Entering the Sacred Space of Hospice

When I first began to walk alongside families on the hospice journey, I encountered the same quiet fears and unanswered questions that so many hold close. Hospice, for many, is imagined as a threshold marking only the end – a place to wait, suspended in sorrow. But in truth, I have learned that hospice is not simply about endings. It is a sacred season, a tender space where caring and presence take center stage, and where time itself is gently reframed.
I have seen how deeply the smallest moments matter here. A hand resting in another’s, the echo of soft laughter filling a sunlit room, the hush of peace when a loved one is cared for with tenderness—these instances, so delicate and fleeting, become lasting treasures that remain in the hearts of families long after the season has passed.
One of the greatest misconceptions I encounter is that hospice is only for the very last days of life. In reality, hospice exists to nurture the time that remains—whether weeks or months. Each day in hospice becomes an invitation: to pause, reflect, and embrace what is meaningful. These aren’t days defined by diminishing possibilities, but by the richness of presence, the chance to gather stories, to say things that matter, and to love in ways both bold and gentle.
Another myth is that hospice extinguishes hope. In my experience, hope
never disappears—it transforms. Instead of being tethered to cure or recovery, hope takes new shape: mornings softened by comfort, afternoons alive with conversation, evenings wrapped in calm. It becomes hope for dignity, for presence, for the freedom to fully live the moments that remain.
I also hear worries that hospice hastens loss, that stepping into this circle somehow shortens a life. The truth could not be more different. Hospice does not quicken life’s closing—it tenderly supports its unfolding. By easing pain, offering comfort, and surrounding both patient and family with care, hospice helps ensure that each day— no matter how many—can be met with intention rather than fear, with love rather than distraction.

Toni’s background in marketing, advertising, and photography collided with her love of animals: she started interviewing pets and their humans for The Chat, The Covington’s monthly newsletter. It’s part icebreaker, part comedy, and part subtle wellness campaign. First she sends a questionnaire to the family to get to know them through their answers, and then she drops by for an in person interview and portrait session.

Pink the Cat: Tail of the Unexpected
Toni: Pink, your coloring is striking — mostly white, some gray but your tail looks like it was added at the last second in a game of “pin the tail on the donkey.” Thoughts?
Pink: What’s wrong with it?
Toni: Oh, nothing. It’s just… distinctive.
Pink: I like my tail. And that wasn’t a very nice thing to say.
Toni: Fair enough. You are beautiful all over.
As she pranced out, she threw a glare over her shoulder and said, “At least I have a tail.”
Mini the Chiweenie: The Culinary Adventurer
Mini is a Chiweenie — part Chihuahua, part Dachshund, all opinions. Rescued by her mom, Dolly, at three months old, Mini arrived at The Covington with big ears, short legs, and one burning question.
Mini: Do I have to sit on your lap for this interview? I love humans, but I’m not comfortable being held. Dolly will tell you — I run when she tries to pick me up.
Once we agreed sitting across from each other would do, Mini eased into things.
Toni: Mini, what’s your favorite part of life here?
Mini: Eating everything in sight. Rabbit droppings are a favorite. Dolly says “no,” but, between you and me, I think she’s missing out.
Toni: Ever gone too far?
Mini: Let’s just say an “all-you-caneat” incident ended in surgery. But it was totally worth it in my opinion.

Toni: Halloween is just around the corner. What will you be this year?
Mini: I don’t get why humans need costumes for treats. I just wiggle and smile and people happily give me treats. Dolly calls me a professional beggar. I prefer “charming neighbor. I’m happy to teach you how to wiggle your behind so you can get treats all the time too.”
DJ the Retriever: Never retrieved a day in his life, but he’s a PeopleWatcher Extraordinaire
Dave, DJ’s human says DJ is cool and mellow. Big, extremely handsome, very friendly, and totally laid back. Dave named him DJ after four generations in his family of David Johns.
Toni: DJ, what do you enjoy most here?
DJ: I like my personal outdoor space where I can sit and watch over things. The birds are entertaining, and I especially enjoy all the twolegged and four-legged friends that walk by and say hello. Mom and Dad are social butterflies, so I’ve met everyone. I’m basically The Mayor around here.
Toni: You are very friendly. I notice you visiting the people sitting in chairs?
DJ: Yes, they always seem so happy to see me, and I like that.
Willie the Terrier: Life Without Chompers
Toni: Willie, does it bother you to not have teeth?
Willie: No. I don’t take time to chew my food anyway. And I’ve noticed, some humans around here don’t have teeth either.
Toni: How are you liking The Covington?
Willie: It’s great. I love to smell the grass and flowers when we take our daily walkies.

