into making these books both enjoyable to read and a learning experience for children. Also, I enjoyed reading about the people who felt they could write a book about their experiences. If I could write a book, it would probably be about eye doctors and vision issues and the multitudes of opinions everyone I know has on the topic. Patch while you still can... Don’t patch, the kid won’t have 3D vision… Only prisms... Don’t do prisms... Only surgery... Only London... Don’t do anything until you try vision therapy.... I think that every doctor and every patient has an entirely different opinion, and it’s hard to feel confident in the hishtadlus you choose. Your magazine is always thought-provoking! (Re: I Could Write a Book About That!, Issue 215) In the article about the family with a bad case of lice, the writer wrote that the Egyptians had it easier than she did because they only had lice for seven days while her family struggled with lice for many weeks. The Midrash, however, explains that after seven days the makkah ended, which means that more lice didn’t appear, but the lice that were already there did not disappear. This was true of all the makkos. After makkas dam, the Nile didn’t return to water; the new water that entered the Nile remained as water and did not turn into blood, but it took a long time until the river cleared and the water was drinkable again. After seven days, the lice didn’t disappear; they simply stopped multiplying, and now the Egyptian had to work hard to get rid of it. In addition, according to the Midrash, some of the lice in Mitzrayim were the size of eggs, some had razor-sharp teeth, and there were body lice, too. Hashem promised never to bring any of the makkos upon us Yidden. As severe as your case of lice was, there is no comparison! H.F.

(Re: Inbox, Not Public Property, Issue 215) I read the letter written by the person who was upset that strangers entered her neighbor’s home — without permission — while it was under construction. I would like to explain. No one wants to upset your new neighbors — certainly not their future neighbors. We’re actually happy for them that they’re building a new house. The reason we go in to look at the house while it’s under construction is because we know that once they move in, we won’t get the chance anymore. I know this is hard to believe, but I once watched a house go up in my area, and every day saw deliveries of huge windows and modern tiles. such a luxurious house piqued my curiosity. (Don’t worry, I didn’t go inside.) But when the owners moved in, they weren’t open and welcoming to everyone in the neighborhood. Their secrecy about their house caused so much petty competitiveness in the neighborhood — who was invited in, who wasn’t; what they saw inside, what they didn’t… It’s not as if they didn’t invite anyone in. They did. But while some people were invited in, other people weren’t. So what are those who were not invited supposed to think? To that new neighbor I’d like to say: By not letting us in, you’re causing hurt feelings, and the jealousy that you’re probably trying to avoid. Being nice can’t hurt. Maintain old-fashioned friendliness, and everyone will fargin you! I showed this letter to a few friends and they added the following points: There are probably 30 heimishe workers going in and out of your neighbor’s house on a daily basis — electricians, plumbers, painters, and so on. So why would I think that they have an issue with people from the neighborhood going in to take a look? Also, if your neighbor is building a beautiful home with stunning landscaping, they can’t claim “ayin hara” as the reason that other people shouldn’t come look at their house. Sincerely, N.P.

PARSHAS SHEMOS The Midwives

When Pharaoh commanded the Jewish midwives, Shifra and Puah, not to allow the baby boys to live, the pasuk says, “The midwives feared Hashem, and did not do as the king of Mitzrayim had spoken to them, and they enabled the boys to live” (Shemos 1:17). Several pesukim later, when the Torah describes the reward Shifra and Puah received, it says, “And it was because the midwives feared Hashem, that He made houses [of kehunah, leviya and malchus] for them” (1:21). Why does the pasuk attribute the reward to their yiras Shamayim? Surely the midwives were rewarded for their mesirus nefesh of not listening to Pharaoh and keeping the babies alive! ONE DAY, a woman came to the Chofetz Chaim, tears streaming down her face. Her husband (who was not shomer Shabbos) was in critical condition, she cried. The doctors could not find a solution, and it seemed he faced inevitable death. Her last hope was a bracha from the tzaddik. Perhaps that would restore him to health. The Chofetz Chaim listened and replied, “Do you know how a prison is run?” The woman didn’t understand the connection and assumed that the Chofetz Chaim had not understood her request. She told her story again, describing her husband’s dire situation, but the Chofetz Chaim simply repeated himself: “Do you know how things work in a prison?” The woman was obstinate. “Rebbi, I came here to ask for a bracha for my sick husband!” The Chofetz Chaim did not answer her request, but continued following his train of thought. “If you don’t want to answer, I will… When a person is caught in a transgression against the authorities, he is locked behind bars. The prison is secured by iron gates and large, heavy locks that are nearly impossible to break open. Armed guards watch over the site, which is well-lit on the outside so none of the prisoners should be able to escape

under cover of night. “These are the measures a human ruler must take to detain offenders. But when Hashem wants to put a person in prison, these security measures are unnecessary. The Ribbono Shel Olam does not need iron gates, guards or light poles. He simply sends the person an illness that confines him to his bed. That’s all! “Your husband, too, is experiencing the punishment of being ‘imprisoned.’ But you should know that just like in our world, when a prisoner behaves properly, his sentence can be commuted, so too in Shamayim; a person who corrects his ways can merit having his suffering curtailed. There’s no other way to break free from the walls of spiritually-inflicted imprisonment!” The Chofetz Chaim explained, “You’re asking for a bracha. Who am I to give brachos? The Ribbono Shel Olam promises that whoever guards Shabbos merits bracha! “A person who opens his shop on Shabbos, or doesn’t close it on time before Shabbos, is not able to receive bracha. Tell your husband that if he promises to close his store on Shabbos, he will recover.” The woman’s response was immediate. “Rebbi, I promise!

