Special Needs Children and Oral Health: Where to start?

By David Delgado, DMD Sometimes just going grocery shopping is a challenge when you have little ones in tow.

Yes, toys and candy! They already know exactly where they are half the time! You practically need a map to avoid all the wrong turns that will cost you extra money, time and the patience to say no. Oh and of course the occasional floor tantrum or meltdown with master manipulation to follow soon after.

This happens to parents more than we like to admit and applies for all children, typical and children with special needs. I happen to fall into both camps, as a father of two, one a child with Autism. I know first hand the fear of the unknown, the tense feeling to a familiar or new environment, whether it be groceries, clothes shopping or a dental visit.

Sometimes at Delaware Pediatric Dentistry a dental chair is not needed to do an examination on a special needs child, you may find us sitting on the floor, singing, humming or making up stories on the spot as we evaluate our patients mouth. Thinking outside of the box to create confidence and trust is something we strive for as pediatric dentists with all children special needs or not. After all, it took my 3yo child with autism three visits to our office for him to finally sit in the chair and for me to do a proper cleaning and I’m the father!

I am certainly not offended but I use this as a simple example that repetitive appointments may be necessary for our special needs patients to feel comfortable enough for us to do a proper evaluation.

The federal Maternal and Child Health Bureau defines Children with Special Health Care Needs (CSHCN) as those who have or are at increased risk for a chronic physical, developmental, behavioral or emotional condition and who also require health and related services of a type or amount beyond that required by children generally. For some parents with CSHCN oral hygiene can easily be overlooked with all other medical issues the child may be going through. Additionally, certain conditions may place the child at a higher risk for dental decay, gum diseases, crowding, extra or missing teeth. These findings are to be reviewed by the pediatric

dentist specifically to the child’s oral health concerns. The U.S. Department of Health and Human Services reports 1 in 5 US households have a child with special health care needs, additionally they are almost twice as likely to have undiagnosed dental problems compared to a typical child.

At Delaware Pediatric Dentistry our goal is to change this outlook. As board certified pediatric dentists our focus is to prevent oral diseases and cavities through early childhood preventive visits and education. As with CSHCN early childhood intervention has been the golden standard for treatment and the same can be said of oral health. The American Academy of Pediatric Dentistry recommends all children to be seen by 12 month of age or after the eruption of the first tooth by a pediatric dentist. An important step in good oral hygiene and preventing cavities before they start. All pediatric dentists are trained in the treatment and care for those with special needs they have 2-3 years of specialized training past the four years of dental school with children and CSHCN.

What to do:

1) Finding a Dental Home.

AAPD recommends finding a pediatric dentist that you feel comfortable with treating your child by 1 year of age. Ask your pediatrician for a referral or search online at AAPD.org 2) No Teeth...No Problem Start your infants oral care now, start by wiping their gums with a wet gauze after feedings this will help remove excess milk in the mouth, clean the gingiva and also help create a habit the child will accustom to if done multiple times a day.

3) Call ahead and fill out all necessary information about your child and list your concerns, speak with the pediatric dentist prior to the first visit if needed.

Health & Wellness R. Alberto Rosa, M.D., FACC Dr. Ramon Alberto Rosa is a graduate of the University of Santo Domingo in his native Dominican Republic. He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), Chief Medical Resident (19941995), and cardiology fellowship (1995-1998).

Dr. Rosa has practiced cardiology in Sussex County since July 1998. He has served as Medical Director of the Cardiac Catheterization Laboratory and as Chief of the Department of Cardiology at Beebe Medical Center in Lewes, DE. Dr. Rosa is trained in non-invasive cardiology, Nuclear Transesopageal Echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Dr. Rosa is board certified by the American Boards of Internal Medicine and Cardiology. He is also a Certified Aviation Medical Examiner and is Board Certified in Nuclear Medicine.

Women and Heart Disease: Reconnecting with a good ol’ friend Recently published research reveals, once again, the lack of awareness about the impact of heart disease in women, among both women and physicians, and how the stigmatization of the disease and body weight continue to be major barriers to women seeking treatment.

Data collected from interviews on 1,011 US women aged 25-60 years of age and physician survey data from 200 primary care doctors and 100 cardiologists (*).

The main goals were to determine knowledge, attitudes and beliefs in regard to heart disease in women. The findings can be summarized as follows: 1. Awareness: Among women, 45 % were unaware of the fact that heart disease is the number one killer of women in the U.S. This level of awareness was even lower among women with lower educational levels and income as well as in ethnic minorities. About 71% of women the issue

of heart health was never raised with their doctors.

Conversely, physicians did not discuss cardiovascular disease with them citing other more pressing health issues at the time of their visit or the lack of reported cardiac symptoms. This suggests prevention was not seen as priority, in the absence of symptoms. As we know, among women the first symptom of a heart problem is often a heart attack or sudden cardiac death.

2. Assessment: Even though a majority of women had an annual physical exam or wellness visit, only 40 % reported having a heart health assessment. About 74 % reported having at least one risk factor for heart disease, just 16 % were told by their doctor that they were at risk. An interesting fact is that 45 % of women cancelled or postponed their doctor visit until they lost weight, out of embarrassment or by feeling overwhelmed by their heart disease, and lack of time to exercise or take better care of

themselves.

