Health & Wellness R. Alberto Rosa, M.D., FACC Dr. Ramon Alberto Rosa is a graduate of the University of Santo Domingo in his native Dominican Republic. He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), Chief Medical Resident (19941995), and cardiology fellowship (1995-1998).

Dr. Rosa has practiced cardiology in Sussex County since July 1998. He has served as Medical Director of the Cardiac Catheterization Laboratory and as Chief of the Department of Cardiology at Beebe Medical Center in Lewes, DE. Dr. Rosa is trained in non-invasive cardiology, Nuclear Transesopageal Echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Dr. Rosa is board certified by the American Boards of Internal Medicine and Cardiology. He is also a Certified Aviation Medical Examiner and is Board Certified in Nuclear Medicine.

“Broken Heart Syndrome” Is a “broken heart” a true medical condition or is it just a figure of speech to describe someone’s emotional state after some great personal loss or stressful situation? It may come as a surprise to many people but the answer is both.

This condition is also known as “stress-induced cardiomyopathy”, a rapid and severe heart muscle weakness, typically as a consequence of sudden emotional or physical stress: unexpected illness, injury or loss of a loved one.

It can also be from a severe medical condition, surgery, an accident, intense fear, domestic violence, severe pain, financial loss, even public speaking or a surprise party.

The exact cause is unknown, but it is clear that most of the affected individuals are postmenopausal women between the ages of 58 and 75 years, in about 90 percent of cases. The symptoms are very similar to those of a heart attack, with chest pain or pressure and shortness of breath being the most frequent ones. It is not completely understood why this condition affects mainly women, but most investigators believe that after menopause the decline in estrogen levels renders patients more susceptible.

It was first observed in Japanese women in the early 1990’s and the name of takotsubo cardiomyopathy was used originally.

This name describes the characteristic appearance of the left ventricle (heart’s main pumping chamber) as seen in cardiac studies that resembles the traditional Japanese octopus trap. There is ample evidence to suggest that the massive release of certain hormones in the bloodstream, such as adrenaline and noradrenaline, as a natural response to stress, under certain conditions causes the heart muscle cells to stop working properly, especially in the lower part and the “tip” of the left ventricle.

Stress-induced cardiomyopathy can be serious medical condition because it involves the sudden onset of congestive heart failure due to the heart muscle weakness, low blood pressure and potential shock or life threatening heart rhythm problems; it is, however, rarely a fatal illness. Furthermore, it is almost always reversible with most patients recovering completely within 2 to 4 weeks.

The main differences between a heart attack and “broken heart syndrome” is

that heart attacks occur as a consequence of blockages or blood clots in the coronary vessels (heart vessels that supply blood flow and oxygen/ nutrients) to the heart muscle cells that cause those cells to die or to suffer permanent damage. In the stress cardiomyopathy patients, the coronary vessels are open and the blood flow is usually normal. It is the “stunning” of the heart muscle cells by adrenaline and other hormones that creates the muscle weakness and this process is, for the most part reversible.

Another interesting finding in this broken heart syndrome

is that it does not tend to recur in the same patient. In other words, once somebody experiences this syndrome, even if other stressful events come along in their lives, the same phenomenon doesn’t tend to happen again.

Learning to cope better with life stresses can prevent it in the first place.

The management of this medical condition is similar to the treatment of congestive heart failure with medications such as diuretics, betablockers and ace-inhibitors as well as aspirin and in some cases blood thinners on a temporary basis.

So yes, it is quite possible

to have a “broken heart” literally, as a true medical illness that is brought about by a heightened physiological response to stress, especially in women. The good news is that in almost every case it is a temporary condition and full recovery is the expected outcome. There is no way to differentiate this situation from a heart attack, before hand. Therefore, it is very important to seek immediate medical attention for symptoms of sudden chest pain or pressure and shortness of breath. It might be the case that your heart doctor could tell you, in the end: “Just a broken heart, already on the mend”.

What Does An IPL Treatment Do? A photofacial treatment addresses unwanted skin pigment, known as sun spots, age spots, or liver spots, pigment that can occur on the face, neck, chest, hands and really anywhere there is excessive sun exposure. And now NEW at Renove, we can also address the unwanted hormonal pigmentary changes known as melasma. Melasma is a deeper much more stubborn kind of pigment that is caused by fluctuations in hormone levels that can occur during or after pregnancy, menopause and with birth control pills. Men can also have these pigmentary changes.

What Is A Photofacial Treatment?

A photofacial or IPL treatment is not a laser, it uses light in the visible spectrum, with numerous simultaneous wavelengths of light, from red light to blue light and the wavelengths in between. This light is applied to the area to be treated

with brief pulses of light, similar to a camera flash. With our NEW IPL addition, we have the ability to treat much deeper and more stubborn pigment, with less discomfort during and after the treatment. If you have had an IPL treatment with us before, you will be very pleased with the level of comfort with our new IPL wavelength.

What Can I Expect During A Photofacial Treatment?

When you come in for your photofacial treatment, your skin will be gently cleaned to remove any makeup and we will apply a cooling gel to the area to be treated. This gel helps the light from the IPL penetrate into your skin and to help keep your skin cool during the treatment. Then the IPL handpiece will be applied to the skin, and you will see a bright flash of light, like a camera flash, and feel a very brief pulse of heat. This will be repeated about 20 flashes for a full facial treatment, which takes no more than fifteen minutes from start to finish. We will remove the cooling gel and apply a zinc based sunscreen for your trip home.

What Can I Expect After My IPL Treatment?

Immediately following an IPL treatment, you will feel like you have a sunburn for about an hour. The evening after your IPL treatment, the treated areas will be more sensitive to heat, such as hot water, so I recommend cool facial washes and

avoid a hot shower that evening to the treated skin only. By the next day, your skin will feel completely normal, and you may resume all prescription skin care treatments.

What you will see after your IPL treatment is the unwanted pigment begin to darken and come to the surface of your skin. This pigment will be exfoliated naturally by your skin over the course of the next week.

It is our goal that you be pigment free by the end of week, though other body parts, such as the chest and hands, may take longer for all the unwanted pigment to come to the surface and naturally exfoliate.

How Long Do The Results Last?

Following your IPL treatment, unwanted skin pigment will be completely eliminated. Stubborn melasma may take more than one treatment, especially if you have darker skin tones, as we must use more gentle settings when there is

Do you try to hide your legs in the warmer months because of bulging or unsightly veins?

Often, it is too late to try to prevent bulging veins, however, walking regularly, drinking plenty of water, and not crossing your legs can help improve the health of your veins.

Varicose veins bulge in some areas or can be red and spidery in other areas.

This condition is very common; it can be painful or hard to deal with, and in some cases, it may indicate a larger health issue.

Spider veins and varicose veins often start when we are younger. They frequently appear at the time of pregnancy, but may appear spontaneously at any time throughout our lives.

Varicose veins may occur in many parts of the body, however, the leg veins are most often affected because these veins must work against gravity. The valves within your veins may weaken or break over time, causing some of the blood to pool in the legs instead of going to the heart. This back-up of blood results in bulging veins.

Chances are if you have varicose veins, then your mother or grandmother did as well. Women are at an increased risk for varicose veins compared to men.

What You Should Know About Vaping and Oral Care

By Richard J. McCann, D.M.D., P.A. The popularity of electronic cigarettes (e-cigarettes) and vaping has soared in recent years. The promise of fewer health risks than cigarettes is appealing to users. But few people stop to consider the impact that the chemicals in e-cigarettes might have on one’s oral health. While it’s true that vaping eliminates the tar in cigarettes, other aspects of vaping can still be harmful to teeth and gums.

