THE ANATOMY OF BREAST CANCER

EARLY DETECTION TO DIAGNOSIS, ALL VITAL FACTS EVERY WOMAN MUST KNOW Breast cancer reigns as one of the leading cancers among women in the world and in the UAE, with women under 40 falling victim to it. Oncologists provide insights into role of AI in early detection, importance of early screening and new age treatments to reduce its incidence. was declared cancer-free and has never been happier to be the perfect mother to her three kids and enjoy family life! “My husband and I were devastated with the diagnosis, but we are absolutely relieved that with early screening we were able to control the outcome. Now, I regularly undergo screening to ensure the cancer never returns!” a beaming Gloria told HEALTH!

What is breast cancer? Gloria Castello was only 35, when she noticed a pea-sized module in her right breast while breast feeding her new-born. Baby James was Gloria’s third child and the young Gloria who had given up her athletic career in marathon running was enjoying her motherhood when this happened. While her gynecologist brushed it away saying it could be attributed to post-childbirth mastitis, the health-

conscious athlete decided to seek a second opinion when the pea-sized nodule felt bigger and harder in a month. The vigilance paid off as, soon after a mammogram, the oncologist softly broke the news – she had Ductal Carcinoma in situ (DCIS)! Her pro-active approach ensured it was stage O! Gloria underwent surgery to excise the tumor and had six cycles of chemotherapy. Within a year she

Breast cancer is a disease that originates in breast cells, characterized by uncontrolled growth, resulting in a tumor. While breast cancer can afflict both genders, it predominantly targets women. According to the American Cancer Society, the incidence of breast cancer has been increasing at 0.5 per cent per year. Shockingly, breast cancer is the most prevalent cancer in the United Arab Emirates, with most of cases striking women under the age of 50.

According to the National Institute of Health, in 2020, breast cancer accounted for 21.4 per cent of all new cancer cases in the region. Knowledge is power, and this statistic underscores the urgency of understanding this disease.

Different kinds of breast cancer

Breast cancer comes in various forms, each with its unique characteristics: Invasive Ductal Carcinoma: The most common type, it develops in the breast ducts and can be either invasive or non-invasive. Ductal Carcinoma In situ: This is a condition that is largely non-invasive but can lead to invasive breast cancer and is limited to the lining of the ducts. Lobular Carcinoma: Originating in the lobules, this breast cancer type increases the risk of invasive cancer if left untreated. Triple-Negative Breast Cancer: This type has a genetic origin owing to the BRCA 1 gene and its mutation. Tripe Negative accounts for nearly 20 per cent of the diagnosed cases of breast cancer and is known to be aggressive with higher risk of metastasis and recurrence risk. Lacking common receptors, it doesn’t respond to hormonal therapy.

Inflammatory breast cancer: This is a rare form of breast cancer that is caused due to blocked lymphatic

vessels that carry the lymphatic fluid. Any rash or swelling on the lymph nodes situated in your arm pits, needs to be immediately screened, as this is an aggressive form of cancer. Other Rare Types of breast cancers include: Angiosarcoma, Phyllodes tumors, and Paget’s disease.

Recognizing the Red Flags

Be in the alert and go in for a detailed screening if you have any of these symptoms: • A lump in the breast or underarm • Changes in breast size or shape • Inverted nipples • Nipple discharge • Skin changes, such as redness or dimpling • Breast Cancer Risk Factors: Know Your Enemy

Early Detection: Your Best Defense

Early detection and diagnosis can be the important difference between life and death for women and it is important women stay vigilant and fearlessly undergo periodic screenings, advise breast cancer specialist.

Self-examination - Your First Line of Defense

Self-breast examination is the first line of defense and women as young as in the age group of 25-35 must conduct self-examination. This also fosters familiarity with breast health. Doctors recommend that all young women in the child-bearing age, conduct monthly breast selfexams By following simple steps in the shower, in front of a mirror, and while lying down, women can maintain breast health and identify early signs of breast cancer, such

examinations every 6-12 months, annual mammograms, and annual MRI scans as indicated. Women can go in for the test for BRCA -1 gene that indicates a genetic pre-disposition to the condition. Genetic counseling is the best line of defense for those at risk, explained Dr. Amal.

Clinical Breast Examination: The Expert Touch Dr Amal Abdelaziz

Consultant and chair of OBG Thumbay University Hospital

Screening and Diagnosis: A Prerequisite for Survival

Dr Amal Hassan Abdelaziz, Consultant Clinical Lecturer and Chair of Obstetrics Gynecology Department, at Thumbay University Hospital, Ajman elaborated, “The UAE’s National Guidelines for Breast Cancer Screening and Diagnosis stipulate that women should commence mammography at the age of 40 and repeat it every two years. However, the frequency and type of screenings may vary based on individual risk factors.” She continued, “For women with an average risk, a combination of breast awareness, annual clinical breast examinations, and biennial mammograms is recommended. Early detection of breast cancer in Stage I increases the chances of survival by nearly 90 per cent!” Meanwhile, women at an increased risk such as those with a family history of breast cancer, should maintain a vigilant regimen of breast awareness, clinical breast

Clinical breast examination (CBE) is a meticulous inspection and palpation of breast tissue, including lymph nodes. Once you are 40 make this mandatory in your clinical screening schedule. This is administered by healthcare professionals; CBE plays a pivotal role in early detection and is an integral part of breast health.

Dr Verushka Mansukhani Specialist General Surgeon, Prime Hospital, Dubai

Why mammograms are the holy grail?

Dr Verushka Mansukhani, specialist General Surgeon, Prime Hospital, who deals with many breast cancer cases in a daily basis, advises every women to undergo an annual mammogram examination above the age of 35 and even earlier in families with a history of breast cancer. “Mammograms are vital tools in the battle against breast cancer. Mammograms have lower radiation exposure than normal X-rays and are designed to exclusively screen breast tissue. While over 60-80 per

cent of all best lumps are found to be benign, the mammogram can easily provide the earliest indication of potential malignancy. The mammogram machine takes detailed images of the breasts that help detect tumors, calcium deposits that can provide early indication of a malignancy,” said Dr. Verushka. Screening mammograms are designed for asymptomatic women, serving as a proactive measure to detect cancer in its infancy. Diagnostic mammograms, on the other hand, are employed when a breast abnormality is detected through clinical examination or routine screening.

Breast Assessment and Diagnosis: Triple Assessment Approach

Triple assessment includes a combination of clinical breast examination, detailed imaging, and needle biopsy. This provides a comprehensive and accurate breast cancer diagnosis and ensures timely and precise identification of potential abnormalities detected during screening. Consult your oncologist and he or she will guide you if you require to undergo the triple assessment.

Diagnosis Navigating the Path

Diagnosing breast cancer involves several crucial steps, beginning with a physical examination by a healthcare provider. Depending on findings, additional tests such as mammograms, ultrasounds, breast MRIs, and biopsies may be recommended. The aim is to catch the disease in its earliest stages for effective treatment.