“Tell him in my name that he must promise to close his shop on Shabbos” My husband will close his shop on Shabbos!” “You are promising… but that’s not enough! I need your husband to commit to this.” “Rebbi,” the woman cried, “my husband is in no state to come here!” “He doesn’t need to,” the Chofetz Chaim rejoined. “You go to him and tell him in my name that he must promise to close his shop on Shabbos.” “I don’t know if he’ll survive until I return home…” The Chofetz Chaim reassured her that she needn’t worry and sent a messenger to accompany her home. The messenger soon returned and reported that as soon as the woman had arrived home and got her husband to commit never to open his store again on Shabbos, there was a marked improvement in his condition. Several days later, the man was fully recovered. Even before any action is taken, with just a strong kabbalah — a conscious decision to improve — it is considered done, and the person can merit bracha already. This man recovered from his illness before he had the opportunity to close his store for a single Shabbos – all through the power of his undertaking to do so. * * * * * The Alshich Hakadosh explains that if the pasuk would simply state the reward of the midwives, one might think that they received it due to their praiseworthy actions. By repeating that the midwives feared Hashem, the Torah is emphasizing that they were already worthy of their reward before they even did anything. Shifra and Puah deserved their reward simply for the fact that they feared Hashem and decided in their hearts not to follow Pharaoh’s command. Three-Year Process Begins To Perpetuate the Legacy of Misaskim’s Yankie Meyer A block-long stretch of 16th Avenue will likely be getting a new name, carrying on the legacy of Rabbi Yankie Meyer, founder and CEO of Misaskim, who was renowned for his efforts to help every person in their hour of need. Brooklyn Borough President Antonio Reynoso contacted Community Board 12 on December 20, advocating for a street to bear Meyer’s name. Meyer passed away in September 2022. “Rabbi Meyer was a leader whose dedication to faith and service can be seen all over Brooklyn and this city,” wrote Reynoso. “In recognition of his profound impact, it would be fitting for a Borough Park Street to bear his name, so that Brooklynites and visitors from all over the world can be inspired by his devotion and selflessness.” Community Board 12 president Yidel Perlstein said that the process of renaming a street takes approximately three years, and that the most likely spot would be 16th Avenue between 58th and 59th Streets, home to not only Misaskim’s offices, but also to the NYPD’s 66th Precinct, where Meyer was a familiar face. Meyer was known throughout the precinct for his efforts to support those going through crisis and tragedy, and his compassion extended to police officers as well. “This is not only about Yankie Meyer and the Yidden,” Perlstein told The Boro Park View. “This is a great, great kiddush Hashem for all the non-Jews he helped as well. When a policeman had a fire, or had a kid in the hospital, or anything, [also] the fire department, he was there for everyone.” Perpetuating the breadth of Meyer’s efforts by naming the street in his honor will also serve as a reminder to the Jewish community to follow in his footsteps, explained Perlstein. “This is one of the few times where we are not only honoring and respecting someone who was a true hero in Yiddishkeit,” said Perlstein. “He was a true hero to all of us, even in the non-Yiddishe world.” “Everybody knows Jack Meyer,” added Perlstein. “He was a household name.” After Yankie’s passing, many tributes came from outside of the local Jewish community. “We mourn the loss of Rabbi Yankie ‘Jack’ Meyer who

was always a great leader and liaison to the community,” wrote NYPD 66th Precinct. “Jack will be missed, and we offer condolences to the family and friends.” “Yankie Meyer was, and always will be, a legend,” asserted Detective Mohamed Amen of the NYPD. “My condolences to the family for the sad passing of a good human being that helped so many families at their hardest time.” “My heart is with New York’s Jewish community after the tragic loss of Reb Yankie ‘Jack’ Meyer,” posted New York State Attorney General Letitia James. “He spent his life serving others, and his legacy will live on.”

NY Switching to ElectricOnly School Buses by 2035

lance cameras captured footage of the robbery, which included the theft of $5,000 in cash, three pushkas holding an unknown amount in donations, a camera and a small aron kodesh that did not contain any Sifrei Torah. Shomrim reviewed footage from nearby security cameras, picking up the license plate of the burglars’ red Toyota Camry on one of its own high-definition cameras several blocks away from the yeshiva. The plates, which were not registered to the Camry, were quickly determined to be either fakes or stolen, and the car was eventually pulled over by police officers who were not involved in the case but also noticed the license plate discrepancy. The two occupants of the Camry were arrested, but found not to be involved in the robbery. The vehicle’s VIN number led police to Racero and the stolen merchandise. Racero was arrested by the NYPD and arraigned on charges that included burglary in the third degree, grand larceny, possession of burglar’s tools, criminal mischief, criminal possession of stolen property and trespass. Bail for Racero was set at $20,000 cash or $50,000 bond.

New Minimum Wage Rates Now in Effect Statewide Get ready to pay more just about everywhere for pretty much everything now that New York’s minimum wage has risen yet again. As of January 1, minimum wage in New York City is $16 per hour. That rate is also in effect in Westchester and Long Island, with a $15 per hour amount in place elsewhere in New York. Minimum wage rates for home health care aides are higher, coming in at $18.55 in New York City, Westchester and Long Island, and at $17.55 in the upstate counties. The rate hikes are part of a multi-year plan designed to have minimum wage workers’ salaries keeping pace with inflation. The plan will have rates rising in 2025 and 2026 as well. According to Governor Kathy Hochul’s office, research has shown that raising the minimum wage can have multiple benefits including reductions in poverty and social assistance spending as well as improved worker productivity. “The elevation of the minimum wage serves as a lifeline for New Yorkers struggling to make ends meet as costs rise,” said Commissioner Roberta Reardon of the New York State Department of Labor. “Steady, multi-year changes allow businesses time 56

In not much more than a decade, all of New York State’s kids will be riding to school on electric buses, with $100 million in funding being made available to school districts to help them acquire the zeroemission buses that will be mandatory in 2035. New York became the first state in the country to adopt an electric school bus mandate last spring, requiring all bus purchases made as of 2027 to be electric under the state’s Electric School Bus Roadmap, which was released in September. Announcing the new funding allocation, part of New York’s $4.2 billion Clean Water, Clean Air and Green Jobs Environmental Bond Act of 2022, Governor Kathy Hochul said that electric buses would provide kids with healthier transportation options. “Ensuring electric school buses are at the forefront of student transport is a critical health investment and demonstrates the benefits of clean energy while reducing harmful emissions to improve air quality and protect our precious environment,” said the governor. The funding also provides rebates to school bus fleet operators who purchase and install eligible charging infrastructure alongside their zero-emission vehicles, with larger amounts allocated to serving priority districts. But while New York State Energy Research and Development Authority CEO and president Doreen Harris lauded the incentive program, saying that it would “create cleaner, healthier commutes to school for our students” that “will pay dividends for decades to come,” others were less enthusiastic. “Each electric school bus costs approximately $350,000, and there are 45,000 school buses statewide,” wrote Assemblyman William Barclay, leader of the legislative body’s Republican Conference, on social media. “The cost to replace diesel school buses with electric school buses could be well over $11 BILLION on the backs of taxpayers.”

