The University of Hawaiʻi at Mānoa John A. Burns of Medicine was established in 1965 as a two year basic medical sciences program and subsequently became a four year degree program effective 1973. All of the Clinical Science Programs are located in the affiliated hospitals with the Department of Pediatrics located at Kapiʻolani Medical Center for Women & Children.

The Community Pediatrics Division consists of generalists, developmental/behavior, specialists. We provide care and teach in the continuity clinics, nursery, developmental/behavioral clinic, adolescent clinic, the detention home, and Hawaii Youth providing pediatric consultative services at Shriner’s Hospital. We are united in underserved children
The Pediatric Emergency Medicine (PEM) Division provides 24/7 specialized care for medical conditions. In addition to front-line clinical services, our team is deeply engaged improvement, administrative leadership, and the education of nurses, medical students, providers.

The Pediatric Hospital Medicine Division at Kapi‘olani Medical Center delivers inclusive, compassionate, patient- and familycentered care rooted in Hawai‘i’s unique regional needs. Our approach blends evidence-based medicine, quality improvement, and health equity while training the next generation of skilled and empathetic physicians.
Education and mentorship remain central to our mission. We continue to refine our inpatient hospitalist curriculum, providing residents from our own program and visiting learners from across the U.S. and abroad a rigorous academic environment. All division members precept third-year medical students, and many serve as PODvisors, longitudinal clinical mentors, advisors for the Pediatric Interest Group, medical school interviewers, simulation leaders, and PBL tutors, in addition to bedside teaching that inspires learners regardless of specialty choice. This year, several members were recognized for their exceptional teaching: Dr. Kyra Len received the prestigious Leonard Tow Humanism in Medicine Award, Dr. Jordan Kono earned the Pediatric Resident Teaching Award, and Dr. Jennifer Sasaki was honored with the Medical Student Hospitalist Teaching Award.
We lead outcomes research and quality improvement initiatives that shape pediatric inpatient care. Our division has developed fifteen evidence-based order sets, with more in progress as we expand our faculty. Dr. Cheryl Okado recently published on our infant fever order set, demonstrating reduced admissions and lumbar punctures through updated AAP guidelines.
This year’s Pediatric Academic Society (PAS) Annual Meeting in Honolulu highlighted our broad contributions. Drs. Maya Maxym, Marissa Fakaosita, and colleagues presented a poster on diversity in the pediatric workforce; Dr. Jennifer Di Rocco co-led a workshop on Entrustable Professional Activities; and Drs. Marissa Fakaosita and Jessica Kosut facilitated a workshop on communication with patients and families infused with a distinctly Pacific Island flavor. This included perspectives from a local interpreter and one of our patient families, who shared valuable insights into relationship-building and the importance of “talk story” as a culturally grounded approach to rapport. Dr. Shilpa Patel and colleagues presented at the Academic Pediatric Association Quality Improvement conference on advancing clinical pathway use through implementation science. Beyond PAS, Dr. Maya Maxym, in collaboration with colleagues from JABSOM, received an intramural grant for the Nurturing Belonging Faculty Development Curriculum, joined by Dr. Barry Mizuo as a participant. Research productivity remained strong, with Drs. Jessica Kosut, Cheryl Okado, Tiffany Wang, Diane Pan, Kyra Len, and Natascha Ching publishing on pediatric COVID-19 hospitalizations in the Journal of Pediatric Infectious Disease. Dr. Kyra Len also continued to publish extensively on medical student education, curriculum development, COVID-19 impacts, and wellness.

DIVISION OVERVIEW
The Neonatal Intensive Care Unit (NICU) at Kapiolani Medical Center for Women and Children
surgical services to over 1,000 babies from across the state of Hawaii each year. The NICU a capacity of 84 beds. Dr Alexandra Ramnath continues as Division Chief, while Medical Dr Kandie George in January 2025.
The Neonatology division provides 24-hour in-house coverage in the NICU and Labor & Delivery consultation coverage for Level II neonatal services at the Queen’s Medical Center. Telemedicine available to community Pediatricians, including neighboring islands and the Pacific outreach by providing local and neighbor island education including in-person neonatal neonatal palliative care services are also provided.