Likely triggers

Dr Verushka educated women on

the most important risk factors. She said, “While breast cancer can happen to any woman, oncologists have certain trigger parameters.” Dr Verushka enlisted the top ones that are linked to poor lifestyle choices that women make and those that can accelerate a malignancy. • Being overweight or obese • Lack of physical activity • Prolonged use of birth control pills • Excessive alcohol consumption • Delayed childbirth or having the first child after the age 30 • Hormone therapy after menopause • Genetic predisposition or strong family history

Reducing Your Risk: Empowering Choices Take charge of your health and lower your risk of breast cancer through:

• Regular exercise • A balanced diet • Maintaining a healthy weight • Breastfeeding • Limiting birth control pill usage • Avoiding hormone therapy after menopause • Mammography (for those above 40, every 2 years)

Treatment Modalities Unveiling the Options

The journey to conquer breast cancer includes various treatment options: Surgery: Options include lumpectomy and mastectomy, tailored to the extent of cancer. Usually, breast surgeons recommend mastectomy to eliminate the chances of any malignant cells. After that too, a patient undergoes chemotherapy, radio therapy regular screening before she can be declared cancer free. Beyond the first year, breast cancer patients are advised to go in for annual screening and later screening once in two years. Chemotherapy: This includes the administration of certain drugs intravenously that disrupt the growth of cancerous cells. The dose and periodicity of the infusion is determined by the oncologist in consultation with the radiologist. The side effects of chemotherapy is well documented as it destroys healthy cells too, decreasing

immunity and causing weakness in the patient.

Radiation Therapy

Used to shrink tumors and destroy cancer cells.

Palliative and Supportive Care

Providing comfort and relief throughout the treatment process.

Immunotherapy, the new frontier

Tailor-made therapies for individual treatments have revolutionized treatment in breast cancer. These include immune inhibitors that disrupt the malignant cells’ ability to hide cancer vaccines for some cancers like cervical cancer caused by the Human Papilloma Virus (HPV) and adoptive cell therapy that changes the composition of an individual’s leukocytes and nano technology that delivers nanoparticles to combat tumor cells. Immunotherapy has been found to be very efficacious in its outcomes.

Types of Breast Reconstruction

Dr Sanjay Parashar

consutant Plastic Surgeon and chairman of Coccona Aesthetic Clinic

Breast Reconstruction

This is a major surgery that women who have undergone mastectomy after breast cancer opt for, for a psychological wellbeing and restoring of normal contours to the body. Evaluating the need, the time and technique for breast reconstruction Dr Sanjay Parashar, eminent consultant plastic surgeon, chairman of Cocoona Day surgery Centre, chairman scientific of the Emirates Plastic Surgery society told Health, “Breast reconstruction is usually performed after the patient has undergone mastectomy, chemo and radiation therapy. Whenever this surgery is required, I recommend patients consult both their oncology surgeon and reconstruction surgeon together on this, for best result.” Going into the second option of the time of the surgery, Dr Parashar continued, “In some circumstances it is also carried out immediately after mastectomy if the patient is fit enough to undergo it at the same time. In most cases, it is a secondary surgery carried out after the patient has completed the surgery and treatment of breast cancer.”

Oct/Nov 2023

8

Elaborating on the popular options of reconstruction surgery, Dr Parashar said, “There are broadly two kinds of reconstruction surgery. One is after a patient has undergone conservative mastectomy. A lot of tissue has been left such as muscles, fatty tissues and skin which facilitates immediate reconstruction or reconstruction by simple method such as using breast implants.” Citing the other method Dr Parashar continued, “In circumstances where the breast has been completely removed partly with muscles, then we use tissue expander where the skin and tissues are expanded and eventually replaced by breast implants.” Dr Parashar points out to other severe cases when patients who have undergone a more drastic

treatment with complete excision of breast tissue, muscle and skin or the patient has undergone radiotherapy which has destroyed the skin, then the patient requires tissue transfer. “One of the techniques is Latissimus Dorsi flap where part of the muscle and skin from the back of the patient is transferred to the breast region. In more complex cases tissues are also transferred from the gluteal or lower abdomen region. Here a pedicle flap or microscopic reconstruction surgery are the usual choices.” Dr Parashar further pointed out that patients require 4-6 weeks of post-operative care and follow up with both the onco-surgeon and plastic surgeon. He reassured, “After this period the patient can resume normal lifestyle and exercises after the complete healing.”

AI, a modern preemptive diagnostic tool Many AI-powered technologies have come up with state-of-art algorithms that are now providing accuracy and precision in detection of breast cancer in early screenings.

Sara Jamal Al Baba

Clinical Dietician, NMC Royal Hospital, Abu Dhabi

Breast Cancer and Nutrition

It is a well-documented medical fact that obesity and alcohol intake may increase the risk of breast cancer incidence in women. Sara Jamal Al Baba, Clinical Dietician at the NMC Royale Hospital, Abu Dhabi said, “I would advise women to be particularly mindful of their intake of meat, fat, and alcohol. Some studies suggest that an increase of dietary fiber may be able to reduce the risk of breast and other cancers.”

In a study conducted in Sweden the AI-powered screenings were successful in a cancer detection rate of 6 per 1000 screenings compared to 5 per thousand using the standard approach! Breast cancer diagnosis is known to be a very complex process and including AI-powered algorithms will only help radiologists and oncologists sharpen their ability for an accurate diagnosis.

Oncologists are opting for AI-supported mammograms for at least 20 percent higher rate of detection than conventional diagnostic tools. It also had a better result in predicting the future incidence of breast cancer in women which meant that the condition could easily be pre-empted and prevented with timely action.

The cancer-fighting elements in food

She continued: “What they need to increase is more antioxidants, high fiber in their diet through fresh green leafy vegetables, fruits, nuts and seeds among other things. Intake of some Phyto estrogens compounds such as isoflavones found in soy and lignans found in cereals can help correct the hormonal imbalance. For healthy fat, women can opt for oily fish such as salmon and trout in their diets. It would be best to consult a clinical nutritionist to draw up a balanced diet regimen based that is tailor made to your bodily needs.” Remember, together we are stronger, and knowledge is our greatest ally in this journey toward a breast cancer-free future. RIZWAN SAJAN TRANSFORMS HIS DREAMS INTO REALITY

FINDING THE SILVER LINING BENEATH EVERY DARK CLOUD Business leaders come in various forms. Some are born into privilege, inheriting wealth and established family enterprises, groomed to lead burgeoning empires. Others, like Rizwan Sajan, founder, and chairman of Danube Group and one of the wealthiest Indians in the UAE, rely on faith in their abilities and belief in their dreams.

