The University of Hawaiʻi at Mānoa John A. Burns of Medicine was established in 1965 as a two year basic medical sciences program and subsequently became a four year degree program effective 1973. All of the Clinical Science Programs are located in the affiliated hospitals with the Department of Pediatrics located at Kapiʻolani Medical Center for Women & Children.

Message from the Editor Our Annual Report continues to be a project that our Departmental Student Assistants help put together. This year the work on this has been spearheaded by two of them – Franchesca Matahum and Joanne Wong. They have both diligently worked with the Divisional leadership in getting the updates and then worked on designing a product which is very pleasing to the reader. It is always a rewarding task for me to review and provide editorial oversight as I become aware of many of the activities in the department which have no other forum to be reported in.

Message from the Chair The faculty, residents and staff welcome you to review our 2022-2023 annual report from the Department of Pediatrics, University of Hawaii, John A. Burns School of Medicine. As stated in last year’s message, we recognize that equity must be a value that serves as a foundation for all work we do. It encompasses every aspect of our academic mission, including clinical care, medical education, research, administration and community service. Being leaders in each of these areas collectively gives us clarity of purpose that this is our kuleana (responsibility). We remain unwavering in our focus on educating future pediatricians to provide the appropriate physician workforce for our state, participate actively in scientific discovery and quality improvement, and partner with all entities involved in pediatric services to be responsible stewards of the pediatric commons. In science and discovery, we are at the tipping point in the application of artificial intelligence (AI). AI, along with continued new scientific knowledge gained will necessitate and accelerate changes in the way we conduct all of our approaches to each aspect of being an academic department. The state counts on us in this scientific trust and in community leadership for children and families. Hawaii is consistently rated as one of the top states in the nation for well-being. COVID and the post-COVID world have handed us one of the greatest challenges the current generation has faced in understanding and addressing well-being. We know that finding joy in work and restoring vitality into the workplace is heavily influenced by a strong sense of connection with community. This justifies our embracement of listening to stories from our patients, families, coworkers and community. It also reinforces that a love of our community factors into how we recruit residents, faculty and staff that feel we belong to each other. Thus it is an individual and collective group responsibility that we gladly accept. Thus, the core of our department is and will remain our people: faculty, residents, students and staff. We are proud of and celebrate their accomplishments herein. True gratitude is extended to the leadership and staff of Kapi‘olani Medical Center for Women & Children, Hawaii Pacific Health; the John A. Burns School of Medicine, University of Hawaii and all pediatric partners in the state. Their support makes our mission to serve Hawaii’s children enduring. The Community Pediatrics Division is a team of general pediatricians, ambulatory pediatric subspecialists, researchers, and family advocates who are engaged in a variety of academic and direct service activities that support a common philosophy that pediatrics must aim to treat children as human beings and consider the full context of their lives—their families, communities, cultures, and personal stories. This philosophy is promoted by engaging in clinical care in a variety of hospital and community settings; by training others about the practical application of community pediatrics; by advocating for policies and practices that will weave these values into the health care system; and by conducting research to develop new ideas and to improve practices. The need for community pediatrics is especially pronounced when looking at underserved and vulnerable children. Therefore, the Division places special emphasis on programs and initiatives that will improve care for these populations. 19

DIVISION OF NEONATAL PERINATAL MEDICINE DIVISION OVERVIEW The Neonatal Intensive Care Unit (NICU) at Kapi‘olani Medical Center for Women and Children (KMCWC) consists of 70 single-family rooms with a capacity of 84 beds. In 2022, we had 979 admissions, of which 153 were very low birth weight infants. The average daily census in 2022 was in the 59-73 range. Kapi‘olani’s NICU measures its outcomes using the Vermont Oxford Network (VON) database, benchmarking to Type C neonatal units with similar risk profiles. Our NICU ranks at or above the top quartile on all its major VON measures.

island families and those with limited access to KMCWC. In addition to the medical clinic, we also provide neurodevelopmental assessments for high-risk infants up to 2 years of age through the NICU Neurodevelopmental Follow-up Clinic. The Pulmonary Hypertension (PH) Team is a multidisciplinary collaboration between Neonatology, Pediatric Critical Care, Pediatric Cardiology, and Pediatric Pulmonology. Additional ancillary services in the PH clinic include dietary counseling, physical, occupational, and speech therapy.

