A MESSAGE FROM YOUR CEO

Dear Team Members,

As we step into the new year, I want to take a moment to reflect on the holiday season—a time to give thanks for the successes of the past year and embrace the promise of new opportunities ahead. I extend my deepest gratitude to each and every one of you; our dedicated Team Members for your contributions in 2024 and I am excited to see what we can accomplish together in 2025.

It’s overwhelming to look back at the exponential growth DSL experienced in 2024 – both in welcoming new communities and Team Members alike. We are now a growing portfolio with more than 17,500 DSL Team Members strong, across nearly 40 states and in excess of 350 communities. The strength of our Company continues to allow us to advance our mission to provide a “better way to age” to seniors in this country and we will never waiver or forget that primary mission, while offering tremendous career opportunities to our Team Members and strong financial results to our capital partners.

In this first Connection volume of the new year, you’ll learn about our newest communities that joined us in the second half of 2024, as well as some familiar communities that have been rebranded to align under our management companies. You’ll celebrate Choreograph Gainesville welcoming its first residents and hosting their “Opening Night” event, plus read about our very own “hurricane hero” Team Members who went above and beyond during 2024’s very active hurricane season, and much more.

PERFORMANCE UNDER PRESSURE

Job descriptions may outline the essential roles and responsibilities of our positions, but they don’t define the limits of our contributions. Going above and beyond for our residents and fellow Team Members is rooted in the DNA of Discovery Senior Living. High performance standards are ingrained in the very fabric of our organization.

While many may interpret our “Performance” Pillar of Excellence to be something you strive to achieve at a high level on your own, it’s our outstanding efficiencies and partnerships during challenging times where we come together to perform at our highest capacity and show why we are exceptional at what we do.

Atlantic hurricane season, which includes the Atlantic Ocean and Gulf of Mexico, runs from June through November 30 each year. With a high concentration of communities along the Gulf Coast and up the eastern seaboard, the threat of hurricanes are not out of the norm for our communities. In 2024, we had three large storms directly affecting 51 of our communities, including Beryl in early July, the earliest category 5 hurricane in the last 20 years, Hurricane Helene with monumental rain and flooding in the Carolinas and Tennessee, and category 3 Hurricane Milton, which made landfall within 50 miles of 15 of our Florida communities.

To say this hurricane season tested the Performance of our Team Members is an understatement. Going beyond the day-to-day care that our residents rely on us for, our Team Members in some communities where it became absolutely necessary, were responsible for evacuating residents to a safe place and managing their care during the evacuation.

When we were put to the test with a hurricane season like this past one, how does a pillar like Performance shine within a company already known for its high standards?

It’s the cross-partnership within our Management Companies, sharing resources and offering help wherever (and whenever) needed.

It’s TerraBella Senior Living President Kellee Agee sourcing everything from garbage pickup to meals for residents and Team Members, to toilet paper for her communities.

It’s Vice President of Facilities Eric Skedel making generators and fuel seemingly appear out of thin air to the communities that needed them. And oftentimes an electrician, too.

It’s National Brand Manager Cindy Crowley making the tough decision to evacuate Assisted Living and Memory Care Residents twice in two weeks for their safety.

It’s Vice President of Culinary Services John Vogelmeier and Vice President of Procurement Todd Howell working endlessly to make sure residents and Team Members had food and water in areas devastated by wind and floods.

It’s all the Team Members who stayed in their community for days to be there for our residents when their own homes were in danger.

It’s every one of the leadership Team Members who asked, “What can we do? How can we help? What do you need?”

Performance is dedication, innovation, and a promise to always do what is best for those we serve. Thank you to all our dedicated Team Members for always showing up and performing at the highest level! We are appreciative of each of you.

A huge shoutout to Eric, Hugh, John, Todd, Cindy, Marisa, Kellee and Linda for their standout efforts, along with everyone else who played a part in supporting our communities during such a challenging time. I just attended Senior Living Innovation Forum (SLIF) and received feedback from an executive at a fellow owner/operator company with a mother in one of our communities. She said the team at Discovery Village Sarasota Bay really went above and beyond to make those Independent Living residents who did not evacuate comfortable, going as far as retaining an RN on-site during the storm for precautionary measures and to put the residents at ease.

All Facilities leaders, including Hugh Guy, Joe Casino, Edward Young, Timothy Sutton and Jason Dearman, worked day and night moving equipment from one community to another. From fueling staff’s vehicles with gas, deploying vendors, patching roofs, moving generators from one division to another, they worked so hard to keep the lights on and allowed us to continue operating in the worst conditions.

