Sudden
and dramatic weight loss causes concern in woman
DEAR DR. ROACH: I am a 75-year-old woman with a weight problem. My weight used to fluctuate from 130-160 pounds.Then I started to notice that I was losing weight slowly but surely on my regular diet. My doctor did not have an answer. Soon I got down to 95 pounds. My family urges me to eat more than I feel comfortable with. I weigh 104 pounds today.
Do you have any idea what could be causing this dramatic weight loss? I still struggle with keeping my weight up to 105 pounds! My height is 5 feet, 4 inches tall, and I really am quite “bony.” Thank you for any help you can give me. -- A.S.

ANSWER: When we see dramatic weight loss like yours (greater than 5% to 10% is generally considered important, and yours is greater than 30%), we think about a few important categories. Of course, some of these aren’t appropriate for you, but I want to give you an idea of what goes through a doctor’s head when we see a person with this much weight loss:
Unfortunately, cancer tends to leap to our minds, even though it’s not the most common cause. A full-body scan isn’t usually indicated or helpful. Instead, a careful and focused history and physical exam will often give clues as to whether cancer is likely.
In addition, doctors want to make sure that the person is up-to-date with their age-appropriate screenings, which include breast and colon cancer screenings in a 75-year-old woman. In a person with a history of smoking, I always consider lung cancer and will usually order at least an X-ray.
A condition that is commonly forgotten when thinking about weight loss is the inability to absorb food properly, and celiac disease is the most common disease to cause this. Diarrhea is common but not always present with this condition.
People can lose calories through their kidneys as well, and diabetes is by far the most likely cause of this, although there are rare other causes. Excess urination is universal in uncontrolled diabetes but can be minimized by the patient. Endocrine disorders, especially hyperthyroidism, often cause weight loss. Mental health issues, especially depression, are very commonly associated with weight loss. Anorexia nervosa is uncommon but not impossible in older adults.
Any serious disease -- heart, lung, liver and others -- can also cause serious weight loss. Some medications can cause unexpected weight loss as well.
Chronic infections are another important cause of unexplained weight loss. Tuberculosis, HIV and viral hepatitis are the most common, but there are many uncommon and rare infections.
As adults get older, some degree of weight loss is expected, but it is the degree and speed of the weight loss that needs to be considered when deciding whether this is a serious medical issue. Since it sounds like your weight loss has come on quickly, this is more concerning but a bit less so since it seems like it has stabilized. Still, were you my patient, I’d recommend a more thorough evaluation than it sounds like you’ve had.
I learned a valuable lesson after I made a home visit to give a patient a thorough medical evaluation about her continued weight loss. I found that she had inadequate food at home and was too embarrassed to tell me.

My dog was small, but next time your off leash dog approaches a less friendly, large dog, it could be really bad, violent, or even deadly. On a recent visit to [local business], I was very disturbed at the state of the outside of the building. Decorative trees that should be trimmed and shaped each year are overgrown. The trees are taller than the building and encroaching on each other to make it look like a jungle. Where shrubs were once planted to enhance the entrance way is now filled with only weeds and dried dirt. What would it take to have a company come in twice a year and trim trees and shrubs and do some clean up? The [local business] should be ashamed at the impression their patrons get when stopping in or driving past one of the most visited facilities in the [local town].

What is Juneteenth?
Juneteenth stands for June 19, and commemorates the emancipation of enslaved African Americans. On June 19, 1865, General Order No. 3 was issued by Major General Gordon Granger at Galveston, Texas, proclaiming, “The people of Texas are informed that in accordance with a proclamation from the Executive of the United States, all slaves are free. This involves an absolute equality of rights and rights of property between former masters and slaves, and the connection heretofore existing between them becomes that between employer and hired labor.”
How is Juneteenth celebrated?
Juneteenth is often celebrated with cookouts, picnics, parades, family reunions, church services, concerts and other community festivities. Many people observe Juneteenth as a day of service. It’s also a time to reflect on history, recognize the wrongs of the past, build community, promote cultural awareness and focus on social justice.
Why didn’t the Emancipation Proclamation end slavery?
Although the Emancipation Proclamation declared all enslaved people in “rebellious” states legally free in 1863, it was not implemented in some southern states. The enslaved people of Texas were not effectively freed until federal troops took control of the state in 1865 and announced that enslaved people were emancipated by executive decree. This day came to be known as Juneteenth.

