What a sterling year we had! We received a million-dollar grant, hired two new scientists, expanded our lab, purchased new equipment, exceeded our budget, and had some wonderful events, to name a few accomplishments. The Pacific Heart, Lung & Blood Institute (PHLBI) focuses on orphan diseases of the heart, lung and blood that receive little attention and funding. One of the divisions of PHLBI, The Pacific Mesothelioma Center (PMC), is currently developing cancer therapies using mesenchymal stem cells (MSCs) derived from placenta and bone marrow. These cells are modified to express large amounts of interleukin 12, which stimulates the immune system to reject cancer cells. In 2017 we filed a patent for our MSC project, and we are excited to see where this takes us. A generous anonymous million-dollar grant enabled the appointment of two world-class research scientists, Dr. Masahide Tone and Dr. Yukiko Tone. Masahide Tone received his PhD at Tohoku University in Japan. His wife Yukiko Tone received her DSc at Tokyo Metropolitan University in Japan. Both scientists have worked at the universities of Cambridge and Oxford in the UK, and the University of Pennsylvania and Cedars Sinai Medical Center in Los Angeles. They are now collaborating with our Scientific Advisor, Dr. Robert B. Cameron, MD, FACS, who is Professor of Cardiothoracic Surgery, Surgical Oncology at UCLA, Director of the UCLA Mesothelioma Comprehensive Research Program at the David Geffen School of Medicine at UCLA, and Chief of Thoracic Surgery at the West Los Angeles Veterans’ Administration. A project for the clinical trial of IL-4-immuno-toxin (IL-4R) mesothelioma therapy, primarily developed by the Cameron team, has found that mesothelioma expresses IL-4R at high levels, and a recent study also suggests that IL-4R expression levels in mesothelioma are significant to survival rates.

Establishing new therapeutic strategies is our highest priority. We are also proud to continue supporting our International Symposiums on Malignant Pleural Mesothelioma. The symposiums give unrivaled opportunities both to the medically savvy and to the general public, including mesothelioma patients, to not only learn first-hand about groundbreaking discoveries, but also to exchange ideas. The symposiums are held on the last Saturday of the month each year in September. Our hard-working board of directors convened in the spring of 2017 to create a new strategic plan which will guide us in expanding our unique capabilities while ensuring continued financial sustainability and strong governance. A special thanks this year to PHLBI’s fabulous executive director, Clare Cameron. Clare’s dedication and fundraising acumen continues to make PHLBI hugely successful, as we transform PHLBI into a cutting edge-non-profit medical research institution. Our board also welcomed new members: Kellie Sutherland as Board Secretary; and Joseph Garland and Michael Arlen as Directors. Our new members bring diverse professional skill sets to us and extensive experience in executive leadership. I am so encouraged by their generosity, creativity and giving spirit.

Dr. Robert B. Cameron, MD FACS, is one of the leading experts in North America on the treatment of pleural mesothelioma. He has been treating mesothelioma patients for over 20 years and is an early innovator of the lung-sparing pleurectomy with decortication (P/D) surgical procedure for mesothelioma. He has authored or co-authored over 40 papers on oncology, thoracic surgery and other topics. Dr. Cameron attended medical school at UCLA, where he obtained his medical degree. He completed three years of training as a clinical associate in surgical oncology at the National Cancer Institute of the National Institutes of Health in Bethesda, Maryland, a five-year residency in general surgery at UCLA, and a two-year fellowship in cardiothoracic surgery at Cornell-New York Hospital and Memorial Sloan-Kettering Cancer Center in New York City.

RESEARCHER SPOTLIGHT Dr. Masahide Tone has led research groups in molecular biology, immunology, immunotherapy, and cancer immunology at the Universities of Cambridge and Oxford (UK), the University of Pennsylvania, and Cedars Sinai Medical Center. His vast area of research expertise is in molecular and cellular immunology, CRISPR gene editing, gene regulation, antibody engineering, cytokines, macrophages, dendritic cells, T-cells and regulatory T-cells (Treg). In addition to his work as a researcher, Masahide has served as an associate editor for the Journal of Immunology and a review editor for Frontiers in Immunological Tolerance. Throughout his research career he has mentored many postdoctoral fellows, graduate and undergraduate students. His most recent work at Cedars Sinai Medical Center centered around the study of development and function of Treg to develop cancer immunotherapy, and drug resistance induced by differential gene expression in tumor environments. Masahide will bring his immense knowledge, vast skills, and leadership capabilities to the PMC lab as Senior Researcher and Director of Research.