I Have Parkinson’s. So What!?
The following includes excerpts from Parkinson’s patient Kathleen’s forthcoming book.
“I laughed that my left thumb was twitching uncontrollably. Surely it was from the stress of planning a big wedding in a very short timeframe. Months later I am at a neurologist’s office drawing backward circles, pinching my fingers, trying to stand straight, balance and not fall backward. Diagnosis: Mild Parkinson’s.
“That was in 2015. Although I am hopeful today, at that time I was confused and afraid. Those of us with Parkinson’s have both movement and non-movement symptoms. We often feel out of control in our bodies and out of sorts in our thinking. We depend on others to help with the simplest to the most private of tasks. We learn to accept our condition and not dwell on the embarrassing moments like falling in public. Managing daily symptoms is challenging to say the least, but believe it or not there is cause to be optimistic.
“Although having Parkinson’s can suck the life out of us, once we accept the diagnosis, we can be on our way to enjoying life again. In the midst of that same suffering, my life has flourished in ways you cannot image.
“Connecting with others with Parkinson’s and participating in support groups can prove invaluable to many. Orange County has an amazing support network. Organizations like Parkinson’s Orange County offer help in
finding the resources anyone may need. RX Ballroom Dance helps reduce some symptoms of Parkinson’s patients through dancing. Their mission is “to use ballroom dance to preserve and enhance the quality of life in people who are confronting neurological conditions.” And Rogue Physical Therapy is for people with Parkinson’s. Their goal “is to give you the tools to live your healthiest and most active lives possible.”
In 2023 Kathleen had DBS (Deep Brain Surgery) to help manage her Parkinson’s symptoms. Her Movement Disorder Neurologist, Dr. Ronak Vora, says DBS works best for “one who typically has had Parkinson’s symptoms for at least 4 years and meets one of the DBS indications (bothersome tremor, motor fluctuations, stiffness and/or bothersome dyskinesia). And one who has been well-informed of the anticipated benefits and potential risks/side effects of DBS therapy by their Movement Disorders Neurologist.”
“The reason I elected to get deep brain stimulation was HOPE! I believed it would open my possibilities of living a more comfortable life. When faced with the decision about which product to choose from, I went with Abbott. My movement disor-
der neurologist, Dr. Ronak Vora, introduced me to Kayla from Abbott months prior to the surgery. She always answered my questions and led me to talk with a few people that had been living with DBS. I learned of their symptoms improving and began to feel more comfortable with the process.”
A specially trained Stereotactic Neurosurgeon does the DBS procedure which can be done asleep or awake. Kathleen’s Neurosurgeon, Dr Alex Taghva, says “I do DBS completely asleep and in one surgery, to ease the burden on the patient. DBS has been approved by the FDA since 1997 and more than 200,000 DBS procedures have been performed worldwide with steadily improving technology and results. Modern imaging and navigation makes it highly precise”. Abbott now has the smallest, longest lasting battery which needs to be recharged as few as 10 times a year.

