ROACH: I have read about using the immune system’s response as a mechanism to battle cancer (such as brain and pancreatic) and to reduce the devastating impact that cancer and its treatment has on humans. I understand current drug trials show promising results. How do doctors and scientists use the immune system to treat cancer? -- J.W.

COMMON CAUSES OF TOOTHACHES TMJ: Temporomandibular disorders can cause tooth pain. TMJ may result from injury or trauma to the jaw. Tooth grinding, also known as bruxism, can cause tooth pain.

ANSWER: The concept of using the immune system to treat cancer is old, but the ability to do so has dramatically improved in the past few years. There are a few specific types of treatments that I’d like to highlight:

Immune checkpoint inhibitors are treatments that release inhibitions on the immune system. (These “strengthen” the immune system, which has beneficial but also potentially toxic effects.) For melanoma, the most dangerous form of skin cancer, these treatments have revolutionized treatment and led to dramatically improved outcomes in people whose melanoma has spread. Response rates are up to 60% in a disease where the prognosis used to be dismal.

WORDS FOR NEW YEAR’S DAY

The unleashed immune system can destroy cancer very effectively, but it also can attack healthy organs, with many people (between 10% to 60% depending on drugs and doses) developing damage to the skin, lungs, liver, thyroid, colon and heart. These toxicities range from mild to life-threatening.

The body’s own immune cells can be genetically engineered to kill cancer cells. Chimeric antigen receptor T cells (CAR T cells) can be specifically changed to recognize cancer cells. This can lead to complete remission with some cancers, especially some leukemias and lymphomas. Unfortunately, they can also attack the body, leading to neurological damage that can be very serious.

Developing cancer vaccines is a highly promising strategy for personalizing the immune system to attack a person’s own cancer.

Although the United States has recently made dramatic cuts to its support of this research, researchers in other countries continue to study this. It can be used in combination with other traditional or immune-based treatments.

Contrary to popular belief, cats can be trained. Although they’re not likely to be as motivated by praise as dogs, cats can respond to food treats particularly something soft and smelly. However, that may take some practice as, according to Petfinder, cats are less instinctively driven to work in partnership with humans. But those tasty treats may inspire them to stay and come and even use a toilet in lieu of a litter box.

DEAR PAW’S CORNER: I have two laid-back cats, my boyfriend has a big sweet dog named “Gabriel,” and we are moving in together in a few weeks. Gabe is very well socialized and he spent his puppyhood in a big house with cats, so I’m not worried about him adapting to a new situation. But I’m not sure how my cats will react. How can I make sure life will be low-stress together? -- Chase T. in Atlanta

SAFETY FIRST WHEN DRIVING WITH PETS Do not leave pets unattended in cars. A study from the Louisiana Office of Public Health found that temperatures in a dark sedan or a light gray minivan parked on a hot, but cloudy day reached higher than 125 F in just 20 minutes. The study also found that cracking the window in such situations had little effect on the temperature inside the vehicle. On hot days, leave pets at home. Owners who must take their pets with them should never leave them in the car, as temperatures inside vehicles rise quickly, putting pets’ health in jeopardy.

DEAR CHASE: You have a great advantage in that your boyfriend’s dog is already used to living with cats.

Before your move-in date, gradually get the cats familiar with Gabriel by bringing him over for a few hours, several days per week if possible. Before his arrival, create a safe space for the cats where they can retreat, because when a big, goofy dog lumbers in the door, they will most likely run for cover.

Once Gabe is settled and calm, have him lie down, with your boyfriend keeping hold of his leash. Bring each cat out, one by one, in your arms, and calmly approach him. When the cat tenses up, hisses or tries to escape, stop and take a few steps back. Stay far enough back that your cat remains calm in your arms but can see the dog. After a few minutes, take your cat back to their safe space and repeat with the other cat.

Repeat this process on each visit to minimize stress and trauma for the cats and the dog. Continue after you move in together. It may take weeks or months for them to be comfortable with each other, but a calm, consistent approach will help.

Penfield Symphony Orchestra Celebrates Its 70th Season B Y S O P H I A KO VAT C H

For 70 years, the Penfield Symphony Orchestra (PSO) has been dedicated to its mission of bringing fine music to the local community and the greater Rochester area. Throughout its history, the Orchestra has featured nationally renowned soloists, commissioned its own musical compositions, and grown into a premier musical ensemble.