The demand for qualified, talented nurses figures to grow in the years to come.According to the U.S. Bureau of Labor Statistics, more than half a million nurses are expected to retire by 2022. Couple that with an existing nursing shortage, and the BLS projects a need for 1.1 million registered nurses in the near future. A shortage also figures to affect Canada, where the Canadian Nurses Association recently estimated that the country would be forced to confront a nursing shortage of roughly 60,000 nurses by 2022. Various factors are contributing to the nursing shortage in both countries, including an increasingly large aging population that will require more and more nurses to meet its health care needs. In the United States, the Affordable Care Act, which was passed in 2010, increased access to health care for millions of people across the country, thereby creating a greater need for nurses and other health care professionals. In addition, a greater emphasis on preventive care in many countries across the globe figures to compel more people to visit their physicians even when they’re otherwise feeling healthy. As more people prioritize preventive care, the need for more nurses will likely increase.

1 large red or yellow bell pepper, seeded and chopped
1/3 cup chopped red onion
1 bag (16 ounces) iceberg lettuce, carrot and cabbage salad mix
1 cup shredded Mexican cheese blend
2 cans (15 ounces each) READ 3 or 4 Bean Salad, drained 1/2 cup low-fat mayonnaise
1/2 cup low-fat sour cream
2 tablespoons finely chopped chipotle peppers in adobo sauce
1/2 cup crumbled tortilla chips (optional)
small avocado, sliced (optional)
In31/2-quartsaladbowlorsouffle dish, layer bell pepper, onion, lettuce, cheese and bean salad. Mix mayonnaise, sour cream and chipotle peppers. Spread over top of salad. Cover and refrigerate up to 8 hours.
Just before serving, sprinkle top with chips and sliced avocado, if desired. Toss to serve.

Though many plants are self-pollinating and others are pollinated by the wind or water, many others rely on insects and animals to become pollinated. The U.S. Department of Agriculture says birds, bees, bats, butterflies, moths, flies, beetles, and small mammals all can work as pollinators. Examples of crops pollinated by pollinators include apples, squash and almonds. Animals and insects help pollinate more than 75 percent of the world’s flowering plants, and nearly 75 percent of all crops, according to the U.S. Fish & Wildlife Service. Unfortunately, pesticide use can diminish the number of natural pollinators. Natural gardening and pest-control can help protect the habitats of pollinators.

Summer Heat Safety
Before we know it, we’ll need to move into hot weather mode and take steps to stay healthy and safe in the heat.
For me, most important is keeping the house cool, since that’s where I spend most of my time. The air conditioning will be set at 76 to 78 degrees, depending on the humidity.
Very soon I’ll need to put up the insulated sun-blocking blackout drapes on the sunny side of the living room where the afternoon heat can bake the outside wall. Each morning I’ll close those drapes before the sun moves around to that side of the house.
Next on my daily list will be to fill my water bottles -- a half-dozen 8-ounce bottles that I can periodically grab during the day. With the little bottles, it’s easy to keep track of how much hydration I’m getting.