are three main types of skin cancer: basal cell carcinoma, squamous cell carcinoma, and melanoma. Because each has many different appearances, it is important to know the early warning signs.
ACTINIC KERATOSIS
An actinic keratosis (AK) is a crusty, scaly growth caused by damage from exposure to ultraviolet (UV) radiation. An AK is considered precancerous because, if left alone, it could develop into skin cancer. AKs are the product of a lifetime accumulation of UV damage and typically appear on areas most commonly exposed to the sun, such as the backs of the hands, forearms, face, neck, and scalp.
SQUAMOUS CELL CARCINOMA
Squamous cell carcinoma (SCC) is an uncontrolled growth of abnormal cells arising in the epidermis, the skin’s outermost layer. SCCs often look like scaly red patches, crusty bumps, warts, or elevated growths with a central depression. SCCs may appear suddenly, and some forms are tender to the touch. They can become disfiguring and sometimes deadly if allowed to grow. More than 1 million cases of SCC are diagnosed each year in the U.S., which translates to about 115 cases diagnosed every hour. Incidence has increased by 200 percent in the past three decades in the U.S., and more than 15,000 Americans die each year from this type of skin cancer.
BASAL CELL CARCINOMA
Basal cell carcinoma (BCC) is the most frequently occurring form of all cancers, with more than 4 million cases diagnosed in the U.S. each year. BCCs may look like open sores, red patches, pink growths, shiny bumps, or scars. Any skin lesion that doesn’t heal or bleeds with even gentle trauma, such as drying with a towel, is concerning for a BCC.
MELANOMA
Melanoma is a very dangerous form of skin cancer. It is caused mainly by intense, occasional UV exposure (frequently leading to sunburn), especially in those who are genetically predisposed to the disease. An estimated 178,560 cases of melanoma are diagnosed in the U.S. each year, and it is fatal for an estimated 9,320 of these patients. Fortunately, if melanoma is recognized and treated early, it is almost always curable.
WHAT PATIENTS NEED TO KNOW
1. Everyone needs to perform a selfskin exam once a month.
2. Melanoma can appear anywhere on the skin, including the palms, soles, under your nails, and places most have never seen the light of day (genitalia). For this reason, it is important to look everywhere. Lock the bathroom door, get out a hand mirror, and check—it could save your life.
3. What to look for: ABCDEs and the Ugly Duckling
A - Asymmetry: If you draw a line down the middle, do the sides match?
B - Border: Is there a nice, crisp border, or does the mole melt into the surrounding skin?
C - Color: Moles should be one color, one shade of brown. Is there a variety of shades or additional colors, such as black, red, white, or blue?
D - Diameter: Any mole larger than 6 mm, i.e., the head of a pencil eraser, has a higher risk of becoming melanoma.
E - Evolution: Has it changed?
F - The Ugly Duckling Sign: Most people have a “signature mole,” their own personal mole appearance and architecture. Is there a mole that seems different from all the others?
4. Everyone needs a full-body skin exam by a board-certified dermatologist once a year, more frequently if you have a family history of skin cancer, a personal history of skin cancer, or live in an area with substantial sunshine.

