R. Alberto Rosa, M.D., FACC Dr. Ramon Alberto Rosa is a graduate of the University of Santo Domingo in his native Dominican Republic. He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), Chief Medical Resident (19941995), and cardiology fellowship (1995-1998). Dr. Rosa has practiced cardiology in Sussex County since July 1998. He has served as Medical Director of the Cardiac Catheterization Laboratory and as Chief of the Department of Cardiology at Beebe Medical Center in Lewes, DE. Dr. Rosa is trained in non-invasive cardiology, Nuclear Transesopageal Echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Dr. Rosa is board certified by the American Boards of Internal Medicine and Cardiology. He is also a Certified Aviation Medical Examiner and is Board Certified in Nuclear Medicine.

Coronavirus-induced heart injury, lessons learned….. so far It has become clear that involvement of the cardiovascular system is common in COVID-19 infected patients. Around 25-33 % of hospitalized patients will show evidence of myocardial injury as defined by abnormal cardiac enzymes, detected on regular blood tests. The affected patients are generally older and have a higher incidence of hypertension, diabetes mellitus, coronary artery disease and heart failure. In addition, myocardial injury is associated with a more severe COVID-19 infection that often times requires the use of ventilators and is associated with higher in-hospital mortality. This disease affects multiple organ systems, in addition to the heart and vessels, lungs, kidneys, liver, intestines and brain. Several mechanisms of myocardial injury have been described with this coronavirus: Direct viral invasion of the heart cells (Myocytes), blood clots in both large and small vessels, inflammation mediated injury, stress-induced cardiomyopathy and actual heart attacks. Most of these

cases have been observed in prolonged coronavirus cases, longer than 14 days as a general rule. Coronavirus enters the human cells by attaching to the ACE-2 receptor on the cell surface through the virus spike protein, in the finger-like projections that make this virus so characteristically familiar to all of us. These receptors are abundant in lung, vessels and heart cells. The virus takes over the cells mechanisms to replicate itself and infect adjacent cells, rapidly extending to multiple organs. It is the body’s inflammatory response to counter the virus that causes the clinical syndrome that we see with fever, cough, general malaise, progressing to pneumonia, kidney failure, heart disease and other organ systems. The inflammatory response can lead to direct damage to the inside of small and large vessels in the major organs causing blood clots to form and create hypoxia (lack of adequate oxygen levels) at the different tissue levels. This is the main reason why the pneumonia caused by

the COVID-19 infection has turned out to be so different from typical ones. Current treatment for COVID-19 infection includes antivirals such as remdesivir, antiinflammatories such as steroids, anticoagulants such as heparin and convalescent plasma. The reality is that the vast majority of COVID-19 patients will have either none to mild symptoms. Those with preexisting conditions

(lung disease, heart disease, high blood pressure, obesity or diabetes) as well as older than 65 will be more predisposed to developing a more serious form of the condition can progress to severe lung failure or heart injury with its potential for a prolong disease or will be in the less than 1 % of patients than will not survive the infection. Prevention and early detection and attention remain the key factors. Wear your masks when out

in public places, especially indoors and keep your distance and your hands clean. Protective eye wear is also important. In the latter months of this pandemic, effective treatment protocols have been developed, and an effective vaccine is coming in 2021. Seek attention early if you are exposed or suspect you may have COVID-19. Do not fear medical care, this virus is here to stay a while longer, and we are learning to live with it!

How Oral Health And Heart Disease Are Connected Presented by Maria E. Cruz, D.D.S. It’s increasingly common to hear that oral health is vital for overall health. More than 80 percent of Americans, for example, are living with periodontal or gum disease, which often goes undiagnosed. This may be because the patient’s teeth feel fine, so he avoids going to the dentist, and visits to the physician rarely focus on oral health. There is now evidence of two specific links between oral health and heart disease. First, recent studies show that if you have gum disease in a moderate or advanced stage, you’re at greater risk for heart disease than someone with healthy gums. And second, your oral health can provide doctors with warning signs for a range of diseases and conditions, including those in the heart. Why Are These Things Related? Oral health and heart disease are connected by the spread of bacteria – and other germs – from your mouth to other parts of your body through the blood stream. When these bacteria reach the heart, they can attach themselves to any damaged area and cause inflammation. This can result in illnesses such as endocarditis, an infection of the inner lining of the heart, according to Mayo Clinic. Other cardiovascular conditions such as atherosclerosis (clogged arteries) and stroke have also been linked to inflammation caused by oral bacteria, according to the American Heart Association.