The making of a true leader

Rizwan’s life story reads like a fastpaced thriller or almanac of a true karma yogi. A man who has seen the entire spectrum of life from abject poverty to the heights of wealth, Rizwan is a true grassroots man whose life began very humbly. A recipient of several awards from the UAE government, this much-loved business genius of Dubai, is one of the top 100 richest Indians in the UAE with a net worth of over $ 2.5 billion! Starting work at the tender age of 12, first borrowing a sum of Indian Rupees 1,000 (Dh 50) from his father, Rizwan bought books at a wholesale to sell to his school friends. At the tender age of 13 when Rizwan’s father passed away untimely, it was this entrepreneurial spirit that emboldened him to be breadwinner for his family, delivering milk to homes, selling books and firecrackers.

"It was a relentless battle for survival. By the age of 16, I realized I needed a more stable job and was open to working abroad. I reached out to my uncle in Kuwait for assistance. Initially, he believed I was too young, which left me disheartened. To my surprise, within two years, at the age of 18, he secured me a job as a sales trainee, offering an impressive salary of 150 dinars in Kuwait, a pivotal moment in my life!" Rizwan spent the next nine years climbing the ranks with utmost humility. Sharing living quarters and resources with colleagues, he continued to work diligently. In just eight years, his salary rose to 1,500 dinars. Unfortunately, the Gulf War in 1991 brought an end to this successful chapter, compelling Rizwan to return to Mumbai.

The road to success...

Down but not out, Rizwan arrived in Dubai, the land of golden dreams, to work for a construction brokerage firm in 1991. The enterprising spirit soon presented him with an opportunity to trade in wholesale construction material and in 1993, Rizwan set up Danube as a construction material trading firm. His first employee was his wife, Sameera Sajan. Within less than a decade, Danube building materials that had started off as a small trading firm, evolved and expanded. What You Need to Know About the Second Most Common Cancer in Men? One in every seven men suffers from prostate cancer. Greater awareness and efficient medical intervention can save precious lives as urologists say that the condition is entirely reversible in the earliest stages. head of the department of urology at the Saudi German Hospital, Dubai, the prevalence of prostate cancer is one in every seven men.

Dr Sanjay Bhat

Senior Consultant and Head of department for Urology, Saudi German Hospital

Prostate cancer is on the rise in men because of a lack of basic awareness and education as well as hesitancy and embarrassment about prostate screening, which could help reduce its incidence. According to Dr Sanjay Bhat, senior urologist and

Oct/Nov 2023

"In fact, it is the most common cancer in men, after skin cancer. However, in the early days, it can be impossible to detect – for many with this type of cancer, the knowledge of it comes during screening not because they are suffering any symptoms," said Dr, Sanjay. While the term cancer is scary, it’s important to understand that some types – such as prostate cancer – can be monitored, treated, and recovered from; for those who catch it in Stage 1, the five-year relative survival rate is almost 100 percent.

As we head into ‘Movember’ soon, which is all about raising awareness of men’s health issues each November - we look at the symptoms of prostate cancer, its stages, and possible treatments.

But first, what is the prostate gland?

The prostate is a walnut-sized gland located under the bladder. Its function is to provide prostatic fluid in the semen and has an important role in reproduction. It surrounds the urethra and is responsible for producing seminal fluid, or fluid that carries sperm. In younger men, it weighs 20-25 gm and four cm wide, 3cm high and 2 cm thick. However, with age it tends to enlarge to the size of golf ball going up to 35gm in weight.

At what age should you be tested?

Dr Sanjay said it was advisable for every man to opt for Prostate cancer screening after the age of 50. Once cleared, he can undergo the screening in an interval of 2-3 years. In many men who may have a higher risk because of genetic family history of prostate cancer, it’s a better idea to start getting checked once they turn 45.

Causes

While the cause of prostate cancer remains a mystery, here’s a look at some factors that increase the risk of disease: Age: Risk of this sort of cancer rises with age. American Urological Association (AUA) states that harm to the DNA of prostate cells is more likely for men over 55. Ethnicity: One in six AfroAmerican men are diagnosed with prostate cancer every year. Family History: If you have a close relative with the disease – father, brother, son – your risk for getting it also increases. Similarly, for men with family members who have breast and ovarian cancer there is a greater risk for the disease. Weight: Being overweight can up your risk for a number of things, including this type of cancer. Studies now link being overweight in one’s 50s to greater risk of advanced prostate cancer.

Early detection is key to saving lives.

American Urological Association classifies this type of cancer into four stages, namely: Early-stage or Stages I and II: In this case, the cancer hasn’t spread

past the prostate. It is also called ‘early-stage’ or ‘localized’ prostate cancer. Locally Advanced or Stage III: In such cases, the cancer has spread but only to nearby tissues. Advanced or Stage IV: Here, the cancer has spread to other parts of the body such as lymph nodes, bones, or lungs. Dr Sanjay said, “It is possible to provide long lasting cure when prostate cancer is diagnosed and treated in stage I and II. The fiveyear survival rate for most people with early stage of prostate cancer is almost 100 percent, while for those with advanced cancer it’s at 32 percent. Doctors therefore advise men to check their prostate regularly and get diagnosed early.

Symptoms

US-based Mayo Clinic lists symptoms of advanced prostate cancer as: • Trouble urinating • Decreased force in the stream of urine • Blood in the urine • Blood in the semen • Bone pain • Losing weight without trying • Erectile dysfunction

How is prostate cancer checked for?

A doctor may conduct one or more of the following tests: Prostate-specific antigen test (PSA): This is a blood test that counts the prostate proteins in your blood; an abnormally high level indicates prostate cancer. Digital rectal exam (DRE): This is a physical exam where a doctor uses a lubricated, gloved finger to feel for any bumps, rigid or enlarged areas of the prostate.

Prostate imaging

Doctors can now use advanced imaging techniques such as an MRI or ultrasound to detect cancer in the prostate. If the doctor suspects cancer, he may recommend a biopsy; a small sample of the prostate tissue is taken for analysis. For those who undergo a biopsy, they will be given a Gleason score – which classifies how many cells look like cancer and how aggressive their growth appears.

Oct/Nov 2023

25

Factors that affect prognosis and treatment options

The prognosis and treatment option, explains US-based National Cancer Institute, depend on: The stage of the cancer (level of PSA, Gleason score, Grade Group, how much of the prostate is affected by the cancer, and whether the cancer has spread to other places in the body).

Treatment types

Active surveillance, surgery and radiation therapy are the standard therapy choices for treatment of early prostate cancer. Other treatment modalities include: • Hormone therapy • Chemotherapy • Targeted therapy • Immunotherapy • Bisphosphonate therapy in case the cancer has spread to the bones. Cancer is a big word, with big real-time consequences. However, it’s not undefeatable. What’s important to note is that with vigilance and screening, it’s not only possible to catch but also to treat it effectively. Remember, early detection can save lives.