Q Does white really go with fish/seafood and red with red meat or is there some overlap?
A When in doubt the white with seafood and red with red meat is a good rule. But of course, there’s definitely some overlap here when getting into the details. There are some fantastic lighter bodied red wines out there that can easily be paired with fish and other seafoods. Some of my favorites are Pinot Noir, Beaujolais, and Grenache, each of them have a light body with very light tannins, won’t overpower the delicate fish. And for red meats you can easily pour a full bodied white wine that has high acidity, think California Chardonnays, white Rhone blends, Rieslings and Chenin Blancs. All of these have nice body that can stand up to high fat red meats, and nice acidity that can shine through some of the more robust seasonings and marinades that are used to cook them.
Q Does the shape/size of the wine glass make a difference?
A This is one of those topics that I wish people took more seriously! In short, yes, the shape and size of the glass makes a huge difference! Wine glasses are designed with very specific shapes and sizes to help deliver wine aromas to your nose. If search “red wine glass” and “white wine glass” you’ll notice red wine glasses are much bigger and more bowl shaped, that’s because there are vapors coming off the wine as the alcohol slowly evaporates, those vapors are filled with compounds that help you taste the wine. White wines a typically chilled and won’t give off as much aromas, therefore the glass doesn’t need to be as large. Another reason for the smaller shape for whites is they don’t need need to aerate like reds do, less surface area reduces aeration and keeps the wine cooler. Reds on the other hand love to aerate and
therefore need all the surface area to slowly oxidize (in a good way). The bowl-shaped narrower opening at the top of the glass does two things that are both very important: 1) It helps trap the aromas into a smaller area to help you small them. Think about how much more difficult it would be to smell wine in a cereal bowl with an 8 inch opening than a wine glass with a 2 inch opening. 2) It helps keep the wine in the glass when you swirl the heck out of it, which you should do! I love swirling wine, it introduces oxygen (good oxidation) into the wine and gets all those great aromas swirling around the glass for you to smell!
Q Should I decant wine before I drink it?
A good sommelier is a helper, a guide to finding the best wine for your food and experience. For this month’s installment, we again approached South County-based sommelier Micah Sampson for a bit of education about the wine/food pairings, whether the size and shape of the glass makes a difference and whether wine should be decanted before its served. Micah is happy to connect personally
A Decanting can do wonders for your wine, but it isn’t always necessary. You should have a nice decanter at home if you’re serious about enjoying some great red wines (don’t worry about decanting white wines), and that decanter should have a nice wide bottom that creates a ton of surface area when the wine is poured in, and a narrow top that keeps too much air from getting introduced and makes it easy to pour a glass. The types of red wines you’re decanting are usually full bodied reds that usually have a some age on them (like 5-10+ years), not those cheap red blends you’re buying for $8.99 at the market. The idea behind decanting is you’re introducing oxygen into the wine and a chemical change is happening, a lot of those volatile compounds (bad aromas) are evaporating off and the harsher tannins are softening up a bit, which allows you to enjoy the wine more. So, if you have a 2010 Barolo or a 2015 Napa Cabernet, let those bad boys decant for 30-60 minutes, maybe take some small sips and smells during that time so you can see how the wine changes.

Your Medicare Plan’s Annual Notice of Change
If you’re currently on Medicare this applies to you if you have a Medicare drug plan or if you are enrolled in a Medicare Advantage plan (HMO or PPO). This does NOT apply to your Medicare Supplement plans, such as Plan F, G, N etc.
The Annual Notice of Change (ANOC) is a document you receive near the end of September showing if your Medicare plan made changes for the upcoming year. Some examples of the changes that you would see on your ANOC are increases or decreases in your monthly premium, changes to your co-pays, deductibles, or if the plan has been terminated
An ANOC will list 2 columns, one is what your benefit is for the current year, the other column will show what it will change to in the new year.
The biggest offender of changes prompting people to make a change to their current coverage is the “Stand Alone Medicare RX plans” that people who use Medicare as their primary insurance coupled with a Medicare supplement plan have. These plans seem to always be giving an increase to the deductible, premium and co-pay amounts. Unfortunately knowing how to go about finding the right Medicare drug plan on their own isn’t something most Medicare beneficiaries know how to do.

Young’s Beach Shack (YoungsBeachShack.com) at Salt Creek Beach is a go-to fave for fresh seafood, hearty lunch options and picnic provisions perfect for a day on the sand. Our son, who can literally eat his weight in seafood, went straight for the tuna poke bowl which tops sticky rice with fresh cuts of fish, sriracha aioli, avocado, mixed greens while our daughter opted for the equally delicious beer battered local catch “fish and chips.” While there is outdoor seating adjacent to the eatery, we took our order “to go” and noshed surfside for a picnic on the sand.
For a more elevated meal, Craft House (EatatCraftHouse.com) – a vibrant gastropub led by Chef Blake Mellgren – offers locally sourced fare and a lively atmosphere that welcomes families.
My son and I are our family’s resident seafood aficionados, while my husband is a steadfast “meat-and-potatoes” man so we loved the diverse menu offerings from the signature Craft House fried chicken dinner and grilled meatloaf - a must-try for comfort food fans crafted (pun intended) of beef, pork and bacon paired with mashed potato and topped with tomato jam. The steak frites was particular hit – 8 oz. wagyu grass fed skirt steak grilled to perfection and accompanied by a yummy chimichurri and tallow (chef avoids cooking with seed oils in favor of beef tallow and olive oil) fries. The Farmer’s Market burrata salad arrived as a blend of mixed greens, tomato,
crispy quinoa, sunny cal farms seasonal fruit and of course creamy burrata.
For my part, I’ve always been more of an “appetizers as an entrée” diner, preferring to sample several dishes to cobble together a “main meal.” This practice provided the opportunity to try the charred snap peas, spicy tuna crispy rice and delish grilled Mexican street corn enhanced with spiced cream, garlic, shallot, cotija, lime and cilantro.