What’s in an E-Cigarette?

E-cigarettes work by heating a liquid, often called vape juice, until it turns to an aerosol or vapor. The liquid is a mixture of water, propylene glycol, glycerin, and flavorings. Vape juice does not contain tar, like traditional cigarettes, but most varieties do contain nicotine.

Is Vaping Safe for Your Mouth?

Removing the tar does remove one chemical that can be harmful to oral health. But the truth is, research on the long-term effects of the other substances in vaping are still in the early stages. So it is not clear how safe they are. Dental professionals do know, however, that some of the chemicals in e-cigarettes have a negative impact on dental health.

Potential Hazard: Inhaling Bacteria Bacteria settling into the pits and crevasses of teeth is responsible for tooth decay that results in cavities. The act of inhaling the vapor of an e-cigarette brings bacteria into the mouth. Just like a diet of sticky, sweet foods, these additional bacteria contribute to a buildup of plaque in the mouth. Users may find themselves more cavity prone, especially if they fail to maintain good dental hygiene.

Making Matters Worse: Dry Mouth The propylene glycol in e-cigarettes causes dry mouth in some users. Dry mouth may seem like just an annoyance, but it can lead to more serious oral health problems. Saliva acts as a natural defense against bacteria in the mouth. The heat and chemicals of vaping take away some of that moisture. The lack of saliva makes it easier for tooth decay to grow, increasing the chance of cavities.

Say No to Nicotine By far, the most damaging ingredient in e-cigarettes is nicotine.

Not all vape juices contain nicotine, but most do. The substance is responsible for several oral health care issues.

Nicotine and Tooth Stains With cigarettes, tar is responsible for most stains on teeth, but vaping can cause them too. Nicotine is colorless, but when it is exposed to oxygen, it turns yellow. Smokers who switch to vaping thinking that their teeth will stay whiter will be disappointed. Many patients who vape end up turning to professional teeth whitening to remove stains.

Gum Disease It has been known by dental professionals for some time how bad nicotine is for dental health. It reduces blood flow to soft tissues in the mouth, which can be at the heart of many problems.

Long-term exposure to nicotine can cause gum diseases like gingivitis and periodontitis. In serious cases, damage to gum tissue can result in teeth becoming loose and even falling out.

Restricted blood flow from nicotine also contributes to dry mouth and the inability to fight off bacteria discussed above.

Lithium batteries One danger to oral health associated with vaping that is not so obvious has to do with the battery used to heat the vaping liquid.

Under certain conditions these batteries can explode, sometimes causing significant injuries. If this happens while the e-cigarette is in or near the mouth, it could inflict extensive damage. The lips, cheeks, gums, and tongue could be cut and teeth could be broken. Reconstructive dental procedures might be necessary.

If You Choose to Vape Studies are still being done to determine the impact of e-cigarettes and vaping on oral health. Although e-cigarettes are not quite as harmful as regular cigarettes, there are hazards.

People who choose to vape should know the risks to both physical and dental health.

A few things can be done to lessen the dangers to oral health.

Limit or avoid nicotine altogether. Choosing vape juice without nicotine can eliminate several worries.

Drink plenty of water to replace the moisture that vaping takes away. Getting rid of dry mouth will help with several issues including bad breath.

Keep up good dental hygiene habits. Routine brushing and flossing are the best defense for oral care issues.

Low Back Pain: Nonoperative Treatment is the Preferred Choice

By William L. Pfaff, MD As a board certified Orthopaedic Spine Surgeon, the most common ailment I treat in my office is low back pain.

Approximately 80% of our population will experience at least one episode of disabling low back pain during their lifetime. For most people, the low back injury and pain will subside completely within a few days to 3 months; however, in some the pain will not subside and becomes chronic in nature. Those who experience an episode of low back pain may expect a lifetime recurrence rate of up to 85%.

It can be difficult to treat low back pain successfully due to multiple sources creating the pain. A person can experience back pain from something as simple as a ‘pulled/ sprained’ muscle. A degenerative disc, although a normal process of aging, can cause pain in some. Arthritic joints in the back of the spine can also cause pain. Pinched nerves and weak core musculature can be responsible for part of the patient’s pain. These potential sources are called ‘pain generators’ and each one may be causing a large amount of the pain, a small amount of it, or none at all.

Once in a while, a patient will inquire as to an operation to ‘take away their low back pain’. As I explain in my practice, it is very seldom that lumbar spinal surgery is indicated to reduce low back pain. Only in rare situations should it be performed. In the majority of patients, there are other alternatives to surgery which can reduce their pain and increase their function enabling them a better lifestyle.

The mainstay for treatment of low back pain starts by treating the acute injury with rest and ice as needed.

Missing a few days of work at this point can be expected. If able, non-steroidal anti-inflammatory

medications (ibuprofen, Advil, Motrin) should be used judiciously. Physical rehabilitation can be very helpful during this time which may be in the form of physical therapy, chiropractic treatments, massage or a combination of these.

These treatments help in the acute period but what can one do about chronic low back pain? Engaging in a regular stretching program can be very helpful at eliminating or reducing the occurrence and severity of low back pain episodes. I encourage my patients to enroll in a yoga or Pilates class or to buy a yoga exercise DVD. Maintaining a healthy body weight will also help reduce low back pain. If you smoke, you are also at a higher risk for low back pain. One should think ergonomically about their workspace and maintain a good posture. Slumping forward makes it difficult for your low back muscles to support your weight.

Be extra careful maintaining your posture while lifting heavy objects as bending from the waist can result in an injury. Instead, bend at the knees and use your legs to do the lifting. For women, avoid wearing heels as they may be contributing to their low back pain.

If these treatments fail to provide relief, I may recommend cortisone injections. These injections may be one of a few different types depending on what is causing the pain. With an acute episode, a cortisone injection into the muscle can provide relief very quickly.

This is what is commonly referred to as a ‘trigger point’ injection. If this doesn’t work or if the pain is of a more chronic nature, a nerve block to the arthritic joints in the back of the spine (facet joints) can be very helpful.

This is a procedure that ‘kills’ the nerves that innervate the arthritic, painful facet joints through needles.

The patient does not require anesthesia and it can be done in the office setting.

It has the potential to significantly reduce low back pain for over a year in properly selected patients that suffer from chronic low back pain and rarely produces any negative side effects (like worsened pain).

Patients with low back pain can be divided into two groups. Those who sustained an acute episode which should resolve within a few days to

3 months utilizing the treatments discussed above including rest, ice, physical rehabilitation and over the counter medications. Patients with chronic low back pain who have failed these treatments will need further diagnostic work up to identify their pain generator. Nonetheless, surgery for low back pain remains the least favorable option for patients and should only be considered in carefully selected patients. Even then, surgery has low success rates and high complication rates when trying to eliminate low back pain in a patient with an arthritic spine.

It’s the most wonderful time of the year.

We’re entering the season that’s supposed to fill us with love, light and laughter. However, if the merry magic of the holidays isn’t sparking you with a warm glow from within, you’re not alone. Whether we’re talking about George Bailey in “It’s a Wonderful Life,” Clark Griswold of “Christmas Vacation,” poor abandoned Kevin McCallister in “Home Alone,” you, or a loved one, sometimes the holidays are more apt to shower us with darkness instead of light.

From stress to financial pressure and loneliness, the season can be more of a thief of joy than a giver of it. According to a survey

by Healthline, a consumer health firm based in San Francisco, 62% of respondents described their stress level as “very or somewhat” elevated during the holidays, while only 10% reported no stress during the season. In another study by the American Psychological Association, nearly a quarter of Americans reported feeling “extreme stress” come holiday time, and statistics show that up to 69% of people are stressed by the feeling of having a “lack of time,” 69% by perceiving a “lack of money,” and 51% are stressed about the “pressure to give or get gifts.”