The Neonatology division continues partnering with Maternal Fetal Medicine to ensure the highest quality patient care. We provide 24-hour in-house coverage in the NICU and Labor & Delivery at KMCWC. In addition, we also provide 24-hour consultation coverage for Level II neonatal services at Queen’s Medical Center. Telemedicine and telephone consultation are also available to Community Pediatricians, including neighboring islands and the Pacific Rim. We maintain our regional leadership and educational center role by providing local and neighbor island outreach education, including in-person neonatal resuscitation training.

The Hanuola ECMO program at KMCWC is the only pediatric extracorporeal life support program in Hawai’i, a joint cooperative collaboration between Neonatology, Pediatric Critical Care, and Pediatric Surgery. In 2020, Hanuola was awarded the ELSO Center of Excellence in Life Support, a Gold Level designation.

Neonatal Transport remains active as we provide services for both inter-island and mainland transports. A small number of our inter-island transports are taken to Kaiser Hospital. We also provided several mainland transports last year. Furthermore, we continue supervising and training our highly qualified transport nurses and respiratory therapists. The NICU Medical Follow-up Program provides ongoing medical care for high-risk infants after their NICU discharge, assessing and monitoring growth and nutritional needs, and managing ongoing medical issues. We provide clinical management and care coordination with multidisciplinary and inter-professional care teams. We offer close in-person follow-up and a Telemedicine platform option for neighbor

Our accredited joint fellowship program with Tripler Army Medical Center is one of three military neonatology fellowships in the USA. Fellow conferences are held weekly. Our division is involved in a variety of research projects, including the annual HPH Summer Student Research Program and the following current national multicenter trials: the ICAF study of extended administration of caffeine in preterm babies, the NIH funded NOVI-ECHO (Neurobehavioral Outcomes of Very Preterm Infants and Environmental Influences of Child Health Outcomes), and the Hawai’i IDeA Center for Pediatric and Adolescent Clinical Trials (HIPACT) (an NIH funded multicenter clinical trials grant, funded under the ECHO umbrella, which has funded trials in our unit such as Advancing Clinical Trials in Neonatal Opioid Withdrawal Eat Sleep Console Trial (ACT NOW ESC).

DIVISION OVERVIEW

The Pediatric Emergency Medicine Division is responsible for: around the clock medical care for pediatric patients with urgent and emergent conditions, pediatric hospital sedation, quality improvement, administration, nursing education, medical student and resident education, and pre-hospital (EMS) education. University of Hawai‘i John A. Burns School of Medicine / Kapi‘olani Medical Center Contributions: Problem Based Learning (PBL) tutoring, ward attending, Grand Rounds, Pediatric Resident conferences, student preceptoring, student exams, research, continuing medical education, contribution to educational and life support programs sponsored by the American Academy of Pediatrics (AAP), the American College of Emergency Physicians (ACEP), and the American Heart Association (AHA). The Emergency Medicine Division contributes to the Academy of Pediatrics, the American College of Emergency Physicians, and the American Heart Association. The Division also contributes substantial faculty resources towards medical student and resident education and has been recognized for teaching excellence on multiple occasions. The PEM division is responsible for scheduling 24/7 attending pediatric physician coverage for the pediatric emergency medical service, utilizing PEM Fellowship trained / Board certified Pediatric Emergency Physicians who care for over 30,000 pediatric patients annually. They also provide 24-hour Pediatric Code Blue coverage for KMCWC. This division collectively has published countless research papers, chapters, received numerous awards and accomplishments in medical education, academic research, and community service. Since 2014, The PEM group has been employed by US Acute Care Solutions, the largest physician owned emergency medicine group in the country, headquartered in Canton, Ohio (usacs.com). 23

SPECIALTY CLINICS The Division staff a number of specialty clinics that are available to Hawai‘i’s children and adolescents. PROGRAM/FACULTY UPDATES Jasmin Jensen, MD, with support from the Hyundai Hope on Wheels Foundation, implemented Extra-Corporeal Photopheresis through the Apheresis Program, making this valuable therapy for graft versus host disease available locally for the first time. Stephanie Lim, MD was selected as Vice Chair of COG AALL2331: A Phase 2/3 Randomized Trial Investigating Daratumumab on a Modified Augmented BFM (aBFM) Backbone in Newly Diagnosed T-Lymphoblastic Leukemia (T-ALL) and T-Lymphoblastic Lymphoma (T-LL). She also opened the Kite ZUMA 4 CAR-T clinical trial, and brought an FDA approved CAR-T product for use in our medical community.