Who Is at Risk? Patients with chronic gum conditions such as gingivitis or advanced periodontal disease have the highest risk for heart disease caused by poor oral health, particularly if it remains undiagnosed and unmanaged. The bacteria that are associated with gum infection are in the mouth and can enter the blood stream, where they attach to the blood vessels and increase your risk to cardiovascular disease. Even if you don’t have noticeable gum inflammation, however, inadequate oral hygiene and accumulated plaque puts you at risk for gum disease. The bacteria can also migrate into your bloodstream causing elevated C-reactive protein, which is a marker for inflammation in the blood vessels. Why You Should See A Chiropractor For Allergy Relief Dr. Tracy Hudson Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. While attending UNCW, Dr. Hudson suffered an injury playing softball and she sought treatment with a local chiropractor. After a few adjustments, Dr. Hudson was back on the field. In the industrialized world, a growing number of individuals are affected by allergies. Currently, the numbers are between 10-30 percent of the total population and are increasing steadily. For Americans who suffer from nasal and seasonal allergies, the costs very quickly add up. It is currently estimated that between doctor visits, prescriptions, and time missed from work, these allergies cost in excess of 3 billion each year. Of course, with Spring just around the corner, the familiar sneezing and itchy, watery eyes can be expected to come out in full force. For allergy sufferers, the welcome break from winter is not entirely pleasant. Seasonal or not, all allergies are the result of an exaggerated response by the immune system to substances that it recognizes as foreign. In allergic persons, the immune system produces a specific antibody known as immunoglobulin E (IgE) in response to pollen, pet dander, foods, dust, and other matters which then produces the symptoms of an allergic reaction. Of course, there are medications to help control the allergic response and provide relief from symptoms. There are even medications that will be lifesaving in the event of severe allergic reactions. However, in addition to treating symptoms, there are other steps which can be taken to boost the immune system and lessen allergic reactions.

HOW CAN A CHIROPRACTOR HELP WITH ALLERGIES? The strength of both the immune and respiratory systems depends largely on proper communication between the brain and spinal cord to control and coordinate their functions properly. When there are interruptions in this communication such as misalignments of the neck, immunity may be compromised. This upper cervical joint irritation can exaggerate or even produce allergic and asthmatic symptoms. Through manipulation of the joints and soft tissue either through traditional chiropractic adjustment or spinal decompression, a chiropractor can help restore alignment and open the necessary pathways of communication between the brain and spine. In addition to promoting a stronger immune system, adjustments may also help regulate the rush of histamine and amount of cortisol produced during high allergy season. The majority of allergies involve release of inflammatory producing substances such as histamine. Adequate production of cortisol, an anti-inflammatory hormone produced by the adrenal gland is necessary to combat this. Understandably, many allergy sufferers are also found to have weak adrenal gland function. What many do not realize is that the adrenal gland is fed directly by the T9 spinal level nerve root, meaning once again, that spinal misalignments can greatly impact allergic reactions.