She broke away from the huddle of babbling aunts and cousins and paced the far end of the hallway. Another text: Yaaaaaaaakov!!!!! WHERE ARE YOU???? — and still no response. Fraidy was lost somewhere in this huge building. She was gone for, how long was it now? Half an hour? Forty minutes? Riva was ready to call police. Oh my goodness!” She pulled her car keys from her bag. “Do you want me to drive you home?” “I would appreciate that,” Riva said. “Yes.” But then she pointed to SpecTac’s door. “But what about the party? I can’t just leave in the middle.” “It’s pretty much over. Leave the rest of your kids here, I’ll bring them home a little later. Or maybe you’ll come back once you see that Fraidy’s alright. Don’t worry about that now.” “Okay. I think I’ll take you up on your offer. Thank you so much, Chanie.” Riva updated her parents and followed Chanie to the door. As they stepped out, the door opened again, and Lani dashed after them. “Hey, Riva. I don’t want keep you — go home, and I’ll take care of the cleanup here. Just tell me what I owe you for the food, I know you laid it all out.” “The food?” Riva repeated dazedly. She waited for Chanie to walk further ahead, and when her aunt was out of earshot, she told Lani, “You don’t owe me anything. I didn’t end up ordering.” “You — what?” Lani blinked. “You mean, Faigy was right? You made everything yourself?” “Not everything. The aunts helped.” “I don’t get it.” Riva clutched her phone. “Look, I need to go now. Fraidy needs me. Forget about the food. I just… at some point I realized how much money it was going to cost, and I felt like it didn’t make sense. Don’t worry, it was no big deal, and I’m glad it all worked out.” Lani’s face was like a spinning color wheel, fading from red to white, then taking on a distinctly gray tinge. But Riva didn’t have the headspace to deal with her sister now. “I’ll talk to you later,” she said, and hurried after her aunt. *****

It was a good thing she’d left so abruptly, or Lani would’ve said things she’d later regret. She remained frozen at the building’s entrance for several long minutes, digesting Riva’s revelation. So. All those “Oh, by-the-ways,” and “I figured I’d justs” — they’d been anything but random. This was a calculated plan, carefully carried out behind Lani’s back. Don’t worry, it was no big deal. She went back inside and returned to the party room. Over at the women’s table, she saw her aunt Bashie Miller shake open a huge garbage bag and two of her daughters pull the tablecloth into it. She knew she should start cleaning up, but instead she made her way over to the row of chafing dishes and gazed at the cooling leftover food. How had she not realized? That was Riva’s vegetable orzo, she’d made it so many times over the years. “Sesame chicken and pepper steak,” Riva had told her, but that was obviously only part of her list. Her sister had misled her, true. But at the same time, the fact registered: Riva’s “scheme” had saved her a ton of money, her entire share of the food cost. Suddenly she was filled with a pleasant warmth. I realized how much money this was going to cost us… Because Riva had done this for her, to spare her the expense. Lani peeled herself away from the food and went to get a garbage bag. Together with her aunts and cousins, they collected all the garbage and then sorted the leftover food into containers. As they moved aside to allow the men to spill the hot water from the roaster pans, Lani spied Chevi. She was sitting alone in a quiet corner, scrolling through the pictures on Lani’s camera. Lani approached her and sat down next to her. “Do you want to tell me what happened?” she asked gently. Chevi looked up and Lani noticed that her face was red. She’d been crying. “It was a mistake,” Chevi blurted. “A mistake…” Lani took the camera from her and absently scrolled through that night’s pictures.

She paused at the end, after the picture of her parents receiving the scrapbook. There was nothing more after that. Chevi had deleted the video. *****

Yaakov and Fraidy were sitting at the dining room table when Riva walked in. When Fraidy saw her mother, she hung her head in embarrassment. “I know, I know…” she muttered. “It was a crazy thing to do.” “I’m glad you’re safe,” Riva responded. She left it at that. It wasn’t the right time to admonish Fraidy about safety, although it took every bit of effort to restrain herself. Instead, they spoke about what had happened, validated Fraidy’s feelings, and helped her calm down. “I think you should take a hot shower and go to sleep now,” Riva suggested after a while. “You’ll wake up tomorrow feeling better, and you’ll see, you’ll be able to move on from this. And I also think Chevi is going to move on. She went too far tonight, and I’m sure she realizes it.” Fraidy shuffled to her room. “That was… insane,” Yaakov said quietly. “You’re telling me!” Riva kicked her shoes off and went to sit on the couch. “I’m literally dizzy.” She was dizzy, and on top of that, her mind trotted off in an unexpected direction — to Kleenup, to Mr. Gabioff, to the torture and abuse she’d been putting up with for so many years. To the slow erosion of her dignity, to the quiet, sneering condescension. And then to the sudden mysterious kindness, which was so incongruous, it felt like another clever move in the cruel game of manipulation that her boss played with his staff. With the image of Fraidy dropping the mic in horror, Riva suddenly gained a clarity that had always been a hazy thought in her brain, but now crystallized into a fact: I need to quit. She heard the bathroom door snap closed, and once again, her heart flooded with pity. “What made

Um, actually, I didn’t even know she was missing.” “What do you mean?” “She walked into the house on me.” Riva stared. “I’m so confused. You were home? What does that mean? We were all in SpecTac when this happened.” “Right, but I left a littler earlier.” “Why?” “I just… needed to come home for a bit.” Work? She stared at him, waiting for him to say more. He sat with stiff shoulders and studiously avoided her eyes. “What?” he said defensively. “It’s nothing. I just… needed a little space. The noise was getting to me.” Riva stood up from the couch abruptly. “Yaakov Stroh. We’re married for…” she fanned open her fingers like dominoes. “Eleven years. And in all that time, I never once heard you complain about noise. Did you grow old overnight?” The fear made her voice louder than necessary. Something was terribly wrong, and her entire body quivered with dread. Finally, Yaakov’s shoulders seemed to give up the fight and sagged at his sides. “I lost my job with Reuven Bleich.” “You —” Of all the things she’d expected him to say, this was not it. “I was worried about it for a while,” Yaakov admitted. “Bleich’s son has been growing interested in the field over the past few months, and he recently enrolled in architectural school. So lately, Reuven started giving him some of the work he normally would give to me.” He paused for a moment, and Riva detected a stab of hurt on his face. “He actually took away the Clove project from me a few weeks ago, before I had a chance to start working on it. His son is working on it now.” Slowly, his words registered in Riva’s head. “That was… Motzei Shavuos, right? You were working in our room, and I knew something was wrong.” He nodded. “That’s when I saw Bleich’s email about Clove.”