Professor Dr Rabi Madi

Urology consultant, Director of Robotic Surgery at Clemenceau Medical Centre Hospital in Dubai

Robotic Surgery: The preferred option

One of the most popular and widely accepted modes of surgery for prostate removal is robotic surgery which has been on the medical scene for at least two decades. Patients are opting for it, because of its small precision incisions that involve only a day-care procedure with less blood loss, therefore minimal risks, shorter recovery time and better outcomes. Professor Dr Rabi Madi, Urology consultant Director of Robotic Surgery at Clemenceau Medical Centre Hospital in Dubai provides a comprehensive explanation of this preferred option. Professor Dr Rabi trained for robotic surgery in 2004 in the US and practised for three years before taking on the procedure. Over the years, he has acquired the practised skill and excellence in doing Robotic surgeries, and is the most sought-after-surgeon on account of his expertise.

Best treatment for radical prostatectomy

Explaining the procedure involved in the surgery, Professor Dr Rabi said that every patient of prostate cancer who was advised surgery is fit for this procedure and the option was the safest one for early-stage prostate cancer. “Treatment for prostate cancer is through radiation therapy or surgery. Therefore, any patient of prostate Oct/Nov 2023

26

cancer who is advised surgery qualifies as an ideal candidate for robotic radical prostatectomy. Professor Dr Rabi added that the surgery had gained credence over successful clinical studies that gave a very positive report on it. “The reason why the surgery is popular is owing to the accuracy and precision that it commands. In the last 20 years, the methodology and the tools have evolved over time and become far more sophisticated. Clinical trials and research have indicated that robotic surgery in relation to conservative or traditional open or laparoscopic surgery, had a far superior outcome.”

Practice and skill of the surgeon count

However, Professor Dr Rabi sounded a word of caution. He said the successful outcome of the surgery depended a lot on the skills of the surgeon at the controls. “Robotic surgery is advanced laparoscopic surgery. It is minimally invasive as the surgeon makes small holes of around 5mm and introduces a 5mm camera and robotic instruments in the patient’s abdomen and he removes the prostate the same way as we do in open or laparoscopic surgery. However, because of the high definition 3D camera and because the instruments that articulate like our hand without any tremors, there is better precision and better vision and provides great outcomes for it. The surgeon totally controls the arms. It is a complex surgery, and a steep learning curve; it cannot be learned overnight. It requires several months of training and after that good practise to master it.” H

Helped this Man Reverse Diabetes

Is it possible to reverse Type 2 Diabetes by following a diet and fitness regimen? New research and several active case studies point out that the right amount of nutrition coupled with regular exercise can work wonders. Oct/Nov 2023

28

Oscar Dsouza Before As we observe the world diabetes day on November 14, the alarming rise of the diabetes epidemic globally and regionally has become one of the prominent global health concerns. The International Diabetes Federation reports a staggering 537 million people affected globally,

After with 73 million residing in the MENA Region alone. In the UAE one in five individuals is reported to have an onset of Type II diabetes.

What is Type II diabetes? Diabetes Mellitus (Type II), a chronic metabolic disease, presents a complex challenge,

If this remains unchecked, then the high blood sugar results in damaging other organs, triggering many other health complications including diabetic coma and death.

from excessive thirst, frequent urination, constant hunger, fatigue, blurred vision, numbness or tingling in extremities, slow-healing sores, and unexplained weight loss. The good news is that diabetes can be managed and even go into remission, provided one is committed towards improving blood sugar levels with proper nutrition and exercise, said Dr Anil Kumar, consultant endocrinologist at Prime Health Care Group.

What is HbA1c?

marked by persistent high glucose levels in the blood. The body requires glucose absorption and assimilation in the cells for energy. However, when the hormone insulin is either underproduced in the pancreas or the cells are unable to absorb sugar due to insulin resistance, there is a unusually high level of sugar circulating in the blood. This is also caused when cells suffer from insulin resistance and are unable to absorb insulin so crucial for glucose metabolism.

HbA1c or glycosylated hemoglobin is an indicator of an individual’s blood sugar levels for three months. The hemoglobin protein in our red blood cells combines with a molecule of glucose and turns into glycosylated hemoglobin. In non-diabetics, the HbA1c reading is between 5.2-5.5. It is about 5.5 to 6 in pre-diabetics and in cases of people diagnosed with Type II diabetes the HbA1c reading can be anywhere between 6 -14 or higher. The higher the HbA1c, the more severe the diabetic condition in the patient.

Causes and Risk Factors

Type II diabetes usually runs in the family. However, environmental and lifestyle factors such as obesity, physical inactivity and poor nutritional choices such as ingestion of fast food and high sugar food play an equally important role in triggering it..

Symptoms

You can go in for a blood sugar test when you notice that you suffer

Case study

Dubai resident and Indian expatriate Oscar Dsouza did the impossible. His HbA1c reading that was 10.5 came down to 5.5 within a matter of six months. His weight dropped from 88 kg to 68g progressively in couple of years which he has maintained now. Oscar, 52, decided to consult an endocrinologist when he started feeling very tired and exhausted. A routine blood test revealed his fasting blood sugar was 180mgdl where a normal range is between 90-100 mg/dl. After consulting Dr Anil Kumar at Prime at Prime Hospital, and being counselled on lifestyle changes, Oscar decided to implement those changes right away. Oscar told , “I began taking the diabetes medication; however, I was determined to follow a health routine and began walking 10,000 steps a day, having healthy meals and doing moderate exercises.” Oscar’s wife Sunita, worked on his nutrition. All processed food was cut out, meal portions were reduced, and cooking modes opted were boiled, steamed, broiled, stir-fried to cut back on calories

Reverse diabetes, avert health complications.

Nutrition, fitness the main mantra

Oscar added, “Previously, rice was my staple and I relished it with red meat. My meal portions had always been large but now I not only cut back on rice and red meat, I ensured my portion sizes were smaller too. I stuck to a three-meal plan with the last meal at 7 pm.” Oscar detailed his meal plans. “My wife tweaked my daily meal portions and i was content to adhere to the new diet. She introduced millet-based idlis (steamed millet cakes), oats porridge and other fresh and wholesome foods for my 7 am breakfast. These helped my blood sugar stay steady. For lunch she introduced a large portion of salad accompanied with ragi chapatis and vegetables. I gave up rice, which was my absolute favorite. Dinner, now, is always at 7 pm which is also frugal with a chapati, a vegetable, a small portion of lentil and an occasional fish or chicken dish. Red meat has been taken off the menu. In addition, I walk for 45 minutes every day, six days a week.”

A miraculous recovery

This progressively improved Oscar’s condition and within three months his HbA1c dropped to 7.3 and his weight started falling off. Oscar was overjoyed and he continued to work hard. Within six months of regular diet and exercise schedule, Oscar's blood sugar levels dropped to normal. His fasting blood sugar readings came down to 94 mg/dl. His HbAIC was down to normal and he was completely able to go off diabetes medication by the end of 2019. Something that he has been able to consistently maintain after the wake-up call.

