of U.S. medical schools report having any type of disability awareness program as part of their curriculum, according to a 2017 study from the American Journal of Physical Medicine & Rehabilitation. (Odgers says she’s been told on multiple occasions to “hop up on the table.”)
Smith wants to see hospitals and clinics supplied with cancer-screening equipment accessible to those with mobility issues. But the changes will have to go deeper than that. Ultimately, he says, it’ll take a “holistic, ground-up e ort” to improve the medical landscape for people with disabilities.
LET’S TALK ABOUT SEX
Sexual dysfunction is prevalent in breast cancer survivors, yet few women are talking about it

MMaybe we shouldn’t be all that surprised by the results of a 2014 study that found that 45% of women with breast cancer saw a decline in their sexual well-being. A er all, hormone treatments and chemotherapy can take a devastating toll on physical health — and that’s not even taking into account the psychological stress and hormonal changes that o en accompany a breast cancer diagnosis and treatment regimen.
The study, which was led by Dr. Hadeel Assad, an associate professor of hematology and oncology at Wayne State University, was conducted among 45 women between the ages of 33 and 73. The women, most of whom had already completed their cancer treatment, were assessed via an anonymous questionnaire.
Only three of the 45 women involved in Dr. Assad’s study saw their concerns about sexual dysfunction addressed by an oncologist, highlighting a huge gap in patient care.
Assad, a medical oncologist on the Barbara Ann Karmanos Center’s Breast Cancer Multidisciplinary Team, found that sexual dysfunction is prevalent in breast cancer survivors. A er their diagnosis, more than 47% of the women experienced a decrease in their libido, a number that went up to 50% following treatment. But these numbers are a ected by both cultural and generational factors, “as some communities view discussions about sexual health as inappropriate,” she says, “further discouraging patients from seeking help.”
Treatment itself, whether chemotherapy or hormone treatment, can clearly have a major impact on sexual wellness, causing problems from vaginal dryness to dyspareunia (painful or di cult intercourse). And while poor body image did seem to play a factor, the data appears to tell a more complicated story.
The biggest surprise from Assad’s study might be that only three of the 45 women involved saw their concerns about sexual dysfunction addressed by an oncologist. “This highlights a signi cant gap in patient care,” Assad says, “where sexual health, an essential component of overall well-being, is o en overlooked in women with breast cancer, whereas this may not necessarily be true when men are treated with similar medications.”
In the 11 years since the study was conducted, Assad says, “awareness of sexual dysfunction in breast cancer survivors has increased,” but it “remains an underdiscussed topic in clinical settings.” She’d like to see future research focus on “the bene ts of hormonal and nonhormonal treatments, pelvic oor therapy, and counseling” and says medical training should “better equip doctors to have these conversations [with patients].”

A few years ago, Dr. Versha Pleasant, a clinical professor of obstetrics and gynecology at Michigan Medicine, was sitting at her kitchen table catching up over the phone with Odessa Norwood. The Philadelphia-based hairdresser had been doing hair for not only Pleasant but all the women in Pleasant’s family — her mother, aunt, grandmother, sisters, and cousins included — for decades.
That day, Norwood told Pleasant that when Pleasant’s aunt, Maxine Jones, who passed away in 2017, had first been diagnosed with breast cancer, she’d come to Norwood and, before beginning chemo, asked her to cut all her hair off.
“I thought that was very powerful,” says Pleasant, who also has a master’s degree in public health. For one thing, her aunt lived in Delaware at the time, and she’d made the drive to Philadelphia so Norwood could be the one to cut her hair.
“Imagine you, as a hairdresser, caring for this person for so many years … only to have them asking one day to cut it off, right?” Pleasant says. “What an incredible honor it is, too, that my aunt trusted her so much that she’s asking her to be a part of this cancer journey and cut it off for her.”
For Pleasant, the conversation also helped spur the idea for Redefining the Crown, a project she spearheaded at the University of Michigan. Pleasant interviewed Black survivors of breast cancer about their experiences, particularly pertaining to hair loss.
“While hair loss can be and is devastating for women regardless of race and ethnicity,” Pleasant says, “hair in the Black community carries a unique sociocultural and political significance to it,” which can give hair loss for Black women its own especially painful resonance.