Evacuations are certainly a time for team performance! Teams across the regions partnered during the hurricane to maintain generators, food supplies, and safe environments for residents, staff, and families. It is during a hurricane event that we see the passion and commitment that our DSL teams have for our residents and each other.

Our Area Director of Operations Joe Casino worked tireless hours on the evacuations, both assisting in departure and assisting in return of the residents. He was a solid rock of dependability that others can look at as a goal. As his regional director, there was nothing I asked him that he did not do with careful attention to detail and motivated those around him to complete the task.

My first blog of 2025: a year that starts with so much promise for Discovery, I can’t help but be excited! Excited not just about our growth and momentum from the prior year, but about the maturation of our management company concept, the excellent support teams and subject matter experts (SMEs) in place, and our progress toward a higher level of functioning than any operator in our field.

My first job in senior living was as a Chief Information Officer, so I am also charged up about the new systems that will be going live in ‘25 (is that a slogan?)! We will see new accounting, clinical, sales & marketing systems all in the same year. Our new purchasing contract not only brings us savings and efficiencies but access to menu systems and capital projects software that we have lacked. We have rebuilt our network infrastructure in a safer, more secure, environment and have turned over the help desk to a larger company for broader, more immediate support in our communities (see announcements from Margo Kidd).

BILL’S BLOG

I shared with our leaders in Dallas on January 6 that the overall growth and new software might make us better – but cannot replace our work as leaders and teammates. We control our destiny through our decisions and actions, through the urgency we bring to every situation. This year should be a year of refinement, excellence, profitability, standards and policies, resident and Team Member satisfaction, risk mitigation, and consistency. Our partners and investors value accuracy and predictable results over mere growth. This year, we will continue to evolve to the highest standards; we go live in ‘25!

Hello everyone!

I recently stopped for a moment to consider, “How long have I been with Discovery?” More on that in a moment…

Home health is the ultimate look forward and look backward business, here is why. Each year, we investigate the new Medicare rules and adapt our business methods to be both legally compliant, to maximize clinical outcomes, and to make sure we are profitable doing so. Typically, we figure out the first two components quickly, and then play a bit of catch up on the financial outcomes, usually coming on like a freight train each fourth quarter. This past year was no different. We look backward to double the check of our clinical outcomes (that are published to the public) to make sure we are doing the important things and small things very well so that our clinical outcomes lead our local markets. We are blessed to have three smart agency leaders that eat these challenges like breakfast, lunch, and dinner. So well in fact, our Texas agency will be getting some extra payments each month in 2025, due to their outstanding clinical outcomes. Well done, Discovery At Home Texas!

So, in my thoughts on my time with Discovery, I struggled a bit to determine how long we have been doing this here at Discovery At Home. Meaning, how many times have we had to work through the maze of new rules, adapt/adjust and then excel! December marked the end of my 11th year and January 2025 marks the beginning of our 13th year serving our residents. It has flown by like a hurricane!

In closing, I want to recognize everyone who worked through numerous hurricanes this year. The commitment of our professionals never ceases to amaze me – No complaints, complete dedication and selfless service. Thank you sincerely for all that you do, and that you did in 2024, to improve the lives of our residents.

SHINE ® MEMORY CARE PROGRAM

Recognized by the Alzheimer’s Association® Curriculum Review

Discovery Senior Living’s (DSL) SHINE® Memory Care Program has been recognized by the Alzheimer’s Association® for successfully completing curriculum review and incorporating its evidenced-based Dementia Care Practice Recommendations in the following topic areas: Alzheimer’s and dementia, person-centered care, assessment and care planning, activities of daily living, and behaviors and communication.

The Alzheimer’s Association, the country’s leading, voluntary health organization dedicated to Alzheimer’s care and research, provides this distinction to a very small and elite group of U.S. Memory Care providers. Its Dementia Care Practice Recommendations include a comprehensive review of current evidence, best practices and expert opinions. Our Memory Care Leadership Team and Community Team Members use these recommendations to inform and influence dementia care standards, training, practice and policies.

“We are honored that our SHINE Memory Care Program Curriculum has once again received the esteemed Alzheimer’s Association recognition for 2024-2026,” said Dawn Platt, Discovery Senior Living National Director of Memory Care Programs. “This prestigious approval, now awarded for the third time, reaffirms our unwavering commitment to providing compassionate, evidence-based care to those living with Alzheimer’s disease and related dementias. I am incredibly proud of our team’s dedication and innovation, which continues to set the standard in memory care. Together, we are making a meaningful difference in the lives of our residents and their families every day.”