Doctor Yukiko Tone is a molecular biologist whose skill set includes gene cloning, DNA/RNA/protein analysis, antibody engineering, BAC (Bacterial Artificial Chromosome) technology and CRISPR gene editing. After finishing her academic studies, Yukiko moved to Great Britain where she studied molecular mechanisms of the thyroid hormone receptor at the University of Cambridge. She then moved

on to the University of Oxford where she worked on regulation of gene expression of cytokines (IL-12, IL-18 and IL-10) and TNF/TNFR superfamilies (CD40, OX40 and GITR, GITR ligand). Later she worked at the University of Pennsylvania where she studied regulatory T-cell (Treg) mediated immune responses. After moving to Los Angeles, she worked as a project scientist where she continued her work studying Foxp3 gene expression in Treg. She recently focused on gene regulation in cancer cells to overcome drug resistance and to reduce cancer cell survivability in tumor microenvironments. Her diverse research background and enormous dedication to her work make her an asset as a Senior Researcher on the PHLBI team.

RESEARCHER SPOTLIGHT Nurse Practitioner Lien Hua-Feng embodies PHLBI’s commitment to patient care and treatment in her work. Through our partnership with the Department of Veterans Affairs, Nurse Lien meets with veterans and their families to discuss clinical care options and treatment modalities. She is a vital resource to thousands of people, from all walks of life and all parts of the country, answering questions on available treatment and support. Because mesothelioma is rare, there are very few experienced medical staff like Nurse Lien, and patient care resources are limited. PHLBI is working to expand our dedicated staff but we cannot do this without more sources of funding. Hiring experienced and dedicated nurses like Nurse Lien is something we are working towards!

CURRENT RESEARCH Our primary focus at PHLBI is centered on developing immunotherapy strategies using mesenchymal stem cells (MSC) against cancers, particularly the aggressive form of mesothelioma. To date, we have amassed significant baseline data that will power our current efforts. Even though the data is not published yet, we are actually well positioned for a successful 2018 based on our new research team. Our new team has assessed our accomplishments and performance in 2017, and have taken steps to better position our institute. A strength in our institute is that the clinical (headed by Dr. Robert B., Cameron) and research teams are directly connected, and this allows us for faster innovation and patient participation. This is the best environment to develop cancer immunotherapy strategies. Our access to patient specimens is an additional advantage. We add on average two tumor samples a week for our research, thereby increasing the robustness of our tissue bank. Our abundant access to mesothelioma samples (40 in our tissue bank), even though mesothelioma is a rare aggressive cancer, positions us favorably for research breakthroughs. Our new research team started at the end of August 2017. They have since reorganized the research operation by upgrading the laboratory and acquiring new and necessary equipment in order to perform world class molecular immunology research. A highlight of their efforts is their recent acquisition of FACS and qPCR platforms. They have already discovered several new findings and are working on publishing these. The new team is actively pursuing three projects (the “Aims” described on the next page), centered on the development of cancer immunotherapy using mesenchymal stem cells.

