As Open Enrollment nears its end on December 7, it’s important to remember that the healthcare decisions you make today will have a lasting impact on your well-being throughout the next year. • Elimination of the Coverage Gap (Donut Hole): In 2025, the “donut hole” will be eliminated, moving to a three-phase structure: deductible, initial coverage, and catastrophic coverage. Some plans may still limit certain drugs to generics.
• Drug Price Negotiations: Medicare will begin negotiating prices for 15 specific Part D drugs, starting in 2025. This aims to lower costs for beneficiaries and improve access to essential medications.
• Drug Deductibles: Some plans may introduce new drug deductibles of up to $590 in 2025. Be sure to check your plan's details to avoid unexpected costs.
• Changes in Medications Formulary and Tier Levels: High-tier medications may have higher costs and deductibles. Ensure your medications are on your plan’s formulary; off-formulary drugs won’t count toward the $2,000 out-of-pocket cap and could require full retail payment.
• Reduction in Plan Benefits: Many plans are reducing their benefits. It is crucial to review the 2025 Summary of Benefits for any changes that could impact your healthcare coverage.
• Discontinuation of Certain Plans: Some plans, like UHC’s $42 PPO and Alignment 007 PPO, will no longer be available in 2025. Affected beneficiaries will be automatically transitioned to traditional Medicare. I’m here to help you navigate this change and explore new coverage options.

Boost Your Brain Health as You Age

Have you ever been told that your “brain will become less sharp” as you age? Fortunately, this is a myth. In fact, cognitive decline is not inevitable as people age and more importantly, there are things you can do to help your brain as you age.
Research shows that there are steps you can take to reduce your risk of cognitive decline and maintain mental sharpness. Incorporating small, positive changes into your daily routine can help support brain health now and, in the future, promoting long-term cognitive health.
Cognitive health refers to the ability to think, learn, and remember clearly, which is essential for carrying out everyday tasks like driving, managing finances, taking medication, and cooking. It’s just one component of overall brain health, influenced by a variety of factors. While genetics plays a role, environmental and lifestyle factors are key contributors to changes in thinking skills and the ability to perform daily activities. Although you can’t control your genetic makeup, you can manage many other factors that affect your brain health.
The National Institute on Aging defines brain health as how well a person’s brain functions across several areas, including:
• Cognitive health – how well you are able to think, learn, and remember
• Motor function – how well you make and control movements, including balance
• Emotional function – how well you interpret and respond to emotions (both pleasant and unpleasant)
• Tactile function – how well you interpret and respond to sensations of touch, including pressure, pain, and temperature
• Sensory function – how well you see, hear, taste, and detect odors
Age-related brain health can be enhanced by focusing on these six key areas that support cognitive function and overall mental well-being:
1. Movement is key – those who exercise have a lower risk of developing cognitive decline. It also promotes good cardiovascular health, and it improves blood flow to the brain, reduces inflammation, and lowers stress
2. Eat right to fuel the brain – limit red meat and foods high in saturated fats (such as butter, fatty cuts of meat, palm and coconut oils, cookies and doughnuts, and processed meats like sausage and bacon) – these are foods that can also disrupt your sleep. Choose lean meats like fish or skinless chicken, use olive or avocado oils over fatty cooking oils, and try to replace some of the high-sugar sweets with dark chocolate, berries and whipped cream, and frozen fruit bars (you can even have peanut M&M’s from time to time to get some good fat and fiber). Coffee and black and green teas have health benefits, as does red wine in moderation (moderation is defined by the Mayo Clinic as 5 oz. per day)
3. Get some sleep and relaxation – sleeping helps our bodies recover from illness, injury, and stress. During sleep, toxins flush from our brains. Sleep helps us
preserve and consolidate our memory and can also improve learning. Remember to stick to a schedule for rising and going to bed every day – prioritize your sleep! Avoid long daytime naps. See #1 above (get moving!), and avoid food and drink that can disrupt your sleep (see #2 above)
4. Strong social connections -- Enjoying close ties to friends and family, as well as meaningful social activities have brain health benefits. Focus on relationships (relatives, friends, and neighbors) and activities you enjoy the most, and stay connected with people of different ages. Find activities that allow you to learn or pass on skills, such as cooking, tutoring, or volunteering. Love on your pets if you have them, as they are a wonderful source for a loving friendship!
5. Engage your brain -- cognitive reserve develops over time through education, learning, and curiosity. Education and learning can reduce cognitive decline (see #4 above). Engaging your brain in challenging ways improves overall brain health. Consider learning a new language, exploring the arts, volunteering, playing games, etc.
6. Managing stress – stress, unfortunately, is a normal part of life, but how you react to stress is what can impact your brain. Chronic stress can damage areas of the brain that control memory and regulate your mood. Remember to focus on the present and practice positivity and gratitude, give your brain a break and try breathing exercises to relax, keep exercising, and if a problem arises, then focus on the problem at hand, not how it could get worse (no “doomsdaying”).
While the first steps can be daunting, these changes are not insurmountable. In fact, making small, simple changes and building up slowly will make an important difference for your brain health. Set a goal for what you want to achieve in 21 days, as that is how many days it takes to break a bad habit (“brain drains”) and create new ones that are good for your brain health (“brain gains”).
Don’t let the myths of aging get in your way. You can make a choice to take action that will maintain a healthy brain.