Perhaps most alarmingly, 45% of Americans would prefer to skip Christmas altogether, according to a survey from Think Finance.

With all of those stats in mind, the phrase “holiday blues” takes on a very real meaning.

And it doesn’t discriminate. No matter geographic location, income level, gender, ethnicity or relationship status, people can feel sad, depressed, anxious, lonely, overwhelmed, bitter or worse during times that are typically billed as joyous and carefree.

Writer Maud Purcell described several of those reasons in her recent article on holiday stress: • Pressure to feel merry: Do you feel joyous when holiday decorations go up and store windows fill with gifts? If you don’t, take comfort in knowing that you are not alone.

The disparity between how you actually feel and what you think you are supposed to feel can cause you guilt and confusion.

• Remembrances of holidays past: Consciously or unconsciously, you have a mental record of previous holidays. Your mood may be contaminated by the specter of sad holidays past. If your current life circumstances are unhappy, however, you may long for the happy holidays you once enjoyed.

• Reminders of loved ones lost: Holidays are a time for reflection. All too often your thoughts turn to beloved family members and friends who have passed away.

• Loneliness: Holidays can be dreadfully lonely if you don’t have a significant other.

Additionally, separation from family members (emotional or geographic) can be particularly painful at this time of year.

4. Be thankful: It’s almost cliché to say during the holidays, but gratitude has been shown to reduce negative emotions like stress and improve connection to others. Focus your energy there instead of automatically defaulting to the negative.

5. Help others: Mark Twain once said, “The best way to cheer yourself up is to try to cheer somebody else up.” When you’re down that can seem impossible, but making someone else feel good or helping them in some way, can be a very effective

stress reliever and alleviate feelings of depression.

As stressful as the holidays can be, they can also bring a lot of light into our lives.

After all, George Bailey realized just how wonderful his life was, Clark Griswold found peace and Kevin McCallister discovered a new appreciation for his family.

Don’t hesitate to take a beat during the holidays and thoughtfully apply some of these simple strategies to find your light in the darkness, too.

• In search of sunlight: Many people are adversely impacted by the relative loss of sunlight they experience during the winter months. This phenomenon even has a name: seasonal affective disorder or SAD.

2. Recognize what you can (and can’t) control: You can’t do much about what other people say and do, or if the car breaks down, or if the weather wreaks havoc on your plans. However, you can control how you react to those situations. Try to be more mindful of your actions when things take a turn for the worse.

3. Find the SUN: Do it literally and figuratively.

When sadness sets in, get 30 minutes of direct sunlight with a walk around the block, and if deep despair takes hold of you, don’t be afraid to seek out professional help from somewhere like SUN.

Beat Common Winter Ailments With Chiropractic Care Dr. Tracy Hudson Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. Aid in Recovery from Winter Sports Injuries, Slips and Falls Skiing, skating and other winter sports are fun, but they can be dangerous. Common winter sports injuries include spinal injuries, knee problems, ankle sprains and more. Sledding is the most common cause of sports injuries during winter, but skiing, ice skating, hockey and snowboarding can also cause sports injuries. The ice and snow of winter make slips and falls more likely. Emergency departments treat more than 2.8 million older people for fall injuries each year, according to the Centers for Disease Control and Prevention (CDC). Injuries from slips and falls can be serious, causing broken bones and other injuries. In many cases, slipping and falling causes sprains, strains and muscle aches.

Chiropractic care for sports injuries, slips, and falls includes stretching for increased flexibility, chiropractic adjustments to address joint tightness, and muscle work to break up scar tissue.

A special note for a seamless surgery Milford resident Lucille Mehl was so pleased with her positive surgical experience at Bayhealth Hospital, Sussex Campus that she went home after the procedure and a one-night hospital stay to handwrite a letter to Bayhealth President & CEO Terry M. Murphy, FACHE. Those who know Mehl aren’t surprised that she took the time to express her gratitude for the quality of care she received; she’s developed a reputation in the community for positivity. “Everything went well, and I think it’s important to write letters when things are good,” she said.

A retired speech pathologist and principal, Mehl relocated to Delaware from upstate New York in 2003 with her husband Tom. They chose Milford as their retirement home and are active volunteers in the area.

In 2013, Mehl received a Governor’s Outstanding Volunteer Award for donating time and talent to the

Prime Hook National Wildlife Refuge’s Nature Center, the Milford Lions Club, Delaware Hospice, the Milford Senior Center, Seaside Jewish Community, First Presbyterian Church in Milford, and Meals on Wheels.

Looking back on her hysterectomy, Mehl admits to having a case of jitters before undergoing the procedure. “I was very nervous. It was my first experience with something like this. I had never had any big surgery,” she said.

In the end, her concerns were unwarranted.

Her experience, according to her letter to Murphy, exceeded her expectations from start to finish. “From the moment I had my pre-admission testing, the surgery and post-operative procedures were excellent,” she wrote.

“Everyone that I had dealings with was knowledgeable, caring and dedicated to the service they gave to me personally.”

In addition to individually recognizing the staff with positive comments on the Bayhealth Honors Card, Mehl said she enjoyed having her own room, a room with a view. “My breakfast [the next day] was delicious, especially the coffee.

The added touch of the Delaware State News on my breakfast tray was over the top,” she wrote.

Mehl also praised her surgeon, OB-GYN Noel M. Anupol, MD, as “walking on water. He listened to my complaints.”

Meanwhile, he credits her for being a well-informed patient. “She is a good person and a great patient, and she asked appropriate questions,” he said.

He described the procedure for this patient, a minimally invasive, gynecologic laparoscopic surgery. “Every case is different, and her procedure was appropriate for her medical problem at the time,” he said.

The patient will have shorter hospitalization, reduced pain and discomfort, a faster recovery time, a quicker return to normal activities, a cosmetically smaller incision resulting in reduced risk of infection, as well as reduced blood loss and transfusions,” he said. “Our philosophy is to serve all patients well and give them appropriate care,” he added.

Since her surgery and recovery went well, Mehl says she has happily resumed an active lifestyle. “I am a nine-hole golfer, I dance, and I do yoga and tai chi,” she said.

And to Murphy she wrote, “Thank you all again for my great care and stay. I’m just so thrilled that the new hospital is here in our community.”

I have smoker’s lines but I don’t smoke!

Are you frustrated about having “smoker’s” lines around your lips and you’ve never smoked?

You are not alone! This is a common complaint of a lot of people, having those pesky vertical creases around your lips that appear when we pucker, use a straw or just speak! And with time and continued motion of our mouths, these creases can become permanently etched in our skin.

You don’t have to smoke to have smokers lip lines Of course smoking makes these creases worse, but it is not the only cause of “smoker’s” lip lines. The cause of these lip lines is due to thinning of the skin around our mouths and losing the strength of the natural white roll of the lip border, where the lip transitions from the pink color of the lip to the normal facial skin. It is this natural white roll that forms the transition or border between the actual lip and the skin surrounding the lip, where lip liner would be drawn.

This white roll border is what usually prevents applied lipstick from bleeding or smudging away from the lip itself onto the surround skin. If you were to observe a child’s lip, this natural white roll of the lip border is typically present and can be felt as a soft firmness that defines the border of the lip.

Lip lines and creases are also caused by the daily use of the mouth in everyday speaking. The facial expressions with using everyday speech causes the mouth to crease in the same areas repeatedly and over time, the creases become permanent.

So what can we do to address this pesky concern of lip lines?