The choice to change is a personal one, but the change itself is not something we need to undertake alone. Substance use or mental health disorders affect about 63 million U.S. adults. Statistically speaking, it is likely that you or your family have been impacted by one or both disorders. As a treatment provider, SUN knows these disorders do not discriminate, and in fact, they often overwhelm parents and working adults. Overdoses have steadily increased throughout our region; SUN Behavioral is now a NARCAN training and distribution point to give community members the tools they need to help save lives. Last year, SUN Behavioral distributed a survey to a few hundred people to ask them what programs were needed most in their communities. An overwhelming number of those people requested more options for outpatient services, available at times convenient for working adults. Professional people who are in recovery – especially those with a co-occurring mental health disorder – needed supports they could work into their daily routines. In response to this need, SUN’s Outpatient Services team got to work. After all, our company name (SUN) is an acronym that stands for “Solving Unmet Needs” in the communities we serve. A new program was born: an evening adult Intensive Outpatient Program (IOP), available 100% via telehealth, launching February 15, 2021. My Tongue Tie Journey Dr. Gerardo Martinez Many of us have heard of the term ‘tongue tie’ or the medical term, Ankyloglossia, which literally translates to: anker of the tongue. It is a condition that has existed in newborns for thousands of years. In the middle ages, Midwives used to cut the tissue restricting the tongue with a sharp fingernail. Over the centuries, the procedure has been performed to facilitate nursing which is usually the first sign of an issue. The term ‘tongue tie’ has become a controversial and oftentimes divisive topic among many healthcare professionals. Even among fellow pediatric dentist colleagues, the procedure can spark heated debates with others having strong opinions for and against it. The issue is so controversial that most of my dental training, including residency in pediatric dentistry, the topic was barely discussed let alone doing a full dive into proper evaluation and treatment. I practiced as a Board Certified Pediatric Dentist for nearly a decade before the issue of tongue ties came knocking at my own door. My own two children were afflicted with the condition. I was not able to recognize all of the signs and symptoms until years after they were born. My son was born in 2015 and we couldn’t have been happier as a family. The whole experience was incredible until it came time to nurse our baby. He would not latch on and would cry hysterically in hunger and desperation. The hospital staff recommended we use formula to help calm him down. My wife wanted to breastfeed our son, and having a bottle of formula within the first hours of life was not part of the plan. After the first couple of sleepless nights, we gave in and started to use formula before we had even left the hospital. We did not have a lactation consultant meeting with us until we were walking out of the room with our bags and a fussy newborn. It was a quick conversation that did not involve actually nursing our newborn. More theory than practice. On our follow up visits, my son was labeled as a “lazy baby” and my wife as a “new mom”. After many continued attempts, my wife gave up on nursing. It became too difficult both physically and emotionally. She pumped for 1 year, 2 days, and 6 hours!

This often results in tongue tied children developing into mouth breathers. Mouth breathing is associated with a host of negative outcomes such as a higher chance to develop frequent nasal congestion, asthma, eczema, and sleep disorder breathing. Our daughter developed all of the above. Both of my children have been released and are doing better. Although, tongue tie problems are not solved but just cutting it. It is about a collaborative approach between primary healthcare providers, lactation consultants, speech therapists, and other medical specialists to help improve the quality of life of the children we serve. Their journey to a full recovery is still on it’s way.

Easterseals Supported Employment: Dream Jobs for Dream Employees By Natalie Scott When most young adults graduate from high school, they typically know which direction they are going to head, whether it be attending college or jumping right into a job. When Mikayla graduated from high school, she and her mom, Michele, knew exactly where to turn and it was the place where she began many years before -- Easterseals! “Mikayla had a good foundation as a child, thanks to the Easterseals Children’s Therapy program. I get to make new friends and see my friends at work. I love it.” When she is not working, Mikayla loves going out to eat and is a huge movie buff. She is also incredibly generous with the money she earns. She often treats her mom and friends to dinner or ice cream. Mikayla stays well-connected with friends and Blue Gold buddies. During the summer, Mikayla enjoys her time swimming and being with her friends at Easterseals Camp Fairlee.

Can People With Peanut Allergies Get The Covid Vaccine? Presented by Alexandra Gallagher Starting August 1, 2021, Alexandra will be a student for PreMed/Biology at the University of Miami.

By Aristos Georgiou NEWSWEEK 12/18/20 The Pfizer/BioNTech COVID-19 vaccine is now being rolled out across the United States as part of the largest vaccination campaign in American history. But can people with allergies to food—such as peanuts—get the shot? According to guidance from the U.S. Centers for Disease Control and Prevention (CDC,) people who have a history of food, pet insect, venom, environmental, latex or oral medication allergies can receive the Pfizer/ BioNTech vaccine.

The American College of Allergy, Asthma, and Immunology has said that the vaccine should not be administered to anyone with a known history of severe allergic reaction one of the components called polyethylene glycol or PEG in particular. This substance has previously been documented as a a cause of anaphylaxis by scientists, although these reactions are rare. The CDC says that individuals who have a history of severe allergic reaction to any vaccine or injectable therapy can still get the Pfizer/ BioNTech vaccine but they should take extra precautions. The agency recommends that such individuals should undergo a risk assessment and discuss the potential harms of taking the vaccine with their doctors. Those with a history of only mild allergic reactions to vaccines or injectable therapies can take the Pfizer/BioNTech vaccine without these extra precautions, according to the CDC.