Raising hopes for all diabetics, Dr Anil explained: “Once you go in for a proper diagnosis, with mindful eating and regular exercise, it is possible to reverse Type II diabetes. The entire family needs to adopt a healthy lifestyle to be able to influence children to do activity that can lower body weight. The benefits are multi-fold and include improved protection of vital organs such as heart, kidney and add years to one’s life.” Dr Anil said he advised most of his patients to aim for a 15 per cent weight loss in the beginning. “If an individual is successful in knocking off 15 per cent of body weight, he or she can be assured of enormous health benefits. The benefits include fewer medications, better blood glucose levels, improvement in blood pressure, and a lower risk of cardiovascular and kidney disease. H

swelling and stiffness in joints. According to the US-based Center for Diseases Control (CDC), about 24 per cent of all adults in America have arthritis. And in the UAE and Gulf countries, the Middle East Foundation puts the prevalence of the condition at about 20 per cent. Arthritis can start as a mild inflammation in a particular joint leading to stiffness and immobility of that joint so much so that it can have a debilitating impact on life. Early intervention with physiotherapy and now, new regenerative therapies can greatly help. The HBG Medical Centre was formed to specifically address the needs of different kinds of arthritis

that can occur in the joints, as well as the auto-immune type that has nothing to with age and the juvenile idiopathic variety that occurs in children below 16 years. Speaking to HEALTH Dr Humeira, the leading Rheumatoid Arthritis (RA) expert and founder director of the Middle East Arthritis Foundation explained, A rheumatologist is a doctor who specializes in arthritis, bone and joint diseases. There are over 100 different categories of arthritis including gout, rheumatoid arthritis, fibromyalgia, gout, scleroderma, lupus, psoriatic arthritis, ankylosing arthritis, back pain and so on. When patients consult us, we examine using the latest ultrasound bedside scans, order blood tests and post diagnosis, prescribe medications to control the pain and inflammation. Patient comfort and trust is priority for us.

Patient-centric Approach

Dr Bhavna elaborated “Our clinic is patient-centered, and we came into existence because as specialist doctors we felt we needed to put the patient’s needs first. We believe in strong communication where we involve the families of patients and work towards treating patients just the way we would like ourselves to be treated.

Aesthetic Interiors

As you walk in the clinic, what is striking, are the tasteful and aesthetic interiors where the walls are adorned with art done by world’s leading artists who suffered some kind of auto immune disorder. Post registration, the patient can meet the specialist. Each doctor’s station is equipped with state-ofthe-art ultrasound scans where the doctor can immediately screen the patient, leading to quick diagnosis and redressal.

and boosts it in a powerful way. It’s a treatment protocol that more and more patients are turning to, and more and more physicians are offering. We are perhaps the only clinic to offer a spectrum of regenerative therapies to address niche needs of our patients using Ortho Biologics, a new frontier in regenerative medicine,” Dr Humeira elaborated.

The Power of Regenerative Therapy

While patients might despair about having to live with painful arthritis,

Dr Humeira pointed out that there has been a paradigm shift in the clinical approach to treatment

Dr Humeira explained how their clinic has ushered in a new hope by deploying the latest therapies in regenerative medicine. “Regenerative medicine is an advanced and innovative alternative to surgery (or adjuvant to surgery) that triggers your body’s own ability to heal itself

of bone and joint diseases. She explained, “In the realm of musculoskeletal diseases, there is a growing shift to seek regenerative solutions instead of surgery as a panacea for these persistent issues. The results are very positive. At our clinic we have introduced a

Dr Ghita explained, “The core values that each one of us follows at the clinic is excellence, integrity, compassion, respect, and teamwork.” Apart from the three RA specialist, Dr Sharmila Tulpule, the consultant orthopedic surgeon and Dr Sri Lakshmi Hegde, the physiotherapist with her team of therapist can help you with the regular exercises required to rehabilitate the affected limb or joint and build up strength.

What is Ortho Biologics?

Ortho Biologics are natural substances such as cells, tissue, blood components, and growth factors that are harnessed to aid in the treatment of orthopedic injuries or conditions. They may be used to replace lost tissue, stimulate regeneration and healing, reduce pain and inflammation, and improve joint function. Ortho Biologics may be obtained from your own body or from a donor. They may be administered in

the form of injections or may require a surgical procedure. They may be used alone as a conservative treatment or following orthopedic surgery to enhance results.

What are the Conditions Treated by Ortho Biologics? Ortho Biologics can be used to treat: • Osteoarthritis • Articular cartilage injuries • Degenerative diseases • Ligament injuries • Tendon injuries and degeneration

Different Kinds of arthritis Genetic and lifestyle triggers Age, genetics, and poor lifestyle pattern are some of the main reasons for different types of arthritis. Some of the major varieties are: Osteoarthritis This is most common type of arthritis, and it begins with the roughening of a cartilage which results in pain and stiffness: • Tiny bits of extra bone, called osteophytes, can grow at the ends of a bone within a joint. • There can be an increase in the amount of fluid inside the joint. • The joint capsule can stretch, and the joint may lose its shape.

Oct/Nov 2023

34

Gout

This inflammatory arthritis causes swelling in joints – it typically affects the big toe and is caused by too much uric acid in the system. The affected joint might become red, hot and peel.

Rheumatoid arthritis:

In this type of arthritis, the body’s immune system begins to attack its own healthy tissues such as joints, causing inflammation.

Symptoms of rheumatoid arthritis can include:

• swollen and tender joints • swelling and stiffness in joints in the morning, which lasts for longer than half an hour • fatigue Spondylarthritis: This refers to a group of conditions that cause pain and inflammation, mainly around the joints of the spine.

Juvenile idiopathic arthritis (JIA): This occurs in children before they reach the age of 16 and could be attributed to autoimmune triggers. Post-traumatic Arthritis: The American Academy of Orthopedic Surgeons, explains that “post-traumatic arthritis is form of arthritis that develops after an injury to the knee”.

Psoriatic arthritis

This is a chronic skin and nail disease which starts with a skin rash leading to itchy, red, scaly skin and later leading to soar, inflamed joints.

Ankylosing Arthritis

This may have a genetic trigger resulting in early morning back pain, stiffness and in later stages resulting in a stooped posture. As can also cause fatigue, fever, anemia and digestive complications to name a few.

When is Ortho Biologics recommended?

Ortho Biologics may be recommended when rest, medications, and physical therapy fail to relieve symptoms.