“And… now?” “Now he called me. I shouldn’t have answered, I know. We were at a party, and I should’ve called him back tomorrow morning. But I thought, if he’s calling me at night, it must be important.” He grimaced. “Very important, to let me know that I was officially out of work.” Riva heard the bathroom door open. Fraidy was out of the shower. She stared at Yaakov wordlessly. “I know I can’t blame him,” Yaakov said. “Family comes first…” Did it matter that there was no one to blame? Nothing mattered anymore. Just a few minutes ago, she’d made the confident decision to leave her job. And now… “It’s not only this job,” Yaakov told her. “I was getting a foot in the door, gaining tons of experience. This was my opportunity to break into the industry. Now I feel like I’m back to square one. It’s going to take forever to build a portfolio and get my name out there.” Fraidy walked into the dining room. “Are you still going back?” she asked. It took Riva a minute to comprehend where she meant by back. “Uh, I don’t think so.” She was too disoriented to go back to a party. She also suspected that Fraidy didn’t want her to leave. She turned to Yaakov. “Do you want to go pick up the kids?” “Yes, I guess so.” When he left, Riva roamed around the house, trying to pack things away but unable to focus on where the stuff belonged. She was sorting a pile of parsha sheets for sheimos when her phone rang. Lani. Riva could barely bring herself to say hello. “I’m just calling to check up on Fraidy,” Lani said. “Is she okay?” “She’ll be okay,” Riva answered curtly. “Aha. I’m… sorry. LEMON-INFUSED BUNDT CAKE This lemon-infused Bundt cake will add brightness and sunshine to your table. INGREDIENTS 4

D IR EC T I O N S 1. Preheat the oven to 350°. 2. Using an electric mixer, cream together the eggs and sugar. 3. Add the rest of the ingredients in the order listed, then mix to combine. 4. Grease a Bundt pan with baking spray that contains flour. Pour the batter into the pan, and bake for 55 minutes to 1 hour, or until a wooden skewer inserted in the center comes out clean.

5. While the cake bakes, prepare the lemon syrup so it’s ready when the cake is done baking: Bring the sugar and lemon juice to a boil in a small saucepan, stirring occasionally. Turn off the heat as soon as the sugar is completely dissolved. Set aside this syrup. 6. Remove the cake from the oven, and let it rest for about 5 minutes. Prick holes in the cake with a fork. 7. Pour the cooled lemon syrup over the cake. Let the cake cool in the pan for another 15 minutes. 8. Remove the cake from the pan. Now prepare the glaze: Mix together the confectioners’ sugar and lemon juice until the mixture is smooth. Pour this over the cake.

1. Preheat the oven to 350°.

3. Combine the flour and baking powder, then set this aside. 4. In the bowl of an electric mixer, beat together the eggs and sugar until it’s light in color. 5. Add the vanilla sugar, oil and soy milk. Mix for a minute to combine. Add the flour mixture, and mix until just combined. 6. In a separate bowl, mix together the streusel ingredients. 7. Pour half the cake batter into the Bundt pan, then top that with half the streusel mixture. Pour in the remaining half of the cake batter, then top with the remaining streusel crumbs. 8. Bake for 55 to 60 minutes. 9. For the glaze: Mix together the confectioners’ sugar, cinnamon and soy milk in a bowl until a smooth mixture forms. 10. Allow the cake to cool completely. 11. Remove the cake from the pan. Pour the glaze over the cake, then tap the cake gently on the countertop to let the glaze run down slowly. 12. Sprinkle the crushed graham crackers on top.

TIPS The perfect glaze is thick, not runny. If your glaze is runny, add confectioners’ sugar. Bundt stuck in the pan? Try these methods: •

Let the cake cool for 10 to 15 minutes before flipping it over.

While the cake is still warm, just out of the oven, run a knife around the perimeter of the pan to help release the cake from the sides.

After removing your cake from the oven, place a kitchen towel in your sink. Carefully pour very hot water over the towel until it’s damp, but not dripping. Drape the towel over the top of the pan, and leave it there until the pan is cool enough to handle. The steam will help release the cake from the pan when you invert it.

Caring for the littlest of patients requires a well-honed skill set, years of experience and a big heart. Luckily for our community, Dr. David Rosenfeld has them all. A pediatrician builds his practice not by expertise alone, but largely on trust. Working with the community for 25 years, Dr. Rosenfeld has developed a devoted following and has his finger on the pulse of the community, literally. But first, some background.

The Making of a Doctor Just days after her yartzeit, Dr. Rosenfeld reflects on the most formative figure in his life, who ignited his passion for medicine. “My mother was a nurse trained during World War Two, but in so many ways she was self-taught,” Dr. Rosenfeld reminisces. “She was fifty years ahead of her time.” Long before the medical establishment discovered the ills of artificial sweeteners, Mrs. Rosenfeld firmly opposed them. “And what do you know?” says Dr. Rosenfeld, “Recent studies show them not just ineffective in combating obesity, but they very well may contribute to diabetes.” But most of all, Dr. Rosenfeld remembers his mother as a nurse with a heart. She’d spend hours on the phone, freely sharing her knowledge without remuneration. The image of his mother assisting others was the impetus for his entering the helping profession. “My path up until med school was fairly typical. I learned in kollel for many years, and lifeguarded in the summer. In that capacity, I became the go-to for ace bandages and other minor issues. I later be-

came an active Hatzolah member, and eventually, I signed up for Touro’s PA program.” At 37, Dr. Rosenfeld was older than most of his classmates, but his EMT background and exposure to medicine was an advantage. Nowadays, there is a proliferation of PA graduates in the community, but back then, Dr. Rosenfeld was one of very few. Now, Dr. Rosenfeld’s two daughters are practicing PAs in New Jersey. Specializing in pediatrics was a natural choice. As a father of a large family (“seven when I entered the program, and nine, baruch Hashem, by the time I graduated”), Dr. Rosenfeld had a close-up of the cough-virus-ear infection cycle and occasional crisis a large family experiences. “Raising children is not an easy job,” Dr. Rosenfeld jokes, “especially when you have a lot of them.”

