At the outset, it is my pleasure to extend warmest greetings and good wishes to all of you-my dear readers and partners for the Holy Month of Ramadan.

I am delighted to inform you all that this magazine will be hosting the prestigious HEALTH AWARDS 2023 in the month of November this year, honoring exceptional contributions in the fields of healthcare from across the region. Keeping the growth momentum going for Thumbay Group, Thumbay Dental Hospital has signed an MoU with Gaea Cynosure to extend our state-of-the-art facilities in providing clinical training facilities for doctors, physicians, and surgeons.

The Gulf Medical University’s College of Pharmacy has hosted the prestigious ‘The Middle East Conference on Pharmacist Administered Vaccination’ recently.

We have also had the pleasure of hosting two special delegations: one was 20 Deans from leading medical colleges of Kazakhstan at the Gulf Medical University, and secondly, leaders from the Young President Organization who had visited Thumbay Medicity’s state-ofthe-art facilities.

Our Group hospitals have always been at the forefront of offering innovative, multidisciplinary and advanced health services that benefit the large section of the population. I am happy that our goal to remain strategic and providing the highest quality has helped Thumbay University Hospital to emerge as the biggest surgical academic hospital network in the northern emirates that offers most advanced surgical procedures and clinical care which enables patients to not only opt for a shorter stay but also makes it highly cost-efficient.

In terms of adding training facilities to our students, Thumbay Dental Hospital has now expanded to 120 chairs and established a Post-Graduate Dental Center at Thumbay University Hospital. This facility has been set-up to serve as a clinical training hub for students enrolled in post-graduate and residency programs, providing them with practical, handson experience under the guidance of experienced dental practitioners and professors.

One of the most important pillars of our focus has been research. Gulf Medical University and Thumbay Research Institute of Precision Medicine marked Research Day with a two-day event, sharing insights from leading minds on the region’s first pilot research on personalized precision medicine program using the Zebrafish Avatars to treat cancer.

YOUR GUIDE TO GOOD HEALTH DURING THE HOLY MONTH

The Holy month of Ramadan presents a unique opportunity to reboot and reset the health button on our lives. This is possible by deriving the health benefits of intermittent fasting, tweaking nutritional intake, paying attention to the quality of sleep and hydration, and focusing inward.

Health magazine, in conversation with mental and physical health specialists and a nutritionist, provides a blue print on how we can reap optimum holistic benefits on all fronts.

How fasting supports good health?

Let’s begin with Intermittent Fasting. Intermittent Fasting (IF) involves eating during a limited window of time and observing total calorie restriction through most of the day. There are different kinds of combinations. Since the Ramadan fast involves complete abstinence from food, from dawn to dusk, IF is at the core of the main pillars of Islam.

Saja Al Mesri, dietician and clinical nutritionist from Enfield Royal Clinic, Dubai, explains, “Fasting during the holy month of Ramadan for 12-14 hours at a stretch can detoxify body, support weight loss, and improve metabolism. Recent scientific research has proved the benefits of intermittent fasting in weight and blood sugar management.”

People need to be mindful and

vigilant about the quality of food and nutrients they are having during iftar and suhoor for optimal health, explains Al Mesri who felt wrong methods lead to health complications.

“Since people restrict their timing to eat, we see many suffering from headaches, fatigue, dehydration, and mood swings. With the correct advice, it is possible to avoid these negatives and aim to be the best version of yourself.”

Never skip your suhoor meal

To begin with, the suhoor meal is your source of energy all through the next day. Unfortunately, this meal is underestimated, and many people skip it, points out Al Mesri. During suhoor people can tank up on nutrition.

People are advised to not skip suhoor and give it equal importance as iftar as this pre-dawn meal can help people go through the day feeling energised and well hydrated.

Al Mesri elaborates, “Suhoor meal should be balanced and should have complex carbohydrates, proteins and fats.”

Balance your meals

Complex carbohydrates come from foods such as whole wheat bread, oats and beans. These are high in fiber which helps stabilize blood sugar for a long time and make you feel full. Also, they are rich in B Vitamins.

One of the most vital parts of the suhoor meal is protein that people get from foods such as eggs, chicken, cheese, and tuna. “Protein is essential to keep you full throughout the day and help keep your metabolism functioning efficiently. One must not forget that protein is essential to maintain muscle mass, explains Al Mesri.

Choose the right micro and macronutrients.

Al Mesri also advised the intake of healthy fats as these are a source of prolonged energy and provide a feeling of fullness. “Good fats add taste to the food which makes your meal tastier. Good fats such as olive oil, peanut butter and avocado should be included.”

All about minerals and vitamins

In addition, to these macronutrients, Al Mesri also advised the inclusion of important micronutrients to the suhoor meal.

Suhoor should be rich in B vitamins that support your metabolism and work to convert the food you eat into blood sugar, in addition to supporting mental and immune health.

If you fall short on B vitamins, this might lead to fatigue and low mood.

The best sources of B vitamins are animal products like chicken, beef, eggs, and dairy and nuts.

Another important micronutrient is iron. Iron is crucial for the formation of hemoglobin, a protein in red blood cells that helps transport oxygen throughout the body. If you’re short on iron, you may feel fatigued, can’t concentrate and unusually cold.

Animal-based iron sources include red meats like beef and egg yolks. Plant-based iron sources include beans and legumes, dark chocolate, and spinach.

A third important micronutrient is magnesium. It supports your body’s production of energy it needs to keep your heart, muscles, and kidneys performing at their best, as well as helping our muscles and blood vessels relax. It also supports the production of feelgood chemicals like serotonin and interacts with certain receptors to help us get the rest we need. Rich sources of magnesium are spinach, nuts, and whole grains.

A fourth micronutrient is calcium which is mainly found in dairy products. Calcium stimulates insulin release in the blood which can help regulate blood sugar throughout the day.

Do not overeat to compensate for fasting hours.

Some fall in the trap of overeating at suhoor which makes them feel heavy, uncomfortable and feel thirsty the next day.