Medicare Part B Costs Increase in 2025
The majority of Medicare beneficiaries don’t’ pay a premium each month for Part A, but they do for Part B. Their Part B premium will be automatically deducted from their social security check and if they aren’t taking social security yet then they get a bill from the Medicare (aka the government). The standard premium for Medicare Part B (outpatient and doctor services) in 2024 was $174.70. In 2025 it will increase to $185.00, and increase of $10.30
The Medicare Part B deductible also increased; from $240 in 2024 to $257 in 2025.
If your modified adjusted gross income (MAGI) is above a certain amount, you will pay an Income Related Adjustment Amount (IRMAA). Medicare uses the modified adjusted gross income reported on your IRS tax return from 2 years ago. The IRS provides this information to Social Security. Notices for your IRMAA amounts have begun to arrive in the mail for 2025, so keep an eye out for that to avoid any surprises in the new year.
There are 5 higher income brackets that cause a person to pay more for their Medicare Part B and an added assessment to their Part D (drug) coverage. In 2025 IRMAA will be assessed when an individual’s MAGI is above $106,000 and a married couple’s MAGI is above $212,000.

In-Home and Walk-Up Tech Support Elevates Connection at The Covington
The Covington, a highly soughtafter, upscale Senior Living community in Aliso Viejo, holds imagination and innovation among its core values. The dedicated team there, continuously seeks ways to expand access to services and support for its 200-plus residents.
Cash Benton, Executive Director at The Covington, recognized a growing need. “We had an overwhelming demand for tech support,” he says. “Residents have various devices - handheld or laptop, desktop or iPads, but there wasn’t a consistent outside service available to assist them.” If residents encountered challenges with their computers or other devices, they had to either arrange tech support on their own or request internal help, which was limited in availability. That’s when a partnership with The Smarter Service became an ideal solution.
The Smarter Service (thesmarterservice.com) provides residents of The Covington with in-home tech support and a convenient walk-up help desk for all their technology needs. “It’s a pilot program that started in August,” Cash says. “We wanted to see what the demand would be like and
how residents would welcome this new service, and we could not have been happier with the outcome and the service.”
The Smarter Service supports residents at every skill and confidence level. “Many of the residents here are extremely tech-savvy,” Cash says. “but, even with their vast array of knowledge, a little extra support goes a long way.”
Residents can schedule in-home visits for personalized help with their devices. Additionally, a Techbar technician is available every Saturday morning in The Covington lobby for walk-up assistance, allowing residents to drop by with tech questions and receive immediate support.
In-home sessions cover a wide range of tech needs – from syncing and setting up devices to file management, app downloads, password resets and more. “Without in-home help, many would either work around the problem, or go without” Cash explains.
The Covington’s goal when introducing the service was to increase resident awareness of the impact technology can have on our lives,
broadening our access to information, and fostering a sense of connection through online access and expertise.
With The Smarter Service program, residents can rely on trusted, regular tech assistance. Cash notes that “in the comfort of their own homes, residents feel more at ease to ask questions, open up about their tech challenges, and actively engage in learning and discovery.”
This has been the experience of Ellie, who has called The Covington home for three years. “We had new WiFi installed,” she says, and her devices weren’t working. “I’m not great with computers, but I’ve got to have it.” Her cellphone and landline were also not working well.
Ellie scheduled time for a tech concierge to come to her home. “I had a list of things I needed help with,” she says. “There was an awful lot on it.” She explains that he looked at the list of items, and “the next time he came back, he had answers to all my questions.”
Ellie says that the Smarter Service tech concierge “is very good and
patient. He’s very understanding. He’s trying to make you self-sufficient.” She says that he doesn’t just want to fix a problem. “He wants me to know what he is doing, so I understand what he has done.”
“Prior to this service, I would rely on my kids or my granddaughter to help me. But they would just fix the problem.” Ellie notes that The Smarter Service “shows me what’s done, then asks me to do it, so I learn.”
Ellie says that she uses technology regularly. “We all live life online now. Not a day goes by that I don’t use a computer, so not having access is a problem.” To get the most from these in-home visits, Ellie recommends understanding precisely what you need. “When you’re having a professional come into your home, you want to be prepared to get the most from the time you have with them.” She says that having access to tech support and having someone who is dedicated to helping her has been a big relief. “It takes away my fear and shows me we can all learn more about technology,” she says.
Alice, a resident of The Covington for about a year, also values the service. “I needed help setting up my new printer and computer, and getting them connected,” she says. She scheduled a few in-home sessions with The Smarter Service, finding it very convenient, and now her devices are working well together.
Alice also has dropped by the Techbar to receive help synching her iPhone, iPad and Apple watch. With so many devices, she finds having access to on-site support invaluable.

