Following our January 2017 merger with Family & Children Services of Silicon Valley, we have expanded our reach in Santa Clara County and strengthened our capacity agency-wide to help our most vulnerable community members move toward greater wellness, resilience, and independence.

At Caminar, we provide a path toward recovery, wellness, and independence, especially for people who often are marginalized. Our name, a Spanish verb which translates as “to walk (with a purpose),” reflects our clients’ personal journeys toward their recovery goals. Our name also symbolizes Caminar’s commitment to walk with our clients.

Following our January 2017 merger with Family & Children Services of Silicon Valley, our scope of service, areas of expertise, and geographic reach have expanded. This growth strengthens our capacity to effectively and holistically address the complex needs of individuals and families, who come to us with interrelated mental health, substance use, physical health, and critical living needs, such as housing and employment.

You are helping us nurture and grow a comprehensive, “whole person” model of care to benefit vulnerable youth and adults. Philanthropic investment allows our clinicians, nurses, case managers, and advocates to use their expertise to innovate, apply data-driven and proven practices, pursue excellence in clinical care, and transform people’s lives.

Inside, you will find a story of how the generosity of our donors created a circle of support around a family during a time of crisis. You also will discover how the compassion of our supporters empowered a young person to move from hopelessness and homelessness toward a full, independent life as part of the community.

Valley (FCS) merged in January 2017, bringing together complementary areas expertise and a combined 120 years of service.

Jobs Plus remains the top provider in co-op

services (since 2013). Caminar is a partner in San Francisco’s Vocational Rehabilitation Employment and Training Program, which was recently selected as one of the National Association of Counties’ (NACo) 100 Brilliant Ideas at Work.

Our Solano Region was selected to implement the

county’s coordinated entry approach to reducing homelessness.

Our FCS Division’s Substance Use Disorder Services

Program launched the Supportive Transition Empowerment Program (STEP), which engages adults with histories of addiction in services prior to their release from jail. Upon release, clinicians provide intensive support to help prevent relapse and promote connections with treatment, housing, and services.

Caminar’s revenue in the 2017 Fiscal Year was $7.6 million greater than in 2016 Fiscal Year. This growth was the result of the merger with Family & Children Services of Silicon Valley in January 2017, and program expansions, such as the addition of Assisted Outpatient Treatment services in San Mateo County.

Solutions We take an evidence-based approach to behavioral health that takes into account a client’s holistic needs and seeks to empower our clients to increase their health, well-being, safety, and independence.

We are building out key services, especially integrated services for patients with complex health needs (such as mental health, substance use, and/or physical health conditions), as well as other proven preventive and supportive services, such as family violence prevention, school-based mental health, and supportive housing, that leverage the unique case management and psychosocial rehabilitation models that are integral to Caminar.

The Road to Resilience: Paving the Road to Wellness Proactive Support Reunites a Family “Being connected to the program gave us the support needed to keep our faith as we endured the hardest period of our lives.

True to the Teen Families and Schools Together (Teen FAST) Program promise, when floods ravaged some residential areas of San Jose in February 2017, affected families had a network of support from which to draw strength and support. As the Alvarez family dealt with the confusion of displacement, they faced an even more devastating challenge: Separation.

Just prior to the floods, Luisa began to experience mood and behavioral instability. The family had not yet had a chance to seek help. Concerned by Luisa’s behavior, social workers assisting families affected by the floods placed Edgar, 15 years old, in a group home.

Devastated by the mother’s sudden onset of mental illness and the son’s placement in the system, the Alvarez family embraced the support of the Teen FAST program at their son’s school.

The Teen FAST team worked tirelessly on the family’s behalf. The mother was quickly connected with health professionals and learned that her destabilized behavior was the result of a severe eye infection requiring surgery. With mom on the road to recovery, the parents moved in with extended family. The team helped Edgar and his father to navigate the process and advocate for reunification. At last the day came when they could welcome their son home with hugs and tears of relief and joy.

The Alvarez family continues to be an active part of Teen FAST. Edgar, an A+ student who works hard on his academics, is now a teen leader in the program. As a leader, he is a role model for other teens facing adversity and instrumental in building the community of support for program families. Our team sees a bright future ahead for him. * Name changed to protect client privacy.

The Teen FAST Program engages families living in areas with high levels of gang violence, poverty, substance use, and other risk factors with two years of education, coaching, and support. Being there for families when they need us is a crucial part of our work that strengthens family functioning, empowers teens to become responsible leaders, builds circles of support among peers and families, and promotes positive life outcomes.

