R. Alberto Rosa, M.D., FACC Dr. Ramon Alberto Rosa is a graduate of the University of Santo Domingo in his native Dominican Republic. He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), Chief Medical Resident (19941995), and cardiology fellowship (1995-1998).

Dr. Rosa has practiced cardiology in Sussex County since July 1998. He has served as Medical Director of the Cardiac Catheterization Laboratory and as Chief of the Department of Cardiology at Beebe Medical Center in Lewes, DE. Dr. Rosa is trained in non-invasive cardiology, Nuclear Transesopageal Echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Dr. Rosa is board certified by the American Boards of Internal Medicine and Cardiology. He is also a Certified Aviation Medical Examiner and is Board Certified in Nuclear Cardiology.

Coronary Calcium Score: What is it and how it can help me stay heart healthier?

The presence of calcium in the coronary arterial wall is a reliable marker for obstructive coronary artery disease. The coronary artery calcification (CAC) score can be calculated based on CT scan imaging.

This is a relatively simple to do, at a reasonably low cost and low risk of radiation exposure. The score generated, also called Agatston score, has important predictive value in regards to the probability of major coronary events, such as heart attacks in the near to intermediate future.

It has been established that a CAC score of 0 is the ideal result for the test. It means that the probability of having significant coronary artery disease is less than 1 %. This confers a favorable outlook in terms of the low likelihood of coronary events such as heart attacks or death.

A score of 1-99 indicates mild degree of coronary disease, 100-399- moderate disease and greater than 400 is compatible with severe disease. This score is mostly indicative of the

extend of coronary artery disease and less of the severity.

According to the American College of Cardiology and the American Heart Association recommendations, the CAC is more reliable in individuals considered to be at intermediate risk for coronary artery disease as calculated by the Framingham risk score which is based on factors such as age, cholesterol levels and elevated blood pressure. A calculated risk of 10-20 % for cardiac events or mortality at 10 years is the recommended patient in whom to obtain a CAC score for further risk stratification and preventive treatment.

The coronary calcium score works best for individuals in their late 40’s to mid 60 ‘s with no previous cardiovascular history and without symptoms.

A moderate to high score (over 100) would indicate a higher probability of heart attacks, stroke or even cardiac death. This will also

lead to making stronger recommendations for more aggressive risk factors modification and reduction by your healthcare provider. In addition, further screening for “silent” coronary blockages might be warranted such as stress testing for those individuals in the higher score categories.

The CAC score is a valuable tool for the right patient in helping determine the level of risk

for coronary disease and possibly heart attacks or even death and should be used in association with other traditional and more established risk factors such as age, high cholesterol, hypertension, sedentary life style, smoking and overweight. Diabetes mellitus is considered an equivalent to coronary artery disease and the CAC is less reliable in those patients.

with your healthcare provider the use of this tool, if you are in the right age group, have no symptoms and you are at an intermediate risk for coronary events as calculated by the Framingham risk calculator (10-20 % chance in 10 years). Please look this up!

WOMEN AND SPRINGTIME FOOT AND ANKLE INJURIES Spring is approaching, and lots of women from the young to the growing senior population will once again venture outdoors to enjoy all that Sussex County has to offer. They will start to use the bike and hiking trails, golf courses, pickleball, and tennis courts, or jog and walk on the boardwalks and enjoy the spring air. For some, it is a personal affirmation to live a healthier lifestyle. For others, it may be a way to recondition after joint replacement, cardiac surgery, surviving cancer, or surgery on your feet. Despite the reasoning, increasing your activity requires some care of your feet and ankles to avoid common injuries, especially with women, because historically, women’s shoes fit tighter in the toes and heel and have caused issues. This history of improper shoe gear, genetics, and increase activity can change the foot anatomy. These factors, in combination with aging, can reduce muscle strength, reflexes, agility, speed, and energy. Consequently, proper physical modifications, conditioning, and shoe gear become critical in reducing the chance of injury.

Here are some helpful tips to avoid foot and ankle exertion or injury: Stretch • The muscles or joints that need to stretch are the ankles, Achilles tendon, calf muscles, quadriceps, and hamstrings.

Proper Footwear • Invest in a good pair of athletic shoes!

• The key to finding the right sneaker is the sole. Each sport may require a different type of sneaker, but it must always provide stability.

• The sneaker should be comfortable as soon as you try them on. There is no break-in period.

• Have enough room in the toe box.

• Dr. Wendolowski can help pick out the proper shoe for each activity.

With activity and all of your good intentions, you may still encounter these typical foot and ankle injuries: Ankle sprain/strain • Typically occurs with a sudden twist of the ankle. There may or may not be immediate swelling.

Achilles tendonitis • (Pain in the back of the ankle) most commonly caused by stop and go action.

Plantar fasciitis • Pain at the bottom of the heel most common when stepping down in the morning and after rest and may gradually subside but never truly goes away.

Reasons can range from flat feet, high arches, unsupportive shoes, weak muscles, and direct injury.

Neuromas • Inflammation of the nerve in the front of the foot due to excessive pressure.

Stress fractures • Caused by repetitive trauma to a bone, typically the metatarsals or long bones in the foot.

Injured toenails (bruised/ingrown/fall off) • Caused by repetitive trauma to the tips of the toes.

Learn how the health of your mouth, teeth and gums can affect your general health Did you know that your oral health offers clues about your overall health — or that problems in your mouth can affect the rest of your body? Protect yourself by learning more about the connection between your oral health and overall health.

What’s the connection between oral health and overall health?

Like other areas of the body, your mouth teems with bacteria — mostly harmless. But your mouth is the entry point to your digestive and respiratory tracts, and some of these bacteria can cause disease.

Normally the body’s natural defenses and good oral health care, such as daily brushing and flossing, keep bacteria under control. However, without proper oral hygiene, bacteria can reach levels that might lead to oral infections, such as tooth decay and gum disease.

Also, certain medications — such as decongestants, antihistamines, painkillers, diuretics and antidepressants — can reduce saliva flow. Saliva washes away food and neutralizes acids produced by bacteria in the mouth, helping to protect you from microbes that multiply and lead to disease.

Studies suggest that oral bacteria and the inflammation associated with a severe form of gum disease (periodontitis) might play a role in some diseases. And certain diseases, such as diabetes and HIV/AIDS, can lower the body’s resistance to infection, making oral health problems more severe.

Pneumonia. Certain bacteria in your mouth can be pulled into your lungs, causing pneumonia and other respiratory diseases.

Diabetes. By reducing the body’s resistance to infection, diabetes puts your gums at risk. Gum disease appears to be more frequent and severe among people who have diabetes.

Research shows that people who have gum disease have a harder time controlling their blood sugar levels.

Regular periodontal care can improve diabetes control.

HIV/AIDS. Oral problems, such as painful mucosal lesions, are common in people who have HIV/AIDS. Osteoporosis. This bone-weakening disease is linked with periodontal bone loss and tooth loss. Certain drugs used to treat osteoporosis carry a small risk of damage to the bones of the jaw.

Alzheimer’s disease. Worsening oral health is seen as Alzheimer’s disease progresses.

How can I protect my oral health?

To protect your oral health, practice good oral hygiene daily.

• Brush your teeth at least twice a day with a softbristled brush using fluoride toothpaste.

• Floss daily.

• Use mouthwash to remove food particles left after brushing and flossing.

• Eat a healthy diet and limit food with added sugars.

• Replace your toothbrush every three months or sooner if bristles are splayed or worn.

• Schedule regular dental checkups and cleanings.

• Avoid tobacco use.