Why Go To The Doctor? Dr. Rosenfeld began practicing in 1999, helping little people and their concerned parents. Always a people’s person, he discovered the key to pediatrics, which is active listening. “We were taught in med school that diagnosis is 80% history. I look at my patients’ parents as active partners, allies in obtaining the best care for our children. And when you listen closely, you hear things you sometimes might not detect on examination.” Despite the proliferation of urgent care practices and our conditioning to on-demand, allthe-time care, Dr. Rosenfeld insists that parents must develop a strong relationship with their pediatricians. “I’d come to a Hatzolah call, and ask the parents to call their pediatrician. Too many times, they wouldn’t feel comfortable calling them. Quick fixes can work occasionally, but in the long term, parents should know that the quality of care their children will receive is dependent on how well the doctor knows their kids.” Doctors no longer make house calls with their mythical black leather satchels, and are no longer paged on their beepers. (Remember those? If you do, you may be old enough to qualify for a shingles shot.) Dr. Rosenfeld allows that in extenuating circumstances, seeking care from a provider other than your regular pediatrician is necessary. But too many times, Dr. Rosenfeld has seen children show up at his office with raging strep throat infections, which could’ve been detected days prior. The culprit? A throat swab that tickled the tongue, but didn’t culture the throat. Another young lady showed up at the office with a tonsil abscess, considered a potentially life-threatening emergency. She’d been out of state, “and I have no idea what that doctor did,” Dr. Rosenfeld says, “but he certainly didn’t have a proper look into her throat. The inflamed tonsil was right there — and it was huge.” Continuity of care is important, too. “How

Dr. Rosenfeld’s Tips To Avoid and Manage Respiratory Illnesses: 1. WASH YOUR HANDS. “And dry them,” Dr. Rosenfeld adds. “No need to develop chapped hands.” Handwashing has always been a crucial mainstay of disease prevention, and it remains the best defense to keep germs at bay, especially respiratory illnesses. Regularly clean areas likely to breed bacteria, such as door handles and faucets. 2. KEEP SICK KIDS OUT OF SCHOOL. Schools are akin to giant petri dishes. While most kids are fine with respiratory illnesses, they can transmit these nasty bugs to vulnerable populations, chiefly the very young and the elderly. If one child brings an illness to school, the next thing you know, it’s all over the community. 3. If your child does develop a respiratory illness, KEEP A WATCHFUL EYE ON IT. Some nasty viruses, especially RSV, can go south in a matter of hours. Specifically, be on the lookout for noisy breathing, breathing using accessory muscles (muscles in the neck and chest), belly breathing, and flaring nostrils. “Lack of appetite can be an indicator of breathing difficulty, since the child will be using all of his resources to breathe. Check the respiratory rate — is he breathing way too fast and struggling? A CHILD OR INFANT IN RESPIRATORY DISTRESS IS A MEDICAL EMERGENCY.”

4. BE PROACTIVE. Many practitioners are prescribing bronchodilators, and inhaled and oral steroids to prevent hospitalizations and severe progression of the illness. Regarding steroid usage, Dr. Rosenfeld mentions that many parents are hesitant, but steroids have been around longer than most anti-inflammatory medications out there, and are effective and safe when used appropriately. “And if you land up in the hospital, guess what they’re giving your kid? Steroids. But in monster doses.” 5. BE IN CONTACT WITH YOUR DOCTOR. Your pediatrician, who knows your child and has seen him in the office that morning, is the best one to make the judgment call. Don’t have a pediatrician? Find one that’s local, confident and competent. It’s the ABC of children’s health.

else will your doctor know if your child has had too many viruses in six months? Or enough ear infections to warrant tubes? Of course, everyone wants to know that in the case of dire emergency, your doctor has your back. But maintaining a relationship with your pediatrician extends to more mundane things, too.” Dr. Rosenfeld’s advice is sound. I’ve experienced this myself, with a young infant diagnosed with RSV. Having my doctor on speed dial enabled us avoid the hospital with its nasty germs. On a different occasion, when a different child was hospitalized, my pediatrician was on top of the case, asking and receiving updates every time the medication regimen was recalibrated. A doctor who has intimate knowledge of your child is more likely to shoulder responsibility than a recent college grad whose main goal is to not kill anyone on their twelve-hour shift. On the flip side, Dr. Rosenfeld reaps endless nachas treating the second generation of patients. “There’s such a satisfaction and joy in being able to tell these little kids that I remember when their mommy or tatty was ‘this small.’ It seems like just yesterday that I charted their height and weight, and now it’s their children’s turn.”

The Doctor in the Mountains In addition to Dr. Rosenfeld’s busy Kensington practice, he’s become the address for quality care upstate. Parents who are not year-round patients are treated to the same expertise, compassion and listening ear. “I’ve been practicing upstate for the entire length of my career, and started by making house calls to patients’ bungalows! But as I gained a following, I began seeing patients in my own place.” The initial upstate office boasted a hybrid exam room cum kitchen. “I’d tell patients to keep their feet

out of my toaster,” he says with a laugh. “But eventually we graduated to a full office. Many of my patients followed me to the city after initially seeing me in the Catskills.” The road leading to Dr. Rosenfeld’s office was an unpaved road — literally. “Until I paved it myself, it was a treacherous drive. One guy got towed off the road, but guess what? He’s still a patient, twenty-five years later.” Dr. Rosenfeld’s expertise with stapling and Steristrips have become legendary. “My first boss was married to a plastic surgeon. And boy, did I learn many tricks to the trade during my years that I practiced there! Not every wound can be glued, and not every gash can be stapled. It takes years of practice to know the difference.”

