Many older adults, especially those dealing with chronic pain or conditions like sleep apnea, restless leg syndrome, or side effects from medications, struggle to achieve the deep, restorative sleep their bodies need.

any families have a special winter destination that becomes part of their story – a place where babies become toddlers, where toddlers wobble on skis for the first time, and where teenagers suddenly look like adults as they carve confidently down the mountain. For us, that place has always been South Lake Tahoe’s South Shore. For well over a decade, our family has packed up the car each winter – snow gear, hot cocoa mix, and far too many mismatched gloves – and trekked to a destination that feels both refreshingly new and beautifully familiar each year. And now, with the City of South Lake Tahoe officially reinstating Vacation Home Rental (VHR) permits after several years of restrictions, our annual trip feels even more special.

Vacation rentals are open again, and for families like ours, that means the return of space, comfort, and a true home base for snowy adventures.

If you notice one or more of the warning signs, see a board-certified dermatologist right away, preferably one who specializes in diseases of the skin.

There are three main types of skin cancer: basal cell carcinoma, squamous cell carcinoma, and melanoma. Because each has many different appearances, it is important to know the early warning signs.

ACTINIC KERATOSIS

An actinic keratosis (AK) is a crusty, scaly growth caused by damage from exposure to ultraviolet (UV) radiation. An AK is considered precancerous because, if left alone, it could develop into skin cancer. AKs are the product of a lifetime accumulation of UV damage and typically appear on areas most commonly exposed to the sun, such as the backs of the hands, forearms, face, neck, and scalp.

SQUAMOUS CELL CARCINOMA

BASAL CELL CARCINOMA

Basal cell carcinoma (BCC) is the most frequently occurring form of all cancers, with more than 4 million cases diagnosed in the U.S. each year. BCCs may look like open sores, red patches, pink growths, shiny bumps, or scars. Any skin lesion that doesn’t heal or bleeds with even gentle trauma, such as drying with a towel, is concerning for a BCC.

MELANOMA

Squamous cell carcinoma (SCC) is an uncontrolled growth of abnormal cells arising in the epidermis, the skin’s outermost layer. SCCs often look like scaly red patches, crusty bumps, warts, or elevated growths with a central depression.

SCCs may appear suddenly, and some forms are tender to the touch. They can become disfiguring and sometimes deadly if allowed to grow. More than 1 million cases of SCC are diagnosed each year in the U.S., which translates to about 115 cases diagnosed every hour. Incidence has increased by 200 percent in the past three decades in the U.S., and more than 15,000 Americans die each year from this type of skin cancer.

Melanoma is a very dangerous form of skin cancer. It is caused mainly by intense, occasional UV exposure (frequently leading to sunburn), especially in those who are genetically predisposed to the disease. An estimated 178,560 cases of melanoma are diagnosed in the U.S. each year, and it is fatal for an estimated 9,320 of these patients. Fortunately, if melanoma is recognized and treated early, it is almost always curable.

WHAT PATIENTS NEED TO KNOW 1. Everyone needs to perform a selfskin exam once a month.

2. Melanoma can appear anywhere on the skin, including the palms, soles, under your nails, and places most have never seen the light of day (genitalia). For this reason, it is important to look everywhere.

Lock the bathroom door, get out a hand mirror, and check—it could save your life.

3. What to look for: ABCDEs and the Ugly Duckling

A - Asymmetry: If you draw a line down the middle, do the sides match?

B - Border: Is there a nice, crisp border, or does the mole melt into the surrounding skin?

C - Color: Moles should be one color, one shade of brown. Is there a variety of shades or additional colors, such as black, red, white, or blue?

D - Diameter: Any mole larger than 6 mm, i.e., the head of a pencil eraser, has a higher risk of becoming melanoma.

E - Evolution: Has it changed?

F - The Ugly Duckling Sign: Most people have a “signature mole,” their own personal mole appearance and architecture. Is there a mole that seems different from all the others?

4. Everyone needs a full-body skin exam by a board-certified dermatologist once a year, more frequently if you have a family history of skin cancer, a personal history of skin cancer, or live in an area with substantial sunshine.

ubstance abuse is often associated with younger adults, but older residents in South County face their own quiet battles. Alcohol, prescription medications, and even illicit substances are increasingly affecting seniors, often triggered by loneliness, chronic pain, or life transitions.

Recognizing the signs can be tricky, as they are often mistaken for normal aging. Look for changes in mood or behavior, such as irritability, social withdrawal, or sudden forgetfulness. Physical signs like unexplained weight loss, sleep disturbances, or frequent falls may also signal a problem. Prescription medications—especially opioids, benzodiazepines, and certain sleep aids—can be misused, intentionally or accidentally, sometimes combined dangerously with alcohol.

Substance abuse in older adults is not a moral failing—it’s a health issue that deserves attention, understanding, and support. By noticing the signs, offering compassion, and connecting loved ones to resources, South County residents can help older neighbors live safer, healthier, and more fulfilling lives.

Ralph Waldo Emerson famously said “who you are speaks so loudly I cannot hear what you’re saying.” Keeping this quote in mind has served me well over the years, particularly in choosing friends … and when seeking out the services of professionals. In my opinion, integrity isn’t a word heard often enough in the business world. Competitive, certainly.

ut unfortunately phrases like “bottom-line oriented,” “unscrupulous,” even “cutthroat” have cropped up in the vernacular when hearing home improvement gone wrong stories from fellow homeowners. That’s why it was so refreshing to come across Jeff McCabe and his Orange County-based One Way Painting.

In business since 1976, Jeff and his team serve all of the OC and specialize in interior and exterior residential and commercial painting. They swear by the rules of professionalism: “no cutting corners. No cheap materials.

The Medicare Prescription Payment Plan

In August of 2022 President Biden signed the Inflation Reduction Act of 2022 into law.

T

he new drug law makes improvements to Medicare by lowering drug costs and improving the sustainability of the Medicare program for generations to come.

For the first time, beginning in 2025, the new drug law requires that all Medicare prescription drug plans, referred to as Medicare Part D plans, both stand-alone drug plans and Medicare Advantage plans with prescription drug coverage to offer enrollees the option to pay out-of-pocket prescription drug costs in capped monthly installment payments instead of all at once at the pharmacy. This will be very helpful for people who have a Part D deductible that needs to be paid first before drug copays begin (in 2026 it can be as high as $615). Centers for Medicare/

Medicaid Services (CMS) has named this program the “Medicare Prescription Payment Plan”.

Now in its second year, Part D Enrollees may opt into the program prior to the beginning of the 2026 plan year, or in any month during a plan year. They do this by notifying the Part D sponsor (insurance company) and they then complete the Medicare Prescription Payment Plan election process. This process will be available by mail, tollfree numbers, or website application. All of these options must be provided by the Part D insurance carrier and must be processed within 24 hours when an enrollee is already enrolled in the Part D plan.

Part D enrollees who opt into the program will pay $0 at the pharmacy for their covered drug instead of the co-pay they would normally pay when filling the prescription. The Part D insurance company must then bill program participants in monthly installments for the co-pays or cost sharing incurred. The pharmacies will be paid directly by the Part D insurance companies.

Spamalot, which first galloped onto Broadway in 2005, features a book & lyrics by Eric Idle and music by John Du Prez and Eric Idle. The original Broadway production was nominated for fourteen Tony Awards and won three, including Best Musical. The musical comedy lovingly ripped off from the film classic Monty Python and the Holy Grail has everything that makes a great knight at the theatre, from flying cows to killer rabbits, British royalty to French taunters, dancing girls, rubbery shrubbery, and of course, the Lady of the Lake.