DIVISION OVERVIEW The Pediatric Hospitalist Medicine Division at Kapiolani Medical Center stands as a core group of 18 hospitalists. Beyond providing exceptional inpatient pediatric care, our members play integral roles within the medical center, John A. Burns School of Medicine, and Hawaii Pacific Health system. Noteworthy accomplishments include achieving subspecialty board status in Pediatric Hospital Medicine by the American Board of Pediatrics. Several division members have already passed the examination, and more are slated to take it this year. Amid the challenges of the pandemic, our division has admirably cared for various presentations of COVID-19, including the multisystem inflammatory condition of childhood (MIS-C), while addressing the healthcare needs of children who had been underserved during this time.

DIVISION OF PEDIATRIC HOSPITALIST MEDICINE MISSION As the cornerstone of inpatient pediatric care at Kapiolani Medical Center for Women & Children, we commit ourselves to excellence. Our dedication lies in delivering outstanding inpatient pediatric care to the children of Hawaii and the Pacific Islands/Pacific Rim. Additionally, we strive to serve as a foundation for emerging research initiatives in inpatient pediatric care while continuing to be innovative educators in pediatrics through the University of Hawaii, John A. Burns School of Medicine and the Pediatric Residency Program.

TEACHING Our focus on mentoring and developing future physicians, particularly pediatricians, remains resolute. Our division members hold esteemed faculty appointments at the University of Hawaii. As educators, we engage with learners from medical schools and universities worldwide, in addition to our own JABSOM medical students and Hawaii Residency Program trainees. Hospitalists take on roles to inspire medical students whether they are interested in pursuing a career in pediatrics or not. All division members are preceptors for third year medical students during their third year pediatrics rotation and many are POD-visors, longitudinal clinical mentors (LCM), advisors for the Pediatric Interest Group (PIG), interviewers for the medical school, simulation based session leaders, and Practice Based Learning (PBL) tutors. We are proud of Dr. Kyra Len, who excels as the Medical Student Director, ensuring exceptional experiences for JABSOM medical students throughout their clerkships. Several members, including Dr. Barry Mizuo, Dr. Diane Ching, Dr. Marissa Fakaosita, and Dr. Jordan Kono, actively contribute to refining and overseeing crucial aspects of medical student education. Dr. Tiffany Wang's remarkable leadership as Chair of the Pediatric Education Committee guarantees a diverse and comprehensive pediatric experience for our residents. 26

Dr. Gina Ramirez provides vital support as the inpatient pediatric wards rotation director, ensuring our residents' fulfilling inpatient ward experience.

RESEARCH, QUALITY, AND PROCESS IMPROVEMENT INITIATIVES Our unique position allows us to conduct outcomes research and implement impactful quality improvement initiatives in pediatric inpatient care. Throughout the year, our division members have been actively involved in institutional, local, and national research and QI projects. As a division, we had a poster accepted to the 2023 Hawaii State Patient Safety Conference entitled, “Results of Labs Pending at Hospital Discharge: Closing the Gap to Improve Patient Safety.” This poster highlighted the continued work that our division has done to ensure that labs ordered during the hospitalization and whose results return after discharge are shared with the appropriate providers with intervention occurring in a timely manner. At the Pediatric Academic Society (PAS) annual meeting, Dr. Maya Maxym and Dr. Marissa Fakaosita presented their work on the “Development and Implementation of a Hawai'iSpecific Anti-Racism Curriculum for Pediatric Hospitalists, Residents, and Wards Nurses.” Dr. Maxym's contributions extended to leading a workshop titled “Becoming a Changemaker: How to Incorporate Advocacy into Your Already Packed Schedule.” We are delighted that Dr. Maxym and Dr. Barry Mizuo participated in the second cohort of attending physicians in the BRIC (Biomedical Research and Innovation Center) Physician Research Training Program, enriching their expertise in their respective areas of interest. Dr. Mizuo’s project was entitled, “The Impact of Rater Training on Pediatric Simulation Assessment of Medical Students.” Dr. Maxym carried on with her work in the DEI arena.

DIVISION OVERVIEW The Pediatric Intensive Care Unit is a 14-bed acute care unit with the capacity to open an additional 4 to 6 beds for intermediate level care patients. Since the opening of the new PICU in the Diamond Head Tower in September, 2016, patients are now all cared for in private rooms, and families have sufficient space to stay with their children during their PICU stay. We admit approximately 600 children per year with a vast array of medical and surgical problems, ranging from sepsis to congenital heart disease, and respiratory failure. Patients from all over the Pacific are transported to the Kapi‘olani PICU, and we have an active, well-trained, transport team. In addition to caring for patients at Kapi‘olani, our physicians assist in the care of critically ill children at Kaiser Moanalua Medical Center. We are also involved in the care of Medically Fragile children throughout the State.

