Dry Cough Persisting for a Decade Might Indicate Asthma

Q.

DEAR DR. ROACH: I have a dry cough that’s been going on for about 13-14 years. I’m a healthy 62-year-old and read that it is called “cough variant asthma.” I would like to know more about this but can’t really find anything about it, except that it will eventually get worse if it’s not treated. I was checked twice in the past 15 years for asthma, and I wasn’t diagnosed. However, I’ve not been diagnosed with any acid reflux or postnasal drip. -- D.S.

SENIOR NEWS LINE

ANSWER:Although most people think that asthma causes wheezing, cough is a very common symptom of asthma. You’ve named the two other most common causes of chronic cough: acid reflux and postnasal drip. It can be tricky to make these diagnoses. Very often, primary care doctors will do a “therapeutic trial” that involves a short-term prescription to see if there is improvement. For acid reflux, it might be an H2 blocker like famotidine (Pepcid), which acts faster than a proton-pump inhibitor like omeprazole. This is often faster, easier, and much less expensive than a diagnostic test like a 24-hour pH monitor (the gold-standard diagnostic test).

Similarly, a physician might give a trial of asthma medication (such as a bronchodilator like albuterol) to see if it stops the cough, rather than immediately testing a person’s pulmonary function with a trial of bronchodilators -- a standard diagnostic test for asthma. Sometimes a pulmonologist will order a “methacholine challenge test” where an airway irritant is given at very low doses to see if it will trigger an asthma response in the lung. Postnasal drip can sometimes be confirmed by a physical exam, but a fiberoptic laryngoscopy is seldom necessary to make the diagnosis. When therapeutic trials are not helpful, diagnostic tests become necessary. I personally order lung testing before and after bronchodilators on all patients with suspected asthma, as the tests give important information beyond a diagnosis. Lung testing can be used to follow the course of the disease and identify whether there may be a separate problem in addition to asthma.

Convenience versus safetysometimes it’s impossible to have both when it comes to tech devices.

There are more smart devices than we could have imagined 10 years ago, and every day brings something new. We have smart door locks, thermostats, refrigerators, indoor and outdoor security cameras, televisions, cellphones, light bulbs, speakers and wall plugs.

Whether it’s data breaches, surveillance, intrusions, hijack of your device or outright cyber attacks and theft of your information, smart devices can make you vulnerable if they are hacked.

Take smart thermostats, for example.Those smart gadgets track all kinds of things about you. Scariest of all is that they keep track of when you leave home and when you come back, information that a thief would like to have.

If you want a smart device, be sure you understand the downside and the risks. Keep the software updated, use strong passwords and understand the settings, especially on any device with a camera or microphone. Your smart television speakers could work in the reverse and capture the sounds in the room, for example. Before you buy a smart device, or to learn more about devices you might already have, go online to search for “dangers of smart devices.”

A SNAPSHOT OF THE CANAL CLEAN SWEEP ACROSS THE REGION:

Memorial, Community, Service, and Gloomy Weather

rom Albany to Buffalo, thousands of volunteers rolled up their sleeves, donned raincoats if necessary, and carried garbage bags as they collected trash and debris in canal towns across the state as part of the annual Canal Clean Sweep event.

Now in its 20th year, Clean Sweep is organized by Parks & Trails New York and the New York State Canal Corporation in honor of Earth Day weekend. According to the Parks & Trails New York website, more than 140 locations had groups registered to clean along

Registered teams were sent t-shirts to wear at the event and garbage bags to collect trash. Anyone can register to start a cleaning group by signing up on the Parks & Trails website. e event saw a diverse rangeofgroups,includingorganizations,businesses,andindividuals with varying motivations that all made it a day to remember for many living along our canal system.

In Memoriam

Newark resident Linda Brunner organized her second annual cleanup group, called RJ’s Army, to honor her son, RJ VanDeMortel, who died several years ago. Brunner and her son became “endto-enders” when he was alive, cycling the entire length of the Erie Canal Trail between Buffalo and Albany. Brunner says, “We have a lot of feelings about the canal trail,” and they also hold a fall 5K along the path in his honor.