Finding Community and Support A program of Caminar’s Family & Children Services of Silicon Valley division, the LGBTQ Youth Space offers youth-driven counseling, peer support, advocacy, and leadership development programming for LGBTQ youth and young adults from 13 to 25 years old, both at its downtown San Jose drop-in center and in community venues throughout Santa Clara County.

The Road to Resilience: Creating New Road Maps From Hospitalization to Home: Tina’s Journey Earlier this year, Tina* entered treatment at Caminar immediately upon discharge from the hospital. She had been referred to our San Mateo County Region’s Assisted Outpatient Treatment (AOT) program, which provides intensive treatment and support for adults with severe mental illness who are disconnected from the mental health system.

Finding Resilience Through the Power of the Arts One of the many ways our donors enhance the health and well-being of our participants is through our Expressive Arts Program. This program brings world-renowned photographer Michael Collopy and art therapists to Caminar’s residential programs for weekly workshops, offering our clients the chance to explore and engage in the therapeutic power of the arts to heal and change lives.

Tina had been living with roommates who were actively engaged in unhealthy lifestyles, and she had been heavily abusing substances. Her mental and physical health were in a fragile state.

At Caminar, Tina began working regularly and diligently with a psychiatrist and a case manager. And, she enrolled in an outpatient substance use treatment program.

With the newfound support and compassionate care, Tina regained her resilience and began to rebuild her life. She invested the time and work to become clean and sober, and her mental and physical health improved. Now, she is living with family, maintaining a healthy lifestyle, working towards securing employment, and committed to the journey of recovery. * Name changed to protect client privacy.

“Alcohol ended up being my worst enemy. FCS was very helpful in guiding me to new ways of coping with traumas in my past. Now that I have this guidance from FCS, I feel like I can do great things. I have seen myself become a much stronger woman. I don’t feel such low self esteem. I feel empowered. I feel less alone. I feel like I am doing the right thing. I feel like, for once, I am going somewhere with my life.”

The Road to Resilience: Moving Toward Recovery Promoting Resilience Through Integrated Health Care With the help of Bridges to Wellness, our integrated primary and behavioral health care program, Michael* has a new-found healthy lifestyle and is working on remaining stable and resilient through life’s up and downs.

Over a year ago, Michael, who is in recovery for severe mental illness, was connected to Bridges to Wellness by his mental health case manager. He received an alarming wake-up call when the Bridges to Wellness nurse reviewed with him the initial wellness lab test results. In addition to having extremely high levels of cholesterol, Michael was pre-diabetic. Smoking 40 cigarettes a day also was affecting his health.

Motivated by the test results, Michael decided to make changes in his life. With the support of the nurse, he set wellness goals and created a plan.

Encouraged by the Bridges to Wellness team, Michael began taking daily walks, which he found reduced his urge to smoke. Through the program’s smoking cessation group, he learned effective strategies, connected with peer support, and eventually quit smoking completely.

Using his savings on cigarettes, Michael joined a local gym, where he works out most days. Now he is maintaining a healthy body weight and his cholesterol and glucose levels are within a healthy range.

Michael is feeling, breathing, and looking better now than he can remember. His own determination and resilience, along with support from our mental health and Bridges to Wellness program professionals, have helped him enjoy a healthier quality of life.

With philanthropic investment, we aim to expand these integrated health services to reach more clients in San Mateo and Solano counties and to add services in Santa Clara and Butte counties. * Name changed to protect client privacy.

Using Resilience to Find Confidence and Achieve Goals Three years ago, April was spending the nights on the streets or in local parks, hidden in the bushes. One day, she found the strength and courage to realize she needed help and to ask for help from the Butte County Department of Behavioral Health.

April was referred to Caminar and became a tenant of Avenida Apartments, our supported housing program for adults who are experiencing homelessness and have mental health conditions, physical disabilities, or developmental disabilities. She also interviewed for and was hired as an employee of Caminar’s Sensible Cyclery vocational training program.

When April began work at Sensible Cyclery, a bicycle refurbishment and resale shop, she had difficulty making eye contact with her colleagues and her supervisor when speaking with them. After a few months, there was a dramatic change: April not only could make eye contact with others, she started to build the confidence she needed in order to achieve the goals she set for herself.

While working at Sensible Cyclery, April set her sights on becoming a computer programmer. After completing the vocational program, she enrolled in Butte College and began coursework in programming. She impressed her professors and was hand-picked for an internship at a local computer programming company upon completion of her classes.