To avoid that, people must practice slow eating, properly chewing each mouthful. An easy way to help eating slowly is to put a timer of 20 minutes and be mindful of using that much time to end one’s meal. Al Mesri also pointed out that it was important to include soluble and non -soluble fibre in one’s meal. Soluble fibre was one that is derived from fruits, vegetables, sprouts

etc while non-soluble fibre comes from chia seeds, nuts, and plant cellulose. Fibre slows down blood sugar absorption and is mainly responsible for a feeling of fullness and satiety for long, helping people avoid overeating and sudden glucose spikes, Al Mesri explains.

Importance of Sleep, in regulating metabolism

Not being able to sleep when it’s dark and keeping awake during sunlight hours affects the circadian rhythms of the body. Dr Kirti Mohan Marya, Group Medical Director at Life Medical Centre and Life Clinics elaborated on why sleep hours were so important in maintaining good health during Ramadan.

What are circadian rhythms?

“Circadian rhythms refer to the internal clock of our body. It’s the natural cycle of physical, mental, and behavioral changes that the body goes through in a 24-hour cycle.”

He continued: “Circadian rhythms are mostly affected by light and darkness and are controlled by a small area in the middle of the brain. Disruption in circadian rhythms can adversely impact one’s sleep patterns, body temperature, hormones, and appetite. When sleep patterns get disturbed and our internal clock is disrupted, our metabolism is disrupted.”

Complete your eight hours of sleep.

Therefore during Ramadan, it is important to make sure to catch eight hours of sleep. Dr Marya further elaborated on how our body’s stress mechanism worked.

“A disturbance in the sleep pattern releases cortisol or stress hormone, which can result in impaired sugar metabolism” he said.

“Therefore, once iftar is over, it is advisable not to stay up for too late or have drinks rich in caffeine that can disturb sleep. People should make sure to hit the bed by 11 pm.”

“Wake up for suhoor and morning prayers and then try and catch at least one hour of sleep soon after.”

“You can compensate the last of the 8th hour by two 30-minute power naps and ensure your eight-hour cycle is completed,” he said.

Importance of hydration

Since the fasting time is between sunrise to sunset, make sure you have plenty of fluids pre-dawn and post-sunset and keep your electrolytes in blood balanced advised Dr Marya.

Keep your electrolytes in good balance.

Elaborating on what electrolytes were, Dr Marya explained: “Electrolytes are minerals such as sodium, calcium, potassium, magnesium chlorine , phosphates etc in our urine and blood. They have an electric charge.

If these get disbalanced due to lack of proper hydration, it can adversely impact our body’s blood chemistry, muscle action and other processes.”

Dehydration can lead to low blood

pressure as well as in cases of people suffering with hypertension or heart condition it can lead to thrombosis and strokes.

“As one must refrain from drinking fluids during the day, the emphasis should be on avoiding fluid loss through excessive sweating or intake of diuretic medication. Make sure to have high water content food such as salads, fruits and fiber in your suhoor meal. That will keep you full and the fiber will help hold water.

During Iftar, people are advised to avoid high salt or high sugar drinks . Take plain water, coconut water, clear soups and replenish lost fluid balance”, aded Dr. Marya.

How can patients with chronic diseases fast?

Dr Mahir Khalil Ibrahim Jallo, Clinical Professor and Senior Consultant of Internal Medicine – Diabetes and Endocrinology at Thumbay University Hospital said, “Patients with diabetes or chronic conditions, like kidney or cardiac disease, need to be in touch with their care provider prior to Ramadan, and seek proper advise on keeping food health and seeking advice on the manner to fast despite chronic health conditions.

More often, every patient has a different medication plan, and their doses needs to be adjusted for suhoor and Iftar. “For example, medications that tend to cause low blood sugar should be ideally taken while ending the fast and insulin doses should be reduced by least 20 per cent from the current dose during Ramadan. The aim is to keep the blood sugar steady during Ramadan. For instance, fasting blood sugar should be around 100 to 120 mgdl, and two hours after food should not exceed 170 mgdl or maximum 180 mgdl. Moreover, if patients find their blood sugars drop below 70 or elevate to 300 at any time during fasting, it is advisable to break your fast immediately, even if it is just one hour before Iftar because this can endanger their health. Likewise, patients suffering from chronic kidney disease, or cardiac issues or any other comorbidity constitute a risk group, wherein fasting is usually not advised,” explained Dr Jallo.

“Diabetic patients should be in touch with their doctor prior to Ramadan, at least a few weeks earlier, to adjust the

In terms of meal-planning, everyone should try to make their meals simple because Ramadan is an experience to practice a healthier life and not indulge in habits that can have bad consequences on overall health. Diabetic patients should have their suhoor as late as possible in the predawn hours. Dr Jallo also cautioned diabetics to go slow on traditional sweets and to make time for physical activity post iftar.

“In terms of planning physical activity, it is very important to consider Tarawi (extended prayers during the night) as a part of physical activity. Given the high temperatures and no liquids from sunrise to sunset, you will compromise your health by pushing yourself too much. It is not recommended to do intense cardio workouts and heavy weight-training exercises while fasting. Also, if you prefer to exercise after iftar, try to keep your meal light and save your hearty meal for after workouts so you don’t feel uncomfortable when training,” conclude Dr Jallo.

Tips on ending fast the right way a sunset.

• End your fast at sunset with two cups of water.

• Keep on sipping a little water until you retire for the night even if not thirsty.

• Add a natural flavour to the water such lemon or mint as that will make it easier to continue sipping.

• Note that water can come also from vegetables and fruits. It is important to state that it is tempting during Ramadan to drink traditional drinks that are very high in sugar and people consider them as hydration. However, these drinks can cause weight gain and should be limited.

• At Iftar and suhoor, it is advisable to have dates. Dates are naturally high in the right kinds of carbohydrates to rejuvenate your body’s energy levels. Eating one to three dates before anything else to break your fast is a sure-fire way to combat the exhaustion you may feel.

• Also, dates are a great source of fibre which helps alleviate

symptoms of low blood sugar such as fatigue, headache, and hypoglycemia.

• Dates can help with digestion. They help ease constipation due to their high fibre content.

• Dates are full of vitamins and nutrients. They contain more potassium than a banana, and also magnesium, iron, copper, phosphorus, and calcium !

Sample Suhoor menu

• Whole wheat bread with boiled eggs and cucumber or tomato with a cup of skimmed milk and 3 dates

• Fat free Greek yogurt with oats, fruits and raw nuts.