Types of Ortho Biologics The different types of ortho Biologics that the HBG Medical Centre offers are:

Platelet Rich Plasma (PRP) PRP is obtained from your own blood after it has been processed to concentrate platelets and certain white blood cells that enhance healing and reduce inflammation. PRP may be injected directly into the damaged tissue during an outpatient procedure or following repair surgery. Over 440 randomized and controlled

trials in PRP indicate its efficacy in ameliorating symptoms knee arthritis giving patients an alternative to knee joint replacement. This is by far the most preferred treatment and Dr Humeira gave details of the process, “During the initial consultation, we will review your medical history, perform a physical examination,

and order any necessary imaging tests. During the procedure appointments, the patient’s blood is drawn, processed to separate the platelets and prepared. A numbing medication is applied sparingly to offset discomfort or pain from the injection. The blood containing the enhanced with the concentrated platelets is injected into the affected joint. Unlike cortisol and other injectable medication, PRP is a natural substance derived from your body and we have had very positive feed back from patients. The entire procedure takes no more than an hour.” Bone Marrow Aspiration Concentrate (BMAC) BMAC is obtained after processing bone marrow that is removed from your pelvic bone. BMAC is rich in stem cells, which produce proteins and growth factors necessary for healing and reducing inflammation.

Adipose Tissue (Fat)Adipose tissue or fat cells have remarkable healing and regenerative properties. It is typically obtained from your waist, processed to remove impurities and inflammatory components, and then injected at the site of injury. With such promising new therapies, introduced by HBG Medical Centre, the management of the entire spectrum of arthritis looks far less daunting. Patients can expect tailored remedies to suit their condition. H

Without much ado, we dove straight into our starters – the Aloo Tikki Sanja Chulha and the Zaika Murgh Tikka, both from the Punjab region. Sanja Chullah refers to the traditional community kitchen in the villages of Punjab and the name itself transported us to the hamlets of this verdant green state. The potato patties, pan-fried, were immaculate in their golden hue and crispy texture accompanied with soft chickpeas. The zaika murgh tikka was the Gharana version of traditional boneless chicken roast. It got full marks for its tenderness, exotic spice marinade and smoky flavor. Both turned out to be ideal starters to the accompaniment of tangy mint chutney and onion juliennes. Starters only whetted our appetite for the main course. As the evening Oct/Nov 2023

unfolded to the leisurely notes of melodious music, the tastebuds were craving for an exciting culinary adventure. Next, we moved to the main course and picked delicacies from two states We ordered a chargrilled mashed eggplant-and peas or ‘baingan bharta’ from Hyderabad and the famous Nalli Nihari mutton stew from Lucknow. For a traditional lentil accompaniment, we chose Dal gharana – a signature lentil broth of soaked black dal, tempered with spices, and slowly cooked for several hours to get a rich, creamy consistency. A yoghurt raita was another dish that was ordered as a cool palate cleanser complementing the fiery flavors. One cannot move ahead without a paean to the glorious Nalli Nihari, a rich lamb shank stew which is a delicacy specially cooked for over four hours. Nihar means dawn and this dish is said to have been originally created for peasants to be put on a slow fire to stew all night and eaten at dawn. It was said to energize the peasants for the entire day. Later, The succulent mutton shanks, blended with the special nihari spices became a delicacy to be served to the

Mughal Kings. The Gharana chefs did full justice to the dish by maintaining a rich balance of delicacy and consistency. The assortment of breads, crisp from the open oven, completed this delightful food narrative that seemed to flow without any punctuation, tantalizing our olfactory and tastebuds. On a sweet note, saffron-flavored kesar kulfi falooda (Indian style noodles) garnished with rose jelly and slivers of pistachio and cashew nuts, was the fitting finale to this gastronomic journey. As the festive Diwali season approaches, Gharana and other restaurants of Holiday Inn Al Barsha offer innovative themed concepts for outdoor catering. Anyone wanting to relive the grandeur of the rich Mughal era and indulge the taste buds, Gharana is a restaurant you cannot miss, It offers the perfectly balanced Mughlai meal experience, an unmissable opportunity for the uninitiated. An Onam Feast at Veg Corner, The Terrace “Pachadi” provides a cooling balance, a yogurt-based side dish with vegetables like cucumber, beetroot, or pineapple, combined with coconut, mustard seeds, and green chilies. To intensify the flavors, a variety of pickles, such as mango pickle, lime pickle, or garlic pickle, are served, adding a spicy twist. For a crispy texture, there are “Papadum,” thin, crispy lentil wafers, and “Banana Chips,” salty and crunchy chips made from bananas.

I was recently invited to sample a lavish Onam feast, served in a very traditional style, which happened to also be my first experience with this special meal. Onam, a vibrant festival in Kerala, India, is celebrated with a grand feast known as the “Onam Sadya.” This feast is a culinary extravaganza served on fresh banana leaves, featuring an array of delectable vegetarian dishes.

the spread, a stir-fried vegetable dish featuring grated coconut and mild spices. For a more delicate flavor, there’s “Olan,” a simple yet delicious curry prepared with ash gourd, pumpkin, and black-eyed peas in a coconut milkbased sauce.

At the heart of the meal is simple, steaming white rice, served hot and plain. One of the stars of the Onam Sadya is “Sambar,” a flavorful lentilbased stew bursting with vegetables like drumsticks, pumpkin, and shallots. Complementing the sambar is “Rasam,” a spicy and aromatic soup made from tamarind juice, tomatoes, and a medley of spices. It serves as a flavorful appetizer and aids digestion. “Avial” takes center stage as a mixed vegetable dish cooked in a luscious coconut and yogurt-based gravy.

“Injipuli” offers a spicy kick, a sweet and spicy ginger-tamarind chutney that adds an exciting contrast to the meal.

“Kalan” brings a unique tanginess, with its thick and tangy curry made from yogurt, coconut, and either raw plantains or yam. A sweet and sour note is struck with “Pulissery,” a curry featuring ripe mangoes, yogurt, and coconut, seasoned with mustard seeds and red chilies.

The meal culminates with sweet delights like “Payasam,” a creamy dessert made from milk, sugar, and either rice, vermicelli, or lentils. The use of a banana leaf as the serving vessel isn’t merely a tradition; it’s a sustainable practice that aligns with Kerala’s eco-conscious ethos. This eco-friendly approach not only reduces environmental impact but also emphasizes the importance of living in harmony with nature, a principle deeply embedded in Kerala’s cultural heritage. Moreover, the Onam Sadya finds its roots in Ayurveda, the ancient Indian system of medicine and wellness. The carefully curated vegetarian dishes are not just about tantalizing taste buds; they are crafted to balance the body’s doshas, promoting holistic health and well-being.

Train Your Sight on top Eye Health Remedies!

The onslaught of LCD lights, pollution, heat, humidity, fine sand particles, can trigger eye infections. Eye specialists offer advice on how you can preserve your sight and maintain it in glowing health.

