Long Term Disability This is an employer paid Long Term Disability, which provides income replacement if you are disabled. All benefitted employees are automatically enrolled in this benefit.

LTD pays up to 60% of average earnings over the past 12 months. LTD has a 30 day waiting period before LTD will pay.

Employees must use all ESL and PTO prior to LTD paying. If an employee is going on leave because of pregnancy the disabled period is 8 weeks for c-section and 6 weeks for natural delivery. It does not go the full 12 weeks of maternity leave.

LTD does have a preexisting clause, this includes pregnancy.

In order to apply employees should call The Standard’s Claim Intake Service Center at 800-378-2395. The employee will need: • Group Policy number: 136592 • Name and SSN • Last day the employee was at work • Physician’s contact information (name, address, phone, and fax)