Neonatal transport remains active as we provide services for both inter-island and mainland number of inter-island transports that are taken to Kaiser Hospital. Furthermore, the division and training for our highly qualified transport nurses and respiratory therapists.
The NICU Medical Multidisciplinary follow-up Clinic provides ongoing specialized care for This interprofessional team includes rehabilitation services, speech-language pathology and telemedicine appointments are offered. The NICU Neurodevelopmental clinic provides infants up to 2 years of age.
The Pulmonary Hypertension (PH) Team is a multidisciplinary collaboration between Cardiology) and intensivists (Neonatology and Pediatric Critical Care) who follow PH patients extremely premature infants.
The Hanuola ECMO program at KMCWC is the only pediatric extracorporeal life support program in Hawai’i. Dr Sheree Kuo serves as Associate Medical Director of Hanuola. This program is a joint cooperative collaboration between Neonatology, Pediatric Critical Care and Pediatric Surgery. In the NICU, ECMO certified physicians provide 24/7 consultation and treatment services.
Our accredited joint Neonatal-Perinatal Medicine fellowship program with Tripler Army Medical Center accepts both military and civilian fellows. Emmanuel Kling is the Fellowship Program Director. Weekly fellow conferences are held as well as participation in the Western Interstate Fellows Tele-conference and fellow bootcamps.
NICU Research includes national multicenter trials such as the Aero-05 nebulized surfactatant trial, NIH funded NOVI-ECHO (Neurobehavioral Outcomes of Very Preterm Infants and Environmental Influences of Child Health Outcomes), the Hawai’i IDeA Center for Pediatric and Adolescent Clinical Trials (HIPACT), and Advancing Clinical Trials in Neonatal Opioid Withdrawal Eat Sleep Console Trial (ACT NOW ESC). Dr Lynn Iwamoto oversees patient safety and Quality Improvement initiatives in the NICU.

PEDIATRIC INTENSIVE CARE
DIVISION OVERVIEW
The state-of-the-art Pediatric Intensive Care Unit (PICU) at Kapiʻolani features 14 beds, beds for intermediate care patients. Each year, the unit provides specialized care to nearly tertiary care unit, we receive referrals from other PICUs, including Tripler Army Medical Center, and hospitals across the islands of Hawaii and the Pacific Islands. We have transport team, that brings critically ill children from across the state to the Kapiʻolani PICU.
Our team manages a wide range of medical and surgical conditions, including sepsis, failure, congenital heart disease requiring surgery, trauma, and brain tumors requiring Equipped with advanced technology—such as conventional and oscillatory ventilators, therapy, and extracorporeal life support (ECLS)—the unit is proud to be recognized Excellence. With the addition of a permanent pediatric cardiac surgeon in Hawaiʻi in growth in both heart surgeries and ECMO patients. In addition to acutely ill patients, we with chronic illnesses facing acute-on-chronic conditions.
Since September 2016, all patients in the PICU have been cared for in private rooms, each families. This allows parents to remain at the bedside of their critically ill children at all accommodate emergency procedures, with the ability to convert into a fully functional teams.

The PICU also serves as a vital teaching environment. Research is another cornerstone of the PICU. We participate in national multicenter studies and conduct retrospective projects, offering research training for PICU nurses, pediatric residents, JABSOM students, SSRP and SSIP participants, and University of Hawaiʻi Honors students, all under the mentorship of our attending physicians.

PEDIATRIC SPECIALITY
DIVISION OVERVIEW
The Pediatric Specialty Division is comprised of physicians and providers with fellowship training in a diverse array of pediatric subspecialties. Our division members participate and other student teaching programs. Many have been recognized for excellence in teaching Pediatric Residency and Medical Student teaching awards. Many of our division members and clinical research activities, which often involve medical students and residents, as the medical school and various healthcare organizations. Most of the division is employed Group (HPHMG). As a group, we provide services for pediatric patients across the state division collaborates on strategies to improve care for pediatric patients across our ambulatory and hospital settings.
Pediatric Specialty clinics are held primarily at the Kapi’olani office tower and the Hale members also provide care at other Hawai’i Pacific Health (HPH) facilities and clinics, Center facilities, and neighbor island outreach clinics. Comprehensive clinical services also include telemedicine services as an integral part of care delivery. Hospital services Medical Center for Women and Children (KMCWC); however, some of our specialties other Oahu facilities, including Kaiser Moanalua Hospital and Queen’s Medical Center.