3. Among the doctors surveyed, only 22 % of primary care physicians and 42 % or cardiologist felt well prepared to assess cardiovascular risk in women. In addition, risk assessment guidelines were implemented by only 16 % of primary care doctors and by 22 % of cardiologists.

There remains a need for the medical community

to try and help counteract the stereotypes that still exist about cardiovascular disease in women. This requires raising the level of awareness about these issues and to help women overcome barriers to increasing physical activity and to develop healthier eating habits instead of focusing in the stigma of just weight loss. Women are natural caregivers, often times neglecting their own personal care, while doing all and then some

for others. It is time to take care of the caregivers as well.

Easterseals Teletherapy is a Win for Families

By Natalie Scott Blaire Marie tilted her head, smiled and let out a giggle when she saw her Easterseals occupational therapist on the screen for the first time.

She is used to meeting with her in person weekly at daycare. When the virus sweeping the nation caused many of Easterseals services to shut down, the therapists jumped into action. Shortly after, Blaire Marie and her therapist were reunited, through a screen, during Teletherapy. Blaire Marie greeted her therapist with a “Hey,” her favorite word these days.

Digital Infrared Thermal Imaging Dr. Tracy Hudson Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. While attending UNCW, Dr. Hudson suffered an injury playing softball and she sought treatment with a local chiropractor. After a few adjustments, Dr. Hudson was back on the field. Digital Infrared Thermal Imaging takes a thermal picture to determine abnormalities or changes in the breast tissue. Thermography can detect irregular patterns years before a mammogram. Best of all, Thermal Imaging is painless, noninvasive, and has no radiation.

DITI’s role in breast cancer and other breast disorders is to help in early detection and monitoring of abnormal physiology and the establishment of risk factors for the development or existence of disease. When used with other procedures, the best possible evaluation of breast health is made.

This 15 minute non-invasive test is a valuable procedure for alerting your GP or specialist to the possibility of underlying breast disease. This test is designed to improve chances for detecting fast-growing, active tumors in the intervals between mammographic screenings or when mammography is not indicated by screening guidelines for women less than 50 years of age. A patient’s thermograms (breast images) are kept on record and form a baseline for all future routine evaluations.

DITI is especially appropriate for younger women between 30 and 50 whose denser breast tissue makes it more difficult for mammography to pick up suspicious lesions. This test can provide a ‘clinical marker’ to the doctor or mammographer that a specific area of the breast needs particularly close examination. The faster a malignant tumor grows, the more infrared radiation it generates.

So, if this is you, let me introduce you to some methods of recharging your body’s health and jumpstarting your weight loss that are tried and true and have been around since Jesus fasted for 40 days.

Welcome to fasting.

What is Intermittent Fasting You may believe that there is only one type of fasting, which may mean to you not eating for a prolonged period of time, maybe as long as a week. But there are actually several types of fasting and there is a whole science as to which kinds of fasting are best, for ease of following and which deliver the best results, for weight loss and overall health. For these reasons, we recommend intermittent fasting for most of our patients, which is an easy and intuitive way to control eating habits based on the time of day, as opposed to just not eating anything for days on end, which most people cannot tolerate.

The intermittent fasting we most commonly recommend is the 16:8 method. Meaning that you will eat in between designated 8 hours during the day, and fast for the rest of the day for 16 hours. And this is not as bad as it might sound initially, because you will be sleeping for a lot of that fasting time.

For most people this means that the hours that you will eat will be between 11am and 7pm. And for most people this will mean just skipping breakfast and not eating too late before going to bed. Intermittent fasting follows our circadian rhythm, or our sleep/ wake cycle, balances periods of food intake with periods of no food intake so that the body spends or burns the calories it ingested rather than storing those ingested calories as fat, which can lead to obesity. This discovery was awarded the Nobel Prize in Medicine in 2017. Well we all knew this, but 3 American scientists proved it!

Alternate Day Fasting In addition to intermittent fasting, we may also recommend alternate day fasting. This fasting entails dropping your caloric intake to 500 calories for the day on two days during the week, in addition to the intermittent fasting. This is not usually 2 consecutive days.

There are other varieties of fasting, that are healthy, effective and promote even more health benefits in addition to weight loss, such as prolonged fasting which causes autophagy, which is the body’s process for breaking down and eliminating cellular debris that can be initiated by fasting.

What is Autophagy a Japanese cell biologist, Dr. Ohsumi, was awarded the Nobel Prize in Physiology or Medicine for his discoveries on how cells recycle their content, a process known as autophagy, a Greek term for “self-eating.”It is a crucial process. During starvation, cells break down proteins and nonessential components and reuse them for energy. Cells also use autophagy to destroy invading viruses and bacteria, sending them off for recycling.

structures. The process is thought to go awry in cancer, infectious diseases, immunological diseases and neurodegenerative disorders.

Disruptions in autophagy are also thought to play a role in aging. It takes 7 days of fasting to reach autophagy.

What kinds of food can I eat to help with weight loss?

You can actually eat any kind of food and still lose weight as it is the quantity of calories that you eat during the day that will determine how much weight you lose. That said, if you were to eat a small amount of calories, but unhealthy calories devoid of essential vitamins and minerals, you are not helping your body’s health long term. Dr. Parsons will recommend a diet that is mostly vegetables, the green crunchy variety, not starchy vegetables such as potatoes or rice, and lean meats such as skinless chicken and fish.