DIVISION OF PEDIATRIC SPECIALTIES DIVISION OVERVIEW The Pediatric Specialty Division is comprised of individuals with fellowship and/or additional specialized training in a diverse array of pediatric subspecialties. All of the division members participate in the residency, medical student, and other student teaching programs. Many have been recognized for excellence in teaching with University of Hawai‘i Pediatric Residency and Medical Student teaching awards. Many of our division members are active in quality improvement and clinical research activities, which often involve medical students and residents. The majority of the division are employed by Hawai‘i Pacific Health Medical Group (HPHMG). As a group, we provide services for pediatric patients across the state and Pacific Basin region. Our division collaborates on strategies to improve care for pediatric patients across our specialties as well as within the ambulatory and hospital settings.

Pediatric Specialty clinics are held at the Kapi’olani office tower and the Hale Pawa’a building. Some division members also provide care at other Hawai‘i Pacific Health (HPH) facilities and clinics, Kaiser facilities, and neighbor island outreach clinics. Comprehensive clinical services in all specialties have expanded to also include telemedicine services. Hospital services primarily occur at Kapi’olani Medical Center for Women and Children (KMCWC); however, some of our specialties also provide consultative services at other Oahu facilities, including Kaiser Moanalua Hospital and Queen’s Medical Center.

DIVISION OF PEDIATRIC SPECIALTIES PEDIATRIC CARDIOLOGY

MEDICAL GENETICS

The Pediatric Cardiology subdivision is composed of a dynamic group of doctors treating children and young adults with congenital and acquired heart conditions. The team operates under the umbrella of the new Mitsui family Pediatric Heart Center at Kapi’olani Medical Center for Women and Children. The members of the team include three pediatric cardiologists and a cardiothoracic surgeon. Besides clinical responsibilities including outpatient pediatric cardiology clinics, inpatient consultations in the NICU and PICU, ECG and echocardiogram evaluations, fetal cardiology clinics, cardiac catheterization procedures and open-heart surgeries, the group is actively involved in scholarly activities, biomedical research and innovation, and teaching at all levels. We offer an elective for MS3 students during their clerkship, a subspecialty elective for MS4 students and pediatric residents, as well as neonatology and maternal fetal medicine fellows. Besides pediatric cardiology, the group provides care for adults with congenital heart disease at Kapi’olani as well as at Straub and Queens.

Hawai‘i Community Genetics offers inpatient clinical services to KMCWC and Pali Momi, as well as outpatient clinical services at Hāle Pawaʻa. Through a partnership with the Department of Health we are also able provide neighbor island outreach, hemoglobinopathy clinic, and management services for our patients with metabolic diseases. Our group provides comprehensive genetics services including coordinating screening for genetic diseases involving single gene and chromosomal disorders, congenital anomalies, inborn errors of metabolism, multifactorial conditions, and common disorders with hereditary factors. We have routine clinics for general pediatric and adult indications, inborn errors of metabolism, hereditary cancer, cleft and craniofacial disorders, and neurogenetic indications. Genetics collaborates regularly with many community providers and multiple subspecialists to provide care to patients throughout the islands.

PEDIATRIC COMPLEX CARE The Pediatric Complex Care Clinic is a multi-disciplinary clinic that provides family centered, high quality and developmentally appropriate, health care services that continue uninterrupted as the individual moves along within systems of services, from infancy to adulthood. The clinic also provides on-going care coordination with multiple specialists, primary care providers and other community partners to promote the healthy development and well-being of children with medical complexity.

PEDIATRIC ENDOCRINOLOGY Pediatric Endocrinology is offered as an elective for Pediatric residents.

PEDIATRIC GASTROENTEROLOGY Pediatric Gastroenterology is offered as an elective for Pediatric residents.

Medical Genetics is offered as an elective for preclinical students in MD5, MS3 students during the Pediatric Clerkship, MS4 students, Pediatric residents, Ob/Gyn residents, and MFM fellows. We also provide learning opportunities for genetic counseling students and community learners.

