CEO Report Grace Health was established in June 2008 and is a 501(c)3 non-profit Federally Qualified Health Center (FQHC) serving the southeastern Kentucky counties of Bell, Clay, Knox, Laurel, Leslie, and Whitley. What started as two Community Health Center sites in Gray and Manchester, Kentucky has since grown to include 11 satellite clinics employing over 300 community members, including more than 50 providers, and caring for 30,000+ people each year. Grace Health offers a variety of services, including primary care, women's care, dental care, pediatrics, pharmacy, behavioral health, school-based health, x-rays and labs, and outreach and education. The center offers compassionate and integrated care that is coordinated by primary care practitioners to provide medical, preventative dental, and behavioral health care, as well as 340B discounted pharmacy for all patients regardless of their ability to pay. Grace Health operates 32 school-based health centers in Leslie, Bell, Knox, and northern Whitley counties, providing medical and preventative dental services to students in those areas. The poverty rate in southeastern Kentucky is 33.7%, double the state and national average. Over 50% of the population in southeastern Kentucky is unemployed or no longer seeking employment. The decrease in Kentucky's coal mining industry and the increasing incidence of substance use disorder have fractured many families and negatively affected employability in the service area.