How can Dr. Parsons help with weight loss program?

Health & Wellness HGA’s 9 Step Prescription for Good Digestive Health Improve Your Vitality & Eliminate Discomfort The key to good nutrition depends on two things: Eating the right foods and maintaining a strong digestive system that allows your body to break down and absorb the nutrients from the foods you eat In order to keep your digestive system in optimum shape, follow these guidelines: 1. Focus on fiber. Ideally, you should be eating 20 to 35 grams of fiber — or about 14 grams of fiber for every 1,000 calories you eat. Fiber is the workhorse that slows down digestion and gives your body time to absorb the glucose it needs to produce energy.

2. Drink lots of water. Water helps move nutrients into your organs and tissues. It helps regulate your body temperature. An adequate amount of fluid also softens your stool for easier elimination.

3. Avoid processed foods. Many processed foods have little or no fiber, are too high in sodium, and often contain preservatives and other additives that you may be sensitive to. Some processed foods also contain lactose, which can give you gas.

4. Cut down on fat. Foods that are naturally high in fat or are fried in fat alter the digestive process and can lead to constipation or diarrhea. No more than 30% of the calories you consume should be fat; and only one-third of fat calories should be saturated fat.

5. Limit your alcohol consumption. Drinking too much alcohol can have a negative impact on every organ in your body.

6. Add yogurt to your food plan. Your digestive system contains healthy bacteria, known as probiotics, which help your digestion and fight off disease. Both yogurt and kefir contain probiotics. Eating them on a regular basis can help replace good bacteria lost by eating a poor diet, illness, stress and some medications and medical treatments.

Social Distancing -Alone Together We are living in times that have never been experienced before. In the past century, there have been just four pandemics. The last one was in 1918.

Life, as we know it has been put on hold.

Events cancelled, school is now in a virtual landscape, and undoubtedly someone you know has been affected by this disaster.

Thanks to modern technology, we are still able to connect with our friends and loved ones either through a Google hangout, Zoom, or House party.

In dark times, it is more important than ever to be a beacon of hope for your friends and family. Families all over the nation are spending more time together than they have in years. With the absence of full calendars, practices and other after school activities, there is more time to connect.

When this time passes (and it will) most of us will come out better for it; more spiritually connected and definitely closer to our loved ones.

Although there is currently no vaccine to prevent COVID-19, the best way to prevent the spread is to avoid being exposed. Those who are in close contact with one another, usually within six feet, spread it from person to person. Respiratory droplets are produced when an infected person coughs or sneezes. The droplets can land in the mouths or noses of people who are nearby or possibly inhaled in the lungs. Another silver lining The County 22 I FallWoman 2020 Magazine

is that once you have contracted the virus it is unlikely you can get it a second time. Health officials have said that coronavirus antibodies are able to prevent a person who had the infection from getting it a second time.

The best course of action at the present time is to minimize your chances of contacting the virus. We can do that in the following ways: • Washing your hands with soap and water for at least 20 seconds, especially after being in a public place.

• You can use a hand sanitizer (if soap and water are not readily available) that contains at least 60% alcohol.

• Avoid touching your face with unwashed hands • Sneeze into your elbow • Clean and disinfect frequently touched surfaces daily – this includes doorknobs, light switches, keyboards, and phones.

It is easy to feel helpless during times like this.

One way to alleviate those feelings are to try and help where you can and right now our nation is facing a major shortage in blood donations. Donating blood is essential to the health of a community and the need right now is dire. The Red Cross has expressed concern over future blood shortages.

If you can, give blood so they can maintain a sufficient blood supply for the weeks to come.

Never have we been given this respite from our daily schedules.

The stress and anxiety from the unknowns can be daunting. Try and take some time for self-care and making yourself a priority during the days ahead.

Addressing The Fires Of Inflammation Functional Medicine Inflammation is the key in the rising epidemic of autoimmune disease, depression, and asthma.

I

nflammation is our body’s innate mechanism to defend itself against negative stimuli such as pathogens, trauma, environmental toxins, and damaged cells. It is a physical response to emotional stress as well. The Latin terminology handed down from Celsus described the key features as: rubor (redness), calor (heat), tumor (swelling), and dolor (pain). All of these, except pain, describe the role of the vasculature. There is increased blood flow, vasodilatation, and increased vascular permeability.

Inflammation can be acute such as with a laceration. Or it can become a systemic, chronic condition with a domino effect of chemical, hormonal, emotional and/or physical triggers. These could be found in foods, the environment, and even stress. When this inflammation continues, the immune system will start to attack healthy tissue resulting in illness and disease. Symptoms could be anything from gut issues, headaches, joint pain, to fatigue and weight gain.

In joints it causes arthritis, in the inner blood vessels, heart disease and stroke. For the thyroid, inflammation can cause Hashimoto’s Thyroiditis. As the immune system becomes weakened, cancer cells can become undetected and grow.

Testing would involve hsCRP (C-reactive protein) which is produced in the liver in response to infections and inflammation. IL-6 (interleukin 6) and ESR are other indicators of inflammation. Toxic metal testing, comprehensive stool analysis and organic acid testing will also detect systemic inflammation. Genetic testing can determine potential links to systemic inflammation. Even if you have normal CRP and then only sleep 4-5 hours a night for a few weeks, your level will elevate. If you are diabetic, due to the glycating effect of blood sugar, you will experience the effects of systemic inflammation.