PEDIATRIC INFECTIOUS DISEASES The Section of Infectious Diseases (ID) is very active clinically, academically and in service to Kapi‘olani Medical Center and the Hawai‘i community. Led by Dr. Marian Melish, they provide inpatient and outpatient services and significant support to the community in the form of telephone consultations. The ID and Cardiology Sections run the Kawasaki Disease Diagnosis and Treatment Program, following children through their acute illness, convalescence and until transition to adult care. The ID Section supports Infection Control and Clinical Epidemiology efforts at Kapi‘olani Medical Center, of which Dr. Melish is Medical Director. Dr. Ching is the Physician Leader of the Kapi‘olani Antibiotic Stewardship Program (KASP). The ID Section offers subinternships and observerships, which are popular among senior medical students from Hawai‘i, nationally and internationally. The Section also focuses on developing academic and career mentoring of pediatric residents, pharmacy residents, medical students and pre-medical students. All members provide educational lectures to medical students,

pediatric residents, and our community of health care providers through the Departments of Pediatrics and Obstetrics-Gynecology Grand Rounds, Hawai‘i Health Partners, Hawai‘i American Academy of Pediatrics and Hawai‘i Immunization Coalition. The highlight of our community education activity is the Annual Aloha Pediatric Infectious Diseases Conference, originally founded by Dr. Raul Rudoy, former Chair of the Department of Pediatrics and a Pediatric ID physician.

The Pediatric Neurology service provides consultative services for outpatients at Hale Pawa’a and inpatients at KMCWC. Our patients range from premature infants to adolescents and our cases involve the full spectrum of pediatric neurologic problems, from seizures/epilepsy, developmental delay, headaches, neurocutaneous disorders, encephalitis, cerebrovascular disorders, and less common demyelinating diseases, neuromuscular disorders, movement disorders and rare neurogenetic disorders. Providers participate in the Pediatric Complex Care clinic and in an out-reach clinic in Hilo. We also support electroencephalography at KMCWC for the coordination and interpretation of routine and prolonged video-EEG studies. Pediatric neurology rotations are offered as an elective to pediatric residents, and other shadowing/learning opportunities are available to medical students at various stages in their training.

Our group has been actively involved during the COVID-19 pandemic through clinical diagnosis and management of pediatric patients with COVID-19 and Multisystem Inflammatory Syndrome in Children associated with COVID-19 (MIS-C), including education of clinicians and patients during the roll out of Pediatric COVID-19 vaccination. Dr. Ching led a Pediatric group developing the MIS-C diagnostic and treatment protocol. We developed the Hawai‘i Pacific Health (HPH) COVID-19 Therapeutics Protocol with our HPH ID colleagues. We provided education to the health care community and public for COVID-19 and served on a Hawai‘i Department of Health advisory board. Finally, Dr. Yanagihara is the principal investigator of a National Institutes of Health funded community-engagement project focused on increasing COVID-19 testing among non-Native Hawaiian Pacific Islanders in Hawai‘i and Guam.

PEDIATRIC NEPHROLOGY Pediatric Nephrology offers consultative services at KMCWC and other hospitals, as well as nephrology clinic at the Hale Pawa’a office and neighbor island outreach clinics. Our clinic provides comprehensive pediatric nephrology services including general nephrology, chronic kidney disease, dialysis, and post renal transplant care. Pediatric Nephrology collaborates regularly with Pediatric Urology and other pediatric subspecialty services to provide comprehensive, multi-disciplinary care to patients. Unique specialized services offered include 24-hour ambulatory blood pressure monitoring. A Pediatric Nephrology rotation is offered to Pediatric residents. A combined nephrology and rheumatology elective is offered to Pediatric residents and MS4 students.

PEDIATRIC PALLIATIVE CARE Palliative Care provides specialized medical care to patients with serious and life-threatening conditions. Palliative Care is focused on identifying, preventing, and reducing the stress and symptoms associated with serious illness, including supporting a family’s values and preferences; helping families in making difficult decisions; identifying and treating symptoms associated with serious illness; providing end-of-life care; coordinating specialty and hospice services where appropriate; and providing emotional and spiritual support to the patient, family and healthcare team.

PEDIATRIC PHYSIATRY Pediatric Physiatry offers both outpatient and inpatient services in a multi-disciplinary approach for pediatric patients with disabilities resulting from disease or injury. Our focus is to develop a comprehensive program to place the pieces of a child’s life together: medically, socially, emotionally and educationally.

PEDIATRIC PULMONOLOGY Pediatric Pulmonology is offered as an elective for Pediatric residents. Next year, an elective will also be offered for MS4 students. In addition to their clinic at Hale Pawa’a, our pediatric pulmonologists also participate in the Pediatric Complex Care clinic and the multidisciplinary Pediatric Pulmonary Hypertension clinic.