AIM 1: Developing cancer immunotherapy strategies using tumor-homing mesenchymal stem cells (MSC). The goal of Aim1 is generating the anti-tumor cytokine expressing MSC for cancer immunotherapy (Fig. 1B). We will generate several potential cytokine expressing MSCs (potential cytokines: IL-2, IFN-Îł, IL-12, and IL-15) and, in three years, we will examine and select which cytokine expressing MSC can effectively activate anti-tumor immune cells. These data will be published to contribute to developing all MSC-mediated immunotherapy strategies. In the fourth year, we will work to develop clinical trial strategies based on the findings. To this end, we must consider safety issues of the modified MSC, compared to other stem cell and immunotherapy developments. AIM 2: Investigating MSC properties to develop cancer immunotherapy strategies. The goal of Aim 2 is generating the best MSCs for cancer immunotherapy. However, these stem cells also express some immune-suppressive molecules that might inhibit cancer therapy. We will therefore analyze gene expression in MSCs, and based on the data, we will modify the MSC that is suitable for cancer therapy. The obtained data is critical for developing immunotherapy strategies. We believe that we can select and/or generate the best MSCs for the Immunotherapy strategy in the next three years. These data are important not only for developing our strategies but also for developing all other MSC-mediated immunotherapy strategies. Therefore, we will publish the data for developing all MSC-mediated immunotherapy strategies. In future years, we will generate, we will generate anti-tumor cytokine expressing MSC (as described in Aim 1) using the best MSCs generated using the strategy proposed in this Aim. The cytokine expressing MSCs generated using plans described in Aim 1 and Aim 2 will be used for clinical trial. AIM 3: Characterizing mesothelioma to develop diagnosis and targeting immunotherapy strategies. Mesothelioma is an aggressive cancer with few treatment options and low survival rate. Establishing therapeutic strategies against this cancer is urgently required, and therefore, we selected this cancer as the first target of our developing immunotherapy (described in Aim 1). In Aim 3, we also plan experiments to identify bio-markers on mesothelioma cells. By identifying bio-markers, we can also develop new mesothelioma immunotherapy strategies. Previously, the Cameron team identified IL-4R as expressed on mesothelioma cells at high levels and developing the IL-4-immuno-toxin mesothelioma therapy strategy. To reach the clinical trial stage, we will further analyze IL-4R expression using mesothelioma samples from our international tissue bank. Normally, our immune systems have the ability to remove abnormal cells,

including cancer cells. However, some cancer cells cunningly escape from the immune-system by producing immuno-suppressive regulatory proteins (e.g. PD-L1, CTLA4, TGF-β, IL-10, and HLA-G). Indeed, many immune cells are disabled around the tumor sites (Fig. 1A). To solve this problem, we will reactivate immune cells using anti-tumor regulatory proteins (e.g. anti-tumor cytokines) delivered by MSC (Aim 1). Since MSCs gather to the tumor sites with their tumor-homing properties, we can deliver anti-tumor cytokines to the cancer site by generating the cytokine expressing MSC (Fig. 1B).

PHLBI provides undergraduate student interns from nearby colleges, UCLA and Loyola Marymount University, early career training and mentorship in medical research. Our interns are given hands-on educational opportunities that are difficult to obtain at their host universities as undergraduate students. University research labs are often constrained by limited budgets, where resources are typically prioritized for post-doctoral fellows and graduate students pursuing advanced degrees. At PHLBI, our interns have the unique opportunity to gain real-world experience and fill meaningful roles in medical research at a young age. In return, PHLBI’s core mission of conducting cancer immunotherapy research benefits from their involvement.

A 10 YEAR SURVIVOR OF MESOTHELIOMA

In late 2007 David had really bad pains in his chest. Saint Jude Medical Center in Fullerton thought he was having a heart attack, but after a stress test, EKG and more, they said “No, you’re fine.” A couple of days later, at his son’s school band concert, the pain was so bad he could not breathe. After various doctor visits without a diagnosis, he finally had a CT scan and that’s when his GP said, “Have you been around asbestos?” “No,” David replied, “and I had no idea what he was implying, none whatsoever.” In the new year they did a biopsy and referred him to City of Hope hospital. There, Dr. Kemp Kernstine performed another biopsy and the diagnosis of pleural malignant mesothelioma (“meso”) was confirmed. It had taken about 3 months to get an answer. David’s treatment started with debulking, which is surgical removal of as much of the tumor as possible, performed by Dr. Kernstine. Today, ten years later, David says the greatest challenge at that time was not the meso, but that he had to sort through and understand his treatment choices. It was a help when Dr. Kernstine referred him to a consultation with Dr. Robert B. Cameron at UCLA, one of the leading experts in North America on the treatment of pleural mesothelioma.