The CDC recommend that vaccine providers should observe people with a history of anaphylaxis—a severe, potentially life-threatening allergic reaction—due to any cause for 30 minutes after the individual receives the shot. Everyone else should be observed for 15 minutes. “If you have a severe allergic response—for instance people have peanut allergies or egg allergies or food allergies—you can still get this vaccine, you just need to wait for 30 minutes in the area where you got the vaccine so if you do have an allergic reaction someone would be able to give you a shot of epinephrine to reduce it,” Dr. Paul Offit, who sat on the FDA advisory board that approved the vaccine, told reporters during a a Poynter Institute webinar on Monday. “And then we for everybody else, we make a recommendation we always make with vaccines, which is that whenever you get a vaccine, you should hang around for about 15 minutes to make sure you didn’t have a severe allergic reaction because one out of every million doses roughly of vaccines is complicated by severe allergic reaction,” Offit said When it comes to allergies, the only people who shouldn’t get the vaccine are those who have a history of severe reactions to any component of the Pfizer/BioNTech vaccine, according to the CDC. Alexandra aspires to be a surgeon specializing in female cancer research and helping children with Treacher Collins Syndrome, a condition that affects the development of bones and other tissues of the face. Alexandra was diagnosed by Dr. Robert Wood, of the John Hopkins, at the age of 4 with a most severe peanut allergy after she went anaphylaxis after eating a peanut product. Preparing for Spring Sports Season ... Warmer temperatures, greener grass, sounds of parents cheering, and spring sports are upon us...We are all excited to see kids be kids again and it is important to make sure they stay healthy to finish out the season ... Spring is finally here and let’s hope for a season full of good weather and no disruptions from COVID. Warmer temperatures, greener grass, sounds of parents cheering, and spring sports are upon us. Unfortunately, many sports have been on hold for the last year and many of you are eager to see your kids back out on the field or court. We are all excited to see kids be kids again and it is important to make sure they stay healthy to finish out the season. In preparing for the season, focus on these important steps: Get enough sleep: sleepfoundation.org recommends at least 8 hours of sleep, but athletes who get 10 hours of sleep a night show ability to achieve greater peak performance Eat a balanced diet: provide your body with good fuel to sustain their peak athletic performance. ‘Healthy Eating Plate’ by Harvard Medical School provides an excellent breakdown of a balanced diet. You wouldn’t run your car on poor fuel, so why do it to your body?

Remember there is an offseason: the trend is to specialize in one sport for an entire season. Statistics show that young athletes are 5-6x more likely to experience an injury when specializing in one sport. Multisport athletes develop a greater range of strength and stability than a single sport athlete. And more importantly, the body needs rests, which can only be achieved with an offseason. BenchMark Physical Therapy is more than happy to guide you through this process by answering any questions. If there are any concerns, we can help refine movement patterns to help your athlete reduce the risk of injury.

Stretch often: build a dynamic stretching program into your daily routine. Mobility before stability- make sure the body can move appropriately before trying to strengthen it. A good dynamic stretching program focuses on all major muscle groups, should elevate your heart rate, and cause you to break a light sweat. Start conditioning early: build your cardiovascular strength prior to beginning sport specific training. You wouldn’t take a test without studying, so why jump into a sport with a training. Start with a jogging, swimming, or high intensity interval training (HIIT) to build your endurance. This will set you up for a long season. Core Stability: the core is the most important part of the body for any athlete as it is the foundation of every athletic movement. Every athlete should devote 10-15 minutes a day working on core, which includes more than sit ups and crunches to build the 6 pack. Planks, yoga, deep breathing, or Pilates are all good places to start. If you have any questions, it is best to consult with a physical therapist or personal trainer. Sport specific movement training: understand the specific movements of the sport and train appropriately. If the sport requires quick twitch muscles, train the body appropriately.