DIVISION OF PEDIATRIC SPECIALTIES PEDIATRIC RHEUMATOLOGY Pediatric Rheumatology specializes in the care of children with rheumatologic conditions such as Systemic Lupus Erythematosus and related conditions (Sjogren’s syndrome, scleroderma, mixed connective tissue disease), Juvenile Arthritis, Spondyloarthritis, Juvenile Dermatomyositis, Acute Rheumatic Fever, and other inflammatory diseases in children or adolescents.

PEDIATRIC SLEEP MEDICINE Pediatric Sleep Medicine is a new specialty to our division, offering specialized consultative and diagnostic services for pediatric sleep disorders.

PEDIATRIC AND ADOLESCENT SPORTS MEDICINE The Pediatric and Adolescent Sports Medicine Department continues to be active in both the medical and the sports community. The physicians provide medical coverage for Castle High School, Hawaii Baptist Academy, Kamehameha Schools, Le Jardin, and Roosevelt High School. They also provide medical care for Chaminade University, Hawai’i Pacific University and University of Hawai’i Baseball. The physicians provide medical consultation for Hawai’i Concussion Assessment and Management Program and the Hawai’i High School Athletic Association Sports Medicine Advisory Committee. Current projects also include working with Team Up for Sports Safety (TUFSS), a national organization invested in reducing death from heat stroke and sudden cardiac death in athletes, collaborating with state organizations to provide continuing education for athletic trainers, and collaborating with other providers to provide baseline concussion testing for professional surfers. Rotations are offered for medical students, pediatric residents, family medicine residents and sports medicine fellows as well as shadowing opportunities for high school and college students. The department remains active in the Summer Student Research Program at HPH.

Resident Research Infrastructure Aloha and greetings from the Resident Research Directors! As Research Directors, we encourage and support the scholarly activity by the residents of University of Hawaii Pediatric Residency Program. Our desire is to train tomorrow’s physicians and healthcare leaders by providing opportunities to practice important skills, like how to develop research ideas, establish goals, practice critical thinking, incorporate feedback, and effectively communicate findings through abstracts at various conferences and by publications. Exploring the ideas of today will help advance pediatric medicine, leading to a healthier future for all children. First of all, many thanks to our pediatric residents! The success of our research curriculum continues to build upon their hard work, passion and dedication. Mahalo nui loa to all the faculty members for mentoring, supporting, and encouraging our residents’ scholarly activity. To support the scholarly activities, we meet with the residents at least every quarter. We review and provide directions in their research work. We help identify and establish their network with the appropriate research mentors. We often help them manage any obstacle that come in their research journey. We also invite internal and external guest lecturers to discuss our resources at Hawaii Pacific Health Research Institution (HPHRI) and University of Hawaii at Manoa and JABSOM. We also conduct educational didactics via lectures on statistics, grant writing, research design, manuscript writing and many more. Every year our pediatric residents present their abstracts at various conferences and publish their work in renowned and well-respected journals. This is reflective of dedication and commitment of our residents and their mentors. This year on May 24th, 2023, we celebrated our 22nd Annual Dr. Raul Rudoy Research Day. We had a total of 8 scientific presentations. This special time reflected the ongoing commitment to research and scholarly activity of the Pediatric Residency Program. A warm mahalo to our wonderful keynote speaker, Dr. Dominic Chow. He shared his knowledge, expertise, and his research journey about “Post Acute Sequalae of COVID-19 in Hawaii” on this day. We were honored to have him, and we will continue to invite such honorable speakers every year on the Annual Research Day.

2022-2023 Pediatric Resident Rural Rotation Providing pediatric residents with rural healthcare opportunities positively contributes to their education in ways that cannot be achieved while training at a tertiary academic center. These rotations also expose training physicians to clinical practice and potential career opportunities in areas of Hawaii where there is a great need for primary care pediatricians or hospitalists. During these pediatric rotations, usually on the neighbor islands, residents become acquainted with the skill and art in providing high level care in a rural area. The goals of the rural health experiences are: 1. To familiarize Pediatric residents with healthcare resources and care models in rural areas to better facilitate care for these patients. 2. To encourage Pediatric residents to consider practice in a rural area. The Department of Pediatrics continues to offer two types of rural health experiences for Pediatric residents during the 2022-2023 academic year: Neighbor island shadowing: Pediatric residents spend a day on a neighbor island as an introduction to rural health care and to stimulate interest in doing a neighbor island IEU (Individualized educational unit) or consider practicing on a neighbor island. With the exception of one resident, all pediatric residents in the program participated in this opportunity. Pediatric residents would either go with the Neonatology team and participate in neonatal resuscitation training for nurses, respiratory therapists and physicians at a neighbor island hospital and then spend the remainder of the day with a community Pediatrician or spend a full day shadowing a neighbor island Pediatrician if there was no scheduled hospital training. The first-year Pediatric residents did their shadowing during their Neonatal Intensive Care Unit (NICU) rotation while the second- and third-year Pediatric residents did their shadowing during their NICU or ambulatory rotations. Neighbor island rotation as an IEU (Individualized Educational Units): Pediatric residents can choose to spend 2-4 weeks on a neighbor island working in a rural hospital and/or with community pediatricians during their second and third year as one of their IEU rotations. Two residents did neighbor island IEUs during the 2022-2023 academic year, both in Kamuela on the island of Hawaii. One resident did a 4-week rotation, and the other resident did a 2-week rotation.

