COVID-19 RESPONSE MATRIX IF YOU HAVE AN ONSET OF COVID-19 SYMPTOMS SYMPTOMS MIGHT INCLUDE: • Fever of 100.4 F or more • Shortness of breath or difficulty breathing • New onset of cough not attributable to or different from chronic seasonal allergies or asthmatic cough • Loss of taste or smell • Diarrhea, nausea, vomiting, chills, muscle aches, fatigue, or congestion not attributable to known causes

SMES COVID-19 RESPONSE TEAM WILL CONTACT YOU AND A DECISION MAY INCLUDE: • Consult your healthcare provider AND/ OR • Consider seeking a COVID-19 PCR test • Stay home for 10 days AND - Remain fever-free for 24 hours (48 hours in non-masked rooms) (without the use of fever-reducing medication) AND - Remain home until symptoms are improving AND • Inform close contacts that you have symptoms of COVID-19, OR • Return to school after 24 hours depending on symptoms and issue

REMAIN HOME • For 10 days, AND consult your healthcare provider - Until fever-free for 24 hours (without the use of fever-reducing medication) AND - Until symptoms are improving AND • Inform close contacts to quarantine for 14 days

QUARANTINE AT HOME For 14 days from the time you were exposed*

AND • Consult your healthcare provider AND • Monitor your health AND • Consider taking a PCR test (if symptomatic) AND - If POSITIVE, stay home for 10 days until fever-free for 24 hours (without the use of fever-reducing medication) AND until symptoms are improving - If NEGATIVE, continue to quarantine for 14 days AND • Inform close contacts if you develop symptoms or have a positive COVID-19 test

AND

A DECISION MAY INCLUDE: • Stay home for 10 days AND - Remain fever-free for 24 hours (without the use of fever-reducing medication) AND - Remain home until symptoms are improving OR • Return to school after 24 hours depending on symptoms and issue

OR Seek medical clearance that states your symptoms are due to a preexisting condition or an alternate medical diagnosis and provide a signed letter to the SMES COVID-19 Response Team for clearance before returning to school.

*Exposure is defined as: Within 6 feet for 15 minutes or more with someone who has COVID-19; Live in the same household as someone with COVID-19; Care for someone with COVID-19; Touched, kissed, shared utensils with someone who has COVID-19; Coughed or sneezed on by someone who has COVID-19.