How to Travel Safely

with an Ill or Aging Loved One Summer is finally here — and so are summer vacations. For some, this means spontaneously packing the family up in the van and driving to the beach for a week of fun in the sun. For others, it means wondering if your loved one will even be able to make it out of the house. However, just because you have an aging or ill loved one, does not mean you need to postpone the family trip. With the right preparations, you can help your loved one enjoy a relaxing, worry-free vacation.

1. Get doctor’s approval. First and foremost, consult with your loved one’s primary care physician and ensure that they are cleared for travel. Talk with their doctor or Delaware Hospice about any concerns or risks in traveling. If necessary, use this opportunity to make sure that your loved one is up to date on all vaccinations and has refills for any prescriptions.

2. Arrange special services ahead of time. If you are planning on flying, contact the airline ahead of time and request wheelchair access at the airport, advanced boarding, or disabled seating, if necessary. You can do this for hotels, rental cars, and other vacation activities, as well.

3. Pack lightly, with essential items easily accessible. Make sure to have all the essentials handy — this includes medication (and copies of prescriptions), water and snacks, emergency phone numbers, sunscreen, and important documentation (passport, Medicare card, and travel insurance). If you are flying, be sure you take their medications in your carry-on.

4. Be flexible. Plan for plenty of breaks and lots of downtime to accommodate your loved one’s needs. Be realistic and patient about how much activity they can or cannot do, and adjust your plans to their capabilities.

5. Take extra precautions when traveling to the beach or somewhere hot. If traveling somewhere where you will be spending a lot of time in the sun, be sure to pack lots of sunscreen, loose fitting clothing, hats, and water. Make sure you know the signs of heatstroke.

Conditions Women Need to Know About Thyroid Disorders Sometimes this disorder ramps our metabolism, causing sudden weight loss, nervousness, irritability, and sweating. Yet you might experience the opposite as your metabolism slows to a crawl: unexplained weight gain, fatigue, and memory impairment are common signs, but symptoms sometimes develop so gradually that it may be years before you decide to see the doctor.

What Happens? Nestled at the base of your neck, your thyroid gland is a tightrope walker, keeping your metabolism in balance by releasing hormones that control the rate at which your body burns energy. Some thyroid disorders, like Graves’ disease, cause your thyroid to pump out too much thyroid hormone. These disorders, termed hyperthyroidism, speed up your metabolism.

HEMORRHOIDAL TREATMENT Over half the population will experience some type of hemorrhoidal problem in their lifetime. How do hemorrhoids develop?

Hemorrhoids are simply swollen and engorged veins in the anal and rectal region. Understanding Peanut Allergies An advanced honors student, Alexandra has been a straight A student since kindergarten. Alexandra is a member of the National Honor Society, a Duke TIP Scholar, received honorable mention two years in a row at Canterbury School’s Science Fair, Regional First Place at HOSA for Nutrition and a Science Olympian member.

Alexandra aspires to be a surgeon specializing in female cancer research and helping children with Treacher Collins Syndrome, a condition that affects the development of bones and other tissues of the face.

Alexandra was diagnosed by Dr. Robert Wood, of the John Hopkins, at the age of 4 with a most severe peanut allergy after she went anaphylaxis after eating a peanut product.

Alexandra feels the best way to prevent a severe reaction is education on food labeling, and fellow student education on this deadly condition. You or your child can correspond with Alexandra by email gallagherscg@aol.com, and she will help with any suggestions and experiences.

Presented by Alexandra Gallagher Signs and Symptoms There’s a difference between peanut allergy and peanut intolerance. Sometimes it can be difficult to know whether you are allergic or intolerant to peanuts.

• Peanut intolerance. If you have peanut intolerance, you usually can eat small amounts of peanuts with only mild symptoms, such as indigestion or heartburn, or no reaction at all. A peanut intolerance doesn’t involve your immune system.

• Peanut allergy. An allergy to peanuts involves an immune system response.

Even a tiny amount of peanuts may trigger a serious allergic reaction. Tests can help determine whether you have true peanut allergy.

Peanut allergies trigger an immune system response. Your immune system reacts to proteins found in peanuts. Exposure to peanuts or traces of peanuts may cause immediate reactions, such as itching, redness, swelling, shortness of breath, wheezing, nausea, abdominal pain, lightheadedness or loss of consciousness (anaphylaxis).The chemicals in peanuts can cause hives to develop on the areas of your skin that have come in contact with peanuts or traces of peanuts. Hives may spread to the rest of your body.

Allergic reactions to peanuts usually occur within minutes after exposure, although reactions within an hour or so after ingestion are possible. Signs and symptoms can range in severity depending on which body systems are involved in a reaction and how much peanut protein you’ve been exposed to.

The most serious and potentially deadly allergic reaction to peanuts is an anaphylactic response. If you are highly sensitive, an anaphylactic reaction can develop immediately after peanut exposure, causing the airways (bronchi) to constrict, making breathing difficult. Blood pressure may drop to life threateningly low levels, making you feel

dizzy or lose consciousness. Other serious signs and symptoms of an anaphylactic reaction include: • Wheezing • Rapid or weak pulse • Blueness of your skin, including your lips and nail beds • Diarrhea • Nausea and vomiting • Dizziness • Loss of consciousness Seek emergency medical care if you or someone else develops an anaphylactic reaction.

