FÖRORD

Kommunikationsträning på engelska för vårdpersonal är tänkt som en praktisk hjälpreda för dig som redan arbetar eller i framtiden kommer att arbeta inom hälso- och sjukvården.

Boken vänder sig till studerande vid vårdhögskolor men även till vårdpersonal som önskar friska upp eller förbättra sina kunskaper i engelska och få en inblick i det språkbruk som förekommer i vård­ sammanhang. Den lämpar sig både för organiserad studieverksamhet och självstudier.

Att kunna använda rätt språk i olika situationer bidrar till att självförtroendet stärks och är speciellt viktigt i tider av ökad inter­ nationalisering. Kort och gott – möjligheten till bättre relationer mellan vårdpersonal och patient ökar.

I början av varje kapitel finns en målbeskrivning (Objectives). Den hjälper dig att inrikta dig på vad som är viktigt. Gå alltid tillbaka till målrutan, när du är klar med ett kapitel. Fundera på om du verkligen behärskar det du har läst.

Före så gott som varje kapitel finns en övning som heter Warm Up. Även om den är tänkt för grupp- eller pardiskussioner ska du inte hoppa över den, om du studerar på egen hand. Förståelsen underlättas nämligen om du före första mötet med en text är inställd på vad den kommer att handla om.

De olika kapitlen består av texter från olika vårdsituationer.

I övningarna som följer på varje text är tonvikten lagd på det kommunikativa. Speciellt viktiga fraser har samlats i en ruta med rubriken Language Focus. Dessa ska du studera noga, innan du gör övningarna.

Rubriken Grammar återkommer i nästan alla kapitel. Här tas huvudsakligen grammatik upp som kan vålla speciella svårigheter för svenskar.

I slutet av några kapitel finns ett avsnitt som heter Advanced Reading. Det är texter av något svårare karaktär, avsedda för den som vill bli bekant med ett mera vetenskapligt språk. Många av orden och uttrycken som förekommer i dessa texter hittar du i Appendix 4 (Nursing research – Glossary of terms).

Summary som finns allra sist i varje kapitel är en kort sammanfattning av innehållet och är tänkt att fungera som en hållplats.

Stanna upp en stund och stäm av med dig själv vad som har varit extra viktigt i kapitlet.

Liksom i svenskan skiljer vi i engelskan mellan det medicinska fackspråket och det medicinska allmänspråket. Det förstnämnda används fackfolk emellan och i vetenskapliga artiklar. Allmänspråket däremot används i kontakten mellan vårdpersonal och patienter. Om vi ska kunna kommunicera på ett tillfredsställande sätt är det viktigt att vi framför allt tillägnar oss det medicinska allmänspråket. I fjärde upplagan finns en hel del nya övningar som speciellt fokuserar på patientspråket, till exempel övningar med phrasal verbs.

För att garantera en korrekt användning av ord och uttryck i bokens patientdialoger har språkdatabasen The Institute for Applied Language Studies medical corpus1 genomgående använts. Som exempel på detta arbete kan följande nämnas. Liksom i svenskan förekommer det på engelska ett flertal olika termer för ’kasta vatten’: urinate, pass urine, pass water, do a wee etc. Vilket uttryck lämpar sig då bäst i den muntliga patientkontakten? Enligt ovannämnda språkdatabas används urinate mycket sällan i talad engelska, däremot oftare i skriven engelska, likaså pass urine, medan uttrycket pass water är det absolut vanligaste i talad engelska.

Det finns också en webbplats kopplad till boken, den får du tillgång till genom att aktivera koden på omslagets insida. Där finns speciellt viktiga men svåruttalade glosor (övningarna Glossary) och de hörövningar som finns till så gott som alla kapitel.

Introduction A hospital is usually a very large workplace with many different professional groups. Doctors have from time immemorial divided themselves into surgeons and physicians. The surgeon’s tool is the scalpel; the physician’s has traditionally been the drug. Some surgeons – general surgeons – perform a variety of operations on almost all parts of the body, while others specialize in particular branches of surgery, such as plastic surgery, orthopaedic surgery, neurosurgery and gastrointestinal surgery.

In all hospitals there are many different kinds of specialists. The rheumatologist specialises in the treatment of rheumatic and allied conditions. He uses a variety of investigative techniques, such as X-rays of joints, tests of muscle function and also blood analysis.

The gynaecologist looks after ailments of the female reproductive system, while the obstetrician provides women with expert care during pregnancy and childbirth. What other specialists are there? Try to find at least five more.

There are many other very important professional groups in a hospital, such as nurses and nursing auxiliaries. They assist patients to recover from illness and injury, encourage them to regain their independence and, in cases of terminal illness, help patients to meet death with as little distress as possible.