Thankfully, we have several options! The first problem issue I like to address when treating these lines, is to restore the strength of the natural white roll that has been lost. This is done by adding a hyaluronic acid filler directly into the white roll. By rebuilding and strengthening this natural border between the lip and the surrounding facial skin, the lips cannot collapse as easily like an accordion and form lip lines and creases, when we are speaking, eating or making other mouth motions. Chronic Insomnia Treatment Is Available Do you have problems falling asleep, staying asleep, or early morning awakenings at least three nights a week despite adequate opportunities for sleep? If you answered yes, you might be an insomniac. According to the Center for Disease Control (CDC), a third of US adults report that they usually get less than the recommended amount of sleep. Not getting enough sleep can shorten your life expectancy, increase your risk of medical conditions, accidents, and impair your thinking. Insomnia is also associated with low energy levels and an increased risk of mental health disorders such as depression and anxiety.

I’d suffered from insomnia for years, trying various types of sleeping pills, herbal supplements, teas, body oils, meditation, hot baths, and many sleep hygiene tips that I’d found on the internet, but nothing seemed to work.

Each night, like clockwork, I would fall asleep quickly, sleep for 4 hours then wake-up. I would then toss and turn in the bed for the rest of the night and drift off to sleep again an hour or two before it was time to wake up to go to work. It didn’t matter if I had a stressed filled day or a peaceful, calm, relaxing day, insomnia like an annoying exlover would show up night after night for our regular rendezvous. Sometimes I would wake up with a recurring song on my mind, other times I would replay the events of the previous day over and over again, or ruminate about how to solve a family or work-related issue. My favorite pastime, however, was watching the clock and worrying about not being able to fall back to sleep. I never felt fully rested, and I hated it because insomnia left me feeling groggy in the morning, and by the afternoon, I needed chocolate, coffee, or some comfort food to keep me going. I packed on a lot of weight and developed other health problems as a result.

I wanted it to get better, but I didn’t know-how. For me, I knew sleeping pills weren’t the answer due to the side effects I experienced, like headaches, dizziness, mood changes, a lapse in time, and memory loss. Even some of the herbal supplements and teas had the opposite effect on me and left me even more awake at night.

As a therapist, I’d attended all the training, read all the self-help books, talked to doctors and colleagues, and watched videos but couldn’t cure my insomnia. Helping Veterans with PTSD, Anxiety, Stress and Sleep

The history of Floatation-REST (Reduced Environmental Stimulation Therapy) dates back to the 1950’s when Drs. Jay Shurley and John Lilly at the National Institute of Mental Health became interested in understanding how the human brain would respond to an environment devoid of external sensory input. The float environment systematically minimizes external sensory input to the nervous system including signals from visual, auditory, olfactory, gustatory, thermal, tactile, vestibular, gravitational and proprioceptive channels. Since its discovery, there have been a number of publications exploring the potential therapeutic benefits of floating. The most consistent observation to date has been significant reductions in levels of subjective stress and increases in relaxation as measured from pre- to post-float.

Concomitant with these subjective findings, floating has also been reported to decrease blood pressure, heart rate, and cortisol levels. A meta-analysis published in 2005 found a large overall effect size for stress reduction, with most past float studies focused on healthy populations.

Clinical research investigating Floatation-REST, although limited, has reported largely beneficial effects across a range of different stress- and pain-related conditions, including: hypertension, chronic tension headaches, chronic muscle tension pain in the back and neck, and stress-related pain with “burnout depression”. Thus far there have been several Floatation-REST studies focused on individuals with clinical anxiety, including generalized anxiety disorder, post traumatic stress disorder (PTSD), panic disorder, agoraphobia, and social anxiety disorder, with many patients presenting with comorbid major depressive disorder. The evidence published thus far suggests that Floatation-REST is a safe and effective technique for the rapid reduction of stress, pain, and anxiety. Clinical trials are currently underway in America, Germany, and Sweden investigating the long-term impact of floating in patients with anxiety disorders, anorexia nervosa, chronic pain, fibromyalgia, and insomnia. As the evidence base continues to grow, we will continue to update this repository of peer-reviewed publications studying the clinical effects of FloatationREST.

Examining the short-term anxiolytic and antidepressant effect of Floatation-REST

This study found that a single one-hour session of Floatation-REST was capable of inducing a strong reduction in state anxiety and a substantial improvement in mood in a group of 50 anxious and depressed participants spanning a range of different anxiety and stress-related disorders (including PTSD, Generalized Anxiety Disorder, Panic Disorder, Agoraphobia, and Social Anxiety Disorder).

The findings from this open-label study suggest that

Floatation-REST may be a promising technique for acutely reducing symptoms of anxiety and depression, although the persistence of these effects is presently unknown. With regard to our primary outcome measure, the reduction in state anxiety was evident in every participant regardless of sex or medication status. Moreover, the anxiety reduction was robust, with an estimated Cohen’s d > 2 across all disorders and subgroups. Beyond the immediate dissipation of anxiety, the float experience also induced a significant decrease in self-reported stress, muscle tension, pain, depression, and negative affect, along with a significant increase in serenity, relaxation, happiness, positive affect, overall well-being, energy levels, and feeling refreshed, content and peaceful, with estimated effect sizes ranging from large to very large across variables.

On key symptom-related variables—such as reductions in state anxiety, stress, depression, and negative affect— the effect sizes were consistently large across all tested diagnostic categories. An exploratory subgroup analysis revealed that those with the most severe anxiety reported the largest effects. This latter finding is notable given the fact that the severely anxious participants reported having the most severe impairments in life functioning, and also tended to be the most resistant to other forms of treatment; approximately two-thirds of the severely anxious participants were currently taking an SSRI or SNRI, and over three-quarters had tried psychotherapy.

Indeed, most participants in this study reported having tried a number of other techniques to help them relax and feel less anxious and stressed. Of potential clinical relevance, nearly 75% of the entire sample, and 82% of the severely anxious subgroup, reported that they had achieved more relaxation with Floatation-REST than any of the other treatments or techniques they had tried in the past. While demand characteristics, expectancy effects, novelty effects, and retrospective recall biases may be inflating these subjective comparisons with other therapeutic modalities, the debriefing interviews (S1 Debriefing interview transcriptions) revealed that the float experience had a powerful positive effect on many of the participants.

In future studies, it will be important to assess whether such positive effects can be maintained, or even further improved, with repeated sessions of Floatation-REST. In comparison to previous float studies, the effect sizes observed in our anxious and depressed sample were about twice as large. Since many of the previous studies tested healthy participants, this noted disparity in effects may be driven by the possibility that Floatation-REST provides the largest effect to those who bring the most stress into the float experience. Such an interpretation is consistent with the larger float-induced changes observed in the severely anxious subgroup, and the relatively small float-induced changes observed in the non-anxious reference sample.

While this study was not designed to directly compare anxious to healthy samples, the limited data we have on this matter suggests that Floatation-REST may temporarily lower state anxiety to near-normal levels.

Although mood and anxiety disorders are heterogeneous in terms of their diversity of symptoms and emotional triggers, recent efforts have attempted to develop more effective treatments that can work in a transdiagnostic manner.

Transdiagnostic treatments have the obvious advantage of being easier to disseminate and more widely applicable, especially given the high rate of comorbidity between the different mood and anxiety disorders. To our knowledge, this is the first Floatation-REST trial in individuals across the spectrum of anxiety and depression, with results showing clear signs of short-term benefit in PTSD, Generalized Anxiety Disorder, Panic Disorder, Agoraphobia, Social Anxiety Disorder, and Major Depression. Previous float research with psychiatric populations did not assess the short-term effects following a single float session, but did assess the long-term effects (up to 6 months post-treatment) following 12 float sessions, and found evidence for sustained long-term benefit in individuals with generalized anxiety or burnout depression. Together, these findings suggest that Floatation-REST may have the potential to be a viable transdiagnostic therapy for relieving symptoms of anxiety and depression.