life an experience,” Pleasant says. “Black women o en don’t get to share their experiences. They may not trust sharing their experiences in a culture that o en doesn’t value the strength and content of their voices. This project is about passing them the microphone.”
According to the American Cancer Society, 1 in 8 women will be diagnosed with breast cancer during their lifetime. But while Black women see a 5% lower incidence rate than white women, they also see a shockingly higher death rate from breast cancer, about 40% higher.
“That disparity is unacceptable,” Pleasant says. She is quick to point out that as with most disparities, this one has likely been caused by multiple factors, including the higher occurrence of certain breast cancer subtypes among Black women, such as triple-negative breast cancer.
Because patients with particularly aggressive cancers are more likely to undergo chemotherapy for treatment, they’re also more likely to lose their hair as a result. This can be not only painful for patients but traumatic. In fact, one study of 38 Black breast cancer survivors found that some participants experienced the loss of their hair as even more traumatic than the loss of their breast tissue.
To prevent hair loss during chemotherapy, some patients have turned to a procedure known as scalp cooling, in which a “cold cap” is placed on the patient’s scalp.
“The mechanism is thought to work by causing constriction of the muscles around the hair follicles, [which] decreases the metabolism of those hair follicles and decreases their exposure to chemotherapy,” Pleasant says.
While the procedure has “demonstrated some bene t” among white patients, as Pleasant notes in a paper on the topic,
“there is limited data on its e cacy among Black patients.” Pleasant says it’s possible the procedure is less e ective for patients whose hair becomes “curlier (and therefore thicker) when wet.”
Because curly hair increases in volume when wet, the cooling mechanism might not fully reach the hair follicles. With this in mind, cold caps could be redesigned “to better accommodate Black patients’ hair needs,” she writes.
In her interviews with breast cancer survivors for Rede ning the Crown, Pleasant found that many of the women she spoke with hadn’t wanted “to tell loved ones immediately that they had breast cancer — but when you lose your hair, it kind of forces your hand. It tells your loved ones, and the world, and strangers, that you’re ill.”
For that reason, or for reasons entirely
their own, many women choose to wear wigs a er losing their hair to chemotherapy. But here, again, Black women may be at a disadvantage.
“A lot of the wigs that are available are not ones that necessarily mimic the texture of some Black people’s hair,” Pleasant says.
A er Pleasant’s aunt cut o her hair, she brie y wore a wig, Pleasant says. A school principal at the time, she told Pleasant that the children teased her because the wig was clearly arti cial: “It didn’t look like her natural hair,” Pleasant says.
Pleasant describes her aunt as a “thick-skinned” woman — not someone easily cowed by schoolchildren. Still, Pleasant asks, “What are small and big ways that we can give people back their dignity during this really challenging journey?”

Mind-Body Connection
Melissa Coulier talks about the cancer journey of her husband, actor Dave Coulier, and how the couple practice wellness principles.

Mindset is one of the pillars Melissa Coulier emphasizes during her training sessions at Live Well Lead Well, the holistic wellness company she co-founded. It has also been a powerful tool for her husband, actor Dave Coulier, during his cancer treatment.
“He has really approached his treatment with making sure that wellness was at the forefront in order to be the perfect companion for his chemotherapy,” she says.
The actor/comedian, best known for his role as Joey Gladstone on Full House, revealed in mid-November that he had been diagnosed with Stage 3 non-Hodgkin lymphoma and was undergoing chemotherapy. He’d received the news the month prior after an upper respiratory infection led to swelling in his lymph nodes.
As he announced the news on his Full House Rewind podcast, he told listeners, “If my encouragement can lead you to get checked out by your doctor sooner than later and can possibly help even just one of you, this little journey that I’m on is all going to be worth it.”
Integrating her wellness teachings at home, Melissa says she’s helped Dave make nutrition a priority, by eating healthy, home-prepared meals, and incorporating movement as much as possible during the day. But she says that above all, his signature sense of humor and optimism have helped carry him through this difficult time.
The couple, who relocated from Los Angeles to Dave’s native St. Clair Shores in 2019, have been open about Dave’s ongoing treatment, with the hope that more people will be motivated to stay on top of regular cancer screenings.
Dave is all too familiar with cancer’s toll, having lost his 36-year-old sister Sharon, who died during the 1991-92 taping of Full House, and his niece Shannon, who died of breast cancer at the age of 29.