How Can I Tell If I Have Skin Cancer?
There are three main types of skin cancer: basal cell carcinoma, squamous cell carcinoma, and melanoma. Because each has many different appearances, it is important to know the early warning signs.
Look especially for change of any kind. Do not ignore a suspicious spot simply because it does not hurt. Skin cancers may be painless, but dangerous all the same. If you notice one or more of the warning signs, see a board-certified dermatologist right away, preferably one who specializes in diseases of the skin.
ACTINIC KERATOSIS
An actinic keratosis (AK) is a crusty, scaly growth caused by damage from exposure to ultraviolet (UV) radiation. An AK is considered a precancer because if left alone, it could develop into a skin cancer. AKs are the product of a lifetime accumulation of UV damage and typically appear on areas most commonly exposed sun, such as the backs of the hands, forearms, face, neck, and scalp.
SQUAMOUS CELL CARCINOMA
Squamous cell carcinoma (SCC) is an uncontrolled growth of abnormal cells arising in the epidermis, the skin’s outermost layer. SCCs often look like scaly red patches, crusty bumps, warts, or elevated growths with a central depression. SCCs may appear suddenly and some forms are tender to touch. They can become disfiguring and sometimes deadly if allowed to grow. More than 1 million cases of SCC are diagnosed each year in the U.S., which translates to about 115 cases diagnosed every hour. Incidence has increased by 200 percent in the past three decades in the U.S., and more than 15,000 Americans die each year from this type of skin cancer.
BASAL CELL CARCINOMA
Basal cell carcinoma (BCC) is the most frequently occurring form of all cancers, with more than 4 million cases of diagnosed in the U.S. each year. BCCs may look like open sores, red patches, pink growths, shiny bumps, or scars. Any skin lesion that doesn’t heal, or bleeds with even gentle trauma, such as drying with a towel, is concerning for a BCC.
MELANOMA
Melanoma is a very dangerous form of skin cancer. Melanoma is caused mainly by intense, occasional UV exposure (frequently leading to sunburn), especially in those who are genetically predisposed to the disease. An estimated 178,560 cases of melanoma are diagnosed in the U.S. each year and is fatal for an estimated 9,320 of these patients. Fortunately, if melanoma is recognized and treated early, it is almost always curable.
WHAT PATIENTS NEED TO KNOW
1. Everyone needs to perform a self-skin exam once a month.
2. Melanoma can appear anywhere on the skin, including the palms/soles, under your nails, and places...for most, have never seen the light of day (genitalia). For this reason, it is important to look everywhere. Lock the bathroom door, get out a hand mirror, and check, it could save your life.
3. What to look for: ABCDEs, and an ugly duckling
A - Asymmetry: If I draw a line down the middle, do the sides match?
B - Border: Is there a nice crisp border, or does the mole melt into the surrounding skin?
C - Color: Moles should be one color, one shade of brown. Is there a variety of shades or additional colors, such as black, red, white, or blue?
D - Diameter: Any mole larger than 6mm, ie the head of a pencil eraser, has a higher risk of becoming melanoma.
E - Evolution: Has it changed?
F - The Ugly Duckling Sign: Most people have a “signature mole,” their own personal mole appearance and architecture. Is there a mole that seems different from all the others?
4. Everyone needs a full body skin exam by a board-certified dermatologist once a year, more frequently if you have a family history of skin cancer, a personal history of skin cancer, or live in an area with substantial sunshine.