Your oral health might contribute to various diseases and conditions, including: Endocarditis. This infection of the inner lining of your heart chambers or valves (endocardium) typically occurs when bacteria or other germs from another part of your body, such as your mouth, spread through your bloodstream and attach to certain areas in your heart.

Cardiovascular disease. Although the connection is not fully understood, some research suggests that heart disease, clogged arteries and stroke might be linked to the inflammation and infections that oral bacteria can cause.

Pregnancy and birth complications. Periodontitis has been linked to premature birth and low birth weight.

Caring for the memory impaired… Helpful Hints Taking care of someone with Alzheimer’s or dementia can be very difficult. Here at Always Best Care, we strive to educate people in how to better take care of loved ones and yourself when dealing with this disease.

We understand that it may make the caregiver feel overwhelmed and isolated.

We want you to know that these are very normal feelings. Here are some tips to help you better understand the disease and how to manage it as best you can.

Typical Behaviors • It is important for people with memory impairment to maintain as much independence as possible.

This will help their overall wellbeing and attitude.

• Expect tasks to take longer. Short-term memory is greatly affected in this disease. It is normal for them to forget what they were doing in the middle of performing a task. Just be sure to redirect them back to the task at hand.

• Aggression and agitation are a normal reaction with these diseases. They may curse, scream, insult and become violent. You need to know this is not your fault and you aren’t doing anything wrong. They aren’t doing it on purpose.

Remain calm, don’t argue and try and redirect them.

Tips • Establishing a routine and sticking to it can reduce frustrations. It also helps them feel independent and in control, so you can experience less agitation.

For example, showering upon waking up and then going to eat breakfast. If you try and switch up the routine, it may confuse them and result in negative

attitude changes.

• Alzheimer’s makes it hard to focus. It is important to reduce distractions. For example, if you are sitting down to eat dinner, make sure you turn off the tv or radio so that they may focus on just eating.

• Stay away from open ended questions. Always try and ask a question that would require a yes or no answer. If you are unable to do so, ask a question giving two choices. For example, “would you like scrambled eggs or oatmeal for breakfast?”

• Limit background noise.

Loud, abrupt noises can cause agitation. Soft tones and a quiet environment are best to keep them relaxed.

• One step instructions.

When assigning a task for them to complete, try using one step instructions.

For example, don’t say “Get dressed.” Instead, instruct them step by step.

“Put your socks on.” And when they have completed that task, instruct the next step. “Put your shoes on.”

Creating a Better Tomorrow with Trauma-Informed Care Overcoming the effects of trauma in Delaware Ann Wayne

Trauma is pervasive throughout our state and country. Nearly 61% of all U.S. adults have lived through at least one adverse childhood experience (Trauma-Informed Care Implementation Resource Center, 2020), and 48% of children in Delaware are being exposed to one or more right now (KIDS COUNT, 2015). Adverse Childhood Experiences, or ACEs, are defined as stressful or traumatic experiences that disrupt the safe, nurturing environment that children need to thrive. They are categorized as chronic sources of stress including abuse, addiction or mental illness in the home and domestic violence.

The ripple effects of this level of trauma are incalculable. Unmitigated, they can have negative impacts spanning an individual’s entire lifetime. That’s where Governor John Carney has stepped in to make Delaware a trauma-informed state. A decision that has the power to immensely improve the daily lives of people impacted by trauma in our community.

Approaching individuals from a position of empathy is the first step in breaking the long-lasting effects of trauma. Our women-led staff at SUN Delaware sees

the ramifications of trauma every day, which is why we ensure traumainformed care is at the core of everything we do.

WHAT DOES TRAUMAINFORMED CARE MEAN? Repeated exposure to trauma can impact brain development in children and rewire their brains’ responses to stress. Later in life, many have difficulties with issues related to emotional regulation (depression, anxiety, chronic diseases and even suicide) because of those early experiences.

Hospice Equals “Hope” for Parents of Medically Complex Child And ironically, it wasn’t until they met their hospice team that they felt they were allowed to hope.

Patty remembers first meeting Nurse Colleen, who gave Patty her “first genuine smile”— not one of sympathy, but of hope. Patty explains it was as if Colleen were saying, “These people have hope for this baby’s life.

No matter how short. It doesn’t matter. Hope in the time that’s given.”

Hospice can seem like a frightening word to a family with a medically complex child. But not for Patty and Zach. To them, “hospice” equals “hope”—hope for the freedom to embrace every joy-filled moment with their baby boy.

When Patty and Zach brought four-day-old Leo home, they were greeted by balloons at the door, two excited big brothers, adoring grandparents… and their Katybug team from Delaware Hospice. It was a joyous occasion; Patty and Zach had been told not to even bother bringing a car seat to the hospital.

Thirteen weeks into her pregnancy, Patty had gotten the phone call with the frightening news: her baby had Trisomy 18— a genetic condition with a high rate of miscarriage, stillbirth, and fatality before the age of one. They were encouraged to terminate the pregnancy.

The family was faced with one difficult decision after another. Ultimately, they decided to enjoy and love Leo for as long as he was able to live. They determined that if the baby was born alive, they wanted him to have “comfort care” rather than significant medical intervention.

As baby Leo defied one odd after another in his first few days of life, the family focused on enjoying each tiny moment and milestone. But at every turn, they were reminded of how serious Leo’s condition was and not to let their hopes get too high.

When it became clear there was a possibility Leo would leave the hospital, the hospital’s palliative team set the family up with Delaware Hospice’s Katybug pediatric program to support them in their home.

A year later, the family’s Katybug Nurse Lynn was at Leo’s first birthday party. Lynn had become part of the family, and Leo was thriving.

Only about 10% of babies born with his condition make it to their first birthday. He was the family’s little miracle.

Once Lynn has helped the family through one more cold and flu season, Leo will “graduate” from Katybug. Even though the step should be a happy milestone, Zach wishes they could keep Katybug forever. Getting in-home services and having someone to call at a moment’s notice—even in the

middle of the night—has been an enormous help to the family, especially since the children’s hospital where Leo is seen is a 2-hour drive.

Zach explains, “If we can use the hospice resources it’s amazing… We don’t want to graduate!”

Children at any stage of any serious illness, whether they’re receiving curative treatment or not, can benefit from the expertise of pediatric palliative and hospice care specialists like the caring team members in Delaware Hospice’s Katybug pediatric program. In the industrialized world, a growing number of individuals are affected by allergies. Currently, the numbers are between 10-30 percent of the total population and are increasing steadily. For Americans who suffer from nasal and seasonal allergies, the costs very quickly add up. It is currently estimated that between doctor visits, prescriptions, and time missed from work, these allergies cost in excess of 3 billion each year. For allergy sufferers, the welcome break from winter is not entirely pleasant.

Seasonal or not, all allergies are the result of an exaggerated response by the immune system to substances that it recognizes as foreign. In allergic persons, the immune system produces a specific antibody known as immunoglobulin E (IgE) in response to pollen, pet dander, foods, dust, and other matters which then produces the symptoms of an allergic reaction. Of course, there are medications to help control the allergic response and provide relief from symptoms. There are even medications that will be lifesaving in the event of severe allergic reactions. However, in addition to treating symptoms, there are other steps which can be taken to boost the immune system and lessen allergic reactions.

HOW CAN A CHIROPRACTOR HELP WITH ALLERGIES? The strength of both the immune and respiratory systems depends largely on proper communication between the brain and spinal cord to control and coordinate their functions properly. When there are interruptions in this communication such as misalignments of the neck, immunity may be compromised.

Gottstein grew up in Northeast Pennsylvania, where he earned a bachelor’s degree in chemistry and a doctor of physical therapy degree from Misericordia University. Ending Dorethea’s years of pain Dorethea Parker Forrester had dealt with chronic back pain for years, after being hit from behind in a 2016 car accident.

