WHAT DOES THE GOLD SEAL MEAN FOR YOU? There are a lot of things to consider when selecting a home health care agency — trust, quality care, personal service and comfort. For more than 70 years, Livingston has earned its reputation every day by focusing attention on the things that matter most to patients and families. We take these things seriously and measure our success with three independent sources.

THE HIGHEST STANDARD OF QUALITY Established in 1988, the Joint Commission’s accreditation process for home health concentrates on operational systems critical to the safety and quality of patient care. It is the standard in home health care. In order to earn and maintain accreditation, we submit yearly reports and undergo a rigorous onsite review every 18 to 36 months. Not every home healthcare agency in Ventura County holds itself to this standard.

FOUR STAR CARE As a CMS (Medicare and Medicaid Services) certified home health agency, we are required to have an independent vendor survey our home health and hospice patients to determine their level of satisfaction with our care. Their survey measures key quality and satisfaction indicators. The results are compiled and assigned a star rating. The information is reported on the official CMS Home Health Compare website (www.medicare. gov/homehealthcompare/search.html). We are proud to consistently receive a four star rating for Quality of Care and Patient Satisfaction. In fact, 90 percent of our past patients and hospice families would recommend Livingston services and would use our services again.

OUR PATIENTS SAY IT BEST Our Gold Seal and Four Star ratings are two external measures of quality, feedback from actual patients is a third measure. We receive cards and letters like this nearly every day, “Everyone that came to help me were great. They answered all my questions. This was the second time I used your services.” Shirley, Home Health Patient.

YOUR CARE MATTERS Our extraordinary team provides compassionate, high-quality healthcare for the best value, one person at a time. As a community anchor and leader in healthcare in Ventura County, we make sure our care is accessible to all regardless of their ability to pay. Our professional caregivers are specially selected, background checked and trained to provide the highest level of professional care in a time of need. Our care team consists of licensed nurses; physical, occupational and speech therapists; medical social workers and dietician as well as certified home health aides. We have certified Wound/Ostomy nurses who provide the highest level of care to patients in need of this highly specialized service. We are proud of our staff and the quality of care they provide. If you have any questions, or would like more information, please contact Jamie O’Brien at (805) 642-0239, ext. 625.

AN ANCHOR IN THE COMMUNITY, A LEADER IN QUALITY HEALTHCARE Here at Livingston we are so proud of our mission to provide care to anyone in Ventura County regardless of their ability to pay. That mission is what has made us an anchor in the Ventura County community for more than 70 years. As an anchor in Ventura County, we are rooted in the community by our mission and committed to the long-term welfare of everyone in the community. Providing care to all is important but it is even more important that the care be of the highest professional standard. To ensure the highest standards are met, Livingston works with The Joint Commission for Accreditation of our Home Health and Hospice programs. The Joint Commission is the gold standard in healthcare accreditation. Their state-of-the-art standards focus on patient safety and high quality healthcare. There are many behind the scenes components to providing a high quality health care program, things you might not think of such as ensuring patient’s rights, validating the competencies of the staff whom enter your home, infection control, data collection and reporting required by our governing bodies – all this and much more being important pieces to quality health care delivery. What does this type of quality healthcare look like to patients and the community? SAFE Providing care that helps patients. EFFECTIVE Providing care based on scientific knowledge and evidenced based guidelines. PATIENT-CENTERED Providing care respectful, responsive and driven by the values of the individual patient and culture of their community. TIMELY Providing care in a timely manner for patients. EFFICIENT Preventing waste and maximizing use of equipment, supplies, ideas and energy. EQUITABLE Providing the same high-quality care for all patients regardless of geographic location, race, gender, or socio-economic status. As a community anchor and healthcare leader we know that everyday our neighbors in Ventura County trust us with their health and the well-being of their loved ones, a charge we take very seriously. 2 Livingston Memorial Visiting Nurse Association•lmvna.org

Volunteer Spotlight

VOLUNTEERS BLOSSOM

AT THE CAMARILLO FARMERS MARKET

E

very Saturday, rain or shine, a dedicated band of volunteers gathers to set up the weekly Farmers Market. They greet vendors, set up tables and pop-up tents, and get ready to welcome shoppers. The market is run entirely by a group that has kept the market going for the past 28 years. For the past six years, Ruff Smith has led this team as the general manager for the market. Ruff has a long history of volunteering. “I was fortunate and never had to work outside the home,” she says. “I thought it was important to give back to my community. I started volunteering at the Ventura County Medical Auxiliary. When my children were in school, I volunteered at their schools. Once they graduated, I looked for something else. “I knew some people that worked at the Farmers Market. I worked as a regular volunteer there for about five years, then served for six years as a Saturday manager.

"I took over as general manager about six years ago,” she said. As general manager of the only genuinely volunteer-run Farmers Market in Ventura County, Ruff is the liaison with all the vendors, sets the schedule and interacts with government agencies. Each vendor donates a portion of their sales to the market. Over the past 18 years, more than $990,000 has been raised to support grief and bereavement counseling services. None of it would be possible without the hard work and dedication of the Farmers Market volunteers. Are you interested in giving back to your community? Consider volunteering at the market! Please visit camarillofarmersmarket.com to download a market volunteer application.

Ruff Smith, General Manager Camarillo Farmers Market

The Camarillo Certified Farmers Market is a project of Camarillo Hospice. Held every Saturday 8 a.m. to noon— rain or shine — at 2220 Ventura Boulevard in Old Town. Over the years, the Camarillo Certified Farmers Market has become a community meeting place where friends, neighbors and strangers shop, socialize and celebrate “the Good Life” and our agricultural heritage. The Market is managed and run by volunteers, and a percentage of all gross sales directly benefits grief and bereavement programs.

Livingston CareGivers has provided vital, non-medical personal assistance in the home to residents of Ventura County for over 15 years. Our clients include not only the frail, disabled and elderly, but anyone and any age that needs personal help at home. When daily needs are managed and addressed, people can remain in their home, which is often a preferred option to long-term placement. Many studies show that home is where the elderly prefer to be and where they do best.

34th Annual

2018

COMMUNITY MEMORIAL SERVICE

Loved Ones Honored at Annual Observance For some, grief is best experienced in the company of others. Together we can acknowledge the pain, the grace to grieve, and strength to move forward. The 34th Annual Community Memorial Service was held under the trees and surrounding beauty of Maravilla Gardens. Community members gathered to honor their loved ones and to recognize their unique grief journey through ceremony, music, community and hope.

Attendees paid tribute to their loved ones by writing their names on butterfly stickers and placing them on the Memorial Quilt. The quilt will be displayed all year in the Camarillo office. We deeply appreciate the generosity of Maravilla Gardens for hosting the service for it is in nature, so beautifully displayed at the gardens, that we can connect to something bigger than ourselves.

Please visit www.lmvna.org/community_memorial for photos of this beautiful event.

VOLUNTEER OPPORTUNITIES Join a remarkable core of volunteers — community heroes that give special gifts of time and compassion to patients and their families.