As we engage in outdoor activities, our eyes become susceptible to various risks. From simple styes, to conjunctivitis and other inflammation, your eyes are one of the most delicate organs that need to be conserved. Understanding Ophthalmic Irritations

Ophthalmic irritations encompass a range of eye conditions, with conjunctivitis and styes being common culprits. High humidity levels, lead to perspiration and accumulation of secretions on our eyelids, causing clogged oil glands and painful red bumps known as styes. Additionally, fine sand particles commonly found at beaches and playgrounds pose

an invisible threat to our eyes. This highlights the importance of wearing protective eyewear to prevent eye abrasions and infections. The most common inflammation in the eyes is conjunctivitis or “pink eye,”. This is characterized by inflammation of the clear tissue lining the inner eyelids and the outer eye surface. Styes, on the other hand, result from bacterial infections of the small glands near the base of eyelashes.

Eye health in digital age? Dr Mahmoud Hegab, Retina Specialist at the Thumbay University Hospital highlighted the detrimental impact the artificial light from the Liquid Crystal Display (LCD) screens had on your eyes. “Most of us spend a lot of time staring at the phone or laptop screen. These screens emit blue light which has the highest energy wavelength of visible light, so much so, that when we stare at the screen for a long time, it can penetrate

all the way to the back of the eye, through the eyes’ natural filters. The rapidly increasing amount of blue light exposure that we get each day through digital device can cause permanent damage to our eyes.” Most of us are so mesmerized by the moving action on the screen, that we stare continuously without blinking resulting in straining our eyes, suffering from eye fatigue and dry eyes which can lead to other problems. Teenagers who play on video games, i-pads, and other electronic instruments without the supervision of elders often complain of double-vision, migraines and extreme eye fatigue. Dr Hegab pointed out that in the modern world, dominated by screens and artificial lighting, preserving our vision has become very crucial. Sight degeneration or impairment can significantly impact one’s quality of life, affecting daily activities and overall well-being. Cultivating healthy habits that promote eye health should be a priority for everyone.

Importance of wearing sunglasses year-round

Dr Hegab adds while many associate sunglasses with summer, protecting your eyes from harmful sunrays is a year-round commitment. "The sun’s rays can lead to retinal burns, cataracts, and other serious eye conditions. Investing in quality sunglasses with UV protection is a small effort that can yield significant benefits."

Protection from smoke, water, dust, or wind at their workplace is crucial: Dr Hegab emphasizes that if your workplace exposes you to these elements, wearing protective goggles is imperative. Shielding your eyes from potential hazards ensures they remain in optimal condition, even in demanding environments.

Why hydration is crucial for eye health.

Dr. Mohamed Ahmed Wassfi, ophthalmologist at the Thumbay University Hospital, explained, “Excessive screen time leads to digital eye strain or computer vision syndrome. Prioritizing outdoor activities and screen-free time to refresh eyes is vital.”

Regular eye-checks crucial.

Both Dr Hegab and Dr Wassfi shed light on the importance of routine eye examinations, which are crucial for early detection and prevention of eye conditions. They pointed out that people with eye problems must avoid relying solely on over-the-counter medications, as these may mask an underlying critical issue. “Consulting an eye care professional ensures that any concerns are addressed promptly and effectively. Routine eye examinations, at least once a year, are crucial for early detection and prevention of eye conditions,” said Dr Wassfi. Dr Hegab sounded a word of caution for people who had genetic predisposition to life-style diseases. “People who have a family history of diabetes or high blood pressure must consult their eye care professional more frequently to ensure that any potential issues are addressed promptly.”

Good eye care will conserve eyesight and eye health, and everyone is advised to be mindful.

Eye Care: Dos Always wear sunglasses with UV protection. Use lubricating eye drops if needed but consult an ophthalmologist first. Maintain proper hygiene, especially for children. Seek immediate medical attention for any eye-related symptoms.

Don'ts Avoid rubbing your eyes. Do not share towels. Stay away from dusty areas. Avoid wearing contact lenses whilst swimming as lenses can absorb water and trap bacteria, virus and other pathogens in the eye H

Dr Wassfi also emphasized on the need for maintaining the moisture balance in both eyes. “Dehydration can lead to dry eyes. Without adequate moisture, our eyes get more prone to infection, itching and can also lead to damage of delicate structure of the eye including the cornea. It is important that young school going kids and adults are mindful of rehydrating themselves with at least 6-8 glasses of water and other rehydrating liquids such as coconut water, lemonade, freshly squeezed vegetable juices and the link to keep the one’s eyes adequately hydrated.”

Consumer Shift to Gen Z and Upcoming Generations Over the past decade, Aafiya TPA has been in the business of simplifying the lives of our consumers, while also closely observing the evolving landscape of healthcare. Throughout this time, we have witnessed significant transformations in the healthcare industry and have adapted our services to meet the changing needs of our members. term baby boomer was coined after World War 2 in the US, where the world saw a sudden spike or boom of babies during this era.

Ali Zaidi

General Manager, Aafiya

In a rapidly changing healthcare industry like ours, it is crucial to have a deep understanding of the needs of the target audience we serve. To achieve this, we must delve deeper into the unique healthcare needs and preferences of each generation. By doing so, we can ensure that we are providing the right care and services to meet the evolving needs of our customers. Join us as we explore the healthcare needs of different generations and how they are shaping the future of healthcare.

The dependants The Generation that was born between the mid-1940s to mid1960s is called baby boomers. The

Baby Boomers have been recognized for their loyalty to traditional institutions and values relationships. We have witnessed these attributes in our grandparents and parents. When it comes to their healthcare needs, unfortunately, the health insurance companies are yet to identify a correct mix to cater to the needs of these generations and if at all they can take an insurance policy, the underwriting guidelines are very stringent that they end paying huge premium eventually relying on their savings and children.

Current target audience The generation born between 1965 to 1979 falls under Gen X. They are

the first generation to experience the internet, they are tech-savvy and understand the value of technology, but they are not dependent on it. For instance, despite the dominance of teleconsultations, many individuals of this generation still prefer to see doctors in person. They may book appointments online and request medicine delivery, but they still desire a personal connection. Similarly, although they have access to technology for online transactions, they often opt to visit a bank to complete these transactions. We can safely say that so far the security relying through technology has yet to win the trust of this generation. When it comes to health insurance, this generation looks for basic healthcare needs and is utilizing insurance for the same. Currently, the active workforce of this generation is 35% at a global level.

and were willing to spend timeconsuming longer-form content, Gen Z prefers quick, visually appealing content that can be consumed on the go. Considering the behaviour of this generation, many brands are moving from the traditional way of working and adapting to the environment which is more appealing to the next generation.

Followed by the most talked about generation, that is the millennials, the generation born between the 1980s to 1996, The generation that grew up when social media was booming making it accessible for the consumer to reach out to business. With access to information, this generation is more likely to seek health insurance through online channels. Many millennials prefer to research and purchase health insurance through websites. They may also turn to social media and online reviews to gather information about different insurance providers before making any decision.