Anxiety and depression affect over a quarter of the population, yet more than three-quarters of patients never receive treatment. This utilization problem is even worse in patients with social phobia or multiple comorbidities.

Novel non-pharmacological therapies for anxiety and depression are desperately needed, and the promising results of this initial Floatation-REST trial warrant further investigation, especially given the insufficient response rates and adherence to currently available treatments, and the paucity of novel medications reaching market over the past two decade. In addition, many medications come with a host of side effects, which further contributes to the poor rates of adherence. The barriers to treatment utilization are complicated, but one of the most common reasons why patients with mood and anxiety disorders fail to receive treatment is due to the notion that they want to solve the problem on their own. In this regard, Floatation-REST may offer an attractive alternative option that enhances selfefficacy and improves treatment utilization by providing anxious patients with the opportunity to learn new ways of coping with distress on their own. In addition, FloatationREST appeared to be well-tolerated by this sample, with minimal evidence of harm, adverse events or major safety concerns arising during the initial float session. Positive experiences outweighed all negative experiences, and consequently, 96% of the participants chose to float for the entire hour, and 100% of the participants requested to float again. The non-pharmacological nature of Floatation-REST, combined with its lack of side effects, ease of use, and rapid onset of benefit, are additional positive attributes that may further improve treatment utilization and adherence.

Transitioning from driver to rider In Sussex County, more than 25% of the population is made up of baby boomers, and while one in four drivers on area roadways is 65+, more than one-third of those over age 70 no longer drive.

The inability to drive does not occur overnight. The transition occurs gradually as age-related conditions begin to advance. You or a parent or loved one may first stop driving at night due to visual deficiencies making nighttime glare a problem. Then driving becomes tentative in bad weather due to slowed reflexes and response time, and finally after sticking closer to home for some time, medical conditions, limited range of motion, diminished mental acuity or faulty memory becomes cause to stop driving altogether. Regardless of how it comes about, when anyone has to retire the car keys, voluntarily or not, it can be very emotional and disheartening. There is most often a feeling that all independence has been stripped away and a life of freedom is gone.

Seniors need transportation Seniors need transportation for many reasons... Just think about all the places you drive to regularly… doctor’s appointments, the bank, your job or volunteer activity, the grocery store or to meet a friend for coffee, to church or a family event, and maybe even a gallery opening or a show. How To Get The Care You Deserve? PART-2

Choosing the Right Home Care Options: What Questions To Ask? Coping with aging, serious illness, recovery, disability or injury can be very challenging! Choosing the right type of care cab be just as tough. It is helpful to know what the differences are among various types of care as you consider your options for the best source of help.

Deciding what type of care is best for you or a loved one at home depends significantly on what the care is intended to do, how long needed, patient diagnosis and level of ability to perform “Activities of Daily Living” (ADL’s).

Home Care is more companionship and regular assistance with ADL’s so you can age comfortably in place (At Home). It is designed for people who need some help with day to day activities in the home or needs someone around in case of emergency. This type care is often long-term, provided by trained aides and is paid by the individual, or covered by some type of personal or Medicaid insurance. So how do you select the right option for your loved one?

What are some of the most asked questions? PART-2 Q: Can you lift clients that are disabled?

• ABC does assist with all types of transfers • Each case is specific to the individual and their level of mobility • Can client pivot on own feet/ankle or not?

• Is an assistive device in home such as Hoyer Lift or Sit-To-Stand?

• We provide Person-Centered-Care and try to train aides specifically on the individual situation Q: Can you transport a client to appointments or do you make arrangements for them?

• Some agencies allow caregivers to drive clients while others do not • Wheelchair lift vehicles is available via public or private transportation services • Agencies will have caregivers accompany clients to appointments • ABC provides transportation using a car service for appointments and other local trips

• During initial intake we review dietary needs and this is made a part of the Care Plan • The aides will shop and prepare meals according to instruction given

Thanks to physical and occupational therapy with Easterseals, Joey has gone from frequently falling because of his physical disability to jumping with both feet. He is also using both of his hands intentionally to play, stack blocks and solve puzzles. Feeding therapy has helped him to swallow and tolerate different foods.

Alexandra is a member of the National Honor Society, a Duke TIP Scholar, received honorable mention two years in a row at Canterbury School’s Science Fair, Regional First Place at HOSA for Nutrition and a Science Olympian member.

Alexandra aspires to be a surgeon specializing in female cancer research and helping children with Treacher Collins Syndrome, a condition that affects the development of bones and other tissues of the face.

Alexandra was diagnosed by Dr. Robert Wood, of the John Hopkins, at the age of 4 with a most severe peanut allergy after she went anaphylaxis after eating a peanut product.

Alexandra feels the best way to prevent a severe reaction is education on food labeling, and fellow student education on this deadly condition. You or your child can correspond with Alexandra by email gallagherscg@aol.com, and she will help with any suggestions and experiences.

HOW TO READ FOOD LABELS Presented by: Alexandra Gallagher Managing life with a food allergy means reading packaged food labels— every time you purchase that food.

What Should I Look For? “May Contain” Statements Other Allergen Statements More Tips for Reading Food Labels The only way to prevent a food-allergy reaction is to avoid the problem food. But you can’t know whether a food contains an allergen simply by looking at it.

Laws and regulations like the Food Allergen Labeling and Consumer Protection Act of 2004 (FALCPA) have made it easier for people with food allergies to identify problem foods and avoid them.

Managing life with a food allergy means reading packaged food labels— every time you buy that food. This is true even if you have purchased the food hundreds of times. Ingredients and manufacturing processes can change without warning. Make a habit of carefully reading labels to ensure you avoid any potential allergens.

While all ingredients in a food are supposed to be listed in the ingredients list, FALCPA only covers the eight most common allergens. These are milk, egg, peanut, tree nuts, soy, wheat, fish and crustacean shellfish.

Note that molluscan shellfish—such as oysters, clams, mussels or scallops— are not required to be labeled as a major allergen.

What Should I Look For? FALCPA-regulated allergens can be called out in one of three ways: In the ingredient list, using the allergen’s common name using the word “Contains” followed by the name of the major food allergen—for example, “Contains milk, wheat” In the ingredient list in parentheses, when the ingredient is a less common form of the allergen—for example, “albumin (egg)” With tree nuts, fish and crustacean shellfish, the specific type must be listed (e.g., almond, tuna, crab).

If you see your allergen featured in one of the above ways, it means the allergen is present in the food. Manufacturers must list an FALCPA-regulated allergen even if the amount is very small.

Non-FALCPA regulated allergens such as sesame and mustard may be present in a food but missing from an ingredient list if they are part of a spice or flavoring. Instead, they may be covered by a general term such as “natural flavorings.”

“May Contain” Statements You may also notice other precautionary language on food labels. These include statements such as “may contain,” “processed in facility that also processes” or “made on equipment with.” These warnings often follow the ingredients list.

Such advisory labeling is voluntary for manufacturers. There are no laws governing or requiring these statements—neither when to include them nor what their wording should be. They may or may not indicate if

a product unintentionally contains, or has come in contact with, a specific allergen.

Likewise, the absence of an advisory label does not mean that a product is safe.

Per the U.S. Food and Drug Administration (FDA), advisory food labels “should not be used as a substitute for adhering to current good manufacturing practices and must be truthful and not misleading.”