He then did eight cycles of chemo. “I felt pretty rough but I never ever thought that it was going to beat me. I wanted to see my boy through high school and graduate from high school and I set that as a goal.” After four or five years his

frequency of scanning was dropped to just a yearly CT scan. “Fortunately, it worked. I got 10 years out of it!” David felt fine. He took up cycling, including participating in several of the Lance Armstrong cycling events for raising cancer awareness. “There was no rhyme or reason why the meso stopped. I put this down to the treatment and also, maybe, to my attitude of being positive and active.” He also never paused in his profession as a machinist in manufacturing aerospace defense, even during chemo. Some days he’d go home early or go in late, “but I went to work every day. We are a small company so I don’t sit behind a desk. I’m out in the shop with sleeves rolled up, whatever it takes.” To this day David has never figured out exactly where and when he was exposed to asbestos. “Back in the early 70s when I was a kid in England, it was everywhere. When I was 5 or 6 years old they were knocking down some old prefabs and the roofing was corrugated sheets of asbestos. We kids were playing around on the demolition site and we were putting these sheets of asbestos over a little fire. Then we rode our bikes and did the Evel Knievel thing over the fire. Things were breaking and going up in the air. That is probably the worst thing you could ever do! But who knows? Who really knows?” And then at Christmas last year, he felt something in his back and shoulder. Upon having his annual scan at Saint Jude’s in March, the CT scan showed some new growth. A PET scan and biopsy confirmed that the cancer was back in the same area in his left lung as ten years ago. David thought he’d have debulking again, but this time was told that it was inoperable. “I was pretty much devastated.” But then Dr. Cameron scheduled David for a new procedure at UCLA, called cryoablation. The radiologist, Dr. Fereidoun Abtin, inserted a hollow needle while David was in a CT scan, so they could see the cancer. Cryoablation freezes and kills mesothelioma cells. “You can see the cancer cells and they change color,” reported David. Dr. Cameron also offered the opinion that it wasn’t going to be effective for David to have any more chemo sessions, because he’d already had it 10 years ago. Instead, as a patient of Dr. Cameron, David will undergo immunotherapy, which will trigger his white blood cells to attack the cancer cells. Next steps will depend on the efficacy of that and the cryoablation. As David puts it, “It’s a mindset. If you’re a positive person you put positive people in your corner and have positive thoughts. Yes, you can have bad days, and it’s 10 years later and I am fighting the same battle. It’s very easy to lay on a couch and feel very sorry for yourself, but then the couch is winning. I get up every morning and thank the big man for another day and I get on with it.” Overall, David’s advice is to “go to work, stay positive, and lead a normal life.”

Cindi Hausheer Cindi Hausheer has myasthenia gravis, myositis and Hashimoto’s disease. “Just a few autoimmune diseases,” she says, almost casually. She also has Type 2 diabetes and is a lung cancer survivor. Cindi retired from teaching in 2013. She not only taught every grade from kindergarten to eighth, she was very involved with math workshops that promoted “girls understanding and enjoying math.” She has had a lot of other interests too, such as glass fusion, making bowls, menorahs, and jewelry. “Visualize a piece of glass, then putting other pieces of glass on top and melting it in a kiln.” She also had a business making and drying pasta, and another making small heat compresses. It was purely “by luck” that she started seeing Dr. Robert Cameron in 2005. At the time she could hardly swallow, and she’d already seen many doctors. She had also suffered through pneumonia. When her son underwent some chest surgery that year, she asked her son’s doctor to recommend someone for her issues. That was Dr. Cameron. That year he performed a thymectomy (surgical removal of the thymus gland), giving her immediate relief for the pain in swallowing. The thymus was found to be cancerous, so she had 6 weeks of radiation, a fatiguing time accompanied by weight loss. In 2010 she was diagnosed with myasthenia gravis. “You can feel very weak and it’s hard to walk. I also have weakness with my hands.”