It is safe to say that I know nothing about pipes. I know a little about drywall, and some about framing; my husband has his general contractor’s license so I’ve picked up a few nuggets of knowledge over the years - albeit in conversation, not in practice.

One year old Sonny just wants somebody to love. Brought to the shelter after having been thrown from a car, Sonny took a little bit of time to learn to trust. However, we soon found out that he is quite the sweet guy and even loves to give hugs. He has an outgoing and playful demeanor and enjoys meeting new people. Sonny would be a great addition to the family, but would prefer to be the only cat in the household.

A good sommelier is a helper, a guide to finding the best wine for your food and experience. For this month’s installment, we again approached South County-based sommelier Micah Sampson for a bit of education about the wine/food pairings, whether the size and shape of the glass makes a difference and whether wine should be decanted before its served.
Q Does white really go with fish/seafood and red with red meat or is there some overlap?
A When in doubt the white with seafood and red with red meat is a good rule. But of course, there’s definitely some overlap here when getting into the details. There are some fantastic lighter bodied red wines out there that can easily be paired with fish and other seafoods. Some of my favorites are Pinot Noir, Beaujolais, and Grenache, each of them have a light body with very light tannins, won’t overpower the delicate fish. And for red meats you can easily pour a full bodied white wine that has high acidity, think California Chardonnays, white Rhone blends, Rieslings and Chenin Blancs. All of these have nice body that can stand up to high fat red meats, and nice acidity that can shine through some of the more robust seasonings and marinades that are used to cook them.
Q Does the shape/size of the wine glass make a difference?
A This is one of those topics that I wish people took more seriously! In short, yes, the shape and size of the glass makes a huge difference! Wine glasses are designed with very specific shapes and sizes to help deliver wine aromas to your nose. If search “red wine glass” and “white wine glass” you’ll notice red wine glasses are much bigger and more bowl shaped, that’s because there are vapors coming off the wine as the alcohol slowly evaporates, those vapors are filled with compounds that help you taste the wine. White wines a typically chilled and won’t give off as much aromas, therefore the glass doesn’t need to be as large. Another reason for the smaller shape for whites is they don’t need need to aerate like reds do, less surface area reduces aeration and keeps the wine cooler. Reds on the other hand love to aerate and
therefore need all the surface area to slowly oxidize (in a good way). The bowl-shaped narrower opening at the top of the glass does two things that are both very important: 1) It helps trap the aromas into a smaller area to help you small them. Think about how much more difficult it would be to smell wine in a cereal bowl with an 8 inch opening than a wine glass with a 2 inch opening. 2) It helps keep the wine in the glass when you swirl the heck out of it, which you should do! I love swirling wine, it introduces oxygen (good oxidation) into the wine and gets all those great aromas swirling around the glass for you to smell!
Q Should I decant wine before I drink it?
A Decanting can do wonders for your wine, but it isn’t always necessary. You should have a nice decanter at home if you’re serious about enjoying some great red wines (don’t worry about decanting white wines), and that decanter should have a nice wide bottom that creates a ton of surface area when the wine is poured in, and a narrow top that keeps too much air from getting introduced and makes it easy to pour a glass. The types of red wines you’re decanting are usually full bodied reds that usually have a some age on them (like 5-10+ years), not those cheap red blends you’re buying for $8.99 at the market. The idea behind decanting is you’re introducing oxygen into the wine and a chemical change is happening, a lot of those volatile compounds (bad aromas) are evaporating off and the harsher tannins are softening up a bit, which allows you to enjoy the wine more. So, if you have a 2010 Barolo or a 2015 Napa Cabernet, let those bad boys decant for 30-60 minutes, maybe take some small sips and smells during that time so you can see how the wine changes.