Sample meal for

healthy Iftar

• 1 date + 1 cup lentil soup + 1 plate salad + a balanced dish of rice and chicken (not greasy or fried)

• 1 date + 1 cup vegetable soup + 1 plate salad + baked sweet potato + baked salmon fillet.

Do’s and Don’ts. Dos

• Walk or work out frequently after iftar.

• Drink minimum 2 litres water.

• Eat five servings of fruits and vegetables daily - Include protein at each meal

Don’t’s

• Cut back on daily desserts.

• Avoid refined carbohydrates such as pastries, white bread and fried food

• Cut back on high sodium foods especially during suhoor as it will trigger thirst.

• Avoid skipping any meal.

• Keep away from high sugar sugary beverages.

• Avoid Skipping meals.

• Avoid too much caffeine.

Note: Ramadan can be a natural detox for our body if done correctly. A tip is to take Ramadan as an opportunity to fix bad eating habits and learn healthier habits that will make you feel strong, energized, and happy. H

YOUR GUIDE TO FAT LOSS AND TRAINING WHILE FASTING

The 30-day fasting period during the Holy Month means refraining from consuming food and drink in the long hours from dawn until sunset. However, Ramadan doesn’t mean you have to put your fat loss or muscle building goals on hold.

Such long hours without food or water can put a large amount of stress on your body – but with some careful planning and smart training advice you can still lose weight or grow muscle during Ramadan safely and effectively. Here is some top tips and advice on navigating your way through Ramadan to help ensure your health and fitness goals can still be achieved.

Can you work out during Ramadan?

If you are thinking about training or doing a workout during the month of Ramadan, the secret is to ‘stimulate, not annihilate’.

You shouldn’t be trying to hit your personal best lifts or starting a new high-intensity weight training regime (unless you’re truly feeling fit and strong after the fast).

It’s always advisable to use Ramadan as a period to maintain your fitness levels rather than drilling yourself into the ground.

Plan workouts that include effective whole-body weight training three sessions a week. This is a great approach to deal with the stress of fasting while still maintaining strength and muscle.

When Is the best time to work out during Ramadan?

Don’t feel you have to spend hours in the gym for an effective workout session. You can get a lot done in 45 minutes if you train hard and with intensity.

If your long-term goal is building muscle, an option during Ramadan could be to lower the load and add more sets to help maintain muscle mass.

Should I do cardio during Ramadan?

While weight training should be the cornerstone of any fat loss plan for both men and women, cardio can also play an important role in creating the energy deficit needed to lose weight.

There are two key types of cardio that you can consider during Ramadan which will help you burn fat optimally – high-intensity interval training (HIIT) and low-intensity steady state (LISS).

What is the best cardio for fat loss?

If you want to burn fat or lose weight during Ramadan, the normal fat loss principles apply.

Creating a consistent calorie deficit (expending more calories than you consume in a day) is key when it comes to fat loss.

The best way to achieve this is through a combination of eating less and increasing your exercise and daily activity levels.

Resistance training, in particular, is important to help preserve your muscle and strength.

Physical activity outside the gym

If you are not doing formal cardio workouts during Ramadan, but you still have a fat loss goal, it is important to stay active throughout the day.

The activity you do outside of the gym is just as important as what you do inside the gym for increasing your energy expenditure and burning fat.

You can increase your daily activity and calorie burn in many ways, including walking upstairs, walking for the train, having a standing desk, cleaning the house, doing chores or playing with the children.

These all fall under the bracket of NEAT, which relates to all other daily activities which burn calories.

The best way to track your activity levels is to get a step counter app on your smartphone fitness watch or buy a cheap pedometer to measure your steps.

A good base target to aim for every day is 10,000 steps which will boost your weight loss efforts without adding any more stress to your system. H

"When people dial down the external noise, focus on listening to the inner voice and look into good deeds, focusing on common approach rather than thinking about self , it creates thinking about the needs of others and importance of giving in charity, take more time out for prayers and spiritual rejuvenation, this can naturally releases endorphins, the ‘feel-good’ hormones that vastly improve an Individual’s mental health. Following a proper process to heal oneself through prayer, fasting, self-introspection will help every individual to be a better version of oneself with enhanced resilience as well as help prepare for future challenges in life, said Dr Tahir.

Five tips on destressing and fortifying mental health during Ramadan.

UAE doctor donates 200 container homes to earthquake victims in Turkey

There are few people in life who can live by the ideals of life where they make true use of their skills to come to the aid of humanity. Dr Khan belongs to this ilk.

Providing a roof over the head of the homeless

After working tirelessly for weeks in his individual capacity to offer whatever help in the devastated villages of Turkey, Dr Khan recently announced the decision to give away 200 container homes to the victims in refugee camps in and around the epicenter. Beginning this Ramadan, Dr Khan flew back again to personally hand over the homes to victims.

“I was so disturbed by the sight of pain and suffering in Turkish villages. The village of Demirkopu was split in two by a wide chasm soon after the disaster struck. I worked in close contact with the crisis management teams and local municipal councils in the villages” said Dr Khan.

Soon after the horrific series of earthquakes measuring 7.8 magnitude on the Richter scale that hit Turkey on February 6, dubbed as the worst disaster of the century, scores of international agencies and volunteers reached the country to work together to help their brethren, re-instilling faith in humanity. One among them was Dr Azam Badar Khan, a Dubai based doctor.

He continued, “I feel incredibly happy to be able to provide these earthquake-proof homes to the homeless families and people and I have the support of some generous people who do not want to be named. It gives me great pleasure to see the smile and relief on the faces of so many traumatized people.” Dr Khan believes there is no alternative but to volunteer his services and his surgical skills for service to humanity.

Changing course

Feeling blessed to be able to come to the aid of the Turkish people Dr Khan recounted to Health Magazine how he had almost

missed this mission because when the earthquake happened, he was enroute on a philanthropic mission of holding a knee replacement camp in the mountainous province of Ladakh in India.

“I was to conduct a free knee replacement surgery for the underprivileged Ladakh in the second week of February and as I was waiting for my connecting flight to Leh from the New Delhi Airport, I watched the heart-rending scenes relayed on international news with a sense of horror and foreboding. “Instinctively I knew, I had to be in Turkey. The next few days were spent changing my course of action and country and I feel precious hours were lost. I still regret having reached Istanbul nearly a week after the earthquake,” said Dr Khan.

