Module 4 Critical appraisal of quantitative research 102721 Research and Theory (Rasmussen University)
Directions: Initial Post:
For your initial post, select one current nursing quantitative research article that may be translated into evidence-based practice. Review the article and complete the worksheet provided. Your analysis will include the required elements of the research process. Identify the steps or elements of the study. Determine the strengths and limitations of the research study including the components of rigor. Evaluate the credibility and meaning of the study findings. Attach the completed worksheet to your initial discussion post.
Competency: Compare research methodologies
Review the article that you selected and complete the following analysis. Be sure to include the permalink for the article you selected.
What is the identified research problem? Does the author include the significance and background of the problem?
The problem identified in this research article was Response to a Mobile Health technology as a Support System for Screening and Management of Tobacco Use.
Background
Smoking is the most preventable cause of death in the United (Cato, Hyun, & Bakken, 2014). More than 50% of smokers have contact with a healthcare provider annually, providing important opportunities for counseling and treatment. About 443,000 premature deaths are attributed to cigarette smoke annually (Cato, Hyun, & Bakken, 2014) Consistently screening for and treating tobacco use and dependence is crucial to reducing tobacco use and dependence.
Did the author clearly articulate the research purpose? What was the purpose of the research?
Yes: The purpose is to describe the predictors of nurse actions in response to a mobile health decision-support system (mHealth DSS) for guideline-based screening and management of tobacco use.
Identify the study methodology.
Data were entered and stored during nurse documentation in the mHealth DSS and subsequently stored in the study database where they were retrieved for analysis using descriptive statistics and logistic regressions.
Did the article include a relevant review of the literature? Explain.
The 2008 update to the U.S. Public Health Service (PHS) Clinical Practice Guideline: Treating Tobacco Use and Dependence recommended that clinicians and healthcare delivery systems consistently identify, and document tobacco use status and treat every tobacco user seen in a healthcare setting using the 5 A’s model:
The article included how Public Health Service (PHS) guideline also recommended individual, group, and telephone counseling, as well as provision of first line medications approved by the U.S. Food and Drug Administration as methods for increasing successful cessation attempts (Cato, Hyun, & Bakken, 2014) Despite the PHS recommendations, clinicians and healthcare systems often do not screen for or treat tobacco use consistently and effectively (Cato, Hyun, & Bakken, 2014)
Describe the theoretical framework for the research study?
Observational design focused on an experimental arm of a randomized, controlled trial.
What are the research objectives, questions, hypotheses, and procedures identified in the article?
Objective of the article is Mobile health (mHealth) technologies can support health promotion and monitoring by expanding communications between healthcare providers and their patients (Cato, Hyun, & Bakken, 2014).
Procedure used was that the mHealth DSS was embedded in the nurse documentation software (i.e., the electronic student clinical log) and was used routinely in clinical encounters after nurses received training on its use.
Describe the results of the study along with identified strengths and limitations. Include appropriate terms for rigor in quantitative research
The research result indicate that Screening was more likely to occur in encounters where patients were female, African American, and received care from a nurse in the adult nurse practitioner specialty or in a clinical site in which the predominant payer source was Medicare, Medicaid, or State Children’s Health Insurance Program. (Cato, Hyun, & Bakken, 2014).
In encounters where the patient payer source was other, nurses were less likely to provide tobacco cessation teaching and counseling.
Strengths
The combination of a reminder to screen and integration of guideline-based recommendations into the mHealth DSS may reduce racial or ethnic disparities to screening, as well as clinician barriers related to time, training, and familiarity with resources.
Limitation
According to the study, nurses in ANP training were more likely to screen for tobacco use, and
all other specialties were less likely to screen
Discuss the findings including relevancy to clinical practice. This study suggests that the use of an mHealth DSS has the potential to increase tobacco screening and guideline-based management. Screening for tobacco use and treatment of tobacco dependence remains the most cost effective and successful clinical preventive approach (Cato, Hyun, & Bakken, 2014)
The study indicates that an mHealth DSS has the potential to reduce screening disparities because of race or ethnicity and increase tobacco use cessation by affecting the nurse provision of guideline-based care