On A Mission to Help Women and Girls Feel “Pretty Powerful”

According to a report from global consulting company Ducker Carlisle, women comprise only 2% of all automotive technicians, and the automotive technician career has the widest gender gap of all service personnel positions. As a female automotive technician in a male-dominated industry, Katie Colella of Palmyra has felt the loneliness of the gender gap.
at’s why in 2017, she launched Pretty Powerful, a nonprofit dedicated to teaching women and girls automotive skills. As co-owner of Reapers Custom, Pretty Powerful is a passion project born from Colella’s journey that started when she was 19, working as a secretary in an auto garage.
Sitting in an office was never the right fit for Colella, who is a hands-on sort of person with a flair for the creative. So, when she watched the guys in the garage painting a car one day, she knew she wanted to try it. e guys made her a deal: If she could convince a customer to let her apply her skills to their car, she was in. So that’s just what she did.
An older gentleman who drove a white Buick gave her the go ahead to repaint his bumper, which is when she realized her true calling. is set her on the path to earn a degree in business management while taking automotive classes. She continued to follow her passion post-graduation with a job in the same shop she started out in – but she missed female companionship.
In search of more women in her industry, she found a camp for middle school girls in Troy, NY. Camp “Rosie’s Girls” (a er Rosie the Riveter) was run out of the Emma Willard School for girls and focused on teaching girls skills from male-dominated industries, like firefighting and carpentry. It was a natural fit for Colella, who found a passion for teaching automotive skills to girls during the four years she taught at the camp.
When the camp shut down, Colella wanted to continue her work, and Pretty Powerful was conceived. She says, “My whole goal is to empower women to be self-sufficient and to know that they can do it. I want to provide them an atmosphere where they won’t be judged. ere’s no judgement in my classes; there’s no silly questions.”

According to Colella, the hardest part of changing your tire really is needing a little extra brute strength, but she has tips for that. When she teaches how to change a flat, it’s done as if it were a real situation. is means no special tools, just the ones most cars come with.

Currently, she’s teaching a class once a month through Mozaic, a chapter of Arc New York, and may soon offer her program through her local library. In addition to automotive maintenance and roadside emergency basics, she also teaches about women in automotive history. For instance, Charlotte Bridgwood was a vaudeville performer born in 1861 who is also credited with inventing the first electronically operated automatic windshield cleaner, called the “Storm Windshield Cleaner.”
As for the older gentleman who let her fix his bumper more than 20 years ago, she’s never forgotten him. He believed in her, and she carries that forward through her work of empowerment and self-sufficiency. Colella says, “I like it when women tell me things aren’t as difficult as they were in their head. Ra lesnakes are a risk for dogs
DEAR PAW’S CORNER: I moved from a suburb to the mountains a couple of months ago,and while my dogs are thrilled to have several acres to run free on, I’m terrified that they will be bitten by rattlesnakes. A neighbor said that they are plentiful out here. How can I prevent this, and what do I do if they get bitten? -- Larry in Tennessee
DEAR LARRY: While there is always a risk of a dog getting injured by something out in the countryside, it’s important not to let fear get the best of you. Instead, focus on prevention. Here are some of the things you can do to minimize their overall risk of injury -- from snakes and other potential risks.
-- Train your dogs to come when you call them; if your property is big enough that they run out of sight, train them to respond to a whistle, which carries much farther.
-- Rattlesnakes (and other snakes) hang out in messy debris piles, wood stacks and crevices with overhangs. They stay out of sight (they’re much more afraid of you and your dogs) unless they are threatened. Clear debris piles off of your property, and stack your firewood on a raised platform, at least 5 feet from your home’s foundation.
-- If your dogs consistently run to one location on your property, follow them and find out what’s so interesting. An animal lair or deer run can be very interesting to dogs, but the same location can attract bigger predators.
-- Consider fencing in a half-acre or acre behind your house and keep your dogs within the fence; you’ll be able to monitor them better and control the space, so critters don’t come in.
Follow this folding technique once the United States flag is lowered from its mast or mount:
1 Begin by holding the flag at waist height between two people parallel to the ground.
2 Fold the lower striped section of the flag lengthwise, up and over the blue field of stars.
3 Fold again, bringing the folded edge up to meet the open top edge. e blue field of stars should now be visible facing outward.
4 Bring the striped corner of the bottom folded edge up and over to meet the top edge in a triangular fold.
5 Turn the outer point of the triangle inward to align with the open top edge.
Display Guidelines
• e flag should be displayed in public only from sunrise to sunset, unless it is illuminated during darkness.
• e flag should not be displayed during rain, snow and wind storms, unless it is an all-weather flag not prone to damage.
• On Memorial Day, the flag should be displayed at half-staff until noon. e flag may also be flown at half-staff to honor a newly deceased federal or state government official by order of the president or governor.
6 Continue this triangular folding pattern along the entire length of the flag.
• When displayed either horizontally or vertically against a wall, the blue field should be uppermost and to the flag’s own right (the observer’s le ). e same guidelines apply to a window display: the blue field should appear on the le to observers viewing it from the street.
• e flag should never be displayed upside down, unless to convey distress or great danger.
• Dirty, ripped, wrinkled or frayed flags should not be used. An American flag that is worn or damaged beyond repair should be retired in a respectful manner.