Prioritizing the needs of the victims

But as soon as he was on site, Dr Khan began working in camp wherever possible, not really worried about the nature or kind of work offered to him as groups had already been configured. He gave away wheelchairs to the crippled, did preliminary checks of patients, assisted in surgeries, and offered a shoulder to devastated families looking to download their grief. In the end, he met like-minded charities and after understanding that having a roof over the head was one of the top priorities, he proceeded to make homes a reality for the earthquake victims.

While Turkey has been a passion project currently, Dr Khan always has been a man of the moment who wastes no time in offering volunteer service including free knee replacement surgeries for the underprivileged in Somaliland, Sudan, Nigeria, Kenya, Mauritius and

some parts of India such as villages in the remote mountains of Kargil, Leh and Ladakh.

Let’s walk again!

Dubbed as Dr Knee, in Dubai, Dr Khan works as a consultant orthopedic surgeon at PRIME and a host of other hospitals and holds an educational program on knee health and knee conservation, as well.

His movement ‘Let’s walk again’, in collaboration with PRIME Hospital, Dubai has been raising awareness on knee osteoarthritis that affects 10 per cent of people in UAE above the age of 49. He has in the last few years, motivated thousands of knee warriors to stand up on their feet and be sporty again, in UAE. Many walked at the special awareness camps during the 30x30 fitness campaign in November 2022 and some volunteered for treks under his guidance.

In addition, Dr Khan also has been raising awareness on knee health

throughout the world and has been conducting free surgeries for knee osteoarthritis patients in many countries. He has also taken a patient to Mount Everest base camp and climbed Mount Kilimanjaro to promote the importance of knee health.

International honor at UK House of Lords

It is evident that serving society and dedicating his free time to public health has been a way of life for Dr Khan. It was indeed little wonder then that his efforts were acknowledged, and he was honored on India’s Republic Day at the House of Lords in UK where he was awarded the Global Inspirational Award 2023.

Humbled by the honor and determined to continue his mission, Dr Khan said: “I want to inspire other doctors as well, to take time out from their busy schedules and make an effort for such outreach in the remote areas of the world, where people have no access to medical facilities or where the medical know-how in regional hospitals is at a basic level. Sometimes, 24 hours in a day are not enough. In the last one year itself the world has been witness to news about shocking and sometimes fatal heart attacks of several celebrities ranging from popular Bollywood and Hollywood stars to global sports icons. The biggest paradox was that many of them had a regular physical fitness routine and some either died after a work-out session or during a treadmill walk at the gym. This is a clear indicator that a heart attack can happen anytime, anywhere, without any warning!

Dr Brajesh Mittal, leading interventional cardiologist of UAE and Deputy Medical Director of Medcare Hospital, Convenor of the annual SHAKE Heart 2023 Conference, that concluded in Dubai on March 16-17, highlighted preventive steps to save lives from heart attacks.

For educating readers, let us first understand the basics about heart diseases.

WHAT IS A HEART ATTACK?

Myocardial Infarction (MI) is the medical term for heart attacks where the sudden block in one of the leading arteries to the heart, blocks blood supply and damages the heart muscle. Damage

to the heart muscle leads to poor pumping of the heart and/or fast heart rhythm which could cause dizziness or loss of consciousness.

Dr Mittal elaborated, “In the UAE, most of the heart attacks occur in people under 50 years of age and a lack of awareness has led to a high incidence of heart disease in the population. It is said, ‘Time is Muscle’ and maximum deaths and long-term damage in heart attacks occurs because patients have not been taken care of in the right time.”

WHAT IS CVD?

Cardiovascular disease is an umbrella term that encompasses all heart diseases, and these include stroke, heart failure, heart attacks or myocardial infarction, arrythmia or irregular heartbeats, Coronary Artery Disease (CAD) induced by high cholesterol that gets deposited as plaque in arteries.

Call an ambulance as soon as you see a patient experiencing a bout of sweating, dizziness or complaining of chest pain.

If you are the victim, do not drive to the hospital, take a cab, or call an ambulance.

Reach the emergency room as fast as possible to minimise damage to the heart muscle.

Give an aspirin to the patient as this can help in dissolution of the potential blockage.

WEIGHT LOSS SUCCESS STORY

From weighing 82 kgs, with a simple, yet disciplined routine involving balanced nutrition, yoga and early sleeping routines, Vandana managed to bring down her weight that stood stubbornly to a perfect 57 kg. No quick fixes or tummy tucks for her. Her journey of losing these 26 kg in three years has been a mix of self-restraint, reconnecting with teachings of her childhood and simplifying her lifestyle.

Of failed fad diets and glam gym routines

Recounting her struggles earlier, Vandana said for many years she had tried many fad diets and gym routines. But nothing had worked. After

losing a few kilos, the weight would come back, no matter what she tried.

“I was anxious as over the years, I had developed high cholesterol and the doctor put me on statins. I enrolled in an expensive detoxification programme at a health farm and there, all the routines they implemented were like what we had followed in childhood. inflammatory response. That is why all the weight loss that happened at gyms, would return. I had to reboot and reset my life.”

I hit the reset button.

The next few months, she worked on decluttering her calendar and listening to her body. She went on a five-month oil-free eating habit, adapting her palate to raw, steamed, and boiled food. “Initially it was challenging, especially when I went over for a social dinner, but I stuck to my guns and did not touch oil for nearly five months. At home I rose early, implemented a strict Intermittent fasting regime, practised yoga, and breath exercises, gave up sugar and retired to bed, post sunset.”

Undaunted by failures.

The inflammation markers were

high in her body and despite all the regimens, it took over five months to break through the insulin resistance and inflammation. Until then, Vandana said she had lost only one kilogram of weight in three months. For over a year she continued with dogged perseverance until lockdown happened.

At present, these lifestyle tweaks have became an integral part of her waking and sleeping hours. Mornings she is up at 6 am, practises a vow of silence that helps her look inwards and decompress as she goes on her 45-minute walk. Upon her return she practises deep breathing, meditation, yoga and has the first meal of the day only by 10 am which amounts to 14 hours of fasting. With those efforts, the great shift happened. Her weight began coming down drastically, her skin tone improved, and she looked calmer and happier.