Other Allergen Statements Phrases such as “peanut-free” and “egg-free” are not regulated. Product labels can bear these phrases but be made in facilities where the allergens are present. Always contact the manufacturer if you are unsure.

Familiarize yourself with your allergen and the foods it often appears in. Food allergens can appear in surprising places and go by less-common names. Knowing your allergen inside and out will improve your sleuthing skills.

If you are unsure whether a product could have come in contact with your allergen(s), call the manufacturer. Ask them about their ingredients and manufacturing practices.

If you encounter a product that doesn’t have an ingredients list, don’t buy it.

Be extra careful with imported products.

Food labeling regulations vary by country.

Imported items are supposed to follow FALCPA and other domestic food labeling laws, but occasionally they do not.

A child with a food allergy can start checking food labels as soon as he or she learns to read. Practice at home and when you’re shopping—with help from an adult.

Colon Cancer ScreeningOne Day Can Save Your Life According to data from the American Cancer Society, colon cancer is the 3rd most common cause of cancer-related death for both men and women.

That is a disappointing statistic, because colon cancer is one of the most preventable cancers.

Unlike many other cancers, there is an easy and effective way to screen for colon cancer. Colonoscopy is readily available and effective. Large, national studies have shown that colonoscopy reduces an individual’s risk of dying from colon cancer by nearly 70%.

Why should I choose colonoscopy?

1. Colonoscopies help detect colon cancer. No other colon cancer screening is as thorough and comprehensive to detect cancer and remove precancerous lesions. It is the most effective means of prevention, diagnosis, and treatment.

2. Colon cancer is typically (but not always) caused by abnormal growths called polyps, or adenomas, that develop very gradually over time to become cancerous. Veterinary Services Peninsula Veterinary Services is a full-service, AAHA-accredited hospital located in Long Neck, Delaware. Owner Dr. April Reid moved from Brooklyn, New York in 2015 to start a practice in this area after spending summers in Rehoboth growing up. Dr. Reid is a graduate of Ross University School of Veterinary Medicine. She previously owned a thoroughbred racetrack practice for 14 years before making the move to small animal practice. Dr. Reid works closely with several local rescue organizations. She is currently serving as the vice president of the Delaware Veterinary Medical Association and pursuing board certification in veterinary practice. What Should I Feed My Pet??

That is the number one question a veterinarian is asked, especially lately with numerous recalls and studies that have implicated certain foods with heart disease. Unfortunately, there is no easy answer to this as there is no one-size-fits-all food for our pets. Here are several areas to consider when choosing a food.

Life Stage This one would seem to be obvious but it can be confusing depending on the breed of your dog. Puppy foods tend to be higher in calories because growing requires energy.

The age at which your puppy should be switched to an adult food is when they have reached 80% of their adult size. This occurs earlier in small dogs than in large dogs.

Large breed puppies are more prone to developmental bone diseases so they should normally eat large breed puppy food due to the calcium to phosphorus ratio. Senior dog foods usually contain increased protein and fiber but fewer calories, as older dogs tend to eat less but also are not as active, so obesity can become an issue. In addition, there are “All Life Stages” foods, which most dogs can eat throughout their lives no matter their age. This is a good choice when there are multiple dogs in the household and it is impossible to separate them for feeding.

Grain-Free?

While grain-free dog food sounds healthier, the truth is that only a tiny percentage of dogs (less than 1%) are actually allergic to grains. Grains serve as a source of carbohydrates for dogs, which they need for energy. Grains also provide a source of fiber, which is

important for gastrointestinal health.

Grain-free food therefore must use other ingredients as a carbohydrate source and these are usually potatoes.

Chickpeas, lentils, or peas. These alternative carbohydrates can be higher in calories and lead to weight gain. In the recent FDA investigation into the increased rate of heart disease in dogs eating grain-free diets, 93 percent contained peas or lentils and 42 percent contained potatoes or sweet potatoes as the carbohydrate source.

Food Allergies Food allergies or food sensitivity in pets can manifest as digestive problems, skin problems and even behavioral problems. The most common allergies are to chicken, beef or dairy products. Elimination

diets or allergy testing may be needed to obtain an absolute diagnosis to determine the culprit. If the sensitivity is to a certain protein, the options are to switch to another protein source or to a hydrolyzed protein. Hydrolyzed proteins are those that have been broken down chemically so the immune system does not recognize them as a protein to which it is sensitive. These foods are often prescription foods.

By-Products and Marketing By-products are not evil. By-products are just a secondary product derived when a primary product is produced.

Chickens are prepared for human consumption and by-products are everything that isn’t in the package sold in the grocery store. The byproducts are high in essential amino acids and have high bioavailability for our pets. These by-products are now being sold in pet food stores as dehydrated treats or cleverly marketed as exotic ingredients in boutique-brand foods.

AAFCO The Association of American Feed Control does not regulate pet food.

Its mission is to protect the consumer through labeling guidelines. In order for a food to be labeled as “complete and balanced” it must meet nutritional standards established by AAFCO in one of two ways-feeding trials or nutrient content analysis.

The current FDA investigation is exposing the fact that the nutrient content analysis may not be enough to ensure a food is safe for your dog. If a food has not met AAFCO standards it must state on the

Backdoor Roth IRA Contributions: A Way to Catch Up on Retirement Savings • The ability to accumulate potentially tax-free assets for beneficiaries

Presented by Chris Theis Retirement planning is complicated. Many individuals put off saving, thinking that retirement is years away—until it isn’t.

Then, in their 40s and 50s, they start to panic and wonder how they’ll catch up.

One strategy, made possible beginning in 2010 by a provision to the Tax Increase Prevention and Reconciliation Act of 2005, presents a way for some individuals to potentially put away more money for retirement, in a tax-advantaged way.

A strategy that works around income phaseouts One of the reasons people may invest in traditional IRAs is because contributions are tax deductible—but only for individuals within a certain income threshold and who are covered by a workplace retirement plan. Individuals with income over the limit can still contribute, but the contribution amounts are not deductible. A similar income phaseout exists for those wanting to contribute to Roth IRAs, whether or not they are covered by a workplace retirement plan; once they make a certain amount of money, the opportunity is no longer available. Or is it?

Although the 2010 provision to the tax act retained modified adjusted gross income (MAGI) limits on Roth IRA contributions, it eliminated the MAGI limits on Roth IRA conversions. As a result, anyone who earns too much to contribute to a Roth IRA (and to make tax-deductible contributions to a traditional IRA) can now fund a Roth IRA by making a nondeductible contribution to a traditional IRA and then converting that amount to a Roth IRA—in some cases, tax free.

Known as a backdoor Roth IRA contribution, this strategy can be highly effective for creating tax-free income in retirement, but it’s also quite complex. So, it is important to understand the rules for using this strategy and what circumstances may make it beneficial.

If it’s so complicated, why bother?

There are many benefits to accumulating retirement assets in a Roth IRA, including: • Tax-free growth • Freedom from required minimum distributions, so assets can grow tax advantaged for a longer period of time

With the backdoor Roth contribution strategy, however, comes another benefit for individuals who have been phased out of being able to contribute to a Roth IRA in the usual way: the ability to put away more money for retirement.

Take, for example, a married couple in their 40s who file jointly. Both employed, they have maxed out their 401(k) contributions, but neither of them owns an IRA because their MAGI has precluded them from making deductible IRA contributions, as well as Roth IRA contributions. Because they didn’t start saving for retirement as early as they should have, they need to find a way to catch up on their savings. What can they do?

They can each make a $6,000 (for 2019) nondeductible IRA contribution and immediately convert it, tax free, to a Roth IRA. Hypothetically, if each of them contributes $6,000 annually with this backdoor strategy, with a conservative rate of return of 5 percent, over 20 years they each could potentially accumulate $208,316.