Her lung cancer was found by Dr. Cameron while he was looking at her lungs with a CT scan (computed tomography, a combination of X-rays and a computer). Every six months since, overseen by Dr. Cameron, she continues to have CT

scans. These days she is at a steady state with her conditions, aided by medications, but she still deals with fatigue and weakness. She “pushes through.” Fortunately, her myositis is in remission. And she hasn’t found Type 2 diabetes to be a burden. She takes metformin (a glucose lowering drug) and watches what she eats. With all that, she has her strategies to keep busy. Her two sons and a daughter live and work locally, so are able to visit and lend support to her and her husband, a soccer referee and coach, who is currently recovering from some surgeries. She enjoys yoga and is taking a philosophy class. How else does she cope? “My friends, I can talk to them. Going to classes and just keeping my mind on other things. It’s too easy to stay isolated in your house.”

CAREGIVER SUPPORT GROUP This marks the second year since PMC began offering monthly caregiver support group meetings. The support group led by PMC Nurse Practitioner Lien Hua-Feng and coordinated by Christina McSherry was created to give caregivers of mesothelioma patients an outlet to ask questions, share stories, and release difficult emotions. The group helps caregivers connect with other caregivers who can directly relate to their experiences.

Cindi gives high praise to everybody at PHLBI, as always friendly and helpful, and has nothing but superlatives for Dr. Cameron. “He is an incredible and caring doctor and person. I love him for saving my life twice.”

“My father was diagnosed with mesothelioma in 2016 and was given an estimated year to year and a half to live. This doctor said that the disease was rare and there wasn’t much he could offer except palliative measures. I refused to give in to this and had started my own research when I came across Dr. Cameron at UCLA who specializes in mesothelioma. While working to get my father seen by this specialist my mother and I were offered the opportunity to participate in a caregiver support group organized by the PHLBI, which was extremely helpful and often times a source of hope for my family. Sharing experiences and tips on how to care for family members helped us to feel less helpless, knowing that whatever my dad was going through was experienced before by someone else and that there were ways to help him was reassuring. The PHLBI also organizes different events, such as a 5k walk/run/hike to fundraise for mesothelioma research which my family has attended the past 2 years. Getting to see the survivors, fighters and supporters work towards helping find other forms of treatment is truly inspiring. The 5th annual “The Greatest Escape” Motorcycle Ride presented by asbestos law firm Worthington & Caron P.C. was held on Sunday, June 4th. The ride originated as a tribute to “The Great Escape” movie, starring actor Steve McQueen, who died of mesothelioma. Since its inception, hundreds have gathered each year to honor those who have lost the fight against mesothelioma and to raise money for research. In 2017 we raised $78,640 and 100% of proceeds from the ride will be applied directly to the Punch Worthington Laboratory at UCLA and the Pacific Mesothelioma Center laboratory on Santa Monica Boulevard, to further our important research on mesothelioma. More than 200 riders, passengers, and attendees enjoyed the scenic ride that kicked off from Top Rocker Harley-Davidson in Canoga Park and also, Antelope Valley Harley-Davidson, and traveled the breathtaking path through the Angeles