2022-2023 Pediatric Resident Rural Rotation JABSOM legislative funds provided support for travel and transportation costs for Neighbor Island shadowing and travel, transportation, and accommodations for neighbor island IEUs. External grant funding covered travel costs for one resident for a neighbor island shadowing. In addition to the neighbor island experiences and IEUs, JABSOM legislative funding has allowed for the purchase of simulation equipment and supplies to train rural health providers beginning in July 2023. This training will include the Pediatric residents and Neonatal fellows, who will accompany the Neonatologists to participate in this training. Three physicians from the Pediatric Residency Program at the University of Hawaii are or will be working on a neighbor island. One physician who completed Pediatric residency in June 2022 is in practice on Kauai. Two physicians will be practicing on Hawaii Island. One physician competed Pediatric residency in June 2023 and has joined the Hawaii Island Community Health Center and another physician who completes Pediatric residency in October 2023 will be working as a Pediatric Hospitalist at North Hawaii Community Hospital in Kamuela.

Pediatrics Interest Group (PIG) The PIG is among the most active interest groups at UH JABSOM. PIG officers are elected at the beginning of the school year. MS1 PIG officers work with the MS2 PIG officers to plan and host PIG activities for the school year. In the school year 2022 to 2023, the PIG hosted the following activities: Aug 2022: Election of new officers (Justin Abe-VP, Shay Nakahira-VP, Richard Chen-Tr, Anita Cheung-Tr, Megan Yung-EC, Johnathan Kim-EC, Kirra Borrello-EC, Sydney Maarat-B, Joyce Huang-B, Jasmine Padmada-M, Selena Vanapruks-B. Meet Dept of Pediatrics faculty, Pediatrics Trivia Game. Faculty: Jessica Kosut, Loren Yamamoto Oct 2022: Splint workshop. Students attended a workshop to learn some fracture terminology and splint application for initial fracture management. Students used cast padding, stockinette, fiberglass, and elastic wraps. Students were instructed in the application of short arm volar, thumb spica, long arm posterior, upper extremity sugar tong, long leg posterior, and lower extremity stirrup splints. This event was sponsored by the University of Hawaii Student Activity & Program Fee Board (SAPFB) and by BSN Medical (the makers of OrthoGlass who supplies the fiberglass splint materials). PIG officers applied for and successfully received SAPFB funding for this event. Faculty: Travis Hong, Kyra Len, Dan Murai, Loren Yamamoto Nov 2022: Chest X-ray and asthma gadgets workshop. Students were taught how to read a chest X-ray and were shown multiple abnormal pediatric chest X-rays.Students taught how to use asthma inhalation devices including, HFA inhalers, spacers, dry powder inhalers, peak flow meters, nebulizers. Students were provided with placebo HFA inhalers. Students demonstrate their inhaler technique under faculty guidance and critique. Students also received bag mask ventilation training of adult and infant manikins. Faculty: Edward Fong, Barry Mizuo, Kenneth Nakamura, Loren Yamamoto Jan 2023: Pediatrics Social with Faculty. Dr. Travis Hong volunteered his home to squeeze roughly 50 medical students (MS1s, MS2s, MS3s, MS4s) potentially interested in pediatrics and 12 faculty into his living/dining/kitchen to have dinner, chat, and discuss medical and non-medical topics. Students competed for valuable prizes in the complex game known as “Been There, Done That”. March 2023: Pediatric Specialty Career night. Students and faculty were treated to a buffet dinner at JABSOM. UH JABSOM Pediatric faculty in nearly every specialty and subspecialty faculty (in primary care pediatrics, nursery, hospitalist pediatrics, neonatology, pediatric intensive care, pediatric emergency medicine, pediatric cardiology, pediatric pulmonology, pediatric gastroenterology, pediatric nephrology, pediatric endocrinology, pediatric neurology, pediatric hematology/oncology, pediatric rheumatology, adolescent medicine, child abuse, developmental/behavioral pediatrics, sports medicine) joined the students for