California Gun Services Safe Gun Storage, Ownership and Disposal
After my grandfather passed away, I moved in with my grandma, doing chores around the house and helping to maintain the garden.
One day, I was looking for my grandfather’s whetstone, to sharpen an old pair of shears, and I stumbled across his Colt Python 357 Magnum. I thought it might be a relic from one of the movies he worked on in the 50’s, but according to my grandma, it was just “one of those things your grandpa just insisted on having.”
Jeff McCabe has gotten more of his fair share of calls from people like myself, who accidentally come across family firearms in their home and don’t have the first idea of what to do with them. The owner of California Gun Services (CGS), Jeff is in the business of safe gun storage, ownership and disposal. The first step? Following California gun laws. I had zero clue that by driving the Colt to the Army Navy to try and turn it in, I’d be breaking a couple laws. According to Jeff there are laws restricting gifting or selling a gun, receiving a gun, borrowing a gun, transporting a gun, even storing guns. New laws come out every year, and it is almost impossible for anyone, even aficionados to keep up with them. Jeff and his team work closely with local law enforcement to ensure the process is easy and safe.
My grandma was afraid the grandkids might find it and would “just feel safer without it.” A sentiment shared by Jeff’s friends, family and fellow church parishionersmany of whom have sought his assistance over the years to ensure the proper storage or disposal of firearms they found themselves in possession of upon the passing of a loved one.
A BUSINESS IS BORN
What initially started as a favor for a friend has grown into a bonafide side business as word has spread about Jeff’s particular skill set and confidence in handling firearms. As the owner of the Orange-based One Way Painting, Jeff often encountered situations whereby he would come across a firearm when moving furniture around for a client prior to starting a painting project.
“Many times, the firearm belonged to my client’s father, grandfather or late husband and she never touched it because she was afraid of guns, but didn’t know how to get rid of it safely,” he says. “So many times these guns are loaded and just stuck in a nightstand or stored under the bed.”
Over the years, Jeff has encountered a variety of different circumstances where individuals seek his assistance in disposing of unwanted firearms. All situations call for professionalism and expertise, but some also require a more delicate approach. One such case was a recent call by a family whose patriarch was once a responsible gun owner, but was now suffering the early signs of dementia. “His family wanted the firearms removed for safety reasons, but the gentleman lawfully owns the guns so it’s important to have a heightened sensitivity so as not to create upset,” says Jeff.
In addition to lay people looking for help, Jeff and his team also assist estate and probate attorneys representing
clients who are surviving family members in possession of unwanted firearms to navigate the process of safe gun disposal.
Raised in a family that had firearms for hunting and sporting, Jeff was taught early on how to handle, be respectful and always be safe when handling firearms. Jeff adds that “ … we as Americans have a great privilege to be able to own a firearm, but with that comes an even greater responsibility to not only protect ourselves legally by complying with Federal and State laws, but to protect ourselves, our neighbors and most importantly, our children from all possible dangers associated with firearms.”
San Clemente resident Rhonda Murphy couldn’t agree more. “There are many things to deal with when a loved one dies. In 2010, when cleaning out my husband’s closet, I came across several of his guns. I didn’t want to keep them and was unsure of what to do with them. My daughters didn’t want the guns and I didn’t want to deal with the hassle of storing them as my grandkids are often at my house. A friend suggested I call Jeff. He was very nice, easy to talk with, trustworthy and knowledgeable. He came to my house, checked all the guns to make sure they were unloaded and removed them. Jeff also told me that one of the guns was a collector’s item and was able to sell it for me. One less thing to have to worry about. I can’t recommend Jeff enough.”
As part of their auditing procedures, California Gun Services gives clients the option of a Voluntary Registration Form (in the case where the weapon was inherited). Many of Jeff’s clients are like my grandma who just prefers that the gun is destroyed. Jeff holds a Federal Firearm License and as such, is able to take possession of firearms without a Gun Transfer. He can also inspect firearms to ensure they are still in working order, and even tell you if the gun, or guns, you have is California Compliant.
“Most people are unaware that firearms that were purchased legally from a gun shop just a year ago may now be illegal in California,” says Jeff. “If we find a firearm that is out of compliance with the law, you’ll receive solid advice on how to make it compliant, sell it out of state, or potentially dispose of it. Our process is approved and certified by local law enforcement, it’s legal, safe and adheres to all state and federal firearms laws.
“Since firearms last more than a lifetime, families often have small and large collections of guns in their home. Knowing what you have and what to do with those firearms is important. Whether it’s from a death in the family, inheritance, or just a need for clearing out some that you know you will never use, we can help. Following California gun laws is essential. We come to you. GUN AUDIT
Jeff will come to you and inspect your firearms to make sure they are in proper working order and that they are California Compliant. If your firearm is not compliant with the California Law, you will receive solid advice on how to make it compliant or possibly dispose or sell it out of state.
GUN DISPOSAL
If a firearm is found to be unsafe to use, cannot be repaired and has no historical value. California Gun Services will permanently destroy it by torching the receiver or frame into three parts or cutting it with a saw into five parts. Jeff will document this with photos and log it for the Alcohol, Tobacco and Firearms and email you the proof of destruction.
GUN TRANSFERS
Private Party Transfers happen between two individuals and will need to take place at a Federal Firearm Licensed Dealer. Since this needs to be done at a Gun Shop, Jeff cannot offer this as an in-home service. If you have inherited or legally obtain a firearm, he can provide you with a Voluntary Registration form or you can download it from the California Department of Justice which is NOT required to do at a gun shop. California Gun Services has a Federal Firearm License and can legally come to you and take possession of a firearm without a Gun Transfer.
GUN PURCHASES OF UNWANTED FIREARMS OR COLLECTIONS
California Gun Services will come to you and appraise a single gun or an entire collection of any size. You will receive an explanation of how guns are legally sold and transferred. California Gun Services can then make you a fair market value offer and/or explain how a gun can be put up for sale on consignment.
AMMUNITION AUDIT
California Gun Services will make sure you are using the right ammunition for the firearms that you have and that it is legal in the State of California. Jeff will inspect the ammunition to make sure that it has been stored correctly, is in good condition and will ensure that the ammunition is being stored properly, using the right type of container and stored in the right location in your home.

