Job dissatisfaction was higher amongst optometrists, locums, those who work for a multiple, and disabled registrants –and (unsurprisingly) the chief causes were busy/increasing workload/pressure, poor pay/lack of reward and sales pressure/retail focus. More than half of respondents (57 per cent) claimed to be working beyond their normal hours, with 41 per cent saying they felt “unable to cope with the workload”. In fact, a third rather worryingly said they “felt unable to provide patients with the sufficient level of care they needed”.

he favourable news is that the majority of respondents (62 per cent) were currently experiencing job satisfaction. The less favourable news is that a fifth (21 per cent) were not; the rest were neither here nor there.

There is no one answer to improving job satisfaction and relieving work pressures, but with 40 per cent of respondents saying they planned to gain additional qualifications/skills within the next 12-24 months – with a focus on delivering better care, expanding scope of practice to deliver more services, and career progression – this could certainly be a step in the right direction.

Further qualifications and skills training are there for taking – for all practice staff, in all disciplines and in a multitude of formats – so why not take a look at what’s out there. If not for yourself, then for a member of your team...

The General Optical Council (GOC) has published two new guides relating to its CPD scheme – ‘CPD record review: a guide for registrants’, and ‘Guide to audit of providers of CPD’ – to help explain what’s involved. The guides contain: an introduction to the record review/provider audit process, including why and when it takes place and how registrants/providers will know if they’ve been selected; and an outline of the process, including what will be looked at, potential outcomes, and next steps following receiving these outcomes. The survey also found that around 40 per cent of respondents planned to gain additional skills and qualifications.

“Rates of myopia in children continue to rise in the UK and the longer axial lengths, associated with myopia, increase the risk of sight-threatening conditions such as myopic maculopathy, retinal detachment, and glaucoma later in life," said Christina Olner, head of professional services, CooperVision UK and Ireland. "We are encouraged by the renewed guidance from the College of Optometrists and welcome its recognition of this important field. Any increased awareness of myopia management will ultimately be to the benefit of young myopic patients," added Christina.

The Sector Partnership for Optical Knowledge and Education (SPOKE), a cross-sector collaboration funded by the General Optical Council (GOC) and led by the College of Optometrists, has published the outputs of its second project. The case studies describe best practice approaches to teaching and learning for student optometrists and dispensing opticians. They have a particular focus on the new GOC outcomes and spiral curriculum, and include effective methods for scaffolding learning.

Professor Lizzy Ostler, GOC director of education at the College of Optometrists, said: “We’re thrilled to see the publication of the Project Two case studies, contributed from across the sector, providing great examples of teaching and learning methods

The joint conference of the Association for Independent Optometrists and Dispensing Opticians (AIO) and the British Association of Behavioural Optometrists (BABO) takes place on 1 and 2 October at Chesford Grange in Warwickshire. A panel session on the reform of the Opticians Act will see representatives from the main optical bodies sharing their views. There will be dedicated sessions on going (wholly or partly) private, looking at the recently released Going Private Roadmap developed for the AIO by Myers La Roche.

NHS England has appointed Louisa Wickham, consultant ophthalmologist and medical director at Moorfields Eye Hospital, as the first-ever national clinical director for eyecare. The appointment is expected to put ophthalmology, the largest generator of outpatient appointments, on a par with other major specialties.

The Eyes Have It partnership is calling on NHS England’s new national clinical director for eyecare, Louisa Wickham (see above), to implement a national strategy to address the issue of referral delays for sight-saving diagnoses and procedures. Speaking on behalf of the partnership, Stephen Scowcroft, Macular Society director of services, said: “We believe a clinically-led national eyecare strategy that prioritises maximising capacity in NHS eyecare services is needed to better support people experiencing sight loss.

There are many issues and questions facing independents, and these are not likely to diminish over time. The reform of the Opticians Act will have a major impact on the future delivery of optical services, and it is so important that the independent voice is heard. This once-in-a-generation opportunity to drive a different future for the delivery of primary eye health care cannot be left to the will of the multiples. Reform of the Act, in whatever form, should lay the groundwork for a single national strategy for eyecare in England, which is properly funded by the NHS, and that brings an end to the current postcode lottery.