Causes Peanut allergy is caused by an immune system malfunction. Your immune system identifies peanuts as harmful triggering the production of immunoglobulin E (IgE) antibodies to neutralize the peanut protein (allergen). The next time you come in contact with peanuts, these IgE antibodies recognize it and signal your immune system to release histamine and other chemicals into your bloodstream.

Histamine and other body chemicals cause a range of allergic signs and symptoms.

Histamine is partly responsible for most allergic responses, including runny nose, itchy eyes, dry throat, rashes and hives, nausea, diarrhea, labored breathing and even anaphylactic shock.

Exposure to peanuts can occur in three ways: • Direct contact. The most common cause of peanut allergy is direct contact with peanuts. This means exposure via all routes of contact usually through eating peanuts, but including kissing or touching someone who’s been in direct contact with peanuts.

• Cross contact. This is the unintended introduction of peanuts into a product.

It’s generally the result of exposure to peanuts during processing or handling of a food product.

• Inhalation. An allergic reaction may occur if you inhale dust or aerosols containing peanuts, such as that of

peanut flour or peanut oil cooking spray.

Common food products that can trigger peanut allergy symptoms if they contain peanut proteins include: • Peanut butter • Peanut flour • Ground or mixed nuts • Baked goods, such as cookies and pastries • Ice cream and frozen desserts • Energy bars • Salad dressing • Cereals and granola • Grain breads • Marzipan (a molding confection made of nuts, egg whites and sugar) • Nougat Peanuts may be present in not so obvious foods including: • Arachis oil. This is another name for peanut oil. Pure peanut oil usually doesn’t trigger allergic reactions because peanut proteins are usually removed during processing. However, peanut oil can become contaminated with peanut proteins.

• Artificial tree nuts. Peanuts can be flavored to taste like other nuts, such as walnuts or pecans.

• Chocolate candies. Some chocolate candies are produced on equipment that is also used for processing peanuts or peanut containing foods.

• Cultural foods. Some African, Chinese, Indonesian, Mexican, Thai and Vietnamese dishes often contain peanuts or are exposed to peanuts during restaurant preparation.

• Nut butters. Nut butters, such as cashew nut butter, are often processed by the same equipment used to make peanut butter.

• Specialty items. Foods sold in bakeries and ice cream shops may come in contact with peanuts.

• Sunflower seeds. Many brands of sunflower seeds are processed on equipment also used to produce peanuts.

Risk factors It isn’t clear why some people develop allergies while others don’t. However, people with certain risk factors have

a greater chance of developing peanut allergy: • Family history of allergies. You’re at increased risk of peanut allergy if other allergies, especially other types of food allergies, are common in your family. A child’s first allergic reaction to peanuts usually occurs between ages 1 and 2.

• Personal history of peanut allergy.

About 20 percent to 25 percent of children with peanut allergy outgrow it. However, even if you seem to have outgrown peanut allergy, there is still a small risk it will recur.

• Altered immune system. According to recent studies, the prevalence of peanut allergy has doubled in young American children in the last five years. Although reasons are unknown, some scientists believe that the destroying of many infectious diseases may have altered the immune system.

Other experts suggest that improved hygiene is involved, proposing that higher standards of cleanliness in Western countries may have confused our immune systems, increasing the chances of developing allergies later in life, when the immune system is usually more prepared to prevent allergies from occurring.

Treatment Medications, such as antihistamines, may reduce symptoms of peanut allergies.

These drugs can be taken after exposure to peanuts to control your reaction and help relieve discomfort. However, the only way to prevent an allergic reaction is to avoid peanuts and peanut proteins altogether. Despite your best efforts, you may still come into contact with peanuts and have a severe reaction. In this case, you may need an emergency injection of adrenaline (epinephrine) and a trip to the emergency room. If you’re at risk of having a severe reaction, you may need to carry injectable epinephrine with you at all times.

Lisa Odenwelder, of Seaford, has been free of chemotherapy for Breast Cancer since May 2019. She was diagnosed at the age of 36, after she persisted with informing her doctors of pain she felt in her left breast, which was initially brushed aside as normal age related changes. However, her PCP, Jessica Baxter, was the first to listen to her concerns. Lisa attributes her and her medical assistant, Diane, surgeons, Dr. Walker and Dr. Pellegrino, oncologists, Dr. ElAyass and Dr. Im, and the wonderful oncology nurses at PRMC to her survival.

Lisa and her husband, both pharmacists, share a son and daughter, who were 3 and 1 years old when she was diagnosed.

Choosing to undergo a double mastectomy, she would later learn through genetic testing and analysis of the cancer, that she was BRCA2+, and at an intermediate risk of cancer recurrence. Knowing her husband and children needed her, she made the ultimate decision to do everything possible to reduce that risk of the cancer returning, and underwent 16 rounds of chemotherapy, followed by reconstructive surgery and radical hysterectomy. Although fearful at times of what the future would hold, or how long she would exist on this Earth, Lisa persevered with the enormous support of her husband, family, friends who are family, and the kindness and generosity of complete strangers. Throughout her battle with cancer, her greatest life lesson is to not ask G-d “Why?,” but to know that He will never leave your side throughout your journey.

In May of 2015, I was diagnosed with ovarian cancer. After 2 major surgeries, my oncologist exclaimed he would not work with me because I refused chemotherapy.