Basic tasks such as taking a shower or standing at the stove to cook a meal caused excruciating pain in the 57-year-old’s back and down her legs.

She had spondylolisthesis, commonly known as a slip, where one of the bones in the spine, slips forward or backward.

After various treatment options, including a previous back surgery, she continued to battle pain. “Thinking I’d always be in this much pain was scary and depressing.

I didn’t want to be on pain medication — I just wanted to be fixed,” she said.

I have smoker’s lines but I don’t smoke!

Are you frustrated about having “smoker’s” lines around your lips and you’ve never smoked?

You are not alone! This is a common complaint of a lot of people, having those pesky vertical creases around your lips that appear when we pucker, use a straw or just speak! And with time and continued motion of our mouths, these creases can become permanently etched in our skin.

You don’t have to smoke to have smokers lip lines Of course smoking makes these creases worse, but it is not the only cause of “smoker’s” lip lines. The cause of these lip lines is due to thinning of the skin around our mouths and losing the strength of the natural white roll of the lip border, where the lip transitions from the pink color of the lip to the normal facial skin. It is this natural white roll that forms the transition or border between the actual lip and the skin surrounding the lip, where lip liner would be drawn.

This white roll border is what usually prevents applied lipstick from bleeding or smudging away from the lip itself onto the surround skin. If you were to observe a child’s lip, this natural white roll of the lip border is typically present and can be felt as a soft firmness that defines the border of the lip.

Lip lines and creases are also caused by the daily use of the mouth in everyday speaking. The facial expressions with using everyday speech causes the mouth to crease in the same areas repeatedly and over time, the creases become permanent.

So what can we do to address this pesky concern of lip lines?

Thankfully, we have several options! The first problem issue I like to address when treating these lines, is to restore the strength of the natural white roll that has been lost. This is done by adding a hyaluronic acid filler directly into the white roll. By rebuilding and strengthening this natural border between the lip and the surrounding facial skin, the lips cannot collapse as easily like an accordion and form lip lines and creases, when we are speaking, eating or making other mouth motions. I use motivational interviewing techniques, a subjective unit of distress scale (SUD) and the stages of change model to assess her desire to change.

During subsequent sessions, I teach her how to recognize and challenge her two dominant ego states (inner critic/out of control emotions). I then show her how if left unchecked, these two ego states like a recording, play over and over again throughout her life yielding the

Addressing The Fires Of Inflammation Functional Medicine Inflammation is the key in the rising epidemic of autoimmune disease, depression, and asthma.

I

nflammation is our body’s innate mechanism to defend itself against negative stimuli such as pathogens, trauma, environmental toxins, and damaged cells. It is a physical response to emotional stress as well. The Latin terminology handed down from Celsus described the key features as: rubor (redness), calor (heat), tumor (swelling), and dolor (pain). All of these, except pain, describe the role of the vasculature. There is increased blood flow, vasodilatation, and increased vascular permeability.

Inflammation can be acute such as with a laceration. Or it can become a systemic, chronic condition with a domino effect of chemical, hormonal, emotional and/or physical triggers. These could be found in foods, the environment, and even stress. When this inflammation continues, the immune system will start to attack healthy tissue resulting in illness and disease. Symptoms could be anything from gut issues, headaches, joint pain, to fatigue and weight gain.

In joints it causes arthritis, in the inner blood vessels, heart disease and stroke. For the thyroid, inflammation can cause Hashimoto’s Thyroiditis. As the immune system becomes weakened, cancer cells can become undetected and grow.

Testing would involve hsCRP (C-reactive protein) which is produced in the liver in response to infections and inflammation. IL-6 (interleukin 6) and ESR are other indicators of inflammation. Toxic metal testing, comprehensive stool analysis and organic acid testing will also detect systemic inflammation. Genetic testing can determine potential links to systemic inflammation. Even if you have normal CRP and then only sleep 4-5 hours a night for a few weeks, your level will elevate. If you are diabetic, due to the glycating effect of blood sugar, you will experience the effects of systemic inflammation.

Seniors need transportation in Southern Delaware Just think about all the places you drive to regularly… to work, doctor’s appointments, the bank, grocery store or to meet friends for coffee, to church, family events, maybe even to a gallery opening or a movie. If you turned in your car keys today, wouldn’t you still want to be able to do all these things and more? Could you count on family or friends to drive you, or would you feel like you were imposing?

While everyone ages differently, many adults between the ages of 65 and 72 begin to limit their driving or stop altogether due to age related conditions such as low vision, slowed response time, or an extended or severe illness. While public transportation companies work to expand and improve services for this “boom”ing population, many older adults still have limited or no access

Joseph Savona, Volunteer Driver When Joe arrived to pick up John and his wife, John had just taken a spill in the grocery store parking lot when his cart hit a crack in the pavement. Joe immediately made sure John was safe, called 9-1-1, loaded the groceries in the car and drove John’s wife behind the ambulance to the hospital. He went into the hospital waiting area and helped John’s wife call her son who lived about an hour away to let him know what had happened.

Once Joe knew their son

was on the way, he took the groceries back to the couple’s home and put the cold items away. If that wasn’t enough, he then drove back to the hospital and stayed until their son arrived.

When Joe received recognition for going above and beyond, he said, “I just did what anybody else would do!”

Joseph Feichtl, Volunteer Driver and ITN Board Member Joe was recently recognized as ITN Southern Delaware’s 2019 Volunteer of the Year and went on to receive ITN America’s 2020 national award because of his tireless efforts, extraordinary customer relations, and continued dedication to ITN’s mission. He is genuinely interested in each individual he drives, keeping in touch with many and helping with errands like picking up

mail, dropping off glasses to be repaired, or fixing a door knob. Riders love his friendly interaction and caring nature and they often call the office to sing his praises and request him as their driver.

In 4½ years, Joe has provided nearly 2000 rides, led new driver orientations, and served hundreds of riders; having had an enormous effect on the lives of each one. He voluntarily looks after the ITN fleet cars, often paying for regular maintenance out of pocket and declining reimbursement.

Joe also donates many hundreds of his own mileage credits back to ITN each year to help members who have difficulty paying for rides.

Joe says he feels that service to others is a blessing to himself. But rest assured, Joe is the real blessing!

Lisa Attebery, DO, FACOS Breast Surgical Oncologist Board Certified General Surgeon As the only fellowship trained breast surgeon in Delaware and a breast surgical oncologist, I am always asked, “What should I do if I find a lump? How is cancer diagnosed? What is new in the treatment of breast cancer? What kind of surgery will I need?”

The diagnosis and treatment of breast cancer is always evolving.

There are many new advances, and more are coming. It is a very exciting time in the field.

STATISTICS Breast cancer is the second leading cause of death in women. In 2019, there will be an estimated 3.5 million breast cancer survivors. 268,600 women will be diagnosed with breast cancer and 2930 men will be diagnosed. Of these, 42,260 women and 500 men will die from the disease. Here in Delaware, breast cancer ranks as the leading cancer diagnosis in women. Thankfully, the cancer death rates have decreased with advances in diagnosis, treatment and education.

GENETICS & RISKS 60-80% of breast cancers are sporadic or random. 10-15% are familial, meaning some family history is present and only 5-10% involve genetic abnormalities. So, who should be tested? Anyone diagnosed with breast cancer that is less than 50 years of age, if you are diagnosed with bilateral breast cancer (in both breasts), if you are a male with breast cancer and anyone diagnosed who has an Ashkenazi Jewish background. Multiple risk factors have been identified, such as

being female, increasing age and race. Caucasians have the highest risk of developing breast cancer.