Mindful nutrition

Nutrition has played a key role in her weight loss as Vandana incorporated healthy, wholesome grains such as millets, sprouted legumes and nuts in her meal. She has also added raw fruits and vegetables, a generous amount of plain water to hydrate herself, into her diet. In the evening, her last meal is over by 7.30 pm, which is usually a soaked and ground ground lentil-based pancake, some dal, and a bowl of salad. Her bedtime is strictly at 10 pm.

The combination of right nutrition, spiritual centring, physical fitness, worked and within a year Vandana was 12 kgs light. Dieticians have pointed out that a slow and steady weight loss usually means the weight loss is holistic and permanent.

“Giving up oil completely and sleeping on time and having plenty of water each day was showing its impact, even though my weight loss was slow. I was losing inches and friends noticed it. I could also experience so many changes in my body. I was calmer; less stressed and felt very much centred,” she said.

Eventually, Vandana lost 26 kg and

has stuck to that number. “I feel so physically fit and energised and my body fat and related cholesterol issues have vanished. My family which includes my two sons, husband, sister, all followed my routine and have lost weight,” said a jubilant Vandana.

Weight loss is not about knocking off a few kilos; it is a holistic 360-degree transformation of mind, body and soul.

Implement the principle of 'early-to-bed and early-to-rise' in your life.

Treat your body as a sacred temple and make sure that the thoughts, food, breath going into is clean.

Work on spiritual and mental health, be mindful and conscientious. Practise moderation in life and have smaller portions of food. Cut out junk food, sugar, oil, trans fats from your school. If possible, be vegetarian. Going grain-free once in 15 days and go without water for 24 hours, once a year is advisable. It helps in inculcating noble thoughts of patience and compassion for others apart from helping in detoxing the body.

Choose wholesome foods like millets, sprouts, nuts and have this as the main meal.

Have generous portions of raw vegetables and fruits in your meal in large salads.

Keep your self well-hydrated and drink plenty of plain water. It will clear your skin tone and make you feel fresh.

People can easily view before-and-after photos and read reviews, which can influence their decision to undergo plastic surgery, he explains. “In addition, social media provides a platform for plastic surgeons to showcase their work and reach a wider audience, which can drive demand for their services.”

Plastic surgery can indeed have benefits beyond just improving physical appearance, says Dr. Ameer. Some of the ways plastic surgery can positively impact people’s lives include boosting self-esteem and confidence: plastic surgery can help individuals feel more confident and positive about their appearance, which can lead to an improvement in self-esteem and overall well-being. Also, improving physical function: certain plastic surgeries, such as reconstructive procedures, can improve a person’s physical function and ability to perform daily tasks. He adds, “For example, reconstruction after mastectomy can restore a form and body shape, and hand surgery can restore fine motor function for individuals with certain hand injuries.” Relieving physical discomfort: Some plastic surgeries can relieve physical discomfort caused by deformities or injury.

SHINGLES, A COMMON VIRAL INFECTION WITH SERIOUS HEALTH IMPACT

Shingles, a virus disease that affects 90 percent of the adult population above 50 years, can be prevented with right action and awareness.

Shingles, a common viral infection with serious health impact, can be prevented with proper education and vaccinations. However, awareness about Shingles is not optimal globally. In a multi-country online survey conducted by a global survey agency, only seven per cent of the adult respondents felt they were at risk of suffering from Shingles in the next 10 years.

What is Shingles?

Shingles, also known as herpes zoster, is a viral infection caused by the varicella-zoster virus, which is the same virus responsible for chickenpox and it can infect people with weakened immune systems, usually above the age of 50 and those with other comorbidities such as diabetes, hypertension, previous history of COVID-19 complications.

Diagnosis

Shingles is diagnosed through a swab of rashes and fluid from blisters. A person suffering from the disease, who has a shingles rash, cannot spread shingles to another person. However, direct skin-to-skin contact with blisters can transmit chickenpox to those who haven’t had chickenpox or the chickenpox vaccine.

Patient speak

Douglas Korth, a patient diagnosed with Shingles shared his story which highlights inadequate awareness about the condition. He said, “When I first contracted Shingles eight years ago, I had no idea what I was dealing with. All I could remember was a lot of pain coming from my nerves, especially on the side of my face where I was affected the most. I was lucky enough to have directly gone under treatment at the time. However, this type of pain is not something I would ever want to experience again in my life, and I am glad that there are ongoing efforts to raise awareness and understanding about it now.”

Incidence

As per the Centre for Disease Control (CDC), about 1 out of every 3 people in the United States will develop shingles, also known as herpes zoster, in their lifetime. Globally, over 90 per cent of adults aged 50 years and over have the virus that causes shingles and so are at risk of the disease.

As we age, the immune system starts to become less effective at preventing and fighting infections and diseases. This age-related decline in immunity

can allow the varicella-zoster virus to reactivate, causing shingles.

Symptoms

Individuals who had chickenpox during their childhood can have reemergence of the virus as Shingles in adulthood. Although the body fights off the varicella-zoster virus and the symptoms of chickenpox disappear, the virus remains in the body and can become active again in adulthood in the form of shingles. The disease causes an outbreak of a painful rash on the skin with burning sensation in the affected areas. The rash eventually changes to fluid-filled blisters in one area of your body which are mildly painful; they break and scab. Early symptoms include fever, chills, headache, tiredness, light sensitivity, and an upset stomach.

An outbreak of shingles typically lasts three to five weeks before the rash disappears. Most often the rash appears on one side of the body, as the virus travels in specific nerves, which transmit signals. Commonly affected areas include the chest, abdomen, and face.

What are common complications with Shingles?

A sizable section of people who are affected by the virus develop postShingles complications such as postherpetic neuralgia which affects at least 10 per cent of those suffering from it. It causes burning pain in the nerves and skin that lasts long after the rash and blisters of shingles have faded away. In some cases, the virus can affect the eyes and lead to blindness. In very rare conditions, Shingles can adversely impact one’s sense of hearing, cause pneumonia, inflammation of the brain (encephalitis) and even death.