Instead, I found an Integrative doctor who said he will work with me as long as I did what I was told. And so came to be a change for the better in every aspect. I learned how far we have come from good nutrition and now more than ever was it time for me to find it again. That being said I “thought” I was eating healthy before.

But when you travel along a journey of this kind you learn a lot. My number one enemy was refined white sugar. Cancer grows and thrives on sugar. So now my label reading was absolutely mandatory and I was finding sugar in everything!

Long story short, after my surgeries and change in eating clean I still found myself in pain due to inflammation. That’s when I found a Program that completely gave me my quality of life back and it is now my joy to offer it to you the reader. My business is Optimal Health. Everyone has their own level and it would be my pleasure to guide and give you structure in finding your own. This program is easy, costneutral, and works! Jocelyn, a local, who was born and raised in Salisbury, Maryland. Previously living in Elkridge, MD, she moved back to the Eastern Shore the winter of 2017. Prior to her leaving, she was an avid volunteer for Women Supporting Women (WSW) for several years. She had the opportunity then to see how WSW ran their local nonprofit and to meet many of the local breast cancer survivors. Upon returning to the shore an opportunity presented itself in taking on the roll as the Mentoring Coordinator for WSW. In furthering her career, Jocelyn registered at Wor-Wic Community College to finish an associate degree in Medical Office Management.

This degree along with nine years of medical experience and knowledge in the gynecology and dermatology fields, made this position a perfect fit. She is eager to help, educate, and support those who have been diagnosed with breast cancer on the eastern shore.

Mike Baker is the founder of C.M.Baker Photography with over 25 years experience. His photography ventures have been quite diverse.

Throughout the year you may find him shooting a wedding, a Quinceañera, in the pits of a NASCAR race at Dover Downs and now at front stage at the Firefly Festival or flying over doing aerials. Giving back to the community has always been an important part of it all. Mike was awarded “Volunteer of the Year” from CDCC (Central De Chamber of Comm.) in 2005, ”Volunteer of the Year” from Kent County Tourism 2010,”Chairman’s Outstanding Partnership Award from CDCC in 2010 along with “Business Person of the Year” from CDCC in 2008. Since 1997, Mike has been the “official” photographer for the KC Heart Ball and for the “Go Red” in Kent County since it’s onset. Treatment options for hearing loss, such as hearing aids, have shown to benefit overall health. Conditions that have shown to have a link to hearing loss include (but are not limited to):

Diabetes The National Institutes of Health News showed in a 2008 study that hearing loss is two times more common in adults with Type 2 diabetes compared to those without diabetes. The nerves and blood vessels of the inner ear can become damaged due to pathological changes that are commonly seen in diabetes patients, resulting in hearing loss.

Cardiovascular Poor cardiovascular health can cause decreased blood flow to the inner ear which, ultimately, can result in hearing loss particularly in the low frequency range. Therefore, if a hearing loss is determined in the low frequency range (think, the bass on a piano), this could be an early predictor for a potential cardiovascular event.

Falls, particularly in the elderly, can be attributed to a variety of factors – one, less commonly discussed, is untreated hearing loss. A study completed by US News found that people with a mild, 25 decibel hearing loss, are 3 times more likely to sustain a fall and for every additional 10 decibels of hearing loss, the risk increased by 1.4 times for falling. Dr. Frank Lin indicates that “people who can’t hear well might not have good awareness of their overall environment, increasing the potential to trip and fall.”

Cognitive Decline Many research studies have shown that hearing loss may be linked to the onset of dementia or Alzheimer’s disease. People who have untreated hearing loss increase the amount of effort needed to understand speech which contends with normal cognition and could result in withdraw from social situations. By withdrawing from social situations, the brain is no longer exposed to typical daily communication which can decrease the amount of grey matter in the brain.

When a person’s ability to communicate effectively decreases with hearing loss, the likelihood of withdrawing from social interaction increases. By slowly withdrawing from social events a person once loved to participate in, the more isolated they become. Eliminating social activities is a red flag for depression. The National Council on Aging found that 36% of patients who begin to wear hearing aids experienced improved overall mental health while 34% increased social engagement.

Understanding the link between hearing loss and other medical conditions is important to a person’s overall health.

It is important for patients to have open communication with their physicians if they suspect hearing loss so that appropriate recommendations and referrals can be made to an audiologist for testing and treatment options.

Cancer Treatments Some chemotherapy and radiation treatments can cause ototoxicity which means the drugs can cause chemical damage to the inner ear or tiny hair cells in the cochlea.

The damage can be temporary or permanent depending on the type of medication used. Additionally, ototoxic medications can also cause tinnitus (ringing in the ears) with or without the presence of hearing loss.

Veterinary Services Peninsula Veterinary Services is a full-service, AAHA-accredited hospital located in Millsboro, Delaware. Owner Dr. April Reid moved from Brooklyn, New York in 2015 to start a practice in this area after spending summers in Rehoboth as a child. Dr. Reid is a graduate of Ross University School of Veterinary Medicine. She previously owned a thoroughbred racetrack practice for 14 years before making a change to small animal practice. Dr. Reid works closely with several local rescue organizations and is currently serving as the president-elect of the Delaware Veterinary Medical Association. In addition, she is working towards board certification in veterinary practice. In her spare time, Dr. Reid enjoys spending time with her two Great Danes, Ambrose and Amelia, and her three cats.