While last year they had a few volunteers join them, this year Brunner says it was just her and her family, which likely was because of the rainy day. at didn’t stop their determined group of five, who found all types of debris to pick up, including plastic oil bottles, cans, juice containers, and more. Brunner says, “It’s nice to keep it clean and available to the community. ere are so many people who utilize it, some people even use it to ride bicycles to work. It’s such a wonderful Earth Day event to keep our little canal trail clean.”

they picked up about 25 bags of trash during a three mile walk along the canal. e group included several members from each organization being represented, as well as BSA Scout Troops 166 and 6166, families, and individuals. Graham says she likes encouraging her kids to get involved, and living so close, her children have grown up fishing, walking, and biking along the paths. She says, “When it comes to making sure that our environment is clean for future generations, events like this contribute to awareness and encourage continuous participation, and not just at these events.”

e Pittsford Rotary Club likes to get local children involved in their cleanup. Pat Mallery, a resident who organized the Rotary’s clean sweep group, says they had two groups that split up, with his group including about 15 volunteers. e club has two Interact clubs involved, one from each of the Pittsford high schools, so they o en have a large group of young adults get into the cleaning action. e last couple of years they’ve also had local Scouts join as well. Mallery says the Clean Sweep event is “a nice outdoor activity; kids are doing something for their community and they get community service credit, so it’s a win-win.”

Caring for your pets in the warmer weather

DEAR PAW’S CORNER: Spring is here, and summer is just around the corner. Can you please remind people that leaving pets in a hot car is a deadly mistake? -- Carol in Syracuse, New York

DEAR CAROL: Absolutely. Folks, even if the weather outside is just warm -- not hot -- the inside of your car can become extremely hot when it’s parked in the sun. Even with the windows cracked a bit, the temperature can still rise by 20, 30 and 40 degrees higher than the outside air temperature. So, if it’s a balmy 65 degrees outside, the interior of your car can reach 105 degrees or higher -- enough to cause heat injury or death for a pet.

That’s why 31 U.S. states now have laws banning pets in parked cars. It’s why police and even private citizens, in some areas, can smash a car window to save a pet. And it’s why pet owners who leave their animal in a parked car may face fines and, in some states, prison. Those laws do not make exceptions for cars that are left running to keep the air conditioning on. If the car stalls or has an automatic switch-off, the interior will heat up dramatically even on a cloudy day.

I wrote a check to give to my child’s teacher. I don’t know whether it fell out of her pocket on the bike ride to school, in front of the school, in the school, or in the garbage with scrap paper as she thought it had, but thank you to whatever good human put it in my mailbox. It’s not a large sum of money, but anyone could’ve written cash and cashed it. Instead, it was put in my mailbox carefully. Thank you! We all need a little reminder that there are good people and this was mine for today.

We were out to eat at a smaller, local restaurant for breakfast this past weekend when a large group coming in needed a few of the tables together. While they were in the process of moving things around a older couple came in to eat and was told they didn’t have a table to seat them. That’s when a younger couple sitting at a table in the middle offered up their available 2 seats to the older couple to all enjoy breakfast together. Truly a sweet sentiment.

Why is it that most gas pump handles don’t have the little hook that catches the handle so that the pump continues to fill your car without you having to hold the handle? Spring is finally upon us, but during the Winter months I actually seek out the pumps (and I know where they are) that have this handle hook! Once you have the handle ‘hooked,’ you can return to the warmth and safety of your car until your tank is full. So simple an idea ... and why do you need my zip code and bonus card number and want to know if I want a car wash... a waste of my time.

Is it just me or does anyone else feel like basic kindness doesn’t exist around here anymore? I’ve noticed lately that so many parents seem to avoid eye contact, rarely say hello, or even acknowledge each other - and I’m genuinely curious why. No smiles, no ‘excuse me,’ no ‘thank you.’ It feels cold and honestly, it’s sad.

I’m not posting this to point fingers or judge anyone - life is busy and we all have our moments. Our kids are watching. They see how we treat each other. But we can do better for each other and for our children. Have you felt this too? How can we help bring back a bit more kindness and warmth into our day-to-day interactions?

DID YOU KNOW? A landmark study conducted by researchers in the United Kingdom discovered a notable link between low to moderate alcohol consumption and breast cancer risk. According to the Million Women Study, every 10 grams (roughly 0.35 ounces) of alcohol a woman consumed per day was associated with a 12 percent increase in her risk for breast cancer. The Million Women Study focused on various areas of women’s health but included 28,000 women who had breast cancer. In addition, the National Institute on Alcohol Abuse and Alcoholism notes that more than 100 epidemiologic studies have examined a potential association between alcohol consumption and the risk for breast cancer, consistently finding that risk for the disease increases as alcohol intake increases.

The tradition of Mother’s Day in the United States dates back more than a century to 1908, when West Virginia native Anna Jarvis held a memorial service to honor her mother, who had passed away three years earlier, and all mothers in Grafton, West Virginia. Mother’s Day would ultimately become a national holiday in the United States in 1914, thanks in large part to Jarvis’ campaigning. As Jarvis, who never married and never had children of her own, grew old, she criticized Mother’s Day as overly commercial, even trying to have it removed from the calendar. Those efforts failed, and Mother’s Day remains popular now, more than 100 years after the idea for it was conceived.