Mission Viejo Wigs is truly a family business. Not in the traditional sense, where its owners and operators share the same genes. But rather in such a way that every person who has walked through Owner Betty Darroch’s doors in the 30+ years she has been in operation immediately felt like a member of Darroch’s extended family.

Prescription Payment Plan

In August of 2022 President Biden signed the Inflation Reduction Act of 2022 into law.
The new drug law makes improvements to Medicare by lowering drug costs and improving the sustainability of the Medicare program for generations to come.
For the first time, beginning in 2025, the new drug law requires that all Medicare prescription drug plans, referred to as Medicare Part D plans, both stand-alone drug plans and Medicare Advantage plans with prescription drug coverage to offer enrollees the option to pay out-of-pocket prescription drug costs in capped monthly installment payments instead of all at once at the pharmacy. This will be very helpful for people who have a Part D deductible that needs to be paid first before drug copays begin. (In 2025 it can be as high as $590). CMS (Centers for Medicare/ Medicaid Services) has named this program the “Medicare Prescription Payment Plan”.
Starting with plan year 2025, Part D Enrollees may opt into the program
prior to the beginning of the plan year, or in any month during a plan year. They do this by notifying the Part D sponsor (insurance company) and they then complete the Medicare Prescription Payment Plan election process. This process will be available by mail, toll-free numbers, or website application. All of these options must be provided by the Part D insurance carrier and must be processed within 24 hours when an enrollee is already enrolled in the Part D plan.
Part D enrollees who opt into the program will pay $0 at the pharmacy for their covered drug instead of the copay they would normally pay when filling the prescription. The Part D insurance company must then bill program participants in monthly installments for the co-pays or cost sharing incurred. The pharmacies will be paid directly by the Part D insurance companies.