DIABETES MELLITUS IN PETS Approximately 10% of the human population in the US are affected with diabetes. Many pet owners do not realize that this disease can also affect their cats and dogs. In dogs, about one in every 300 dogs will be diagnosed in their lifetime and in cats the number is slightly higher with one in 280 becoming diabetic.

Being “cats and dogs”, of course their type of diabetes are different from each other but their symptoms are the same.

Pets usually present for drinking and urinating more often and in larger amounts than usual. Weight loss despite a ravenous appetite is also a presenting complaint. If your pet develops any of these symptoms it is important to see your veterinarian as quickly as possible so that your pet can be diagnosed quickly and treatment can begin.

Insulin is produced by the pancreatic beta cells. Diabetes occurs from one of two situations-either the cells do not produce enough insulin, or the animal’s body has a systemic resistance to insulin. In either case, there will be elevated blood glucose, which can interfere with the body’s absorption of nutrients.

The main causes of diabetes in pets are obesity, chronic pancreatitis, Cushing’s disease, autoimmune disease and certain breeds (Dachshunds, Yorkshire Terriers, Maine Coon and Siamese cats are examples) are also prone to developing it in their lifetimes.

If your pet is diagnosed with diabetes, your veterinarian will have

you administer insulin at home, along with diet change, exercise and monitoring the blood glucose level. It is important to follow the veterinarian’s instructions, as untreated diabetes can lead to diabetic ketoacidosis, cataracts, and kidney disease.

DIABETES IN CATS It is possible in some cases for cats to go into diabetic remission and no longer require insulin if owners are diligent about food and insulin administration in the first 60-90 days of treatments. The rate of cats being able to stop insulin administration varies from 15-60% in various studies. Your cat will be placed on a special veterinary diet, begin twice-daily insulin administration and the owner will be asked to monitor the cat’s glucose readings throughout the day. This is easier for owners now that there are continuous monitoring devices, which attach to the animal and are read by a scanner or phone.

There are applications on phones, which you will be asked to use to keep records of readings as well as food and water intake. Cats are therefore more like humans with type 2 diabetes who are sometimes able to discontinue insulin with changes in diet and exercise.

DIABETES IN DOGS In contrast, diabetic dogs are more like humans with type 1 diabetes and will be on insulin for the rest of their lives to maintain control of the disease. Diet, exercise and monitoring are still the mainstays of treatment.

carbohydrate, high fiber diet. Also, diabetics are predisposed to urinary tract infections because the body “dumps” the sugar that cannot be processed and that attracts bacteria.

Another effect that diabetes can have is the development of cataracts. One study showed that 80% of dogs go blind by year 3 of treatment.

Having their pet diagnosed with diabetes is often difficult for owners to handle. Your veterinarian team should go over everything with you

and anyone else in your home that will be helping to care for the pet.

There are different insulin products and syringes and the team should teach you how to handle the insulin your pet is placed on as well as how to administer the insulin. Food choices will be explained and things such as snacks will be addressed.

After the initial overwhelm, you will soon be comfortable with everything and your pet’s clinical signs will stop. Pets with treated diabetes can live normal healthy lives.

A beachy boutique specializing in swim and resort wear for women of all shapes and sizes

Owner Donna has always had a passion for swimwear. Studying fashion merchandise at University of Delaware and being a local resident at the beach in southern Delaware, she started making custom swimwear back in 1991. For the next 19 years she would fit women of all shapes and sizes into custom made and branded swim which in return grew an amazing cliental. Her daughter Devon became her shadow and grew up sitting on her cutting table watching her every move. Devon learned sewing by making scrunchies to match each of the custom swimsuits-whoa! Scrunchies! Flashback! Then she began doing fittings on women and that passion she witnessed in her mother became her own.

Like mother, like daughter Devon went on to study fashion merchandise at University of Delaware. In 2006, Donna got an opportunity to open her own swimwear boutique in Rehoboth Beach,DE. So with her daughter by her side and the support of her amazing family, the Pineapple Princess was born! Now, you might ask why the name Pineapple Princess?.... Right before officially opening the boutique, the family suffered some tremendous losses. Two of Donnas sister in laws (Devons aunts) passed away. One loved Hawaii and the other had a favorite song. By Heather A. Long, Esquire Whether you’ve been injured at work or in a car accident, chances are that the injury has had a significant impact on your life. When that happens, we become emotionally invested in both the healing process as well as the justice process.

If you have the option available, Mediation can streamline many issues in your case, including settlement.

What is Mediation?

Mediation is a type of settlement conference. Prior to the conference, both parties select a neutral person (or persons) to act as the mediator. The issues that the parties aim to resolve at the mediation are identified, and each party sends paperwork to the mediator and explains their point of view. One of the parties begins the negotiations process prior to the start of mediation. After the initial offer or demand is made, the mediator handles all further negotiations between the parties, by relaying offers and demands back and forth.

Do I need an attorney?