CoolSculpting Non-Surgical Fat Removal The biggest trend in cosmetic surgery is the expansion of non-invasive body contouring technology, one of the best of which is CoolSculpting, which is available at Renove Medical Spa, the aesthetic medicine practice of Dr. Michelle Parsons, located in Rehoboth Beach. CoolSculpting is an FDA Cleared, non-surgical device that uses precisely controlled cooling to destroy fat cells, thereby eliminating unwanted love handles, bra rolls, and stomach fat. Arms and legs can be treated as well. Unlike some other non-surgical technologies that release fat from fat cells, CoolSculpting destroys the fat cells thereby eliminating treated fat cells. The technology behind CoolSculpting was developed by Harvard scientists who noted that fat was destroyed by cold under certain conditions. your unwanted fatty area will be pressed and chilled to -11C while keeping your skin protected. At this temperature the fat within the fat cells crystallizes which damages and kills the fat cells. Your skin and other tissues are protected and not damaged during this process. Your body will then eliminate the killed fat cells over time. “The best news is that we do not make many new fat cells as an adult. Fat cells may get bigger or smaller as they store more fat or lose fat, but typically, as adults we do not make new fat cells. This helps the results of CoolSculpting be long lasting if there is no subsequent weight gain. Of those patients who do gain weight after a treatment, the weight may not return to the treated areas”. What are the most common areas treated? Many people on a weight loss program continue to have stubborn hard to lose areas, no matter how much dieting, sit ups or weight they lose. Such stubborn areas as typical around the abdomen, the love handles and inner thigh areas. Once you are over forty, losing fat in these areas is extremely difficult. These are the areas that CoolSculpt can address. “Most of our patients are interested in losing fatty areas in the upper and lower abdomen, the muffin top, waist, bra fat, back fat, love handles and the “mommy

pooches”. We can also treat under the chin fat, the jowls, arms, thighs and the thigh saddle bags.” Are there any side effects from CoolSculpting? The side effects from a CoolSculpting treatment are minimal. It is possible to have bruising after a treatment, which would resolve in several days. It is not uncommon to have a mild decrease in sensitivity in the treated area lasting 1 to 2 weeks. In some instances patients will develop a burning tingling sensation 5 days after the treatment which can be controlled with an over the counter lidocaine patch. What does CoolSculpting feel like? The treatment room is very comfortable where you will relax on a padded recliner. When CoolSculpting is applied to the fatty area to be treated there is a cooling sensation for the first 5 to 10 minutes. Gradually this sensation lessens to the point there really is no discomfort during the treatment. Palliative Care:

How it can help you care for you or your loved ones Palliative care is often a misunderstood healthcare treatment in our community. To “palliate” means to ease or eradicate suffering. An overreaching goal of palliative care is to “meet the patient where they are” and to provide support, information, and guidance throughout their disease process while they continue active, curative treatment for their serious illness. With years of longevity and care in the community, Delaware Palliative is the premier in-home palliative program offered in Delaware. A Palliative Care Team includes a Nurse Practioner, a Social Worker, and a Care Navigator who provides in home support for not only the patient but also the caregivers. Palliative care differs from other in home healthcare support by: • Providing care alongside continued prescribed treatments and other post-acute care options, i.e., home health services and rehabilitation. • Being appropriate at any stage of a serious, life-limiting illness; a terminal prognosis is not required. Through home visits, Delaware Palliative provides assistance with the physical and psychosocial burdens of a serious illness. In collaboration with the primary provider, the Palliative Care Team works with the patient and caregiver to identify goals of care related to symptom management and advance care decisions. Incorporating Delaware Palliative into your current healthcare team allows time for communication of better understanding of the illness as well as options for care and treatment. Early palliative care referrals can limit out of control symptoms and caregiver burden. This intervention allows patients and families to maintain overall control of the serious illness and fosters increased independence and functional ability. The most commonly seen diagnoses include cancer, dementia, end stage renal disease, congestive heart failure, and chronic respiratory diseases. Referrals can be made by loved ones or medical professionals. Patients at any age and stage of serious illness may benefit from and receive palliative services. Medicare, Medicaid, and some Medicare Advantage health insurance programs cover palliative services. Patients are responsible for co-pays, deductibles, and visits not covered by their insurer.

Delaware Palliative Story Mrs. T is a homebound 89 year old. Even with the assistance of her daughter it is very hard for her to get to doctor’s appointments. Delaware Palliative was called in so medical care could be provided in her home. In the months prior to admission to Delaware Palliative, Mrs. T experienced frequent and multiple ER visits and hospitalizations for upper respiratory infections. In consultation with Mrs. T’s primary care physician, the Delaware Palliative Nurse Practitioner was able to obtain portable medical equipment for her home and was able to provide hands on education. Mrs. T’s daughter has formed an excellent relationship with Delaware Palliative and calls as soon as she observes a change in her mother. Home assessments, early interventions, including medication prescription, caregiver education, and support has prevented further ER visits and hospitalizations for five months now.