Treatment and prevention

Although there isn’t a cure for shingles, the disease symptoms can be treated

with antiviral medications, pain medication and antiinflammatory treatments and the virus can be prevented though vaccination. The shingles vaccination is available and recommended for those at risk. Vaccination has been shown to be more than 90 per cent effective in preventing shingles and postherpetic neuralgia.

In its efforts to raise public awareness on Shingles, the UAE’s Health ministry in collaboration with GSK, a global biopharma company has created several videos on awareness that can be viewed on Shahid, its Arabic Video on Demand (AVOD) platform Shahid. The promotion that can be viewed throughout the Holy month of Ramadan, aims to drive the conversation about Shingles between people and their loved ones to have important conversations with their trusted doctors about Shingles disease and prevention.

Inclusion, diversity, and equality: how to foster these qualities in your children

Inclusion, diversity, and equality are all big buzz words across multiple sectors. But the question remains; how do we inculcate these concepts in our children? On this note, Mai Elsayed, Clinical Hypnotherapist with Cognitive Behavioral Therapy Skills and Reiki Healer, explains more.

Inclusion and diversity defined

Inclusion refers to the act of creating an environment in which everyone feels valued, respected, and supported. According to Elsayed, in an inclusive society, people of all backgrounds and identities are given equal opportunities to participate in all aspects of life, regardless of their race, gender, religion, age, or ability. “An inclusive society recognizes and celebrates differences and promotes a sense of belonging for all individuals,” she says. Diversity refers to the range of differences that exist among individuals, including differences in race, ethnicity, gender, religion, and age. She adds, “A diverse society recognizes and values these differences and understands that they enrich our lives and make us stronger as a community. It is important to promote diversity because it leads to greater innovation, creativity, and problem-solving, as people with different perspectives and experiences can bring unique ideas and solutions to the table.” While equality refers to the state of being treated fairly and without bias or discrimination. In an equal society, everyone has access to the same opportunities and resources, regardless of their background or identity. Equality is essential for

building a just and fair society where everyone has an equal chance to succeed and achieve their potential. “Together, inclusion, diversity, and equality are imperative for society because they promote a sense of belonging, respect, and fairness for all individuals, regardless of their background or identity,” she says, and by embracing and celebrating differences, we can create a more tolerant, openminded, and compassionate society that values and supports all its members.

It is incredibly important to practice the qualities of equality, diversity, and inclusion at a young age, as these are foundational values that help shape children’s attitudes and behaviors towards themselves and others, says Elsayed. “By instilling these values early on, parents can help their children develop a strong sense of empathy, respect, and understanding for people of different backgrounds and identities, which will serve them well throughout their lives.”

Expose children to diversity: Exposing children to people of different backgrounds and cultures is an excellent way to teach them about diversity and foster a sense of openness and empathy, says Elsayed. “This can include reading books and watching movies that feature diverse characters, visiting culturally diverse neighborhoods, and participating in multicultural

events and activities.”

Encourage dialogue: Encouraging children to ask questions and have open and honest conversations about diversity and inclusion is a great way to help them understand different perspectives and experiences. She explains, “Parents can create a safe and non-judgmental environment where their children feel comfortable asking questions and sharing their thoughts and feelings.”

Teach about equality: Parents can teach their children about equality by highlighting the importance of treating everyone with respect and fairness. “This can include discussing issues of discrimination and prejudice and the harm they cause, as well as highlighting examples of people who have stood up for equality and justice.”

Helpful tips to incorporate these concepts in our homes

Make an effort to celebrate and appreciate differences in your family. Talk about the importance of diversity and how it contributes to a more interesting and dynamic world. Be mindful of your language and avoid using derogatory terms or making derogatory jokes that could be hurtful to certain groups. Instead, use language that is inclusive and respectful of all people.

Encourage your family members to learn about different cultures and traditions. Take time to learn about other cultures through books, movies, and documentaries, and encourage discussion about what you’ve learned.

Set a positive example by showing tolerance in your own behavior. This could include practicing patience, understanding, and empathy towards others, especially those who are different from you.

Encourage open communication in your family, and make it clear that it’s okay to ask questions and learn about different perspectives.

Avoid stereotyping people based on their race, gender, or other identities. Instead, encourage your family to get to know people as individuals, rather than making assumptions. H

Laparoscopic and roboticassisted surgeries for cancer patients

In cancer related surgeries, surgeons still perform open surgeries although oncology surgeons are fast transitioning to laparoscopic techniques in certain cancers. She cited the example of cervical cancer surgery as an important case study. In conventional surgery, for as finer nerves are difficult to dissect and might lead to weak bladder post-surgery. On the other hand, if it is a laparoscopic robotic assisted surgery with you can get the best magnification to perform nerve sparing surgery, minimizing the complications.

Dr Usha Kiran also pointed out that it was important to clear the misconceptions people had about minimally invasive procedures which clearly had an advantage over conventional surgical methods. “The biggest misconception patients have is that they think only if a surgeon cuts open widely, can he or she get a clear view. While this may be true to some extent, in most cases, especially in obese patients, laparoscopic surgery gives a better access and vision because there is a better view on camera and the laparoscope can go behind into difficult-to-view places and give the surgeon the much-needed clarity and perspective,” added Dr Kiran.

Dr. Usha Kiran stated that technologically, laparoscopy provided surgeons a better view of the interior of the abdomen, especially as more and more hospitals had begun relying on robots to perform certain tasks.

“Robotics-Assisted Laparoscopic surgeries are more efficient and accurate and allow for minimal errors. Moreover, Robotics-Assisted Laparoscopic Surgeries allow complete circumduction which is difficult for humans to do. They have eliminated the limitations of a human body and hand movements, offering increased precision with a much better vision of fine blood vessels, and fine nerves. All of this leads to less complications for the patient later.”

Practice makes for minimizing margins of error

Surgeons who intend to take up this career path need to acquire special training in institutes like those in the United Kingdom. “The learning curve depends upon the operator’s learning abilities and the number of cases that they perform. After training, a minimum of six months is required to work in a place where these surgeries are happening, so that there is enough practice for one to get adept at it,” advised Dr Usha Kiran.

Dr Usha Kiran has been able to successfully remove enlarged fibroids and uterus using the laparoscopic option and advises surgeons to continue with refresher courses once their training has been completed.