Prevention, the Best Medicine With decades of experience dealing with our community, Dr. Rosenfeld maintains that not much has changed in the realm of pediatrics. “Kids are still contracting the same illnesses, though we may be wiser with our treatment modalities. I am seeing more ADHD and behavioral issues, but it’s difficult to know if there is truly an uptick in cases, or simply more awareness, leading to more diagnoses.” However there is less of a stigma discussing the topic. Our community is tops, Dr. Rosenfeld avers, but if he could change one thing we do, “it would have to be the way we eat, when we eat, and what we eat. Lots of us are killing ourselves with food.” Dr. Rosenfeld specifically fingers our food-centered culture as the culprit, adding that “we can’t even send our kids onto the bus without reams of refined sugar.” Regarding physical

activity, Dr. Rosenfeld is heartened by some improvements he’s seen over the decades, but mentions that “there’s only so many times I can roll down my car window and yell at a boy scooting on his electric scooter sans helmet. Take a jog, ride a bike — they each have definite health benefits over the scooter, and are far safer. And no, a helmet dangling from the handlebar does not confer any cranial protection.” When I ask the good doctor about his competition, Dr. Google, he grins. “Yeah, some parents have their preconceived notions about what the diagnosis should be. But none of them seem to have been reading medical journals, and I tell them so.” But easy access does mean more knowledge, and Dr. Rosenfeld has been seeing younger, more savvy parents willing to try alternative methods of care combined with rigorous follow-up, and he respects that. “My mother was a vitamin guru way before there was a health food store on every corner, so I get it. But balance, as always, is key.”

It’s an Emergency Siyata d’Shmaya can only account for some of the rather eventful cases Dr. Rosenfeld has been involved in. One such case involved a toddler; Dr. Rosenfeld was the third doctor he was visiting. His face was grotesquely swollen, he had a raging fever, and a serious infection around the eye. “While taking his history, I noticed his nose. It looked like he’d fallen and bumped it, but then again, his whole face was in such bad shape, it was hard to tell. But upon exam, I discovered a lithium battery up his tiny nose. I have no idea how it fit in there!” The little boy was lucky. The battery could’ve exploded. Another young man fell off his scooter (“Was he wearing a helmet? No.”) and his CT scan came back normal. But after developing headaches, his father called Dr. Rosenfeld, reporting that over Shabbos he had spoken to other doctors and the ER team five times. The other doctors had dismissed him, and the ER told him not to come back. Dr. Rosenfeld insisted he return, and sure enough, a repeat CT revealed a brain bleed that necessitated surgery, not once, but twice. Of course, Dr. Rosenfeld prefers the humdrum regularity of a standard day at the practice, with its ear infections and strep throats. eena Kress has been volunteering at Hatzolah Air since January 2022. She joined this non-profit emergency medical air transport provider at a pivotal moment, when it was looking to grow from a startup to a fullfledged rescue organization. That’s when Deena came across a post in which Hatzolah Air announced that they were looking for women to do case management to help them expand their operations.

For Deena, the timing was perfect. With her youngest child in high school, she found that she had the leeway to take on some volunteer work. The fact that she already had (and still has) a full-time job as an occupational therapist at a local school was no deterrence for Deena — and not for Hatzolah Air, either. “We like busy people,” they told her when she signed up for their training course. After all, if you want something done, it’s wise to ask a busy person to do it.

ask if anyone is available to take a call with a patient’s doctor,” Deena says. “The responses are incredible. I’ll get messages like, I’m in a Daf Yomi shiur but I can take it if it’s an emergency.” Hatzolah Air’s doctors are the ones who arrange for all of the necessary equipment and personnel that are required for a flight. They have contact with hundreds of specialists, doctors and EMTs who are willing to drop everything to accompany a patient on a transfer flight. In fact, all of Hatzolah Air’s personnel, from dispatchers and case managers to EMTs and doctors, are volunteers. Only the pilots are paid. Amazingly, Hatzolah Air has never had to delay a flight due to a lack of volunteers.

Up in the Air At any given time, each case manager is usually working on one or two immediate missions — those in which the transfer is imminent — plus a couple of missions that are scheduled to take place in the very near future. Unless the mission is a sudden emergency, in which case the entire mission can be completed in a matter of hours, regular missions take a few days and up to two weeks to complete. Deena works at her job as an OT from 8:15 a.m. to 3:30 p.m., but her volunteer work for Hatzolah Air has no official hours. She is on call whenever she is 104

needed, including into the early hours of the morning. “I start my day at 7:00 a.m. by checking everything that I need to do for my open cases. If there’s an active case — meaning, a transfer that is taking place that very day — I review the schedule and all the details with my team. I’m lucky that my workplace is very accommodating; I have several breaks throughout the day, and I use that time to make all the necessary phone calls and to check on how my cases are progressing.” Medical transfers can take place at any time during the day or night, and the case manager must be on call for the duration of the transfer in case any problems crop up. If the case manager is at work or on vacation, other case managers step in to make sure that everything is running smoothly. As a case manager supervisor, Deena is on all the case

This makes it easier for her to take over for a colleague if necessary. “We also have a system in which the up-to-date case files can be accessed by everyone,” she adds, “which makes for a smooth transition during the hours that the original case manager is not available.” Do things ever go wrong during an active case? “All the time!” Deena exclaims. “Almost every flight has a curveball.” Even though Hatzolah Air takes no responsibility for ground transportation, if an ambulance on either side of the flight falls through at the last minute, the case manager scrambles to get another one in place. Then there are details like the weather, which may result in a change of airport. Human error is also a factor, like not double-checking that the hospital has an available bed for the incoming patient. “There are so many details to take into consideration, it’s almost inevitable that I’ll make a mistake somewhere,” Deena admits. With experience, she’s learned to mitigate those mistakes by creating a checklist, but that’s still not foolproof. Knowing that someone’s life is literally dependent on your efforts can cause anyone to buckle under the pressure. Deena learned quickly not to let mistakes get to her. “I tell myself that I’m doing the best I can, and that if it’s meant to be, then Hashem will do the rest. It’s only when the patient arrives at the hospital that I can finally sigh with relief.”