2020 was a difficult year for many reasons. Stress levels were elevated. Consuming the wrong foods and drinks. Not exercising. Skipping our doctor visits. All in all, just a year to forget. Social Distancing -Alone Together We are living in times that have never been experienced before. In the past century, there have been just four pandemics. The last one was in 1918. Life, as we know it has been put on hold. Events cancelled, school is now in a virtual landscape, and undoubtedly someone you know has been affected by this disaster. Thanks to modern technology, we are still able to connect with our friends and loved ones either through a Google hangout, Zoom, or House party. In dark times, it is more important than ever to be a beacon of hope for your friends and family. Families all over the nation are spending more time together than they have in years. With the absence of full calendars, practices and other after school activities, there is more time to connect. When this time passes (and it will) most of us will come out better for it; more spiritually connected and definitely closer to our loved ones. Although there is currently no vaccine to prevent COVID-19, the best way to prevent the spread is to avoid being exposed. Those who are in close contact with one another, usually within six feet, spread it from person to person. Respiratory droplets are produced when an infected person coughs or sneezes. The droplets can land in the mouths or noses of people who are nearby or possibly inhaled in the lungs. Another silver lining The County Woman Magazine

is that once you have contracted the virus it is unlikely you can get it a second time. Health officials have said that coronavirus antibodies are able to prevent a person who had the infection from getting it a second time. The best course of action at the present time is to minimize your chances of contacting the virus. We can do that in the following ways: • Washing your hands with soap and water for at least 20 seconds, especially after being in a public place. • You can use a hand sanitizer (if soap and water are not readily available) that contains at least 60% alcohol. • Avoid touching your face with unwashed hands • Sneeze into your elbow • Clean and disinfect frequently touched surfaces daily – this includes doorknobs, light switches, keyboards, and phones.

It is easy to feel helpless during times like this. One way to alleviate those feelings are to try and help where you can and right now our nation is facing a major shortage in blood donations. Donating blood is essential to the health of a community and the need right now is dire. The Red Cross has expressed concern over future blood shortages. If you can, give blood so they can maintain a sufficient blood supply for the weeks to come. Never have we been given this respite from our daily schedules. The stress and anxiety from the unknowns can be daunting. Try and take some time for self-care and making yourself a priority during the days ahead.

Veterinary Services Services Veterinary Peninsula Veterinary Services is a full-service, AAHA-accredited hospital located in Millsboro, Delaware. Owner Dr. April Reid moved from Brooklyn, New York in 2015 to start a practice in this area after spending summers in Rehoboth as a child. Dr. Reid is a graduate of Ross University School of Veterinary Medicine. She previously owned a thoroughbred racetrack practice for 14 years before making the move to small animal practice. Dr. Reid works closely with several local rescue organizations and is currently serving as the vice president of the Delaware Veterinary Medical Association. In addition, she is working towards board certification in veterinary practice. In her spare time, Dr. Reid enjoys spending time with her two Great Danes, Ambrose and Amelia, and her three cats.

ALLERGIES IN PETS Pets presenting for allergies and the resulting skin issues is a common problem in this geographical area. Allergies in both human and pets are sensitivities to things in the environment or food that may not be harmful, but for some reason the individual’s body may see it as such and mount an immune response. Histamines are released and systemic signs such as inflammation, swelling and itchiness begin. Allergies are very common in pets and can develop over time. Allergies tend to present as respiratory, dermatological issues or both. They can be seasonal or non-seasonal and acute or chronic. Humans are most familiar with respiratory allergies during certain seasons of the year and our pets can also be affected in the same ways. Typically pollen, trees, weeds, grass, mold and fungi cause respiratory allergies. The pet may suddenly develop red, watery, itchy eyes and “gun-fire” sneezing. This is uncomfortable for the pet and worrisome for the owner. Acute cases are usually treated with allergy medication or the offending cause is removed and the clinical signs disappear. If the clinical signs do continue, the pet may develop secondary issues such as respiratory infections, which may need to be treated with antibiotics. Though we tend to think of respiratory allergies as developing to sources outside the home, they may also develop to indoor substances such as laundry detergents, home fragrances,

candles, carpets etc. Also remember that homes are not airtight and outdoor air can enter and cause symptoms even in a pet kept completely indoors. Allergies that present as skin issues are a bit more complicated to manage. The pet usually presents to the veterinarian for the first time with a skin infection. The pet may be licking and chewing skin and paws or scratching, shaking and rubbing the ears. As pets are exposed to a variety of things both indoor and outdoor, the pet is usually treated and the symptoms ease. However if the pet’s skin/ears do not improve or the owner notices a seasonal aspect to the skin problems it is possible that allergies are involved.