American Legions, Girl Scouts, Boy Scouts and other organizations may collect flags as well. Ceremonies are most o en conducted on June 14, Flag Day.
• If a flag ceremony drop-off location is not available, anyone can hold a flag retirement ceremony, provided it is conducted in a respectful and dignified manner. Flags may be ceremoniously folded and burned in compliance with local and state fire codes and ordinances, or folded and buried in a wooden box. Flags made of synthetic materials that could be potentially hazardous if burned may be recycled through flag companies or various organizations. FUN WAYS TO KEEP KIDS OCCUPIED OVER SUMMER BREAK Reading: Parents may be surprised by how much their children enjoy a good book. A recent poll from the National Education Association found that 70 percent of middle school students read more than 10 books a year. The National Literacy Trust reports that roughly 45 percent of children between the ages of eight and 11 enjoy reading “very much.” When suggesting to children that they read more this summer, parents can note the many ways that reading for pleasure differs from reading for school. Point out that kids won’t have to submit book reports and emphasize that they can choose their own books. Depending on children’s ages, introduce kids to a series like “Harry Potter,” which is a set of seven fantasy novels that has helped millions of young people across the globe discover and develop a fondness for reading. An eco-friendly lifestyle is anything but selfless, as such a way of life can pay both immediate and long-term dividends, helping men and women in the present day while also contributing to healthier futures for generations to come.
As environmentalists and eco-friendly ordinary Joes no doubt already know, there are myriad ways to go green. Even mealtime at home can be made considerably more eco-friendly with relatively little effort.
Stop handwashing dishes. Men and women who dread having to hand wash their dishes after a nice meal have an ally in science. A study from researchers in Europe found that people who handwashed place settings for 12 people used as much as 27 gallons of water, while washing the same amount of place settings in a dishwasher required just four gallons of water. So instead of standing over a sink and washing dishes by hand each night, men and women can simply load up the dishwasher, making sure only to run it when it’s full so even less water is wasted.
Avoid the stove when possible. Empty nesters, couples without children or single men and women who want to save energy come mealtime can use toaster ovens in lieu of traditional stoves when preparing small meals. According to EnergyStar®, toaster ovens consume between onethird to one-half the amount of energy as conventional electric ovens when cooking small meals, making them a far more eco-friendly option for men and women accustomed who typically cook small meals for one or two people.
Shop with reusable shopping bags. Cooks know that great meals begin at the grocery store, so there’s no reason why eco-friendly cooking cannot begin there as well. Estimates suggest that as many as 100 billion plastic bags are used in the United States alone each year. EnvironmentMassachusetts reports that such bags are used for an average of 12 minutes, even though their life expectancy is as high as 1,000 years. By relying on reusable cloth shopping bags instead of single-use plastic bags, shoppers can dramatically reduce the carbon footprint of cooking their own meals.
Recycle materials. Glass jars, plastic butter tubs, milk cartons, and other items necessary to prepare meals at home are all recyclable. Many local governments have weekly if not biweekly pickups of recyclable materials, making it easier than ever to ensure these items are given new life after beingused.Menandwomenwhoresideinareaswithoutsuchpickupscan go it alone, collecting recyclable materials and periodically transporting them to nearby recycling centers.