No, there is no requirement to have an attorney represent you in mediation; however, a skilled injury attorney will be able to provide invaluable advice as to the value of your claim, the applicability of the law to your particular facts, and will help guide you through the process. Most injury attorneys offer free consults and work on a contingency fee, meaning they get paid out of the settlement funds. This puts them on the same side of the bargaining table as you- the more they can get for you, the more they earn for themselves. A consultation with a local injury attorney can help you understand the strengths and weaknesses of your case, the value of your injuries, and can pave the way to mediation. In an injury case, the mediator sits down (either in-person or virtually) with the injured party and their attorney to discuss negotiations. There is typically a confidentiality agreement that is signed, and the opposing party is not in the same room and not on the same conference call. This way, the injured person can talk freely and openly without fear that the mediator will relay anything that is said.

After the mediator conferences with the injured party, they then meet privately with the opposing party to relay the opening demands. The opposing party then has the opportunity to have the same confidential conversations with the mediator, and relays the offer to the injured party by way of the mediator. The mediator then returns to the injured party to let them know what the offer is, and negotiations continue in this manner until hopefully the issue is resolved.

Why go to Mediation?

Mediation can be an informal, cost-effective tool to resolve many different kinds of claims. The costs associated with engaging in mediation are typically a fraction of the costs associated with going to trial or other types of hearings.

Even better, many times mediations can be conducted within an hour or two, as opposed to sitting in trial for several days or weeks. Many injured parties and seasoned attorneys prefer this expeditious and (relatively) inexpensive process over the rigors and uncertainties of a jury trial.

What happens after Mediation?

After mediation, the parties and their attorneys will execute a settlement agreement, and final settlement documents will be exchanged. The settlement agreement is usually signed prior to leaving the mediation conference, and then final settlement paperwork is typically sent and executed within the next 7-10 days. Once the final settlement documentation is returned and approved, the settlement funds are usually available within 30 days.

You pay a level premium each year which is applied to the cost of insurance and to cash value.

This asset called whole life insurance with it’s “pool of money” is available for a life event (if necessary) and gives

you permission to spend down your retirement assets while still leaving a financial legacy to your family.

Why a Long Term Care Rider? It’s the most cost effective way of having Long Term Care (LTC) coverage available if and when you need it.

Long term care costs can financially ruin you and your family

or worse...you can’t afford the care.

The cost of a pure LTC policy is high.

Insurance companies can’t guarantee level premiums each year because they are paying claims at a high rate.

So, even if you get a pure LTC policy, you don’t know how much or how often the premiums will increase, and if you can’t afford the increase, they can decrease your benefits.

Monthly long term care costs can be in excess of $5,000 a month.

These costs vary from state to state.

A pure LTC policy is a “use it or lose it” policy meaning that you can only use it if you have a claim. As a rider on a whole life policy, the premium is much less and you have a permanent insurance policy with the ability to have LTC coverage.

Millions of shoppers are bypassing brick-and-mortar stores, as well as their respective websites, in favor of purchasing those gifts or personal items from individual online sellers. For many people, sites like Craigslist and eBay offer a chance to save on everyday items and luxury products, both new and used—all from the comfort of their own homes.

Yet, while online shopping may be a convenient way to find deals and oneof-a-kind items, it’s important to protect your identity and financial information, particularly when dealing with individual sellers. Before you purchase anything listed on an online classified ad, auction, or marketplace site, keep the following precautions in mind.

Online classifieds On classified sites run by newspapers and popular venues like Craigslist, sellers tend to offer used items and may be open to negotiation. Since most transactions are done in cash, there’s little buyer protection.

To stay safe: • Never wire funds. If a seller asks you to wire payment using Western Union or MoneyGram, you’re likely dealing with a scammer.

• Safeguard your personal information. Sellers on Craigslist and similar sites don’t need your personal financial information, such as credit card numbers. To keep your information safe, it’s best to pay with cash.

• Don’t go it alone. Always take someone with you when meeting a seller. Be sure to tell a friend or family member where you’ll be and take

your cell phone with you.

• Pick up in a public place. Choose a busy location to meet the seller.

If you’re picking up the item at the seller’s house, it’s particularly important to have a friend or family member join you.

Flowers grown in a garden or in containers are nature’s hummingbird feeder. The right combination of wildflowers and exotic fl owers appropriate for your growing zone will attract hummingbirds, increase the number of small insects that hummingbirds require for their survival, and will bring beauty and interest to your garden.

The ideal hummingbird flower contains nectar at the base of a single tubular flower; this shape and lack of fragrance discourages insect use.

Hummingbirds need to feed every 15 minutes and may visit as many as 1,000 fl owers each day, so flowers need to be numerous and constantly regenerating. Red flowers are popular, but flowers of other colors also contain nectar and attract hummingbirds. Seek flowers with a long bloom period and make sure that a great hummingbird flower is in bloom at every point during your hummingbird season, especially during the spring and fall migration periods. Additionally, it is important to provide “cover” in the form of mature trees and thick shrubbery.

Type: The best feeder is one you are able and

willing to clean. If the feeder ports are above the pool of liquid there is less tendency to drip, a messy problem that attracts ants.

Size: Use a feeder whose size matches your

population. Don’t fi ll it upif all the nectar isn’t being used. Several small feeders are better thanone large feeder and will support more hummers, helping deal with territoriality.

Recipe: Dissolve 1 cup table sugar (cane

preferred, beet is OK) in 4 cups water. Do not use any other sugar— not turbinado, brown sugar, etc.—and never use honey or artifi cial sweeteners. Spring water is best, but most tap water is OK; don’t use distilled water.