may not have an actual allergy, but intolerance to certain proteins and ingredients in food. The symptoms seen with intolerance are mainly gastrointestinal in nature. Most owners will switch foods, but a food trial for allergies actually takes 4-6 weeks on a limited ingredient diet with no treats. Ingredients are then slowly added back in the diet until the culprit is discovered. This is labor intensive for the owner and intradermal or serum allergy testing will give an answer much more

quickly with less expense in the long run as pets may be allergic to more than one food item. Uncommon things that pets may be allergic to are dogs to cats (and vice versa), feathers, cigarette smoke, drugs, perfume, cleaning products, rubber and plastic. If you feel your pet has allergies, the best course of action is to see a veterinarian and discuss your pet’s individual health concerns.

Areas of the body most affected by skin allergies are paws, ears, eyes, the rectal area and the abdominal and inguinal areas. Since the pet is itchy, he or she will lick and chew until the skin barrier is broken and that is when a secondary bacterial or yeast infections can begin to affect the skin. In order to find out what the allergen(s) is, allergy testing is available. The gold-standard allergy testing is the intradermal test and is most commonly available through boarded veterinary dermatologists. There is also serum allergy testing which is done from the animal’s blood. Allergy testing is available for both environmental factors and food. Food allergies are less common than owners think, as the animal

Cathy Basenese

ReStore Manager, Sussex County Habitat for Humanity Cathy began her career with Sussex County Habitat for Humanity ReStore in April 2016. It quickly became clear that Cathy had found a second home in her role with ReStore. Over the years she has moved through the ranks beginning as a cashier at the original store in Georgetown, helping to relocate and set up the new store in Lewes as Project Manager which eventually led to her becoming the Lewes ReStore Manager, a position she takes very seriously. During what was a very tough year for everyone, Cathy somehow managed to do more with less; with reduced hours to the public and no volunteer participation for the past several months, she and her team of twelve employees were able to keep ReStore sales and morale strong. They cut the hours of public access to the ReStore to only six hours a day, five days a week. The staff used the additional hours of the day to manage the influx of donations and keep the store and its merchandise sanitized. They were able to continue curbside pickups and drop offs which helped strengthen sales. Sales were so strong, in fact, that the Lewes ReStore was able to use proceeds to build two new homes in Sussex County! You won’t find Cathy resting on her laurels; however, she’s constantly designing and facilitating innovative projects to help promote sales. Cathy attributes the success of ReStore to the staff, clients and amazing support from the community. It’s good to know that during these challenging times, a community can count on someone like Cathy and Cathy can count on her local community.

Michelle Serrano is originally from Lancaster, Pennsylvania her, She and her family moved to beautiful Sussex County Delaware in 2006. She began her employment with Always Best Care in December 2019.

Chris and his brother, Tony, started Budget Blinds back in 2005 and now they and their highly experienced staff cover the southern half of Delaware from just south of Dover on down and three counties in Maryland. Both he and his brother are native Sussex Countians in fact their family has been here for many generations and self employment was a model presented to them early with one grandfather an area farmer and the other owning a shoe shop just off the circle in Georgetown. Growing up in a close family, they were taught the virtues of love, grace, loyalty, and hard work. They were always taught to be accountable to one another, to work first, play later and if it’s worth doing, it is worth doing right. They carry those values into both their personal lives and in the business that they run.

After more than fifteen years of Budget Blinds they have acquired a great deal of knowledge about window coverings and running a business in general but they feel that one of their best assets is the desire to learn more and improve along the way. With only 10 full time employees, including the two of them, and one part time, they have nearly 70 years of experience when combined. That is a particular point of pride because they feel it is an indicator of what really makes a good company, which is employees that love what they do. They know that a person who is enjoying their job, will do it better and with more care. That leads to what Chris and Tony feel is their most important skill and that is choosing employees. “If we make sure our employees and our clients are happy, our happiness will always fall right in line.” says Chris. Chris is Married to his wife Julie who is a speech pathologist for a local elementary school and they have 2 boys, Gavin and Parker who are 12 and 10 respectively. He loves just being around his family, reading, golfing and fishing.