Not Only Medical A typical Hatzolah Air mission is when a patient needs to access higher level care at a hospital across the country. Many times, the cost is absorbed by the patient’s insurance company. In other cases, the insurance company refuses to pay for the flight, or the flight needs to take place before the insurance company gives its approval. In such cases, Hatzolah Air funds the flight itself through donations. Not every transfer is scheduled solely for medical purposes, a fact that Deena discovered during one of her very first cases. “I was in charge of bringing a group of five kids from Chicago to Camp HASC. It was a big job! I had to be in contact with 106

each child’s family and get the necessary paperwork from all of them. It was like arranging five flights in one. But this remains my favorite case to date. I went to meet the plane at Westchester Airport, and it was like greeting long-lost friends. The staff members had decorated the plane, and the group was so fun and energetic. I also learned a lot about the wonderful Chicago community. Since the HASC counselors had to be in camp early, the community sent staff from the kids’ schools to accompany them and then paid for their return flights.” Deena developed a special connection with those campers and their parents, who sent her photos of the kids all summer long.

It Takes a Village The best part about her volunteer work for Hatzolah Air, Deena says, is seeing how people creep out of the woodwork to help. There’s the time a stranger arranged ground transportation. “A family from out of town had trouble getting an ambulance, but they got me the number of a local Yid who could help,” Deena relates. “For some reason, I understood that he was a member of the local Hatzolah. Turns out, he was just a person who wanted to help! He made the arrangements and paid for it himself to the tune of $1,500. I don’t think the family even realized.” A recent case Deena worked on involved a newborn baby who needed lifesaving surgery. It was such a complicated case that the doctors’ phone call took about an hour — an unusually long time. Special equipment was required for the transfer, and a doctor offered to sponsor the purchase of the equipment so that Hatzolah Air could use it in the future. “Baruch Hashem, the surgery was a success,” Deena says. “The family contact was the baby’s grandmother, and she has been updating me on the baby’s progress ever since.”

Mission Accomplished Every case manager will tell you that they develop a deep connection with the patient’s family contact, and by extension, with the patient themselves. It can happen that a patient takes a turn for the worse, which means the transfer is delayed, sometimes indefinitely. “It’s hard when that happens,” Deena acknowledges, not just because she’s rooting for her patient, but also because she may never hear from the family again. “Obviously, the family is dealing with so much, it’s perfectly understandable that they don’t have time to update us. When they do, it’s especially meaningful.” As a case manager, it’s important for Deena to know that her patients are getting the best care. “Many times, issues occur before the flight, usually with the hospital or the ambulance, which is stressful for the family. They also inserted a nasogastric (NG) tube through his tiny nose, to deliver food and medicine to his tummy. In the months following my baby’s premature birth, the words of Asher Yatzar brought tears to my eyes: If one of them were to open… if one of them were to close… it would be impossible to stand before You… I knew that Hashem had decreed that my baby enter this world with a body that wasn’t functioning properly, and only He could heal him. When my baby was two weeks old, a central line was inserted so nutrients would go directly into his veins, circumventing the digestive system. That procedure came with its own risks, because the line was at risk of infection — and it was right next to the baby’s heart. The only thing we could do was daven, and daven some more. hen my baby was a month old, the doctors discovered an intraventricular hemorrhage. A few months later, he was flown by private jet to Cincinnati Children’s Hospital to have the ileostomy reversed and a G-tube inserted into his stomach so his body could absorb nutrition in this way. Even during this tumultuous time, the chesed of Hashem shone brightly. The staff at the hospital was extremely caring and competent, making us feel that we were in good hands. Of course, my baby’s life-threatening condition meant that he couldn’t have a bris for a long time. When he finally did have one, we called him Chaim, a never-ending tefillah to Hashem that He grant our baby the precious gift of life. When we were at the hospital in Cincinnati, the doctors were concerned about my baby’s liver, since his skin was yellow, indicating high levels of bilirubin, and he wasn’t gaining enough weight. I learned a tremendous amount about the amazing organ called the liver. It’s responsible for so many functions, among them growth, as well as healthy hair and nails. In fact, since Chaim’s nails hardly grew, I first cut them when he was eight months old. I also learned about another important job of the liver: to clot blood. The liver produces vitamin K, which is responsible for blood clotting. The importance of this task, which we take for granted, was brought home to me in a way I’ll never forget. The doctors monitored Chaim’s liver, and continued to monitor it even after we were sent home. Bloodwork showed that his liver was malfunctioning. One day, the feeding tube became irritated and began to bleed. Within moments, Chaim’s tiny undershirt was soaked with blood. I put pressure on the wound and called the doctor. He told us to go to the hospital. Chaim was bleeding so much, I had to change his undershirt another three or four times before we left. Even as we waited

for the doctor to come into our cubicle, I had to change it again. After my baby was administered vitamin K, his blood began to clot and the bleeding stopped. It was clear that Chaim needed a new liver. It wasn’t an emergency — yet. We put his name on the list of transplant candidates, since it can take a long time to find a donor. Meanwhile, I was tested and found to be a perfect match. Another wondrous ability of the liver is that a piece can be removed, and the liver will grow back to its original size in the donor’s body. At the same time, the transplanted piece will shrink to the right size in the recipient’s body. The transplant surgery was scheduled for the week after Sukkos. Chaim was thirteen months old. I was thrilled to have the opportunity to give my child life — but at the same time, I was very apprehensive. I had other children waiting for me at home. It would 118

be no simple matter for me to be laid up for weeks after the surgery, recuperating. And I would have to tend to Chaim, who would also be recuperating. On top of that, there were many restrictions in place because of COVID-19, further complicating matters. Still, I knew there was no choice. Sukkos arrived. We were determined to celebrate it properly, with joy. Then, on Chol Hamoed, Chaim’s G-tube kept coming out and he became dehydrated. We rushed to the hospital. His condition was rapidly deteriorating, making a transplant essential. According to his PELD scores, he had reached end-stage liver disease, rendering him the next candidate in the country to receive a liver transplant within a 500-mile radius of his home. As we were sitting in the emergency room, waiting to see a doctor, a liver suddenly became available. And the Master Doctor ordained that the liver be given to Chaim.