Midwives are nurses with special training in antenatal care, childbirth and postnatal care.

There are several other professional groups in a hospital.

Prosthetists and orthotists prescribe, design, fit and monitor orthoses and prostheses.

Who treats the patient with therapeutic massage? Who helps patients who are disabled by physical illness or a serious accident to relearn muscular control? Who is responsible for financial, personnel and educational services? See appendix 2.

There are many different categories of nurses in the UK. Here are a few: NHS

The Swedish system

They have many similar job duties.

The medical secretaries have knowledge that is specific to the medical field such as medical terminology. They schedule appointments, answer telephones and ensure medical records are accurate among other things.

The ward clerks perform clerical and receptionist duties. They fill out forms for laboratory tests and record patient information etc.

Reading Carol Gore is employed as a senior ward sister on a 22-bed mixed sex ward caring for patients with both acute and chronic lung and cardiac conditions. Her day’s schedule is different each day. She basically works three different shift patterns.

The first shift is what is known as an early. When she works this shift she gets up in the morning at about 6:00. She leaves for work at 6:45 and travels by car out of town to the hospital where she works,

which is in a neighbouring town. The journey takes her about 25 minutes. When she arrives at work she usually chats with the girls who have been working the night shift, asking them what sort of night they have had. During this time she collects together all the case notes for the new admissions that have come in over the previous day and night.

These patients are always known as emergency admissions. From between 7:15 and 7:30 the rest of the staff arrive. They include another two qualified staff and two unqualified staff. Once all the staff have arrived they begin the handover.

They practise a system of nursing known as team nursing. This is where a qualified nurse cares for a team of patients with an unqualified auxiliary to help. Each nursing team has its own handover. Carol – as the ward co-ordinator – listens to the handover from the busiest team or if there have been no real changes she does not attend the handover.

The handover is done at the patients’ bedsides to allow them to be involved with their own plans of care.

Whilst the handover continues she prepares for the post-take ward round. At 8:00 the ward clerk arrives. Carol communicates to her the jobs she needs to do for the day which include things like getting case notes or X-rays, arranging appointments, communicating with other departments. The ward clerk is her right-hand man and assists her a lot in enabling her to effectively fulfil her role.

During the next hour she greets the patients, arranges staff meal breaks, opens the mail and deals with any outstanding problems or communications. Next on the agenda is what is known as the posttake ward round. This is where the patients who have been admitted as emergencies during the previous day and night are seen by the consultant and the rest of his team who have been working overnight.

This ward round takes a varying amount of time depending on the number of patients admitted. Once she has finished this ward round she sorts out all the medical instructions and communicates information to the other members of staff caring for these patients.

At approximately 10:00 staff begin their coffee breaks and whilst they are away for their breaks Carol oversees their team of patients.

She usually goes for her own break at 11:00. On her return to the

ward she continues to do ward rounds. Depending on what day of the week it is there are between two and eight ward rounds. She also communicates continually with the ward doctor who is based on the ward from 9:00 am to 5:00 pm.

At 12 the afternoon staff come on duty and again a handover takes place. Carol personally hands over to the nurse who is going to be in charge of the ward once she goes home. This usually takes between 15 and 30 minutes. After this has been completed Carol goes to her lunch. What time she goes and how long she takes for her lunch break depends on how busy the ward is. She is allowed 45 minutes for her break and this is unpaid time. She sometimes goes to the library and does a literature search or just reads the journals.

After her break is over she returns to the ward and usually in the afternoon she continues to deal with the ward management issues. These will include things like staff development issues, attending meetings, communicating with relatives and patients and delivering care to patients. The shift finishes at about 4:15 pm.

When she works an afternoon shift she commences her shift at 12:30 pm and receives a handover report from the nurse who has been in charge of the ward for the morning. Her afternoon continues as when she works an early shift. During this shift until 8:45 pm she undertakes direct care of a team of patients. This includes doing a medicine round, ensuring that all medical and nursing interventions are undertaken, giving out meals and feeding patients. A large amount of time is also spent writing the nursing records which provide up-todate information on the patients’ progress throughout the shift.

She also checks that the controlled drugs are all present and correct.

At 8:45 pm the night staff arrive and Carol does the handover for the group of patients she has cared for during the shift. At 9:00 pm she goes home.

She works one more shift which is exactly the same as the early shift but she finishes at 1:00 pm.

Because of the nursing overload Carol works a lot of unpaid overtime and misses meal breaks. This occurs with all members of staff on the ward. It is therefore difficult to leave duty at the allocated time.