Mike Baker Mike Baker is the founder of C.M.Baker Photography with over 25 years experience. His photography ventures have been quite diverse. Throughout the year you may find him shooting a wedding, a Quinceañera, in the pits of a NASCAR race at Dover Downs and now at front stage at the Firefly Festival or flying over doing aerials. Giving back to the community has always been an important part of it all. Mike was awarded “Volunteer of the Year” from CDCC (Central De Chamber of Comm.) in 2005, ”Volunteer of the Year” from Kent County Tourism 2010,”Chairman’s Outstanding Partnership Award from CDCC in 2010 along with “Business Person of the Year” from CDCC in 2008. Since 1997, Mike has been the “official” photographer for the KC Heart Ball and for the “Go Red” in Kent County since it’s onset. Flowers

Flowers grown in a garden or in containers are nature’s hummingbird feeder. The right combination of wildflowers and exotic fl owers appropriate for your growing zone will attract hummingbirds, increase the number of small insects that hummingbirds require for their survival, and will bring beauty and interest to your garden. The ideal hummingbird flower contains nectar at the base of a single tubular flower; this shape and lack of fragrance discourages insect use. Hummingbirds need to feed every 15 minutes and may visit as many as 1,000 fl owers each day, so flowers need to be numerous and constantly regenerating. Red flowers are popular, but flowers of other colors also contain nectar and attract hummingbirds. Seek flowers with a long bloom period and make sure that a great hummingbird flower is in bloom at every point during your hummingbird season, especially during the spring and fall migration periods. Additionally, it is important to provide “cover” in the form of mature trees and thick shrubbery.

By Heather A. Long, Esquire Nurses, therapists, medical technicians and other healthcare workers put their own health on the line every day as they care for their patients. This statement was put to the ultimate test over the last several months in the setting of COVID-19, as the frontline workers who we count on to take care of us began to fall ill themselves. This makes us examine who is caring for these caregivers. In response to the COVID crisis as it applies to frontline workers, they are protected by workers’ compensation benefits just like every other Delaware worker. The problem arises, however, that currently frontline workers are held to the same standards of proof that the injury occurred in the course and scope of their employment. This means that these frontline workers, who put everything on the line for their community, their neighbors, their peers have to PROVE that they contracted COVID while at work and not in some other manner. This concept is disgraceful and unjust, but for right now it’s all we have.

If you are injured at work, particularly if you work in the healthcare field or are a frontline worker, immediate and accurate documentation regarding the COVID trace contacts is absolutely necessary. In order to make sure that your occupational disease is properly documented, I highly recommend: 1. that you inform your supervisor in writing about your injury; and 2. consult with an attorney to ensure that you will receive all the benefits to which you are entitled. I offer free consultations with no attorney’s fee to be paid until your case is settled. This gives you the peace of mind you need in order to focus on the most important thing- your health.

As of July, fourteen states have enacted or amended their workers’ compensation laws to provide a legal presumption that certain COVID positive frontline workers contracted the virus while in the course and scope of their work. Delaware is not one of those fourteen states and therefore, COVID positive frontline workers here in the First State must still PROVE that they contracted the virus at work. Not only must Delaware workers prove when, where and how they contracted COVID, but as COVID can be considered an “occupational disease” the Statute of Limitations to file this claim has been cut in half from 2 years to 1 year AND you must notify your employer within 6 months of when you discover (or should have discovered) that your occupational disease is connected to your work activities. The unfairness of these stringent reporting requirements and burden of proof that is placed upon our Delaware frontline workers is nothing short of tragic. As with every other aspect of healthcare, documentation and tracking is key to proving that the frontline worker became infected through their work activities. Therefore, it is necessary to inform your supervisor and document the illness right away. Ask your supervisor to fill out a “First Report of Injury” and give you a copy. Also, make a note of any contacts with other COVID positive persons through your work activities, including names, dates and the nature of the interaction. If you are lucky enough to have your workers’ compensation occupational disease claim accepted by the insurance company, you may be entitled to benefits including medical expenses, wage replacement, mileage reimbursement, permanent impairment and disfigurement compensation.

This pediatric dental practice is everything. Starting out with the amazing greetings from the receptionist. Extremely clean and kid friendly. Both dentists are AMAZING! They cater to our every need, no matter the time. They are professionals and speak fluent Spanish, which is a major plus. My kids love them. Switching from another practice was the best choice I ever made.

By far, one of the most professional and kind pediatric dentists I have ever encountered. The entire staff went above and beyond to make sure my children were taken care of, and most of all, went out of their way to make my babies feel comfortable.