AS CHAIM WAS WHEELED TO THE OPERATING ROOM, I KNEW THAT HIS LIFE WAS ONLY IN HASHEM’S HANDS. I FELT SO CLOSE TO HIM, LIKE I COULD TALK TO HIM AND TELL HIM WHAT WAS ON MY MIND

The transplant took place on Hoshana Rabbah, a day of tefillah. It was a heart-wrenching experience, but also an incredibly uplifting one. As Chaim was wheeled to the operating room, I knew that his life was only in Hashem’s hands. I felt so close to Him, like I could talk to Him and tell Him what was on my mind. When I first saw Chaim after the surgery, my heart dropped. He was on a ventilator and looked so pale and still. Even though I knew that patients on ventilators can’t talk, I asked Hashem to let me know that my baby was okay, that he was going to make it. Chaim opened his eyes, mouthed “Mommy,” and closed them again. I had gotten my answer. The night after the surgery, I was at Chaim’s side, bent over my Tehillim. The machine monitoring his heart rate beeped, but I didn’t pay any attention. The machine had beeped so many times that I hardly noticed the noise anymore. But suddenly, the room filled with doctors and nurses. It was only then that I realized how critical my son’s condition was. They bent over my son and tried to bring him back to life as I continued to daven. After three and a half hours, they finally stabilized him. Another tefillah, another miracle. In the weeks following the transplant, the possibility of rejection hovered over us. Rejection of a new organ is common after a transplant, and we kept davening for Chaim’s body to get used to its new organ. We went for routine blood work and got the news before Chanukah: Chaim’s body was rejecting the new liver. We rushed to the hospital and received medication. Baruch Hashem, we were home that night in time to light the menorah — a Chanukah miracle. Today, four years later, Chaim is a bright, sociable child at the top of his class. One day in school, he was playing with toy food, and his teacher asked if he was making a seudah. “I’m making a seudas hoda’ah,” he replied. Every year, on the day of his transplant, we commemorate our miracle with a seudas hoda’ah. Every time I look at Chaim, my heart swells with gratitude. I will never stop thanking Hashem and asking Him to give us the continued ability to stand before Him with shevach and hoda’ah.

Patient satisfaction is directly linked to the waiting experience.

Everyone hates waiting rooms. In a recent study, 97% of patients claimed to be frustrated by wait times. Eighty percent agreed that their frustration would be minimized or eliminated entirely if they were told accurate wait times, and 40% claimed to be willing to see a different physician if it meant shorter wait times. When asked if they were ready to pay to cut down on the wait, 20% enthusiastically agreed.

Some people are itching to get back to work or back home to tend to their families, some are simply anxious to get their appointment over and done with — and most have better things to do than sit in a chair. (Unless you’re in a super busy stage where you don’t get to sit in a chair in peace — ever!) It’s the perfect breeding ground for impatience and stress. Waiting room anxiety can be slashed significantly by setting appropriate expectations. Would you wait more calmly if you knew how long you will be waiting?

DÉCOR DEBATE Studies show that a clean, pleasant space for waiting — the first and possibly longest contact patients make with their healthcare providers — will increase trust and approval in their medical care.

Spending hours in the hospital as outpatients in the renal ward can be grueling, and a hospital in Scotland recognized that. They decided to improve the waiting room décor and then surveyed 44 renal transplant patients to hear how the new ambiance enhanced their experience. The patients were pleased, and this is the order of preference in which they ranked the new features: Comfortable chairs, magazines and puzzle books, paintings on the wall, views from the window, computers and potted plants.

There is growing belief that healthcare providers should sooner invest in hi-tech means to keep waiting times in check, and scale down on the need for large waiting rooms altogether.

Clinics can provide real-time messaging to patients via text alerts or mobile queue tools, allowing patients to join a virtual line, where they can go grab a coffee instead of sitting and waiting. In 2019, Jaber Al Ahmad Hospital in Kuwait opened its doors. Besides its state-of-the-art medical facilities, the hospital has a massive medical mall housing multiple levels of experience. The focus is getting patients to move rather than having them sit while waiting. Weather-permitting, patients also get to enjoy the hospital’s outdoors.

STATE OF THE WAIT At 4 hours, 2 minutes, Maryland has the longest average emergency room wait time. North Dakota has the shortest, at 1 hour, 44 minutes. New York ranks number 5, at 3 hours, 21 minutes. Average wait times at standard healthcare provider offices in the United States is 18 minutes.

WAITING ROOM EPISODES One family physician curated a playlist of background music by asking his patients which songs they’d like to hear. He sometimes checked his appointment schedule and made sure to play specific patients’ favorites on the day of their visit! The result? A happy patient population!

Whenever Mr. G. walked into his family physician’s office, the patients near him groaned. Mr. G. thought the extended wait time was the perfect time for reviewing his sheet music. Ta-dada-DA-DA! Watching the overall blood pressure in the room rise, the secretary kindly showed him to an examination room. Singing aloud became Mr. G.’s trick to bypass the lines…

IS A LONG WAIT TIME A GOOD SIGN? Stuck in the waiting room forever? It may indicate mismanagement or negligence. But it may also indicate a super thorough and compassionate healthcare provider who treats each patient like family, so when you finally get your turn, you get your every question answered with care.

Longest wait in a doctor’s office: 19½ hours Melvin Barnswallow went to see his doctor at 1:30 p.m. on March 10, 1994. He was seen only at 9 a.m. the following morning! Turns out, he’d left his hearing aids at home and missed hearing his name called. The nurses mistook him for the cleaning crew when they noticed him tidying up the space around his seat (he must’ve exhausted all other activity ideas). When the office opened the following morning and Melvin was found sleeping in a chair, he wasn’t even upset, and claimed he’d had his best sleep in months! Better yet, sleeping at the doc seemed to be the best medicine, as his symptoms had disappeared by the time he was examined.

For a Laugh I know a man who sells chairs designed for waiting rooms. He has a high position. He’s chair man of the bored.