Potential target audience Setting all the generations apart, Gen Z, born after 1997, has grown up with technology as an integral part of their lives. Gen Z’s approach to consumption is based on the value and usability that a product or service adds to their lives. For instance, they prefer subscription models like Netflix, where have the freedom to choose what to watch and when to watch it. the current generation has a shorter attention span and tends to consume content on social media in bite-sized pieces. While earlier generations were more detail-oriented

As an insurance industry, it is important to recognize and address the unique needs of this techdependent generation. By providing more choices and personalized offerings, can we become the Netflix of insurance and stay relevant in this fast-paced era? That being said, 75 percent of the total workforce will be a blend of millennials, and Gen Z by 2030. Gen Z differs from previous generations in terms of their insurance needs and preferences From virtual reality to digital twin technology, the healthcare sector is creating waves across the globe, Artificial Intelligence (AI) is reshaping healthcare by assisting in diagnosis and providing personalized healthcare insights. Blockchain, addressing privacy concerns, ensures secure health data management. Additionally, Augmented Reality (AR) and Virtual Reality (VR) enhance medical education, offering interactive and engaging learning experiences. Chatbots and virtual health assistants powered by AI provide instant healthrelated information, catering to Gen Z’s need for prompt responses.

Key differences are as follows:

1. Adapt Changes – Gen Zs are more likely to switch plans if they find a better deal elsewhere.

2. Digital over Traditional – The

current generation prefers alternative healthcare options, such as teleconsultation and wellness program.

3. Preventions over cure – Today’s

generation takes prevention measures to stay fit and healthy.

4. Quality over convenience-

Whilst older generation give preference to reputation, quality of service, and credentials of doctors, that is not the case with the current generation.

In conclusion, AI, blockchain, augmented reality, virtual reality, and health applications in the healthcare sector are changing healthcare for Gen Z. These developments fit well with the fast-paced, technologically savvy lifestyle of Gen Z, setting the way for a future that’s healthier, more informed, more digitally sophisticated. H

ASK THE EXPERTS From pediatric questions to general medical queries, our panel of experts are here to answer your questions.

Dr Amla Khopkar Nazareth

Specialist Gynecologist and Obstetrician, PRIME Healthcare Group

Q: What are the routine gynecological screenings that every woman must undergo annually after the age of 40, to rule out breast, cervical and ovarian cancer? How can these screenings make the difference? We have all heard the adage that prevention is better than cure. However, there is a thin line between preventative measures and paranoia. These days we are bombarded by information and feel wise beyond our age. I would say women must use that wisdom to understand that there is a schedule to get screened regularly for Breast / Ovary / Cervix cancers.

Ovarian checks should be carried out annually under a gynecologist after the age of 40. On advice of your gynecologist, women also need to undergo the BRCA gene screening, only once in their lives. This will be recommended by the gynecologist who will arrive at the decision after reviewing the family history. Genetic mutation of BRCA GENE causing hereditary breast cancer. Go for subsequent follow-ups as recommended by your gynecologist. As for cervical cancer, women must know that it preventable with vaccination. Make sure you are vaccinated to protect against the Human Papilloma Virus (HPV). Women need to take the course if they haven’t received it before and are between 25 years to 65 years of age. The vaccination is administered to all girls and boys from age 9 onwards till 25 years of age.

A – Self-examination- up to twice monthly

Even if one is vaccinated, the screening for cervical cancer must continue. The screening schedule and duration is as follows:

B - Breast Surgeon visit - Annually after the age of 40.

1. From the age 25 onwards till age 50 years every 3 years

C - Screening mammogram- annually or as directed by the Breast Surgeon after the age of 40.

2. Above the age of 50 till 65 years - every 5 years

For Breast

If any abnormalities like lumps or puckering of skin over the lump noted than that warrants an examination by the breast surgeon even if you are below the age of 40.

3. Voluntary withdrawal from screening can happen if a woman is pregnant or has never consummated her relationship, or when a woman is above the age of 65 years and not sexually active.

Q: Is it possible to reverse Type II Diabetes with a balanced diet? Can you share an ideal menu that highlights the absolute must-haves and food items to be avoided in our regular meals? In Type II Diabetes the body has poor insulin absorption, but with healthy eating it is possible to control blood glucose in combination with prescribed medications. The easiest way to plan a diabetic meal is to follow the Plate method. This implies, fill half your plate with non-starchy vegetables, a quarter with lean protein, and one quarter with carbohydrate foods like whole grains, starchy vegetables, or fruit. While it would be difficult to provide a sample menu that suits all, as each one has personalized nutrition requirements it is always better for diabetics to plan meals to achieve better blood sugar control. Most meals must include lean protein such as ground turkey and chicken; for non-starchy vegetables, it ideal to include salad greens, onions, collards, and yellow squash. In the carbohydrate foods, one can recommend sweet potatoes, small portion of potatoes, fruits such as oranges. This is an illustrative sample. It is always advisable for diabetics to consult a clinical nutritionist to get a proper meal plan.

Q: What is the best exercise regimen after 50 years of age to be healthiest version of yourself? As we age, maintaining our physical health becomes increasingly important, and adopting a suitable fitness regimen can make all the difference. For those over the age of 50, a balanced and sustainable approach to fitness is crucial. Keeping in view holistic fitness, here are few tips one can follow: 1. Cardiovascular Exercise: Incorporate regular cardiovascular exercise such as brisk walking, cycling, or swimming helps improve heart health and maintain overall fitness. Aim for at least 150 minutes of moderate-intensity aerobic activity per week. 2. Strength Training: Engaging in strength training exercises like weightlifting or bodyweight exercises is vital. Building and maintaining muscle mass is not only beneficial for strength and mobility but also plays a significant role in supporting bone health. 3. Flexibility and Balance: Yoga, tai chi, and stretching exercises enhance flexibility and balance, reducing the risk of falls and injury. 4. Functional Fitness: Emphasis on functional exercises that mimic daily activities, helps one stay independent and agile in everyday life. It improves neuromuscular co-ordination that is pertinent to our daily needs. 5. Mental Health: it’s well known that our mind controls our body. Taking time out for mental wellness and stress-relief from daily routine must not be overlooked. 6. Strength training: For good bone health, maintaining good muscle tone is important. Muscles act as protective cushions around bones, absorbing shocks and reducing the risk of fractures. When we engage in strength training, it promotes bone density and strength. Additionally, muscles exert tension on bones during exercise, stimulating bone growth and remodeling. A good muscle tone improves our blood circulation and escalates aesthetics of our physical shape, eventually giving us that feel-good factor. Remember, it’s never too late to start investing in your health, and a strategic fitness routine with a balanced diet is a valuable asset on your journey to a healthier and happier you.