The right individuals for this strategy The backdoor Roth strategy is commonly used with individuals who earn too much to make deductible IRA contributions or contribute to a Roth. For these individuals, it’s more advantageous to hold assets in a Roth IRA instead of a traditional IRA, as the investment earnings in the Roth will grow tax free. Other individuals who may benefit are those with a long time horizon to retirement, as well as those who have maxed out their 401(k) contributions. In addition, because the backdoor contribution starts with a contribution to a traditional IRA, individuals must be younger than 70½.

and the conversion. For this reason, it is best to convert immediately after making a nondeductible contribution.

Individuals with existing IRAs. For individuals who already have IRAs, things can be more complex, although still beneficial. The pro rata rule requires individuals who contribute to and maintain money in multiple IRAs, including SEP and SIMPLE IRAs, to aggregate the account balances. If the IRAs have been funded with both nondeductible and deductible contributions, or if there is any pretax money within the accounts, income tax will be owed on the previously untaxed amounts. This means that individuals using the backdoor contribution strategy may owe taxes on the conversion.

Rules for filing. There are specific tax filing requirements for calculating and tracking after-tax amounts. Failure to track after-tax amounts or report these amounts properly can have tax consequences. Because proper filing and recordkeeping are so imperative— and using this strategy can be complex—it’s

important for individuals to work with both their financial advisor and CPA. Unlock opportunities to boost retirement savings Although a simple solution is to save early and often, it’s important for individuals to understand all the strategies available to help them catch up or get ahead. When used properly, funding a Roth IRA with backdoor contributions can be a great way to boost retirement savings.

Car Accident- Who Pays?

Long, Esquire Car accidents are mentally, physically and emotionally traumatic, and are made even worse when trying to figure out which insurance company is responsible for paying the different types of benefits. There are a few things that you can do right after the accident that will save you a TON of work later on: 1. Call the police. The local police department will arrive and take a report with the names, addresses and statements of all parties involved. If applicable, they will also interview witnesses and add those statements to their report. This report will be important to you in proving that you were not at fault, and also for obtaining the other driver’s contact and insurance information.

2. Take photographs of the damage to your car, the damage to the other vehicle and their locations in the roadway if possible.

3. Open a Claim with your insurance. Your insurance company will assign a claim number to review your medical expenses and damage to your car.

4. Call a Personal Injury attorney for a free consultation. Most Personal Injury attorneys offer free consultations, and do not charge a fee until you receive a settlement. The sooner you do this, the smoother and easier your claim can be established and reviewed.

There are many different types of insurance in play for car accident cases.

The first insurance company that you’ll need to report your accident to is your own auto insurance company. The first order of business is usually getting your car repaired. You can go through your own insurance company for this, or the insurance company for the car that hit you. If you have rental car coverage, you’ll also want to get this started as well.

The second coverage that you will need is called Personal Injury Protection- or PIP, for short. This pays your medical bills and lost wages up to your maximum policy limit. There is a 2-year time limit on this coverage.

In order to start this portion of your claim, you will need to fill out an application for benefits. Beware, these applications must be completely and fully filled out and all injuries must be documented in order to avoid any denials of the claim.

The other person’s insurance is called Liability Insurance. This is what pays you the pain and suffering settlement at the end of your claim. These kinds of settlements can vary greatly, and are generally based on the nature and extent of the injuries as well as the consistency and complexity of the medical treatment. It may be helpful for you to keep a journal of instances where the injuries have prevented you from participating in your usual hobbies, curtailed previously enjoyed experiences or otherwise affected your usual standards and activities of daily living. You should not settle with the Liability Insurance Company until you know the nature and extent of your injuries; otherwise, you may not receive full compensation for your claim. Under no circumstances should you settle with the Liability Insurance Carrier until you discuss your claim with an attorney skilled in

Albert and Elmina Miller started this company back in 2007, but it wasn’t until January of 2017 that Albert decided to take the leap of faith and go all-in with the real estate business. He was raised in the Amish community in Dover, Delaware and at age twelve began working in the construction field with his family.

By the time he turned sixteen, he was a full-time bricklayer and from there continued in the masonry field for the next three years. After turning twenty, he started his own business framing houses for big developers throughout Delaware and Maryland’s Eastern Shore. That same year, Albert and Elmina made the hard decision to leave the Amish community. This decision was tough, but they remain forever grateful for the hardworking and committed foundation that they were raised in.

Wilkins Wildlife & BedBug 911 Jay Wilkins, the owner of Wilkins Wildlife & BedBug 911, started his business just a few short years ago in 2017. With the last $344 to his name, Jay decided to start this business with the purchase of four humane wildlife traps. He then started advertising and posting videos of his clients’ needs on Facebook. Not knowing how a brand new business would succeed, he had enough faith within himself and his future clients that he would continue to push forward and soon was able to see his business start to grow.

Jay was not new to this line of work, as he worked for a pest control company years prior to taking the leap on his own. One job a day led to two jobs a day and now has become a full day and at times, full weeks. His passion for solving human/wildlife conflict is unmatched and the education he openly shares with clients speaks volume about his integrity.

Wilkins Wildlife & BedBug 911 is a 100 percent humane wildlife removal business. Jay and his staff will do what they have to do to remove animals that may have made your home, their home. Once Jay removes the animal from your structure or property it may be taken to a rehabilitator for observation or released within state guidelines and laws. To understand these laws and guidelines the state requires nuisance wildlife control operators to be permitted by the state in order to operate legally.

When Jay started this business, he was a one-man operation. With the growth of the company, Wilkins Wildlife & BedBug 911 has recently added two new employees to the team. It welcomed a lifelong friend, Beth Blake, to manage his office and keep Jay on track with his appointments. She has since become its backbone and she is the voice you hear when calling for services. She makes sure that clients are informed on the services they provide, as well as keeping them updated on the process happening at their home. It also welcomed Jay’s son Aulstin Wilkins to the team. While Aulstin had a job working as a detailer at a local car dealership, he would help his father when he could fit in his schedule.

His passion grew once he was able to see the results he had provided for a client, whether it be a bedbug treatment or the removal of an animal. It didn’t take long before reviews starting pouring in.

Wilkins Wildlife & BedBug 911 does an array of animal removals. Jay and his team will resolve issues with snakes, birds, raccoons, squirrels, coyotes, possums, foxes, groundhogs, skunks, and bats. While some animals can be caught by hand, many of them require trapping. Once a trap is set in place, sometimes a camera is placed in the area of the trap and is monitored by Jay himself. Once an animal is trapped, that animal will be picked up the same day and released according to state law. Wilkins doesn’t stop there. Jay isn’t about just capturing an animal, he wants to find their entrance points and repair those issues so the homeowner will never have to worry about other animals reentering the structure. He also offers exclusions on mice, rats, and rodents.

It’s that time of year again and there are several things to consider as the cooler temperatures are approaching.

Will Have Your Landscape Blossoming Next Spring burlap” and “bare root.” Balled plants can be planted at any time during the year, while bare root plants can only be planted after the leaves drop (that is, in fall or winter).

Bare root plants are less expensive than balled ones, so simply by planting in the colder months, one can reduce cost significantly with no difference in results.

By Fred Winward Many homeowners wait until spring to think about their landscape.

Inspired by the warmer weather and the blooming flowers, they start imagining what improvements they would like to make to their landscape. Often, they’re disappointed to find that all of the landscape contractors they contact are busy until midsummer or later.

This is the “spring rush”.

It is important to realize that each season, not just spring, is the right season to work toward a better landscape. The most effective landscaping program includes yearround activities, timed precisely for the maximum effectiveness.