Next is African American, Asian and then Hispanic. Estrogen exposure whether from medicine, bio-identical hormones or foods (soy), obesity, excessive alcohol use, personal history of breast cancer and radiation exposure on the chest also increase your risk.

DETECTION Women should start having a 2D or 3D mammogram at the age of forty. If you have a family history, you should have a baseline mammogram at 35 years of age. An Ultrasound is a used for palpable masses and to aid in identifying lesions on a mammogram. An MRI is indicated for certain types of dense breasts, a new breast cancer diagnosis and to evaluate implants or lesions not well seen on mammograms. A breast MRI or Ultrasound are not generally used as screening tests.

BIOPSIES Biopsies are usually performed at the hospital but can sometimes be performed in the doctor’s office.

An ultrasound, mammogram or MRI can be used to assist obtaining the tissue. It depends on what test clearly identifies the lesion. The average time from abnormal mammogram to a biopsy should be 48-72 hours.

SURGERY There are two options for surgery.

One is breast conserving, called a lumpectomy. A lumpectomy is just what it sounds like.

The doctor removes a lump or the cancer from the breast while preserving the majority of the breast. The second is a mastectomy, which removes

anywhere between 92-98% of the breast tissue. A lumpectomy is most often followed by radiation which helps prevent the cancer from returning. In fact, a lumpectomy with radiation is EQUAL to a mastectomy in terms of survival. The real benefit for a mastectomy is risk reduction and personal choice. Most mastectomies can be completed with the preservation of the nipple and reconstruction can be started immediately.

NEW TECHNOLOGY Advances in the diagnosis and treatment of breast cancer are happening quickly. Ultrasound guided breast biopsies can be performed right in the office with a new tool called the Mammotome. This probe device can extract tissue samples that are then sent to the lab for testing. After the tissue is removed, a Magseed is placed at the site of the biopsy. The Magseed is a tiny, radiation-free magnetic seed that is detected by the Sentimag. The Sentimag then uses magnetic signals to accurately pinpoint the Magseed, showing the doctor where the lump or cancer is located. The Sentimag is used in the operating room when the patient is asleep.

The placement of the Magseed avoids the uncomfortable and painful placement of a wire in the breast prior to surgery while the patient is still awake and shortens the time needed at the hospital for the surgery. Prior to surgery, Magtrace is injected into the breast so the lymph nodes can be identified. Magtrace is a dark brown liquid of organically coated iron oxide particles.

It avoids the need for nuclear medicine and the subsequent unnecessary exposure to

radioactive material to the patient and staff. Dr. Attebery is the first surgeon on the East Coast to use this advanced technology.

YOUR CANCER DIAGNOSIS An important tool in diagnosing and treating breast cancer is doing a tumor genomic assay or test. These tests analyze a sample of the cancer tumor to see how active certain genes are. This activity affects the behavior of the cancer. The Oncotype DX test is used to estimate a person’s risk of

recurrence and/or the risk of cancer developing again in the same breast. Depending on what type of cancer it is, this test can also tell how likely a patient is to benefit from radiation or chemotherapy. Of the genomic tests used on breast cancer, the Oncotype DX test has the most thorough data supporting its use to make treatment decisions.

Because of this strong research, the Oncotype DX test is the most common test used in the United States to make treatment decisions.

Gum Regeneration No Scalpel & No Sutures

What is gum recession?

Gum recession refers to the loss of gum tissue along the gum line. This can occur as a result of periodontal disease (gingivitis, periodontitis, advanced periodontitis), the natural aging process, or abrasive habits when it comes to brushing the teeth.

Love (And Easterseals) Can Keep Us Together

By Natalie Scott Love is not just words said on your wedding day, but how one lives those words out each day. Love is shown in health, but especially in sickness.

Emory and Otisia “Tish” did not know 37 years ago when they said those words “in sickness and in health” that sickness would come sooner than they thought. Though caregiving for Tish, who had her first stroke at 45, has not come without trials, it is Emory’s great attitude, support from family and a team of caring professionals at an Easterseals Adult Day program, that has really made a huge impact in how they continue to live their lives.

“I don’t think I knew what love was until Tish got sick. I had no idea I could handle it. I have to help her with everything,” Emory says.

“Easterseals has been a lifesaver. The staff takes great care of her. I know it’s their job but they don’t treat it like a job.

You can see they really care. I am so overwhelmingly grateful. She is well taken care of and safe. I know all of her needs will be addressed.”

Easterseals adult day services enable Emory to continue to care for Tish at home. It gives him the break he needs to re-energize, take care of his needs, and run errands. Tish attends the program five days a week and enjoys painting, ball toss and parachute exercises. She also enjoys the camaraderie felt amongst the participants and staff in the program.

“As long as I can take care of her, I will take care of her here at home and thanks to Easterseals I can do that. Our life is not stress free but it is peaceful,” Emory says. “While she is at Easterseals I don’t have to be on guard. I can relax, take a nap to catch up on sleep, go shopping or make dinner. I know Easterseals is great for her and truly great for me.”

Easterseals Camp Fairlee was just opening its doors in 1954 for three weeks of adventure for children with disabilities. For many of the children who entered those doors it would be their first time away from home.

Recently, Don had the opportunity to visit the camp he had not seen since 1957, a camp he attributes for the great changes in his life. “Going to Camp Fairlee changed my life. For the first time, I felt equal. Everyone was in the same boat,” says Don, who experienced physical disabilities due to polio. “Camp changed my outlook on life. I got the confidence from Camp Fairlee that I could do anything. Because of Camp Fairlee I am the man I am today!”

In 1957 Don was named Easterseals ambassador.

“As we were driving in, it brought back a lot of memories,” Don says. “I had tears in my eyes when I saw the Manor House.”

These days, Camp Fairlee’s summer sessions are available for both children and adults of varying disabilities, including specialized sessions for those with autism. The summer sessions include a lot of activities such as swimming, arts and crafts, horseback riding, canoeing and so much more. When Don was a camper at Camp Fairlee, his favorite activity was fishing. In fact, in 1955 he

Easterseals is There When Caregiving Gets Tough

By Natalie Scott When Frank was diagnosed with Parkinson’s disease in his 50s, his wife, Evelyn, was overwhelmed. As Frank became more fragile, she needed more help than she ever anticipated. Thankfully, she found Easterseals Community Outreach Program and the many supports for caregivers.

Easterseals connected Evelyn with answers and resources to help her navigate the challenges of caregiving. Not only did Easterseals help her find support from other caregivers walking the same path, it also provided respite grants which have allowed her to take a break knowing Frank was in capable hands.

“Some days I am so busy taking care of Frank that I forget to even brush my teeth,” Evelyn says with a laugh. “Respite is like heaven. I love it because I can go away and not worry. I am happier when I am able to take a break and it is something I look forward to. I couldn’t survive without it.”

Caregiving can be an exhausting job, especially when the caregiver does not get time to recharge. Respite is an option through Easterseals Lifespan Respite Program, which provides funds to support family caregivers providing care to a person of any age or disability.

Using the respite grant to pay for Frank’s care, Evelyn was able to go away with her friends to Florida on a trip she dubbed “Grannies Gone Wild.” The group was able to enjoy the beach, the pool, staying in their pajamas all day and eating on their own schedule. She was also able to visit her sister, who she is not able to see often, in Myrtle Beach. During these trips, Evelyn is able to take a break from not only the mental exhaustion, but the physical demands of caregiving.

“I think it is important to see how others are dealing with the same thing as you are. You learn more resources from others,” Evelyn says. “It is great to know you are not alone.”

