The team focused outreach efforts on two main areas in Downey: around City Hall and along Firestone Boulevard. Since more clients have been appearing in these areas, case managers emphasized engaging individuals and connecting them to the Salvation Army Bell Shelter and interim housing opportunities.

When he finally enrolled, the case managers were able to complete a referral to Housing for Health and enrolled the client in street outreach to keep him updated on the response from the Department of Health Services.

An elderly gentleman enrolled with City Net eight months ago to return to the workforce. Case managers advised the client to explore Social Security retirement benefits and determine if he would qualify. However, the client was hopeful that he would

find work and dismissed the suggestion at the time. For six months, case managers helped connect him to employment resources and provided transportation to job fairs. But when he began to worry that he would never find an opportunity, case managers suggested revising his Social Security Benefit options. With their help, he completed phone interviews with the local Social Security office, and within a month, he set up direct deposit for Social Security retirement payments. At the same time, case managers helped him reunite with a former neighbor who was looking to rent out a room. By the end of November, the client was housed and looking forward to rebuilding his own support network. •

When City Net first began working in Downey, the outreach team engaged a gentleman who was staying near the railroad tracks along Firestone Boulevard. During their early conversations, the client explained that he had never sought out shelter placement due to nightly curfew requirements. He feared that if he were to obtain employment that fell outside the curfew window, he would lose his shelter placement and any progress he might have made towards permanent housing. While building up trust with the client, case managers knew that pushing shelter on the client prematurely could hurt their progress and focused on offering other services first. Thanks to this plan, the client applied for general relief, Medi-Cal, and obtained a free government phone to stay in contact with his case managers. Two months ago, after multiple conversations about the client's housing goals, the team assisted him with completing a Department of Housing Services' application to Housing for Health. By the end of November, the client was matched to an interim housing opportunity at Rancho Los Amigos! The timing perfectly allowed the client to enter a shelter right before Thanksgiving!

Positive exits indicate clients moved off the streets or out of shelter into a more stable living situation. Temporary housing includes rehabilitation programs, institutional care facilities, and all transitional housing placements. Permanently housed clients rent a private residence where they may receive ongoing subsidies or have moved into a residence with family or friends permanently.