“Every day presents new challenges,” she says. “Your body is transforming and changing right in front of you, which can have a negative effect on your mental state if you’re not prepared and armed with [a positive mindset] prior to going in.”
Mindset, movement, and nutrition — the three pillars of Live Well Lead Well — have not only helped her make a difference in Dave’s life, Melissa says, but transformed hers.
Her introduction to holistic healing came in 2016, from another member of the Full House family. Melanie Samuels was working as a production coordinator on Fuller House. Dave introduced the two, and they became fast friends.
“I think just the connection that we had felt very safe,” Samuels says.
Today, as co-founders of Live Well Lead Well, the two host retreats, workshops, coaching sessions, and corporate wellness programs aimed at female leaders, who studies show are much more likely to experience workplace burnout than their male counterparts.
“There’s a societal pressure for women to be overextended, to strive for perfectionism, to be people pleasers, and to also experience imposter syndrome at work,” Samuels says. “Those three things are amplified by the fact that we are trying to wear so many different hats, as mothers, as partners, caretakers.”
Early in their friendship, Melissa opened up to Samuels about her struggles with lupus, an autoimmune disease in which the immune system mistakenly attacks healthy tissues and organs.

migraines, and rashes, Melissa was finally diagnosed in 2006 after a blood clot in her brain caused her to black out. She went on to be prescribed six different medications for lupus — four to mitigate her symptoms, two others to treat the other medications’ side effects, she says.
For years, she kept her diagnosis private due to worries that it might impact her career as a freelance photographer in Los Angeles. Then one day, on a five-day advertising shoot, she’d had enough.
“I remember not being able to get out of bed on that fifth day. I was like, ‘OK, I have to shift some things around in my life, because I can’t live like this,’” she says. “I felt like I was living in a body of a 100-year-old, and at the time I was in my mid-20s.”
Melissa’s story resonated with Samuels, who felt burned out after over a decade in the television industry. The grueling shoot schedules, she says, made her deprioritize her health and well-being to stay afloat at work.
“My emotions were so heightened all of the time, and I realized it was because I was wearing so many different hats and also trying to build a personal life for myself,” says Samuels, who struggled with an eating disorder as a teen. “It was such an ‘all or nothing’ sort of industry. And my past was ‘all or nothing’ in health. I think that that whole parallel was triggering for me.”
Eventually, she quit and left to pursue functional nutrition. After Samuels obtained a certificate, Melissa became her first client. The two worked together and came up with a plan. Today, through adjustments in diet and lifestyle, Melissa says she’s been able to manage her lupus without medication.
On the other hand, Dave’s road to recovery remains an uphill battle. He’s had multiple surgeries and rounds of chemo. When we spoke in March, Melissa was unsure she wanted to give an update, as she couldn’t predict how his health would be at the time of publication in early May.
“Things change day by day,” Melissa says. “But I think that his positive mindset and the innate fighter in him really give him a one up.”

Telling someone they have cancer is never easy, but what helps ease the pain and relieve some of the shock and anxiety is when the diagnosis is accompanied with answers, solutions, and a positive outlook. Dr. Margolis has had to give plenty of patients difficult news during his 24 years in medicine, but he says that what’s changed over time is the increase in positive solutions that are now included with the diagnosis.
“In the past we would be trying to get people more time, but now we’re focused on meaningful cures that allow people to get back to living and doing what they want to do,” he says. “What we’re doing now is targeting what’s best for the patient, what type of treatment they can tolerate, and what side effects they can manage — and then coming up with a treatment that best fits their profile and their goals. We identify very specific types of cancers. These types of cancers have their own specific treatments, and it’s making a huge difference.
“One of the things we’re doing today is more personalized medicine,” Dr. Margolis continues. “We’re doing special genetic molecular testing on everybody’s tumor. We used to just say you have lung cancer, but now we find out what driver mutations there are and we target things to that mutation. Treatments are no longer generic; they’re tumor-specific.”
Another example of whole-patient care is offering Cold Caps/Scalp Cooling Systems at infusion centers located throughout metro Detroit. And when fertility preservation is the goal of a patient, options are discussed.

