SUCCESS STORY

Document-Ready Clients (This process takes about 3-6 months on average to gather ID and Social Security cards, birth certificates, and medical documentation)

After receiving a referral from the Homeless Solution Manager of Downey, City Net met with a woman who had been experiencing homelessness for several months. Upon meeting with her, case managers learned that she had been struggling to adjust after being unhoused when she fell on hard times, but she was excited to enroll with City Net (continued on page 3).

City Net worked with Code Enforcement to post notices of clearing at abandoned encampments along Firestone Blvd and around railroad tracks. City Net has been partnering with The Bell Shelter in Bell, CA to connect clients seeking shelter to essential programs that have helped them get off the street during the harsh winter weather.

• (Continued from page 1) Case managers learned that she had recently left the hospital and was worried about staying out in the cold, so entering a shelter was her primary goal. During that meeting, the case managers submitted referrals to shelters in the area for her. Within a week, the client was approved for the Salvation Army Bell Shelter and case managers transported her directly to the shelter to ensure that she could enter the program and rest assured that she would not spend another night out in the cold.

• Residents in the community contacted City Net's outreach team regarding an encampment located on the railroad tracks. When case managers responded to the call, they met a gentleman who explained that he was a military veteran who had been recently released from jail and had no safety net after leaving. The team recognized that reconnecting the client to his benefits would be a significant first step in his program and immediately offered to help him obtain a new ID card He accepted the offer and received transportation to the DMV for his ID and after he got his temporary copy, the team took him to the Department of Public Social Services There, his case managers assisted him with applying for Medi-Cal and Cal Fresh so that he could start working on caring for his health right away. Now that he has his benefits established, he continues working with City Net to connect with the Veterans Affairs office and apply for general relief aid before he starts working on housing navigation. The client thanked City Net for understanding that addressing his immediate needs and health provided him the peace of mind to start planning for his long-term future

Positive exits indicate clients moved off the streets or out of shelter into a more stable living situation.

Temporary housing includes rehabilitation programs, institutional care facilities, and all transitional housing placements.

Permanently housed clients rent a private residence where they may receive ongoing subsidies or have moved into a residence with family or friends permanently.