When should patients opt for laparoscopy?

Dr. Usha Kiran’s advice to patients is that if they have a gynecological condition like a fibroid or a variance system which requires removal, or if they require the removal of the uterus, a fallopian tube, or an ovary, they can opt for laparoscopy. Unless a surgeon is ready to try laparoscopically, patients should not really opt for an open surgery. Laparoscopic surgery should always be the first choice as open surgery is resorted to by default. However, in case of complications, sometimes open surgeries are required to be performed.

New methods to prevent cancers with laparoscopic techniques.

Some adversaries of laparoscopic surgery have argued that in procedures

where the extraction requires several small pieces to be made of the parts, as in case of tumor or fibroid removal, there have been possibility of leaving behind a malignant piece that could later trigger cancer in some cases. However, new research has shown that application of very stringent standards in the extraction of small pieces using an airtight morcellation or ‘morsafe’ bag has helped eliminate this possibility completely. “The airtight bag seals off the extracted pieces from meeting the rest of the space or cavity. These are completely dropped in the bag and extracted with zero chances of any contamination,” she explained.

A safe, versatile option

In conclusion, Dr Usha Kiran feels in the debate between the two procedures, laparoscopic surgery wins hands down. “All surgeries contain a certain element of risk. But laparoscopic surgery offers numerous benefits over traditional open surgery.”

• Quicker chances of recovery

• Robotic-assisted surgeries allow far more precision, accuracy, and range of angles in vision that conventional hand guided surgery may not be able to offer. This results in better outcomes. H

Erroneous diagnostic criteria

Dr Shrivastav explained, “The nomenclature PCOS is a misnomer as the condition does not originate in the ovaries. What is seen as a rootcause, is, only, the symptoms. It all begins with insulin resistance which is precursor to diabetes that the women may have as part of her family history.

As per the conventional Rotterdam Consensus, PCOS in a patient is diagnosed when two or three criteria are met. These are:

• Irregular or scanty periods

• High levels of androgens present in blood

• Presence of multiple tiny cysts on the ovarian surface that is visible during an ultrasound examination.

Insulin resistance the root cause

Insulin resistance is the earliest manifestation of metabolic syndrome. It occurs when the cells in our muscles, fat, and liver don’t respond well or are unable to respond to insulin and metabolize glucose from the blood for

conversion to energy. As a result, their glucose is high in the blood, the pancreas makes more insulin to help glucose enter the cells. So initially the sugar levels in the blood may be within the normal range. The sugar gets stored as fat and the individual who is insulin resistant puts on weight which has different side effects. One of the fall outs of advanced insulin resistance is diabetes. In case, of young girls who are insulin resistant and put in excessive weight, their ovaries begin to produce more of the male hormone, testosterone. The direct impact of high testosterone is slowing down of egg production and the symptoms of this are delayed menstrual cycle, acne, hirsutism or (facial hair) and the appearance of very tiny cysts on the ovaries. These are symptoms and not the root cause,” said Dr Shrivastav.

Conventional treatment for PCOS

In most cases the conventional treatment that doctors deploy for PCOS is laparoscopic cautery or

drilling of the ovarian surface, putting the patient on contraceptive pills to regulate the delayed menstrual cycles are very common. While cautery or drilling do have an impact, because these decimate the ovarian surface, there can be serious side effects of bleeding, post procedural adhesions or scarring and sometimes a diminution of the egg reserve and contraceptive pills interfere with the natural rhythm of the menstrual cycle.

Best line of treatment, lifestyle change

Instead of just focusing on symptomatic treatments, it is advisable to address the cause of PCOS which is insulin resistance. “What we do when women come in for infertility treatment is to first screen them for IR and put them on insulin sensitizers. Before patients can undergo any other treatment, the advice that is given is about correction of lifestyle. This includes better diet and nutrition, focus on physical work out, including weights to stimulate muscles as

muscles building increases insulin sensitivity. Even a five per cent drop in body weight and reduction of fat, in many cases results in natural conception at our clinic, even before patients can be taken through other fertility treatments such as ovulation induction,” explained Dr Shrivastav.

Call to review and revise diagnosis and treatment.

Dr Shrivastav thinks is time for the medical fraternity to call for better understanding of PCOS, review the conventional Rotterdam criteria approach and look for holistic health solutions to reduce insulin resistance with lifestyle changes. “These steps will naturally register a drop in symptomatic PCOS and thereby reduce the incidence of high infertility in women suffering from it.

Top Tips to minimize PCOS through lifestyle changes and food choices.

• Cut down on high sugar and processed foods.

• Canned or tinned products must be avoided as these contain hidden sugars, high salt and preservatives which can trigger insulin resistance

• Include dark green, leafy vegetables in your diet such as kale, spinach etc

• Include dark red fruits such as red grapes, blue-berries , black berries, cherries in your diet as they are rich in anti-oxidants and low in glycemic index which helps blood glucose

• Have stir fried broccoli or cauliflower in your meals as these are high in fiber and micronutrients that increase insulin sensitivity.

• Exercise regularly for at least 45 minutes each day, follow weight bearing and muscle building regimen.

• Cut back on saturated fats such as margarine, fried food, red meat as these food groups have a direct impact on increasing insulin resistance and aggravating PCOS. H

STEM CELLS: IS IT ALL HYPE

Most high to mid-end anti-aging skincare products are now adding some derivative of stems cells to their formulations. But do these really help or in is it all hype? Here to shed some light on the subject, HEALTH speaks with Dr. Hoda Makkawi, an anti-aging medicine specialist and a consultant for family medicine and integrative holistic medicine.

Defined

This refers to a rather broad category of cells that participate in tissue generation, regeneration, and renewal, as well as the healing process, explains Dr. Makkawi. “Also, actual human stem cells are not suitable for topical skin care since they are fragile and easily destroyed, as well as too large to be absorbed,” she says, so instead, brands pioneering this ingredient will extract one human stem cell to grow hundreds more, which in turn generates the “helper” ingredients that plump your skin.

What are stem cells meant to do in terms of anti-aging?

Studies have found that the introduction of ‘youthful’ stem cells into the body may rejuvenate the existing cells, working to reverse some natural effects of the aging process, she says. “This is why the skincare industry is taking a closer look at this as an ‘anti-aging’ superpower.”