Also housed in Easterseals Resource Center is Assistive Technology, or “tools for independence.” There are hundreds of pieces of assistive technology on display, like pill reminders and smart home devices, which promote independence and quality of life. Alexandra is a member of the National Honor Society, a Duke TIP Scholar, received honorable mention two years in a row at Canterbury School’s Science Fair, Regional First Place at HOSA for Nutrition and a Science Olympian member with regional awards.

Alexandra aspires to be a surgeon specializing in female cancer research and helping children with Treacher Collins Syndrome, a condition that affects the development of bones and other tissues of the face.

Alexandra was diagnosed by Dr. Robert Wood, of the John Hopkins, at the age of 4 with a most severe peanut allergy after she went anaphylaxis after eating a peanut product.

Alexandra feels the best way to prevent a severe reaction is education on food labeling, and fellow student education on this deadly condition.

By Roni Caryn Rabin, Published in the The New Times Published Sept. 13, 2019 Updated Sept. 23, 2019 About 1.2 million American children have peanut allergies. A new drug, Palforzia, does not cure the condition, but it can blunt lifethreatening reactions.

A Food and Drug Administration panel on Friday recommended approval of the first-ever drug to treat life-threatening peanut allergies in children, a condition that confounds and frightens families across the country.

The recommendation all but assures the agency will approve the drug, called Palforzia and made by Aimmune Therapeutics. The F.D.A. typically follows the advice of its expert advisory committees.

Some panel members expressed reservations about the safety of the drug, since patients receiving treatment experienced more allergic reactions than those given a placebo. There is also little longterm safety data about a treatment that may be lifelong, the critics noted.

The drug’s goal is not to cure the allergy, but to reduce the risk that an accidental exposure to small amounts of peanut will set off a lifethreatening reaction. It might also relieve some of the fear and anxiety many families experience as they struggle to cope with a child’s severe peanut allergy.

“This is one of the most important unmet needs of medicine,” Dr. James R. Baker Jr., director of the Mary H. Weiser Food Allergy Center at University of Michigan, told the agency’s advisory committee. He was paid by Aimmune.

The demand for treatment among patients and their families is enormous, Dr. Baker noted. “Right now the only approved approach to this allergy is to avoid peanuts, and the amount of effort and cost involved in making sure everything your child is exposed to is peanutfree is overwhelming to most families,” he said, adding that even scrupulous efforts to prevent exposures fail, resulting in lifethreatening medical emergencies.

“The quality of life of patients and their caregivers is adversely affected due to fear and anxiety about accidental ingestions,” he added.

The regimen begins with trace amounts of the protein, which are carefully measured and increased incrementally under medical supervision as tolerance develops.

Allergists who treat children with peanut allergies applauded the decision, though they acknowledged the treatment regimen was demanding and not appropriate for everyone.

“Until now, patients with lifethreatening peanut allergies had no options but avoidance,” said Dr. Sandra Hong, an allergist at the Cleveland Clinic. Still, “it is an exciting step for patients and their families who live in constant worry about coming in contact with even the slightest amount of peanut.”

The new drug is an oral immunotherapy regimen that aims to reduce sensitivity to peanut allergens. It gradually exposes children to small amounts of peanut protein over the course of six months, until they can safely eat the equivalent of two peanuts.

The treatment is not always effective and it is accompanied by side effects, including severe allergic reactions to the peanut exposure.

One in five children who were treated stopped because of side effects; 14 percent experienced severe allergic reactions, double the percentage in a comparison group receiving a placebo.

About 1.2 million American children have a peanut allergy. The allergies are believed to cause more deaths from anaphylaxis — an acute physiological response that includes lowered blood pressure, shock and constriction of the airways — than any other food allergy, though the precise number is not known.

Because children generally do not outgrow peanut allergies, they must avoid peanuts, peanut oil and foods contaminated with traces of peanuts for their entire lives.

Tessa Grosso, 16, a high school junior from Menlo Park, Calif., told the panel she was allergic to peanuts, dairy and many other foods until she was 10, when she was treated with an early form of oral immunotherapy at Stanford University.

Now, she said, she eats just about anything and has peanut butter every morning before school.

“Before I was treated, my life was like navigating a minefield,” Ms. Grosso said in an interview. “I couldn’t go to anyone’s house, and a crumb of something on my hands could have killed me. At one point, a glass of milk spilled on my hand and

I went into anaphylactic shock and almost died.”

Now, she said, her diet is unremarkable. Though she still avoids shellfish and carries epinephrine to treat an unexpected reaction, “My life changed forever.”

She appealed to the committee to recommend approval of the drug.

“Every child deserves this therapy that provides freedom, safety and the ability to live without fear of food,” Ms. Grosso said.

But several experts at the hearing raised questions about the drug’s long-term safety and effectiveness.

Nina Zeldes, a senior fellow at the National Center for Health Research in Washington, pointed out that only one study had examined the drug’s effectiveness.

The participants were mostly white, she also noted, so little is known about how children of other backgrounds may respond to treatment. Long-term consequences of exposure to the drug are unknown.

“This is the first treatment for peanut allergies, and if this drug is approved despite the unanswered questions, it will set a precedent for future drugs to treat food allergies,” Ms. Zeldes said.

Only 4 percent of the 124 children who had been given a placebo powder were able to consume that amount without a response.

Most of the participants were aged 4 to 17, and when they enrolled, all of them suffered from severe peanut allergies triggered by the equivalent of one-third of a peanut or less.

The trial, carried out at multiple medical centers, was designed and sponsored by Aimmune, the drug’s developer.

The drug itself consists of peanut flour. It is carefully measured and packed into capsules or foil sachets of varying doses that can be easily opened and mixed with food.

The starting dose is three milligrams, which is gradually increased to 300 milligrams, the equivalent of one peanut, over a six-month period, followed by six months of maintenance therapy of the same amount. In order to retain their tolerance, patients would need to continue taking the maintenance dose, possibly indefinitely.

Several of the treated children developed eosinophilic esophagitis, an inflammatory disease of the esophagus that is triggered by exposure to an allergen.

The group urged the F.D.A. to require additional research data about risks before approving the drug, “because it will be difficult if not impossible to obtain it afterward,” Ms. Zeldes said.

Company officials say that over 1,100 patients have enrolled in the clinical trials, and they continue to follow patients to evaluate the longterm safety and effectiveness of the therapy.

A trial of the new drug published last year in The New England Journal of Medicine found that after a year, two-thirds of the 372 children who received the treatment were able to tolerate at least 600 milligrams of peanut protein — the equivalent of two peanuts — without having an allergic reaction.

In July, the Institute for Clinical and Economic Review, a nonprofit research organization in Boston that evaluates medical evidence, concluded in a report that there was inadequate evidence to justify the immunotherapy treatment over simply avoiding peanuts.

Chris and his brother, Tony, started Budget Blinds back in 2005 and now they and their highly experienced staff cover the southern half of Delaware from just south of Dover on down and three counties in Maryland. Both he and his brother are native Sussex Countians in fact their family has been here for many generations and self employment was a model presented to them early with one grandfather an area farmer and the other owning a shoe shop just off the circle in Georgetown. Growing up in a close family, they were taught the virtues of love, grace, loyalty, and hard work. They were always taught to be accountable to one another, to work first, play later and if it’s worth doing, it is worth doing right. They carry those values into both their personal lives and in the business that they run.

Chris Russell is a recognized leader in the home improvement industry with 23 years experience.

He has received multiple national awards including 5 time GAF President’s Club Award recognizing the “elite of the elite” in residential roofing; the Trex PRO Platinum certification, the highest level of recognition a deck builder can receive; and Sunesta Awnings & Outdoor Comfort Bronze Elite Award and Sunesta’s 2019 Dealer of the Year. Store Manager, Furniture and More Deborah joined the team of Furniture and More in August 2005.