April’s next step was to move into her own apartment in the community. She could have stayed longer at Avenidas, yet she felt ready and wished for someone else to have the opportunity to benefit from the services as she had.

“Although we were sad to see April leave Avenida Apartments, we are so proud of her and all she has accomplished in pursuing her goals,” said Scott Walker, Program Manager, Butte Region.

Innovative Solutions Support the Road to Resilience Given our location in the heart of the Silicon Valley, leveraging technology to enhance client care comes naturally.

Thanks to the generosity of our board member Carole Middleton, we now offer a pilot group of clients access to bStable, a mental health software application that creates a digital health diary.

This valuable tool improves communication between the client and health care provider as clients record data on a variety of health and wellness factors, including sleep, medications, energy levels, mood, relationships, substance use, and exercise.

Leslie Wolff, a registered nurse who runs our program, commented: “The more clients use bStable and the more I work with the technology, the more efficient and meaningful the meetings become.” Additionally, by using technology and maintaining consistent records of one’s health and wellness, Leslie sees that clients feel more empowered and confident in their daily lives.

We look forward to leveraging effective technological innovations for the benefit of more clients.

Philanthropic funding and partnerships allow us to empower local individuals and families to build their resilience and reach their potential.

Our social enterprises advance the social mission and financial sustainability of our organization. Any proceeds from these enterprises are invested directly into Caminar programs, supporting resilience in both our clients and our organization.

Our master lease of the 38-unit Linden Street Apartments in Redwood City allows us to use half of the units to provide supported, independent housing for low-income Caminar clients with disabilities. The other half of the apartment units are rented at market rates, offsetting the supported housing service.

Sensible Cyclery sells quality reconditioned bicycles at an affordable price in Butte County. Our vocational program participants overhaul, repair, tune, and ready used bicycles for resale, while receiving training to succeed in the

workforce. Not only does Sensible Cyclery offer a quality product to the community, it helps employees reach their fullest potential and thrive. Stop by when you’re in Chico!

Pro-Touch provides affordable janitorial and landscape services for businesses and people on a limited income in Butte County.

Individuals who are in recovery for behavioral health conditions and who are preparing to enter or reenter the workforce receive vocational training and skills that prepare them for community employment.

Olivos provides private, concierge mental health and wellness services to individuals in various stages of recovery.

Through our boutique, highly-specialized approach, we tailor services to the unique needs of each person. Whether they have a highly acute and serious mental illness, are dealing with the long-term effects of trauma, or are seeking to improve their mental health and wellness, we help them reach their goals.

Each treatment plan is thoughtfully designed based on the care needed and may include case management, psychiatry, therapy, vocational rehabilitation, supported education,

medication management, life skills development, independent living, housing, and companion services. Throughout the journey to wellness, we strive to ensure families remain connected, involved, and supported. We delight in seeing clients rediscover aspects of their lives that may have been lost as they regain functioning and independence.

In 2017, over half of our Olivos clients graduated from care. And, since Olivos was created in 2012, 97% of our clients report having found meaningful activities that have enhanced their lifestyles.

Community Leadership Events Through leadership events, we strive to inform the community about cutting-edge behavioral health research while celebrating the journey to wellness.

Our 3rd Annual Mental Health Symposium, held on May 18, 2017, at Filoli Gardens, featured experts and ground-breaking research in the mental health field.

Dr. Kuhn, Clinical Research Psychologist from the VA National Center for PTSD and Clinical Assistant Professor at Stanford University School of Medicine, spoke on “Using Technology to Help Manage PTSD.” Dr. Adelsheim, from Stanford University, spoke about “The Youth Mental Health Imperative: Why We Must All Get Involved.”

held in our Solano County Region during Mental Health Awareness Month in May, celebrated our clients’ achievements on their paths to improved well-being and health.

Fundraising Events Because of the generosity of all who supported our annual fundraising events, over $650,000 was raised to expand the circle of support and safety net for our community’s most vulnerable residents.

Circle of Support Luncheon The spring 2017 event highlighted our addiction recovery services. Featured speaker Sam Quinones (author of Dreamland: The True Tale of America’s Opiate Epidemic), testimonial speaker and local parent D’Anne Burwell (author of Saving Jake), and honoree Kathy Forward (NAMI Santa Clara County), all spoke to the role of the community in addiction recovery. We extend a special thank-you to lead underwriters John A. and Cynthia Fry Gunn.