PEDIATRIC CARDIOLOGY
The Pediatric Cardiology subdivision is composed of a dynamic group of doctors treating children and young adults with congenital and acquired heart conditions. The team operates under the umbrella of the new Mitsui family Pediatric Heart Center at Kapi’olani Medical Center for Women and Children. The members of the team include three pediatric cardiologists and a cardiothoracic surgeon. Besides clinical responsibilities, including outpatient pediatric cardiology clinics, inpatient consultations in the NICU and PICU, ECG and echocardiogram evaluations, fetal cardiology clinics, cardiac catheterization procedures, and open-heart surgeries, the group is actively involved in scholarly activities, biomedical research and innovation, and teaching at all levels. We offer an elective for MS3 students during their clerkship, a subspecialty elective for MS4 students and pediatric residents, as well as neonatology and maternal fetal medicine fellows. Besides pediatric cardiology, the group provides care for adults with congenital heart disease at Kapi’olani as well as at Straub and Queens.
PEDIATRIC COMPLEX CARE
The Pediatric Complex Care Clinic is a multi-disciplinary clinic that provides family-centered, high quality and developmentally appropriate health care services that continue uninterrupted as the individual moves along within systems of services, from infancy to adulthood. The clinic also provides ongoing care coordination with multiple specialists, primary care providers, and other community partners to promote the healthy development and well-being of children with medical complexity.
PEDIATRIC ENDOCRINOLOGY
We specialize in diagnosing and managing a wide range of hormone-related conditions in children and adolescents. This includes disorders of growth, puberty, thyroid, adrenal, pituitary, and electrolyte balance, as well as both type 1 and type 2 diabetes. We provide evidence-based, patient-centered care and emphasize a comprehensive approach that blends clinical experience with current guidelines. Pediatric Endocrinology is offered as an elective to residents. As part of the rotation, learners have the opportunity to learn how to evaluate, counsel, and manage pediatric endocrine disorders while working alongside a dedicated and collaborative team.
PEDIATRIC GASTROENROLOGY
The pediatric gastroenterology group is a specialty clinic that focuses on treating children and young adults with a variety of digestive and nutrition-related disorders. The group covers outpatient services for all the Hawaiian Islands, and the inpatient services at Kapi’olani Medical Center for Women and Children and Kaiser Permanente Moanalua Medical Center on Oahu. The members of the team include three pediatric gastroenterologists and two medical assistants. Clinical responsibilities include covering outpatient consultations, inpatient consultations in the pediatric ward, NICU, PICU, and Emergency Department. The team also performs endoscopic procedures at both medical centers. The service collaborates with the dietitians on addressing all nutrition-related disorders for pediatric patients. The team also collaborates with the
pediatric surgery service in the placement of gastrostomy tubes. Our physicians care for a multitude of digestive disorders and have interests in treating inflammatory bowel disease, functional gastrointestinal disorders, liver disease, pancreatic disorders, eosinophilic and other food-related disorders, obesity, failure to thrive, and metabolic disorders. Our physicians use a holistic approach utilizing integrative medicine principles by advocating for healthy nutrition and stress reduction. Educational opportunities exist for all medical students and residents as an elective offering.
MEDICAL GENETICS
Hawai’i Community Genetics offers inpatient clinical services to KMCWC and Pali Momi, as well as outpatient clinical services at Hāle Pawaʻa. Through a partnership with the Department of Health, we are also able to provide neighbor island outreach, hemoglobinopathy clinic, and management services for our patients with metabolic diseases. Our group provides comprehensive genetics services, including coordinating screening for genetic diseases involving single-gene and chromosomal disorders, congenital anomalies, inborn errors of metabolism, multifactorial conditions, and common disorders with hereditary factors. We have routine clinics for general pediatric and adult indications, inborn errors of metabolism, hereditary cancer, cleft and craniofacial disorders, and neurogenetic indications. Genetics collaborates regularly with many community providers and multiple subspecialists to provide care to patients throughout the islands.