this event. Students rotated through pairs of subspecialties every 10 minutes in small groups of 2 to 3 students to ask questions to the faculty. At the end of this, all students and faculty returned to the big group for a big group photo. Students were taught how to perform venipuncture phlebotomy using the Vacutainer system under the guidance of faculty and two former phlebotomists. Departmental Research Report Departmental Research Report (2022-2023): Grants and Researchers: The Departmental faculty have established a good track record in procuring extra-mural grants for many research and service products. For the past several years, the cumulative amounts of these grants have been in excess of $ 1 million annually and the department ranks very highly among the other JABSOM departments in attracting these funds. Active PI’s with grants for research and service contracts in the Department include, Akshatha, Venkataraman Balaraman, Andras Bratincsak, Rachel Coel, Marissa Fakaosita, Pia Francisco Natanauan, Travis Hong, Lynn Iwamoto, Jessica Kosut, Sheree Kuo, Wade Kyono, Jannet Lee-Jayram, Kyra Len, Marian Melish, Charles Neal, Cheryl Okado, May Okihiro, Kara Wong Ramsey, Kelsey Rhodes, Loren Yamamoto, and Richard Yanagihara. The Department is in the process of actively tracking all the research work done in one centralized place to allow for more effective collaboration amongst investigators. IDeA States Pediatric Clinical Trials Network (ISPCTN) Research This NIH funded network is currently in the second funding cycle as a site to recruit patients in IDeA states sponsored pediatric and adolescent clinical trials. Active projects under collaboration (not all funded under ISPCTN) are: Eat Sleep Console (ESC) approach for management of newborns with Neonatal Opioid Withdrawal Syndrome (NOWS). The site Junior Investigators leading this are Drs. Akshatha and Kara Wong Ramsey under the guidance of Dr. Charles Neal. This study is in the final stages of completing the two year follow up on consented patients for neurodevelopmental outcomes. The Primary paper from this study was published in the New England Journal of Medicine in April 2023 with both Drs. Akshatha and Wong-Ramsey contributing to authorship. MoveUP study – Improving Pediatric COVID-19 Vaccine Uptake Using an mHealth Tool. Actively recruiting patients from two HPH based clinics – Pediatric Clinic at KMCWC and KMC Clinic at Lihue, Kauai. Dr. Kelsey Rhodes in the site PI for this study. We enrolled six patients across our two sites. (ISPCTN sponsored) Bronchiolitis Recovery and the Use of High Efficiency Particulate Air (HEPA) Filters. Actively enrolling with our site hoping to enroll 10 patients. Dr. Brian Wu is the site PI for this study. (ISPCTN sponsored)

Post-Acute Sequelae of COVID-19 (PASC-RECOVER) is actively recruiting patients at Kapiolani Medical Center for Women and Children. Drs. Jessica Kosut and Travis Hong lead this study at Kapiolani Medical Center and we have done well with recruiting over 60 Pediatric and Young adult patients into this protocol. Our site has been selected for participation in the PhiTT trial which is the use of Phenteramine in the management of overweight children/adolescents. Dr. Pia FranciscoNatanauan will be the study PI for this project. Biomedical Research and Innovation Center (BRIC at HPH) Two of the Departmental faculty members Drs. Andras Bratincsak and Russell Woo along with Dr. Cass Nakasone provide leadership to this center. The center sponsors a one-year program in clinical research to interested faculty and is a pathway for faculty development. Funding for the program comes from HPH Medical Group with optimized use of all of HPH wide shared resources supporting research. Two of the Departmental members were BRIC supported faculty this year – Maya Maxym and Kelley Hutchins. The Program is actively developing web-based resources to support faculty needs for research. HPH Summer Student Research Program (HPH SSRP) This program sponsored by HPH to engage college students in an eight week curriculum in the area of Biomedical Research and exposure to practice of medicine in Hawai‘i. 12-14 students are selected annually. They work with Research mentors primarily on clinical projects. This year several pediatric faculty members were research mentors to the students – . Children’s Oncology Group (COG) Protocols Dr. Wade Kyono acts as the site PI for all pediatric oncology studies done under the auspices of COG at Kapi‘olani Medical Center for Women and Children