There are diverse landscape maintenance projects to be done in the fall, including cutting back annuals and herbaceous perennials, and raking leaves.

Fall is a great time to transplant because the shorter days and lower temperatures place less water on freshly planted perennials.

The fall season is generally a much less hectic time than spring in the landscaping professional’s schedule more accommodating this time of year.

WINTER Winter is a time for introspection.

As the shining days cause us to spend more time indoors, we find ourselves with more time to reflect, to evaluate, and to plan.

Though it’s not commonly thought of as the time to work on one’s landscape, quite the contrary is true. A successful landscape, like any worthwhile project, demands careful planning…and what better time to plan than now. Work with your landscape designer to define your goals for the upcoming year, unhurried by the tight schedules of spring.

There are a number of outdoor maintenance projects for which wintertime is a fine time for pruning; removing old, dead, or diseased branches will result in a rejuvenated plant in spring, with improved flowering fruiting and form.

Early winter and pre-spring are considered the “off season” in the landscaping profession. As a result you may be able to obtain discounted rates during this time of year.

SPRING While many people plant in the spring, it’s actually a better time to work on the “hardscape” walkways, driveways, retaining

walls, and fountains. A spring deck or patio project will bring countless hours of enjoyment per the summer months.

Slide on into 2020 with these fresh ideas for those hard to cover Sliding Doors!

Often while on an in-home consultation, the topic of handling the sliding door comes up.

It’s always accompanied with some sort of joke, or look of bewilderment. I usually look forward to figuring out my customer’s way of articulating their concern. To paraphrase, it generally goes like this. “I need to cover this—I HATE VERTICAL BLINDS.” Of course, I need only remember back to my first apartment to understand why. Cheap vertical blinds are everywhere. Their top treatments are either bent up and trying to fall, or already placed in that closet, behind the water heater. The vertical louvers are seldom part of a complete set. They like to break and fall, and usually there is tape or unmatched colors mixed in for good measure.

The good news- the custom blinds industry has you covered with more modern and beautiful options!

Vertical Honeycomb ShadesHoneycomb shades are quite popular these days, so why not for sliders? Several manufacturers make honeycombs that are rotated on their side, and anchored to the stack side, and make a solid option for this application. Not only are these available in many colors and textures, but they are some of the most insulative coverings available. When you want to use the door, simply slide the shade into the closed position.

You want it half open- no problem. Generally, magnets are used to help in the fully closed position.

ShuttersShutters are one of the hottest products in the window covering industry, thanks in part to myriad of home improvement shows that seem to always drop in shutters as one of the final touches. Shutters are also a great insulator and are extremely easy to use. If you have kids or pets that are prone to a little rough housing, these are a nice hearty choice.

Sheer ShadesAnother trendy approach comprises multiple layers of sheer fabric, arrayed in a varying degree of light blocking and privacy choices. Essentially, when the vanes are opened, you can see right through the sheer backing. When you want that privacy, close the vanes and you’re all set.

Natural Woven Wood DrapesIf you are a big fan of a natural look, several manufacturers market woven shades that are turned for use in vertical applications. Generally cordless and extremely light, these can be optioned with base bamboo materials that are cost competitive to custom vertical blinds. If you want to add darkening or higher levels of privacy, linings can be applied to the back.

“Old school” Vertical BlindsYes, you’re reading this correctly. These are still made! As a provider of many different window treatments, I want to go on the record to say that vertical blinds are a great product.

If you skip the dollar store version, you will find a smooth, effective, and long-lasting covering for a large glass door. No other treatment combines versatility, ease of use, and affordability quite like this venerable leader.

Modern vertical blinds are available in high end vinyls and fabrics. For a contemporary look, we recommend outfitting your vertical blind with a stained wood valance, which can easily be matched to flooring, cabinetry, etc.

As always, we recommend consulting with professionals to ensure your product is right for your application. This will also help improve the finished look and long-term reliability of your new treatment. Also- do make sure your seller offers you some sort of warranty for this or any window covering. Not only is this good insurance in case of damage or faulty components, but custom blinds retailers are not looking to warranty something that will fall apart.

Meet Successful Women Entrepreneurs in the Restaurant Business Marilyn Spitz

Dana Banks

I came into the restaurant business through love. In 1980 I met a man Windsurfing on Rehoboth Bay. He was one of the original owners of The Back Porch Café. We fell in love and 4 years later I was surprised to learn that he had left his share of the business to me at his death.

I have worked in the restaurant business since I was 14. I always loved it, and I have been blessed with a great staff/ friends and family. Every day brings something new. Never the same day twice.

Here I am 39 years later a part of an amazing journey. I have primarily worked in the background, doing the books, the flowers, bar back, well…many hats. Our food is phenomenal, thanks to our chef and our wine list deep thanks to my partner, Keith. We have all learned basic carpentry and plumbing skills in our efforts to maintain the century old building that we inhabit. The love I have for this business reveals itself on busy nights with the diner’s voices filling the air, the chef calling out the servers names as the orders come up, the bus boy reaching for a tub as I grab a bread basket, basically, controlled, glorious chaos.

What a team it takes to pull together a successful evening night after night!

Our guests have often become fast friends and a community has grown up around us and our little town. After all these years, though, I find that it is the relationships with my coworkers who warm my heart and make it all worthwhile.

Now, at age 69, I don’t carry bus tubs anymore but I still spend down time sailing on Rehoboth Bay.

I had hoped that I would open my own place and that happened in 1997. The Parkway has been going strong ever since. The best thing that happened along the way was taking over the kitchen and learning how to cook. I certainly enjoy hearing the positive comments. I try to do it all and am a bit of a control freak. When I’m not at work, I enjoy my dogs and helping out wherever I can.

Susan Wood I opened the Cultured Pearl Restaurant in 1993 out of necessity. I loved sushi and I got very tired of driving to Annapolis to get some! With a background in drafting, design and construction management and the help of some wonderful family and friends, building the restaurant was the easy part. Running it was a different story! Through the years I have been everything from a dishwasher to a plumber to a sushi chef. It is the diversity of the profession and the instant gratification received when a customer leaves satisfied and happy that makes owning a restaurant a very rewarding career. Of course, being able to remodel every few years and fulfill my urge to build things doesn’t hurt either! With my wonderful husband as a partner, our two beautiful daughters by our side and an amazing, dedicated, long time Pearl Family Team, the restaurant is our life and we greatly appreciate every single customer that makes it possible for us to open our doors every day and leave with a smile on our face every night!

Maria Fraser I bring over 14 years past experience in the food service industry. I served a ten year span at the Country Squire, a restaurant in Rehoboth Beach, starting out as a bartender/server and worked my way up to the manager. Then I worked as a bartender at the infamous Starboard in Dewey Beach. I also owned and participated in operations, of my own business, Maria Fraser LLC, a town and Landscaping Company, serving the community of Rehoboth Beach for over 30 years.

While going through a divorce, becoming a single parent with 3 sons still in high school and college, I decided to open The Cafe 26 when the opportunity come along in 2010. I was not daunted by the hard work, and was accustomed to working with the public which I enjoy very much. I knew it would be difficult but necessary by wearing all of the “hats” in this business from dishwashing to cooking, to have a successful business.

I am grateful to have behind me an excellent hard working, loyal staff, which are family to me. Though I learned the hard way, I am very proud of what I have been able to achieve thus far and grateful for the confidence my patrons have shown in me, my staff and Cafe 26. My life is always centered on my family especially on my 3 sons, Will, Nolan and Prescott, as they continue to achieve their hopes and dreams by working hard and never giving up whenever challenges come along.