The 6th annual 5K Walk/Hike for Mesothelioma took place on October 22nd, 2017, at the Paramount Ranch in Agoura Hills. With over 350 walkers, the event raised over $160,000 to find better treatments for malignant pleural mesothelioma. In the words of Master of Ceremonies, CHiPS star Larry Wilcox, “Asbestos is everywhere, and once you inhale it it’s a major problem. It’s important that people get educated about it, and the walk is a great way to increase awareness and funding.” Presented by Worthington & Caron, P.C, the event brought together thirty-two teams, as well as supporting organizations and individual walkers, who gathered at the park at 9 a.m. Each registrant received a complimentary T-shirt, fruit products generously donated by Dole, and sunscreen, before leaving on the hike. Afterwards, participants enjoyed a catered lunch by local Agoura Hills favorite Italia Deli. The event also offered David’s Tea, Duverger Macaroons, Monster energy drinks, Mustache Mike’s Italian Ice and a concert by The Six Billion Dollar Band. The live auction featured items such as a photo safari for two in South Africa and a Maui luxury package. 7TH INTERNATIONAL SYMPOSIUM ON MALIGNANT PLEURAL MESOTHELIOMA The 7th International Symposium on Malignant Pleural Mesothelioma was held on Saturday, September 30, at the UCLA Meyer & Renee Luskin Conference Center. The focal point of the symposium was on clinical therapies and research data to prevent, detect and treat malignant pleural mesothelioma. Health care professionals including physicians, nurses, and scientists joined students, mesothelioma patients and their families to attend this informative and educational conference. The symposium was co-hosted by UCLA and the Pacific Mesothelioma Center (PMC). The morning session was opened by symposium chair, Dr. Robert B. Cameron, MD, FACS, who presented The Future of Mesothelioma Surgery. “Although surgery in mesothelioma has decreased due to a perceived lack of efficacy among oncologists and other physicians, there are therapies, such as preoperative radia-

tion, cryoablation and Betadine lavage, which are keeping surgical results ahead of other therapies.” said Dr. Cameron, Professor of Clinical Cardiothoracic Surgery and Surgical Oncology. The highlights of his presentation centered on how these therapies supplemented standard of care treatments through preoperative, intraoperative, and postoperative therapeutic modalities. The symposium’s other distinguished faculty included: Anna Nowak, PhD of the University of Western Australia; Luana Calabrò, MD, University Hospital of Sienna, Italy; David S. On Wednesday, January 25th 2017 Global Images USA hosted an art gallery viewing and cocktail evening showcasing iconic works from world-renowned photographers and artists at a private residence in Hollywood. Guests had the opportunity to enjoy beautiful art, great company, and award-winning locally brewed beer, cocktails, delicious food, and entertainment. A portion of proceeds from the sale of these art pieces benefited The Pacific Mesothelioma Center. This event was geared towards art collectors, photography fans, and aficionados of Hollywood history. The prints and original art works for sale covered a broad spectrum of themes, offering something of interest for everyone. The eclectic assortment included high fashion prints, intimate portraits, behind the scenes old Hollywood elite, aerial photography, and conceptual oil and acrylic paintings. Guests had the opportunity to purchase these works through a silent auction with a minimum bid and a buy-out option. The lists of artists whose works were for sale included Helmut Newton, Frank Worth, David LaChapelle, Timothy White, Daniel Furon, Arney Freytag, Stephan Wayda, Ann McFerran, and many more. Additionally, some of the artists were present at the event, offering perspective and background on their work and personally signing their work.

PHLBI HOLIDAY OPEN HOUSE PHLBI hosted its annual holiday open house on December 6th, 2017. The annual open house showcased the innovative research conducted at PMC at their in-house research laboratory in Los Angeles. This event was held at one of the two laboratories that PHLBI operates. The other laboratory is the David Punch Worthington Laboratory, located at the David Geffen School of Medicine at the UCLA Medical Center.

Attendees met the PMC researchers, learned about the exciting research taking place in the lab, ate delicious food from French- Moroccan eatery Chez Marie, and watched student interns perform live demonstrations. Individuals new to PHLBI are encouraged to attend next year’s open house so they can learn more about the institute and its exciting research, and learn more about events held throughout the year to raise awareness and funds for research.

VOLUNTEER SPOTLIGHT One thing Robin Olin has always strived to do is to help others in whatever way she can. After moving to the US from Montreal, Quebec she has worked as an advocate in a number of nonprofit settings including animal welfare, homelessness, and human empowerment. Her current position as a cold cap therapist involves working with cancer patients to preserve their hair while undergoing chemotherapy, something she finds truly rewarding after her own battle with breast cancer. Robin started volunteering with PHLBI in 2014. Robin’s roles have included serving on committees for PHLBI events, assisting with multiple events, and going door to door fundraising for the organization’s 5K Walk for Mesothelioma. Robin’s dedication, tenacity, and willingness to help PHLBI further showcases how our greatest assets are our volunteers.