The Penfield Symphony Orchestra started in 1955, known as the Penfield Community Orchestra for its first 20 years. The group first organized within the Penfield Central School District as a part of their adult education program.

Its founders included Penfield High School’s music director and its principal, who sought a way to extend students’ musical development after graduation.

The orchestra soon became a community organization and was sponsored by the Town of Penfield until 1982, when it became an independent nonprofit. Around that time, the PSO commissioned its own works, including a one-act opera that premiered in 1985, and implemented the Young Artist Competition, inviting high school musicians from across the greater Rochester area to perform a concerto movement, with the winner performing with the full orchestra at a concert.

Today, the PSO still runs the Young Artist Competition (now named for Jo Amish, an early violinist with the PSO) and maintains a connection to Penfield High School, where they perform each concert. The school’s auditorium is one of the few venues large enough to comfortably hold the full symphony orchestra, usually 80-85 strong. Sometimes more musicians are called upon to perform specific works.

Seats in the Orchestra are highly sought after, with a long waiting list to even audition. Alan and Carol Lowne, who currently serve as co-presidents of the PSO, have been involved with the group since 1999, when Alan landed a seat in the violin section. Carol had to wait five years for the sole English horn seat, which she still holds.

Carol says, “It is an absolute privilege to play in this group.

The standard is so high that you really feel you are bringing the best music you can to the community.”

As co-presidents, the Lownes work behind-the-scenes to keep a complex ensemble like a symphony orchestra operating at such a high standard. They’ve split the job for the last seven years, focusing their efforts on building a deeper connection with the audiences who support the Orchestra’s mission and music.

Carol serves as the face of the PSO, welcoming audiences to concerts in a manner that’s more upbeat and direct than one might expect at a symphony. She says the effort to make music more approachable extends into the performance, in which audiences hear more context around each composition before it’s played. “We encourage the music director to explain the pieces, what they are and why they chose them,” Alan says. “We’re building a bridge between the music and the audience.”

Dr. Jared Chase has been PSO music director since spring 2024, following a lengthy search to replace the previous music director, David Harman, who had conducted the Orchestra for more than two decades. Chase has conducted numerous choral and instrumental groups, as well as musical theatre, opera, and the New York State Ballet, where he served as music director for seven years. He is also Director of Wind Studies at Nazareth University and music director for the First Presbyterian Church of Pittsford.

He says he plans an entire season at once, rather than concert by concert, often building a season around keystone pieces and masterworks. He also makes selections that highlight current strengths of the overall orchestra.

If I could just apologize If I could just apologize to the very nice older couple that visited “our” bargain basement store on Monday, December 8th from Brighton. They were obviously seniors getting along together well. That’s so nice to see!

A coworker and I found the square new rug which they’d been looking for. They wanted blue colored, not white. Unfortunately, when I suggested buying slippers so the rug would stay clean, I remembered a slip and fall I’d had and ran out to the front door and overreacted. Please, customers, come back to our store, I apologize from the heart, it was “a senior moment!”

The city speed limit is 30. Why post that when so many ignore the speed limit? Some do 40-50, zigzagging, no turn signals, no stopping at stop signs, cutting others off when it’s “right on red when clear,” etc., etc. Will the police please monitor, hopefully every day, the main road? Visibility is a deterrent to at least slow them down.

Drivers & pets To add to your vent, as a retired flag person, I saw this many times. My added advice is, if you ever get in an accident, and your air bags go off, how will your pet fare?

Actions give neighborhood trashy look People, please reconsider parking or storing cars on the front lawn. The practice has grown from one house to multiple houses. One car has no plates; one is wrapped in tarp. The rest get driven daily then back on the lawn. One house has now added a second car, giving their place that car-lot look.

To wedge it in they blast over the curb and sidewalk to get that second car to fit. Over time, muddy ruts have formed and walkers have to go to the road or get mud all over their shoes.

People still care about the appearance of their homes and neighborhood. This parking practice kills the grass, looks very junky, and the weight of a car or two sitting on underground service lines could possibly damage them. Apparently, it is against village code but code enforcement says they don’t have time to deal with this and suggest calling the police.