Deborah has been in Retail Sales since 1965. Her first retail job was in her mother’s retail business.

Deborah says and I quote “ That was the toughest job I ever had but those were the years that left a permanent influence on my management skills even to this day. I speak to my Mom every evening and at 94 years old she is still giving me advise on life and business. At the end of the conversation I think how lucky can one person be.”

For over 14 years Deborah has had the privilege to be the team leader for a very skilled and experienced sales team. Deborah says their business is more than just selling furniture, “Its about lifestyle”. No two customers are a like so we are never bored. The team enjoys being a part of the customers vision of what they want their home to look like with the best product, at the best price.

Deborah says her sales team today is always thinking of the bigger picture for the customer.

Bobbi J. Barends, Ph.D.

Vice President and Campus Director, Delaware Technical Community College Dr. Barends has served in a variety of roles in higher education including adjunct faculty and Assistant Professor of Occupational Therapy at Elizabethtown College and Occupational Therapy Assistant Department Chairperson, Assistant Dean of Instruction, Acting Assistant to the Campus Director, and Dean of instruction at Delaware Technical Community College.

She became vice president and campus director for the Owens Campus in January 2017.

Dr. Barends’ career has included teaching, advisement, faculty and staff development, campus and collegewide planning and assessment, oversight of marketing and public relations, fundraising and grant writing, program development and outreach to K-12, higher education, and community partners.

She is passionate about student success and the important role that community colleges play in economic development and social mobility.

Dr. Barends earned a bachelor’s degree in Exercise Science from the University of Pittsburgh, a master’s degree in Occupational Therapy at Misericordia University, and a Ph.D. in Education from Walden University. She has served on numerous boards and committees including the Carl M. Freeman Foundation’s F.A.C.E.S advisory board, Friends of CASA, and the 2015 Vision steering committee. Dr. Barends is a Court Appointed Special Advocate (CASA), a member of Sussex County Economic Development Action Committee (SEDAC), and a board member for Kent-Sussex Industries and Sussex County Habitat for Humanity.

Dr. Barends is a proud mom of her three children Jacob, Kennedy and Thien Thanh. She lives in Selbyville, Delaware with her husband, Andrew, and her two youngest children. She is an advocate for individuals with special needs and is passionate about service and giving back to her community.

Mike Baker Mike Baker is the founder of C.M.Baker Photography with over 25 years experience. His photography ventures have been quite diverse.

Throughout the year you may find him shooting a wedding, a Quinceañera, in the pits of a NASCAR race at Dover Downs and now at front stage at the Firefly Festival or flying over doing aerials. Giving back to the community has always been an important part of it all. Mike was awarded “Volunteer of the Year” from CDCC (Central De Chamber of Comm.) in 2005, ”Volunteer of the Year” from Kent County Tourism 2010,”Chairman’s Outstanding Partnership Award from CDCC in 2010 along with “Business Person of the Year” from CDCC in 2008. Since 1997, Mike has been the “official” photographer for the KC Heart Ball and for the “Go Red” in Kent County since it’s onset. Kaitlin Childress A&E Properties

Kaitlin and her husband, Zach, moved to the Dover area in September of 2017.

They met A&E owner, Albert Miller, and his wonderful family, quickly became friends, and Kaitlin joined the A&E Team the end of October that same year.

Kaitlin assisted Albert in a myriad of administrative tasks before diving into the design side of the business. To this day she is Albert’s Executive Assistant as well as the Lead Designer of A&E Building Service overseeing the design selection and home staging of A&E Products. The A&E Staff has grown incredibly in the past 2 years and Kaitlin has been a key component in forming A&E’s culture and making A&E a refreshing and life giving place to work.

All of the staff at A&E are passionate about what they do and love the people they get to do it with! Kaitlin is an avid coffee drinker, dog lover, and fan of weekend getaways!

A&E Property Solutions is a real estate investment and rental property company, and its sister company, A&E Building Service, is a home building and renovation company. Together, they make up A&E Properties, a full service real estate development and investment company.

Kevin Gilmore

Executive Director, Sussex County Habitat for Humanity Kevin has served with Habitat for Humanity since 1992. He started as a volunteer with his Campus Chapter at Elon University in North Carolina. After college, Kevin worked with Habitat International at their headquarters in Americus, Georgia.

From there he went on to serve with Habitat for four years in Guatemala.

In 2003, he completed a graduate student fellowship with Public Allies Delaware focusing on non-profit management and building volunteer programs. After earning his Master of Public Administration degree at the University of Delaware, Kevin came to work with Sussex County Habitat for Humanity in 2004 as their first paid employee. The organization now employs over 20 people and works with more than 2000 volunteers to carry out its mission.

Kevin grew up in Milford, Delaware and is pleased to be in Sussex County serving the community which nurtured him as a youth.

Christina Stanley

Vice President MHDC Christina Stanley is an accomplished affordable housing and community development professional.

She has a broad knowledge of affordable housing and community development, and extensive experience in Low Income Housing Tax Credit (LIHTC) syndication and multifamily lending.

Christina strives to improve the lives of low to moderate income communities, families, and individuals. She is passionate about her loving family, community development, affordable housing, and the promotion of healthy food access for low income communities and individuals.

Christina currently serves as Vice President of Milford Housing Development Corporation (MHDC) and East Coast Property Management.

Prior to joining MHDC, she served in various capacities at the Delaware Community Investment Corporation/Cinnaire (DCIC) for over fifteen years directing operations, managing portfolio risk, underwriting debt and equity, securing loan and equity investors, administering loan servicing, and originating debt and equity projects. In addition to her work with DCIC, Christina worked for several years with NCALL Research as a Housing Development Specialist. At NCALL, she provided capacity building training and technical assistance to nonprofit housing sponsors in all areas of the housing development process. Christina developed her passion for rural development issues as a research analyst at Jefferson Medical College while examining physician location patterns in an effort to assist with promoting health service access in rural communities. She has served on several nonprofit boards throughout the region. In 2008, she received the Ambassador Award from the DE Chapter of the National Association of Housing & Redevelopment Officials.

Christina has a BA in Economics from LaSalle University, 35 credits earned in graduate studies in Urban Affairs & Public Policy from the University of Delaware, and extensive affordable housing and community development courses including NeighborWorks, IREM, NAHMA and University of DE.

Dr. Goldstein is a board-certified gastroenterologist who graduated from Georgetown University for her medical degree and then went on to complete her Internal Medicine residency at North Shore University Hospital and did her Gastroenterology fellowship training at the University of Buffalo. Health & Wellness Harrison Senior Living Communities Receive National Recognition for Improving Quality of Care Harrison House of Georgetown and Harrison House of Snow Hill are being recognized by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL) for their achievements in the quality of care they provide. The AHCA/NCAL’s Quality Initiative program is a multi-year effort to challenge long-term care providers to achieve quantitative results in four areas. Both communities earned recognition for their work in two specific areas: Functional Outcomes and Appropriate Use of Antipsychotics. Veterinary Services Peninsula Veterinary Services is a full-service, AAHA-accredited hospital located in Millsboro, Delaware. Owner Dr. April Reid moved from Brooklyn, New York in 2015 to start a practice in this area after spending summers in Rehoboth as a child. Dr. Reid is a graduate of Ross University School of Veterinary Medicine. She previously owned a thoroughbred racetrack practice for 14 years before making the move to small animal practice. Dr. Reid works closely with several local rescue organizations and is currently serving as the vice president of the Delaware Veterinary Medical Association. In addition, she is working towards board certification in veterinary practice. In her spare time, Dr. Reid enjoys spending time with her two Great Danes, Ambrose and Amelia, and her three cats.