“I’m more of a doer.” This is how Lenny Barnes, vice president of Heritage Vision Plans, views his role as a second-generation member of the family business. At the start of his career, Barnes wanted to strike out on his own. He spent time in L.A. as a musician, where he gained exposure to different people and different walks of life.
It wasn’t until Barnes began his first corporate job in the dental industry that he grew to appreciate company culture and understand expectations from a financial standpoint. When a territory manager position opened in New Jersey, he jumped at the opportunity to dive deep into the business, arranging educational programs and interfacing with everyone from oral surgeons to office managers.
Having spent time on both coasts, Barnes returned to Michigan, landing in the heart of Detroit’s automotive industry,
where he gained a deep appreciation for quality control and attention to detail. “At every process of putting a car together, someone’s entrusting their safety to you,” he says. “That was a huge education for me: Every part matters. That’s what I brought to Heritage.”
In 2004, Barnes got the call from his sister asking him to rejoin the family business. By then, he could see how the lessons from his automotive career would apply to Heritage. Barnes’s vision was to ensure the highest standard of care, from claims processing to the call center to accounting, and to replicate it for every customer interaction.
To Barnes, that means providing a positive experience on a consistent basis. “You can’t win a full week in one day,” he says. “But if we take each day, and our team takes each day, that’s how we string the
consistency together to have a great experience for clients. Because we do try to win every day at satisfying and servicing our customers.” ■

Dr. Thomas Gilbert feels very lucky to be able to practice in the city where he grew up. A strong sense of community was instilled in him during his childhood in Royal Oak, where he played every sport under the sun with his neighborhood friends and enjoyed movies at the old Royal Oak drive-in.
Dr. Gilbert’s passion for dentistry and excellence in treatment started early. Both of his parents were teachers and administrators at Detroit Country Day School, where he excelled. He majored in biology and psychology at Albion College and received his Doctor of Dental Surgery degree from the University of Detroit Dental School in 1985.
His love of learning continues in both his professional and personal life. Dr. Gilbert is an avid reader who loves mysteries and suspense on top of reading dental journals and current research papers to provide the most innovative and comfortable treatment for his patients.
Dr. Gilbert is a firm believer in continuing education. One class that he has taken is Invisible Braces by Invisalign, and now his Invisalign patients range from young teens to 80-year-olds. He has also learned how to place and restore implants, which has opened up a whole new option of treatment for his patients. Dr. Gilbert was also one of the first to complete the specialized course in Lumineers.
Family is one of Dr. Gilbert’s top priorities. He and his staff at Royal Oak Dental treat their patients as if they were family. Dr. Gilbert met his wife in 10th grade and married her 10 years later. Together, they have four children, a dog, and two cats.
Dr. Gilbert’s focus is to provide the most professional level of treatment and comfort to his patients. As Dr. Gilbert will tell you, he has had a lot of treatment done on his own teeth, and he knows what his patients go through. ■

ADULTS OFTEN underestimate the creative capacity of children, who can be just as inventive as their elders, if not more so. At play, young people are wonderfully adept at improvising. That appears to be the case in this circa-1913 photo, which shows a group of boys on an unidentified Detroit street engaged in shinny, a kind of poor man’s hockey that’s played on ice, turf, or the street. There are few rules, and equipment is extemporaneous. Sticks can be improvised from broom handles or tree branches; a ball, tin can, or chunk of ice may substitute for a puck; and goals might be constructed from rocks or garbage cans.