Successfully completing the Alzheimer’s Association Curriculum Review helps professional care workers remain on the leading edge of person-centered care for those living with dementia. It reflects DSL’s commitment to using the organization’s recommendations in our SHINE Memory Care training. Team Members who complete this

training are eligible to become certified in the Alzheimer’s Association’s essentiALZ®, an exam that demonstrates knowledge of quality dementia care practices.

“The Alzheimer’s Association is pleased that Discovery Senior Living is aligning its training program with our Dementia Care Practice Recommendations,” said Emily Shubeck, Alzheimer’s Association Director. “By taking this step, DSL has demonstrated its commitment to providing evidence-based training with a person-centered focus at its core.”

High-quality dementia care training can lead to improvements in communication between caregivers and people living with dementia, a reduction in dementia-related behaviors, and an increase in job satisfaction and retention for Team Members.

To celebrate, the community hosted a special “Opening Night” event on December 7 fit for a Hollywood premiere. Residents and potential new residents were in attendance for the evening of glitz and glamour, complete with gourmet hors d’oeuvres, live musical performances, and special tours of the new, resort-style 55+ active living community, located in the heart of Gainesville, Florida’s Celebration Pointe.

A special thank you goes out to the many Team Members across Discovery who helped make Choreograph Gainesville’s official debut a successful one.

Love was in the air for 28 of our communities in September as residents enjoyed a special “Senior Prom” night and viewing party to kick off ABC’s The Golden Bachelorette season. ABC partnered with the communities to provide a prom kit to help them celebrate in style with everything from Prom King and Queen sashes, gold decorations, and trivia to add to the festive night.

Residents fully embraced the theme, dressing up in their prom best, and our Culinary Team Members prepared spreads fit for kings and queens! Thank you, ABC, for choosing our communities to participate in this special evening celebrating love at any age!

Enjoy these new recipes to share with loved ones this season, courtesy of Culinary Services Director Lisa Green from Sage Lake in Covington, Louisiana.

INGREDIENTS

6 mirliton (chayote squash)

1 onion diced

1 green bell pepper diced

1 red bell pepper diced

1 yellow bell pepper diced

¼ cup minced garlic

½ bunch parsley diced

½ cup granulated garlic

½ cup granulated onion

INSTRUCTIONS

1 cup vegetable oil

3 cups water

1/3 cup Tony Chachere’s (or seasoning of choice)

2 pounds shrimp 50/60 (any size you prefer)

½ can claw crabmeat (lump crab is okay)

2 cups Italian bread crumbs

1. Steam or boil the mirliton for 1 1/2 hours or until tender, trying not to break them up.

2. In sauté pan, add vegetable oil over medium heat. Add onion, green bell pepper, red bell pepper, yellow bell pepper, and minced garlic. Sauté for 7 minutes or until tender.

3. Add 3 cups of water to the pan, parsley, granulated garlic and onion, and Tony Chachere’s. Stir in well.

4. Add shrimp and crabmeat. Cook for 10 minutes or until shrimp is thoroughly cooked. Set aside.

5. On cutting board, slice mirlitons down the middle. The seed needs to be taken out. With a spoon, scrape the inside of the mirliton directly into the sauté pan with the rest of the ingredients. Set aside the shells on a baking sheet. Mix all mirliton in sauté pan well, breaking it up as you go. Add bread crumbs to sauté pan mixed in well for thickness.

6. Add the stuffing to the mirliton shell. Bake in the oven for 15 minutes and enjoy!

INGREDIENTS

FOR PUDDING:

6 large eggs

2 quarts heavy cream

½ gallon milk

1 loaf diced raisin bread

6 French(po-boy) bread diced (regular bread 4 loaves)

½ pound unsalted butter melted

2 cups sugar

1 cup light brown sugar

½ cup vanilla extract

1/3 cup cinnamon

FOR SAUCE:

2 cups heavy cream

2 tablespoons unsalted butter

1 cup brown sugar

½ cup dark rum (optional)

1 tablespoon cornstarch

INSTRUCTIONS

1. In a large bowl, combine bread, butter, heavy cream, eggs and milk. Mix until bread is wet and smooth.

2. Add sugar, cinnamon, light brown sugar, and vanilla. Combine well. Add to coated hotel pan.

3. Bake for 25 minutes at 325 degrees. Bake an additional 10 minutes if necessary.

4. While baking, start on sauce. Add all ingredients to a saucepan. Whisk together on medium heat.

5. In small bowl, add cornstarch and water, mix well together. Pour cornstarch into saucepan while whisking for thickness. Add rum (optional).

6. Pour sauce over bread pudding. Serve and enjoy!
