With an increasingly aging population, the role of the caregiver becomes an even more important one, both functionally and economically. Caregiving is the ultimate act of love. nwh_constellation_03 Connections Program and the Westchester Public/Private Partnership for Aging Services are collaborating to offer caregivers emotional support, counseling, and the resources necessary to navigate the health care system free of charge. If you need a shoulder to lean on; a good listener to hear your voice; assistance in identifying and addressing health system barriers; an unbiased intermediary to facilitate communication between you and your care recipient; or a linkage to community resources, a care navigator is available to assist you.