PETS AND DENTAL DISEASE February is National Pet Dental Health month. Dental disease is by far the most common health problem that dogs and cats develop.

By the age of three, 80% of dogs and 70% of cats have some degree of active dental disease. It is also the most under diagnosed disease of companion animals because our pets are unable to communicate their discomfort to us and because pets continue to eat no matter how much disease and pain they are experiencing due to their survival instinct.

The number one thing pet owners can do at home to prevent dental disease is brushing their pet’s teeth to remove the plaque. Plaque is a colorless film that is constantly forming on both human and animal teeth. If it is not removed by brushing within 24 hours, it becomes tartar/calculus that cannot be removed by brushing. In pets, the tartar builds up and begins to accumulate at the gum line. This is how periodontal disease begins.

Periodontal disease is inflammation/ infection that develops around the tissues of teeth. As periodontal disease progresses, pet owners usually become aware that there is a problem in the mouth because the breath begins to smell badly and an oral examination by a veterinarian will likely result in recommendation of a professional cleaning.

Daily brushing is recommended with three times weekly being the minimum amount recommended.

Brushing should be started as soon as possible in the pet’s life. Even

though the baby teeth are going to be replaced by adult teeth, daily brushing helps acclimate the pet to the experience. There are two kinds of brushes-finger brushes and the longer brush which resembles a human toothbrush. The finger brush is good in the beginning, but transitioning to the longer brush will most likely be necessary as the pet grows. DO NOT use human toothpaste as they may contain fluoride in toxic doses, xylitol or a chemical called Sodium Lauryl Sulfate (SLS), which is what makes human toothpaste foam. Even small doses can cause gastrointestinal issues for your pet. There are special toothpastes made for pets that are safe and also tastes like chicken and other flavors pets prefer.

During a professional dental cleaning, pets are sedated, IV catheters placed and the pet is intubated with an endotracheal tube. In addition to providing gas anesthesia to the pet, the tube protects the trachea from any debris that would otherwise go into the lungs during a dental cleaning. From this point, a veterinary cleaning is much like a human dental cleaning. The teeth are first examined and charted to determine the degree of mobility,

gum recession and loss of bone around the teeth. Full mouth dental radiographs are taken to assess the health of the gums and bone beneath the gum line, which is where the majority of dental disease takes place. If there are any teeth that need to be extracted, local blocks are administered and the affected teeth are removed. Finally the teeth are cleaned and polished and the pet wakes up from anesthesia with a healthy mouth free from periodontal disease.

If owners are unable to brush their pet’s teeth, there are a variety of other products that can be helpful.

The Veterinary Oral Health Council (VOHC) evaluates products and their seal can be found on products that have been tested and proven to effective. Chew toys, treats, bones, water additives and even food are available to help keep pet’s teeth healthy.

Besides better breath, proper oral hygiene in pets helps increase their lifespans. The mouth is a constant source of bacteria that can damage organs such as the heart and kidneys. Research has shown that professional dental cleaning can actually improve kidney function in cats that have been diagnosed with kidney disease/failure.

Sasha’s Jewels, LLC I’ve had several retail shops my first at the age of 18 in the Talleyville Shopping Center, Wilmington, DE and the next and the Booths Corner farmers market in Boothwyn, PA . I had shops in the Booths Corner farmers market for about 20 years.

By the time I was ready to move on from Booths Corner I had three shops. I left the retail business to raise my young family in 1990.

My store is called Sashas Jewels LLC because when I started my business I was only 16. I didn’t feel as though I would have the credibility with the public to be able to represent a jewelry business properly.

I named my business from my middle name which is Sasha. I am Madeleine Sasha.

I’ve been in the jewelry business for over 40 years. I started my business when I was 16 and still in high school because I wanted to be able to buy things for myself as many teenagers can imagine. I started taking the graduate gemology classes in the 10th grade from the Gemological Institute of America in California. At that time it was a correspondence class where they would send you paper lessons and as you completed them you would mail them to the school and the school would grade your lesson and mail it back to you. As you can imagine juxtapose to this day and age where you can just email or work online, class work took a long time. The pictures in my original classes weren’t even pretty they were hand drawn pictures of inclusions of the Gem Stones.

It was necessary for me to attend many gem shops and trade shows in order to get an actual look at the gemstones which I had been reading about.

After graduating high school I worked for a Russian jeweler for free so that he would teach me Jewelry Repair. It was a lot of fun I really enjoyed it. He would rave about how easy I was to teach and that all he had to do was tell me what to do and I did it. This was a great boost to my confidence and acknowledgment that I was also good at what I already enjoyed doing.

I always maintained the workshop and at home and maintained my wholesale and private customers.

More recently I had a shop in Berlin, NJ for 7 years. I’m currently located on Coastal Hwy in Fenwick Island, DE As well as my retail business I’ve always done jewelry repair work and appraisals for the trade servicing jewelry dealers that have antique stores and/or do the traveling antique shows nationwide.

I’ve done a lot of repair work on antique jewelry and have become adept at that art as I am familiar with the styles of antique jewelry and what they’re actually supposed to look like before and after being repaired. A piece of antique jewelry that’s repaired should never look as though it’s been repaired. The repair should be in keeping with the design of the piece. Some antique parts are difficult to find therefore I’m always buying jewelry even if it’s broken because it may have parts that I can use for another customers repair.

I am always buying. It doesn’t matter to me if your items are gold, silver or costume, or if you don’t know what you have! You’re welcome to bring it in and I’ll take a look at it “ there is no fee for me to evaluate your items”. If you have many items then it’s probably a

good idea to make an appointment.

I’ve had some people bring in as many as three suitcases filled with jewelry costume gold silver all mixed in together. I’ve had some of my customers bring in duffel bags filled with stuff that was barely separated. I’ve gone into homes occasionally where the table tops were filled with jewelry and boxes and the whole table was covered with unknown items.

One woman found a pair of earrings at a yardsale, she paid a dollar for. I paid her $40 for them.

I go to yard sales quite frequently and I’m always treasure hunting.

If you come into my shop and you asked me to evaluate your piece, I will and I’ll tell you what I’m willing to pay for it. Then it’s your decision as to whether or not you would like to except the offer, there is no obligation.

I tell all of my customers that come into the shop, “Bring you items in, you might be pleasantly surprised”.

Because it’s true sometimes they just don’t know the value of what they have. No matter the size of the estate I’m always interested and able to purchase. Larger estate collections of jewelry may take more time and require several visits.

My goal is to make the process as smooth and stress free as possible.

If you’re going to sell your jewelry to anyone you should know their qualifications for evaluating your jewelry. I’ve gone to estate sales and bought jewelry from antique jewelry dealers and secondhand stores that simply put, did not know what they were doing.

I’ve heard some customers of mine say that the chain got broken so they threw it out or a ring lost a stone they threw it out well all of that can be fixed but basically speaking jewelry is not the same as clothing it has a value even broken.

Jewelry can be worn out and could still be worth hundreds of dollars you just never know. A little known fact is some metals that are marked 14 karat are not and some jewelry that is without a mark could very well be precious. Do not just discard an item without having it tested.

I can test it for you, we can find out together because I will do it right in front of you. I am happy to teach anyone what to look for. I’ve had many customers whom I’ve taught what to look for and they go out the yard sales and they find things they bring it to me.

Just this past fall I bought a two stone diamond ring for five dollars at an antique show and the woman had no idea what she was selling, therefore she probably had no idea what she should have paid the person from whom she had bought it.

I’ve had customers bring me jewelry and told me that another jeweler told them a repair could not be done. Well, I really love a challenge! So I will go ahead and figure out a way to get that done for my customer. Sometimes it’s a simple fix and sometimes it’s a little bit more involved. Either way it’s all fun and games to me, as I really enjoy it.