The verdict

Companies often claim a ‘stem cell’ product when the truth is they are products derived from a complex composite of media that is enriched with growth factors, proteins, and cytokines (signalling proteins) in which the harvested stem cells were grown. Dr. Makkawi reveals that research concludes, it is really the antioxidant properties and active extracts from the actual plant ‘stemcells’ than regenerative properties from which our skin is benefitting.

Lasers for brown skin

Question: I am a skin type 4 woman with pigmentation. Can a laser treatment help me?

Dalya Sager, Laser Specialist responds, “The answer is yes. There are three types of technologies that can easily and safely treat pigmentation. Various conditions are melasma, sunspots, freckles, and age spots. The safest being IPL or BBL Broad Band light, it’s a broad non-specific light (in the form of heat) that penetrates the skin to damage the pigment, the body will work to repair the damage by reducing the appearance of the discoloration by removing the treated pigment via the excretory system. Pico Laser technology is most effective at removing pigment from darker skinned patients. It utilizes photomechanical energy (no heat) to penetrate the skin at a specific wavelength (670nm and 1064nm) to shatter and damage the pigment in the epidermis. This triggers a healing response that removes the fragments of pigment from the surface of the skin. Several treatments are needed to achieve the desired results.“

Diagnosis of keratoconus

Question: I have recently been diagnosed with keratoconus. What is this?

Dr. David Gritz, Staff Physician, Cornea and Refractive Disorders responds, “Keratoconus is usually detected in the teenage years or 20s but can also start in childhood. However, the exact cause of keratoconus is unknown, and there are no eye diseases or injuries that explain why the cornea’s structure weakens and changes shape. This makes the role of preventative screening and timely detection even more critical, so accurate treatment is provided to manage the extent of distortion. Permanent vision loss from keratoconus is preventable through early detection and treatment, as we have innovative cutting-edge technologies available today. Therefore, we encourage people to take the preventive route and opt for screening tests. Keratoconus occurs when the cornea, the transparent outer lens of the eye, which normally has a dome shape with a ball-like appearance, is unable to maintain its round shape and extends outward in the form of a cone. While the exact cause is largely unknown, some research studies suggest that keratoconus runs in families and occurs more often in people with specific medical conditions.”

A baby with colic

Question: My baby has been diagnosed with colic. What is this in layman terms?

Dr.Rimzim, Specialist Pediatrician responds, “Infantile Colic is an acute abdominal pain that causes excessive crying spells in an infant. It peaks around six weeks of age and usually resolves at three to four months of age. Diagnosis was previously done by the rule of three: crying that lasts for more than three hours per day, occurs more than three times per week; but more recently (according to New Rome IV criteria), it is defined as recurrent and prolonged periods of infant crying or fussing without any specific reason. The infant may feel very irritable, fussy and gassy, drawing their legs up (flexion). The diagnosis of infantile colic should be made after excluding other organic causes of excessive or undue crying. Causes of colic are not clear, but several physiological and psychological factors may be contributing to this condition. The physiological causes are the most common cause of this condition and are due to aerophagia (swallowing of air) or flatulence related to the digestive issues. Burping the baby after feeding prevents excessive gaseous distension and prevents colic up to some extent. For the treatment of colic, babies can be given a carminative mixture, colic drops, and probiotics. Colic can occur in both breastfed infants and formula fed infants. For formula fed infants if colic is very troublesome then the type of formula could be changed to a comfort/hydrolyzed formula.”

Upon arrival, we were srved two authentically Persian beverages—Anartini, a pomegranate concoction with

Some highlights we loved included assorted hot starters like slow cooked meatballs with herbs, dried apricots, lentils, and walnuts, called Kufteh Tabreezi, as well the ash rashteh, a soothing thickish soup with beans, noodles, hints of meat and topped with dried mint and fried onions. We also loved the warming stews including the Khoresh Bamieh, okra and meat, as well as grills including Persian lamb chops—kabab sheeshlik,

prawn tamale, with king prawns, kale, basil and corn; the augergine tacu tacu was delicious with a traditional Peruvian panca huacatay sauce and feta cheese, adding a medley of tastes and textures in every bite. The chicken pachamanca also had roasted sweet potato which is a Peruvian staple, alongside uchucuta cream and crispy cancha. We also enjoyed some of their key signature sides, including panca mushrooms and potato fries with a spicy huancains sauce. The beverages are also Peruvian and I absolutely loved the chicha morada, a beverage made from purple corn, along with fruit and spices.

Known as a Picanteria in Peruvian style, the vibe here is casual, fun, and a social space where you enjoy authentic Peruvian cuisine with music and friends. the brunch menu is divided into snacks for the table, ceviche and tiradito, mains, sides and desserts. We were served roasted corn nuts, green plantain crisps and beetroot and cauliflower causa, which is a traditional chilled potato cake with beetroot tarter, mango gel made to resemble egg yolk, and cauliflower florets.

From the ceviche, we tucked into their ceviche andina, with fresh sea bass, and also the Amarillo leche de tigre, namely avocado cream, red onions, sweet potato crisp, which we loved. The salmon tirdito was slightly different to the typical ceviche and served in a platter with yuzu leche de tigre, pickled cucumber, croilla and crisp kale.

The mains were an eclectic mix of vegetarian and protein based dishes, some standouts included

The desserts included lucuma mousse, a flavorful kale, basil, corn nut pesto, giant corn and cream pudding; the chicha morada granita—coconut and mango chia pudding pineapple preserve and our unanimous favorite; the picarones—deep fried Peruvian styled donut made with sweet potato and pumpkin, and served with warm chocolate sauce.

Amongst the Levant mains, we loved the Sayideh rice pilaf, while from the Indian corner, the dal makhani and aloo corn was the perfect accompaniment to the freshly steamed basmati rice. There is also a large counter with a section for bespoke pizzas and pastas where we indulged in the classic margarita pizza and pink sauce pasta.

Desserts here are amazing with a focus on Arabic desserts, but lots of other treats to satisfy any sweet tooth. I loved the sticky toffee date pudding, baklava, Um Ali, red velvet cake, chocolate brownies and lots more.