Medical Genetics has been offered as an elective for preclinical students in MD5, MS3 students during the Pediatric Clerkship, MS4 students, Pediatric residents, Ob/Gyn residents, and MFM fellows. We also provide learning opportunities for genetic counseling students and community learners.
PEDIATRIC INFECTIOUS DISEASES
The Section of Infectious Diseases (ID) is very active clinically and academically at Kapi’olani Medical Center and in the Hawai’i community. The members provide inpatient and outpatient services and provide significant support to the community through telephone consultations. The ID Section and the Cardiology Section run the Kawasaki Disease Diagnosis and Treatment Program, which follows children through their acute illness, convalescence, and periodically until they transition to adult care. The ID Section offers sub-internships and observerships in ID, which have been popular among senior medical students from Hawai’i, nationally and internationally. The Section also focuses on developing the academic and career mentoring of pediatric residents, pharmacy residents, medical students, and pre-medical students. All members provide educational lectures to our community of health care providers through the Department of Pediatrics, Hawai’i Pacific Health Partners, Hawai’i American Academy of Pediatrics, and Hawai’i Immunization Coalition. The members also coordinate numerous ID teaching lectures and case conferences to medical students and pediatric residents throughout the year. The entire ID Section supports the Infection Control and Clinical Epidemiology Effort at Kapi’olani Medical Center, of which Dr. Marian Melish is Medical Director. Dr. Natascha Ching is the Physician Leader of the Kapi’olani Medical Center for Women and Children Antibiotic Stewardship Program (KASP).
Dr. Marian Melish founded the First International Kawasaki Disease Symposium (IKDS) series in 1984 and nurtured these symposia into a regularly recurring event. She attended the 14th IKDS in Montreal in August 2024 and was involved in the planning for the 15th IKDS to be held in Japan in 2028. She and Rodolfo Bègue gave a coordinated series of lectures on Infectious Diseases, Medical Microbiology, and Antimicrobial Therapy in support of all the Basic Science Units in the JABSOM Pre-Clinical curriculum program.
Dr. Rodolfo Bègue has mentored 2 pediatric residents in their research projects: Dr. Daisuke Sato in a study of Invasive Pneumococcal Infections and Dr. Sung Hwan Choi in a study of Kawasaki Disease and MIS-C in the COVID era. Dr. Rodolfo Bègue is an NIH Study Section member.
Dr. Natascha Ching collaborated with the Pediatric Infectious Diseases Society (PIDS) to bring 15 learners from KMCWC to attend the Pediatric Academic Societies (PAS) Conference and its Pediatric ID Career Workshop in April 2025, introducing 7 pediatric residents, 3 neonatology fellows, 3 medical students, 1 premedical student, and our Pediatric ID nurse practitioner to the field of pediatric infectious diseases. Dr. Natascha Ching was also an invited speaker in the PIDS Career Workshop at PAS. She also mentored 2 pediatric residents in their research projects, Dr. Nanea Cavaco and Dr. Safiuddin Mohammed.
Bryscen “Makana” Prothero, APRN, spoke at the 2024 Hawai’i chapter of the National Association of Pediatric Nurse Practitioners (NAPNAP) and the Hawai’i Academy of Physician Assistants 35th Medical Conference. He also spoke to the Health Careers Summer Interns at their Career Development Seminar. He was recognized by HPH as the Nurse of the Year, Excellence in Patient-Centered Care, and Patient Experience. He became a member of the Pediatric Infectious Diseases Society (PIDS) Advanced Practice Provider (APP) Subcommittee.
The highlight of our ID Section community education activity is the Annual Aloha Pediatric Infectious Diseases Conference, where all our faculty are speakers. Dr. Natascha Ching is the Program Director. The 16th Aloha Pediatric ID conference was held in November 2024 with Dr. David Kimberlin as the keynote speaker. Through Dr. Natascha Ching’s efforts, the ID division has sponsored or will bring three internationally known visiting professors to the Pediatric Department during 2024-5: Dr. David Kimberlin in November 2024, Dr. Jason Newland in April 2025, and Dr. Sean O’Leary, who will be the keynote speaker for the 17th Aloha Pediatric ID conference in November 2025.