I attend many trade shows as there always new developments in jewelry and styles and design.

I am always learning something new! I keep my eyes open for new ways to do things and new designs that I find very exciting.

I love to get old pieces or exciting things that are different and unusual looking. I will make a mold of them and recast them in several different styles recasting them with the lost wax casting method. This comes in handy and is a lot of fun when you have a particular family piece that was inherited and maybe there are two or three sisters that would like to have that piece as a remembrance of their mother or some other matriarchal family member. I can make several of the same piece and even though it’s not the original piece it’s still intrinsic to the memory.

Earrings are another fun project.

Sometimes you get a pair of earrings and do you have two sisters that would both like to have the earrings. I can make pendants out of the earrings and each sister can have a pendant to wear. I do this in my shop quite often with my earrings were friends or sisters come in and they would like to have something that goes together and I’ll take a pair of earrings out of my showcase and I’ll make them what I call sister pendants or best friend pendants.

I have a large selection of natural gemstones and vintage Gem Stones to replace in vintage rings that have either lost or broken stones. I also have a large selection of vintage castings. I recently had a woman come into the shop with a vintage stone that her mother had left her I had a Filigree casting which fit her stone exactly. Which is rather difficult as the older stones are not calibrated as the newer stones are they were very unusual sizes.

Putting a vintage stone into a new ring just doesn’t suit the style of the era of which the stone originated.

When workers are injured, they do the best that they know how to do in navigating the Workers’ Compensation system. My first meeting with an injured worker is spent imparting knowledge to empower them to do better, once they understand how the process works and what their rights are. Yes- RIGHTS! There are several benefits that injured workers are entitled to as a matter of law, and which they often either do not know about or do not fully understand. Understanding your rights will help you get the best medical treatment available in order to make the best medical recovery possible. This article will give you a very broad overview of those rights. Keep in mind, every case is different, so a free office or telephone consultation would be the most valuable source of information to tailoring these benefits to your particular issues. Workers’ compensation is generally a “no fault” benefit because it does not matter whose fault your injury is, only that you are injured while in the course and scope of your employment.

As every work-related accident is unique, all of the rights listed above will not necessarily apply to every case. What are injured workers entitled to under the Delaware Workers’ Compensation Act? 1. Disability Benefits Your wage rights under the workers’ compensation laws in Delaware are based on your average weekly wage (AWW for short). Workers’ compensation wage benefits can pay up to 2/3 of your AWW, up to a limit of $725.89.

This benefit is designed to replace your wages when you can’t work for 3 or more days due to your work injury. This benefit is also available on a partial basis if your doctor restricts your work hours or work duties, which results in you earning lower wages.

2. Medical Benefits If you are injured in the course and scope of your employment, you are entitled to medical benefits for the rest of your life as long as the treatment is reasonable, necessary and related to the work injury, and your Delaware caregiver is also certified under the Workers’ Compensation Statute. (If you see an out-of-state doctor, they do not have to be certified under the Workers’ Compensation Statute.) A list of certified providers can be found on the Department of Labor’s website.

The worker’s compensation insurance company must pay at least one medical bill every 5 years in order to keep this benefit active!

3. Mileage You are entitled to mileage reimbursement of forty cents (.40) per mile, roundtrip for your doctors appointments.

4. Scarring/ Disfigurement If you were unfortunate enough to have suffered a scar or other physical disfigurement as a result of your work injury, you are generally entitled to compensation. Any scar and/or disfigurement that is visible when properly clothed can be awarded a monetary settlement.

The amount of the settlement is determined by the location, size and visibility of the scar.

Also, if you walk with a limp as a result of your work injury, or need to use any devices permanently (crutches, wheelchair, etc.) you may also qualify for this benefit.

5. Permanent Partial Impairment If your work injury is permanent, you may be entitled to a lump sum payment. The amount

This asset called whole life insurance with it’s “pool of money” is available for a life event (if necessary) and gives

Why a Long Term Care Rider? It’s the most cost effective way of having Long Term Care (LTC) coverage available if and when you need it.

Long term care costs can financially ruin you and your family

The cost of a pure LTC policy is high.

Insurance companies can’t guarantee level premiums each year because they are paying claims at a high rate.

So, even if you get a pure LTC policy, you don’t know how much or how often the premiums will increase, and if you can’t afford the increase, they can decrease your benefits.

Monthly long term care costs can be in excess of $5,000 a month.

These costs vary from state to state.

WHO HAS THE TIME? In the spring of last year, I was approached by the Leukemia & Lymphoma Society to be a candidate for the Woman of the Year campaign in 2020. It is a 10 week fundraising effort which runs from March 5 through May 15th here in Sussex County, of which I was not too familiar. I immediately and politely declined, citing that I “did not have the time,” which I already knew with a family vacation already planned and my nephew graduating from college.

A&E Properties DE is owned and operated by Albert and Elmina Miller. They reside in Dover, Del. with their two wonderful daughters, Kayla and Ashlie. Over the years they have built their family on a strong foundation of faith, generosity, and integrity and they strive to incorporate these principles into all that they do.

Albert and Elmina Miller started this company back in 2007, but it wasn’t until January of 2017 that Albert decided to take the leap of faith and go all-in with the real estate business. He was raised in the Amish community in Dover, Delaware and at age twelve began working in the construction field with his family.

By the time he turned sixteen, he was a full-time bricklayer and from there continued in the masonry field for the next three years. After turning twenty, he started his own business framing houses for big developers throughout Delaware and Maryland’s Eastern Shore. That same year, Albert and Elmina made the hard decision to leave the Amish community. This decision was tough, but they remain forever grateful for the hardworking and committed foundation that they were raised in.

Slide on into spring with these fresh ideas for those hard to cover Sliding Doors!

Often while on an in-home consultation, the topic of handling the sliding door comes up.

It’s always accompanied with some sort of joke, or look of bewilderment. I usually look forward to figuring out my customer’s way of articulating their concern. To paraphrase, it generally goes like this. “I need to cover this—I HATE VERTICAL BLINDS.” Of course, I need only remember back to my first apartment to understand why. Cheap vertical blinds are everywhere. Their top treatments are either bent up and trying to fall, or already placed in that closet, behind the water heater. The vertical louvers are seldom part of a complete set. They like to break and fall, and usually there is tape or unmatched colors mixed in for good measure.

The good news- the custom blinds industry has you covered with more modern and beautiful options!

Vertical Honeycomb ShadesHoneycomb shades are quite popular these days, so why not for sliders? Several manufacturers make honeycombs that are rotated on their side, and anchored to the stack side, and make a solid option for this application. Not only are these available in many colors and textures, but they are some of the most insulative coverings available. When you want to use the door, simply slide the shade into the closed position.

You want it half open- no problem. Generally, magnets are used to help in the fully closed position.

ShuttersShutters are one of the hottest products in the window covering industry, thanks in part to myriad of home improvement shows that seem to always drop in shutters as one of the final touches. Shutters are also a great insulator and are extremely easy to use. If you have kids or pets that are prone to a little rough housing, these are a nice hearty choice.

Generally cordless and extremely light, these can be optioned with base bamboo materials that are cost competitive to custom vertical blinds. We may not always remember everything that happened at an event; however we don’t have to worry about that because photos will always be a way to remind us about our life experiences, good times and cherished moments.

All 4 of our different entrees were excellent, as were the variety of sides you get to choose with each meal. Everything was fresh and tasty and the staff were very attentive and helpful in guiding us through our options. We left very satisfied and looking forward to a return! Thank you! - JM