This summer, Théa spent time successfully engaging with this patient group, raising awareness of the link between DED and the benefits of visiting a optometrist. Our press event saw patients together with a panel of experts, including Sarah Farrant and Francesca Marchetti, sharing their knowledge and experiences, resulting in an array of media coverage. Having never experienced any eye problems to suddenly suffering with “bizarre” watery, gritty and irritated eyes, 53-year-old Margaret recounted her fairly typical and frustrating journey. This involved trying home remedies at first, being told it was hayfever, visiting her GP and pharmacy, and finally seeking help from a local optometrist. She told the audience that as well as physical symptoms, there were mental challenges of living with DED: “There’s not a day that goes by where you are not dealing with it,” she said. “It’s a very debilitating condition.” Margaret went on to say that living with DED had made her very aware of the key role that optometrists play: “You take your eyes for granted until something happens. You don’t appreciate what they mean to you.” With the help of her optometrist, Margaret was diagnosed with DED and managewomenHelpingtotheirsymptoms

The meibomian glands are modified sebaceous glands, which produce and secrete lipids to promote stability of the tear film and reduce evaporation of the aqueous layer.The production of all three layers is influenced by the sex steroid hormones –androgen, oestrogen and progesterone. The meibomian glands and lacrimal glands have oestrogen and androgen receptor sites that stimulate secretion.

Approximately 61 per cent of perimenopausal and menopausal women are affected by dry eye disease (DED). In fact, women over 50 are nearly twice as likely to experience the condition compared to men of the same age1. Are we delivering the best service for these women, and do they know it? Are we making the most of this valuable patient base? Is this an opportunity for growth in your practice?

During menopause, the production of these sex steroid hormones is reduced, which can lead to tear film imbalance. The reduction in androgen can impact the quality and quantity of lipids produced by the meibomian glands, and the reduction in oestrogen and progesterone, may suppress the sebaceous glands which can, in turn, lead to a reduced lipid production. This disruption to the tear film can then start a cascade of events, including inflammation and the appearance of ocular surface disease including DED. When we include time spent on screens, wearing contact lenses for long periods of time, cigarette smoke exposure, heating, air-conditioning as well as certain medications and other chronic conditions, it really is a recipe for the onset of those

During my optical career, I’ve been very fortunate to work with some innovative and some would say ‘game-changing’ products. When I was at Bausch + Lomb back in the 1990s, we launched the world’s first multipurpose contact lens solution – ReNu – and just before the millennium, the UK’s first silicone hydrogel lens – PureVision. A clinical study coordinated by VTI has shown that when the fitting guide is followed to the letter, a successful NaturalVue fit can be achieved in an average of 1.1 visits3 Thus, if we say zonal multifocals average 2.5 visits versus 1.5 for NaturalVue (being generous to zonal multifocals), that’s at least one whole appointment saved. Not only that, but in addition to saving the cost of that appointment, another NaturalVue patient could be successfully fitted in the time freed up, generating additional revenue and more profit.

CONCLUSION Wasn’t it Einstein who referred to our mental state if we continue to do the same things, yet expect different results? For me, continuing to plough the zonal multifocal furrow is a similar analogy. So, if you haven’t tried NaturalVue yet, what are you waiting for? ZONAL MULTIFOCALS NO LONGER ADD UP Traditional zonal multifocals create two images for the brain to interpret. Those images are ever-present and so when one is in focus, the other is blurred, to a greater or lesser extent. This blurred image is believed to create the image ‘ghosting’ patients report. This is particularly bothersome on print whilst reading and creates haloes on lights during night driving.

It is widely accepted that it typically takes two to three visits to fit a zonal multifocal contact lens; in fact, many manufacturers’ own fitting information confirms this. This is arguably okay if this time is appropriately charged for and it ends up with a successful patient; but what about when one or neither scenario is met?

ONCE IT’S FITTED – IT’S FITTED But the commercial benefits don’t end there. In fact, it’s only the tip of the iceberg from a chair-time saving perspective over the lifetime of the patient. This is because once NaturalVue is fitted, it’s effectively fitted for life. By that I mean that the only reason to change the specification of NaturalVue is if the distance power changes. How so? Well, the single EDOF design manages to cover a continous range of Add powers right up to +3.00D. This means that if you were to fit an emerging or pre-presbyope successfully with NaturalVue and their distance prescription didn’t change for the next 30 years, then the patient would simply continue to wear the same specification lenses for that time. The practice simply continues to collect the direct debit for lens supply and an annual check-up. No wonder we call it game-changing. In fact, it seems almost too good to be true doesn’t it? Perhaps this, and being creatures of habit, explains why many practitioners continue to fit the tried and tested lenses they feel comfortable with; often doing so by limiting the type of patients they fit, whether by age, prescription or other factors. EDOF allows multiple focal points along the visual axis to be clear. CONSERVATION AND CREATIVITY Our rapidly changing climate, and increased awareness of the need to look after the planet and its fragile eco-systems, have prompted companies to produce items with sustainable features. “We have a massive story to tell,” says David Strathie, sales and marketing manager for Continental Eyewear, part of the Millmead Group.