PEDIATRIC NEPHROLOGY
Pediatric Nephrology offers consultative services at KMCWC and other local hospitals, as well as a nephrology clinic at the Hale Pawa’a office clinic. Our clinic provides comprehensive pediatric nephrology services, including general nephrology, chronic kidney disease, dialysis, and post-renal transplant care. Pediatric Nephrology collaborates regularly with Pediatric Urology, Pediatric Rheumatology, and other pediatric subspecialty services to provide comprehensive, multidisciplinary care to patients. Unique specialized services offered include office urine microscopy analysis and ambulatory blood pressure monitoring.
A combined pediatric nephrology and rheumatology elective is offered to Pediatric residents and MS4 students.
PEDIATRIC NEUROLOGY
The Pediatric Neurology service provides consultative services for outpatients at Hale Pawa’a and inpatients at KMCWC. Our patients range from premature infants to adolescents, and our cases involve the full spectrum of pediatric neurologic problems, from seizures/epilepsy, developmental delay, headaches, neurocutaneous disorders, encephalitis, cerebrovascular disorders, and less common demyelinating diseases, neuromuscular disorders, movement disorders, and rare neurogenetic disorders. Providers participate in the Pediatric Complex Care clinic and in an outreach clinic in Hilo. We also support electroencephalography at KMCWC for the coordination and interpretation of routine and prolonged video-EEG studies. Pediatric neurology rotations are offered as an elective to pediatric residents, and other shadowing/learning opportunities are available to medical students at various stages in their training.
PEDIATRIC PALLIATIVE CARE
Palliative Care provides specialized medical care to patients with serious and life-threatening conditions. Palliative Care is focused on identifying, preventing, and reducing the stress and symptoms associated with serious illness, including supporting a family’s values and preferences; helping families in making difficult decisions; identifying and treating symptoms associated with serious illness; providing end-of-life care; coordinating specialty and hospice services where appropriate; and providing emotional and spiritual support to the patient, family and healthcare team.
PEDIATRIC PHYSIATRY
Pediatric Physiatry offers both outpatient and inpatient services in a multi-disciplinary approach for pediatric patients with disabilities resulting from disease or injury. Our focus is to develop a comprehensive program to place the pieces of a child’s life together: medically, socially, emotionally, and educationally.
PEDIATRIC PULMONOLOGY
Pediatric Pulmonology is offered as an elective for Pediatric residents. Next year, an elective will also be offered for MS4 students. In addition to their clinic at Hale Pawa’a, our pediatric pulmonologists also participate in the Pediatric Complex Care clinic and the multidisciplinary Pediatric Pulmonary Hypertension clinic.
PEDIATRIC RHEUMATOLOGY
Pediatric Rheumatology specializes in the care of children with rheumatologic conditions such as Systemic Lupus Erythematosus and related conditions (Sjogren’s syndrome, scleroderma, mixed connective tissue disease), Juvenile Arthritis, Spondylarthritis, Juvenile Dermatomyositis, Acute Rheumatic Fever, and other inflammatory diseases in children or adolescents.
PEDIATRIC SLEEP MEDICINE
Pediatric Sleep Medicine is a relatively new specialty to our division, offering specialized consultative and diagnostic services for pediatric sleep disorders.
PEDIATRIC AND ADOLESCENT SPORTS MEDICINE
The Pediatric and Adolescent Sports Medicine group continues to be active in both the medical and the sports community. The physicians provide medical coverage for Castle High School, Hawai’i Baptist Academy, Kamehameha Schools, Le Jardin Academy, Punahou School, and Roosevelt High School. They also provide sports medical care for Chaminade University, Hawai’i Pacific University, and the University of Hawai’i Athletics. Physicians provide medical consultation for the Hawai’i Concussion Assessment and Management Program, the Hawai’i High School Athletic Association Sports Medicine Advisory Committee, and also act as the Physician Champion for Hawai'i Water Safety Coalition.
Current projects also include working with Team Up For Sports Safety (TUFSS), a national organization invested in reducing death from heat stroke and sudden cardiac death in athletes, collaborating with state organizations to provide continuing education on concussion, heat stroke, cervical spine injury, and Stop the Bleed training for athletic trainers.
Rotations are offered for medical students, pediatric residents, family medicine residents, and sports medicine fellows, as well as shadowing opportunities for high school and college students. The department remains active in the Summer Student Research Program at HPH.