Eighteen years ago, on the sun-soaked Orange County beaches where surfers chase the perfect wave, a small team of innovators set out to solve a slippery problem. Today, VersaTraction™ stands as a testament to ingenuity, resilience and the power of a simple idea transformed through years of trial, error and relentless improvement.
FROM SURFBOARDS TO SAFETY: THE EARLY DAYS
VersaTraction’s™ journey began in 2007, but its roots stretch back even further. The company’s original incarnation, Waxless Surfboard Systems Inc., was founded by Eric Deitch and Jason Neu after two years of experimenting with alternative traction solutions for surfboards.
“We manufacture a patented nonslip safety traction that helps prevent you from slipping on wet surfaces,” explains Co-Owner Harris Farber. “We have been in business for 18 years now, started back in 2007 as an alternative to waxing your surfboard. Back then we bonded resin directly to the deck of your board and worked our non-slip engineered process over the top once the resin cured.”
The team’s background in surfing gave them a unique perspective. “We all surfed so we knew how to work on boards, fixing dings and such, but really it was just one genius idea and a lot of trial-and-error,” says Harris. Their
early solution was simple but effective: a resin-bonded, non-slip surface that eliminated the need for messy wax.

Your Medicare Plan’s Annual Notice of Change

In the Medicare Insurance world, we refer to it as an “ANOC” (we love using acronyms, unfortunately we’re just about the only ones that know what they mean).
If you’re currently on Medicare this applies to you if you have a Medicare “Stand-Alone” drug plan or if you are enrolled in a Medicare Advantage plan (HMO or PPO). This does NOT apply to your Medicare Supplement plans, such as Plan F, G, N etc.
The Annual Notice of Change is a document you receive in the mail (or online if you signed up for that) that explains the changes that your Medicare plan may have made for the upcoming year. Some changes that you would see on your ANOC are; if the plan will be terminated for 2026, increases or decreases in your monthly premium, changes to your co-pays, deductibles, and the annual out of pocket maximum and the plan’s drug formulary. (The
prescription drugs that the plan gives you access to at a set co-pay or co-insurance amount)
You will receive your Annual Notice of Change every year around the end of September. It’s not a fancy envelope with bright red letters reading, “Open Me, I’m Important to Read” rather it’s generally printed on an inexpensive paper resembling a newspaper, and it usually reads and looks like a pamphlet. All plans have some type of change for the
upcoming year. An ANOC will list 2 columns, one is what your benefit is for the current year, the other column will show what it will change to in the new year. Sometimes the change is good; a lower premium, deductible, co-pay or maximum out of pocket known as a MOOP (see, there’s another acronym for you!)
What you want to make sure of is that you are aware and informed when the change is not good news; your plan will not be offered in 2026, a higher deductible, higher co-pays or premium or MOOP. At that point, you can decide if it’s going to affect you enough to want to look at making a change to your coverage for the new year.
The biggest offender of changes prompting people to look elsewhere is the “Stand Alone Medicare RX plans” that people who use Medicare as their primary insurance coupled with a Medicare supplement plan pay for. These plans seem to always be giving an increase to their deductible, premium and co-pay amounts. Unfortunately knowing how to go about finding the right Medicare drug plan on their own isn’t something most Medicare beneficiaries know how to do.
