Introducing our new SEND Support The New SEND Support Team delivered by the Children and Family Well-being Service, Lancashire County Council, has been created and based on a model used in Herefordshire, where Early Help Services support The Children with Disabilities Team by working with families who have been assessed as needing ongoing support. The team comprises of 4 Family Support Workers based across different areas of the county and covering Lancashire as a whole. At this time the catchment age group is 016yrs however over the next 12 months we will be looking to develop and grow the service offering it to 0-18years and looking at supporting the transition to adulthood and adult services. We will provide a holistic and whole family approach and will co-ordinate multiagency practitioner working. We will signpost to appropriate or relevant agencies, will provide advice and guidance, will support with the EHCP process and with reviews of care plans, packages, and direct payments. The new service is being launched on Monday 6th February 2023.

Team The criteria that will be considered for a child, young person, or family to access the service appropriately is: • Level 3 CoN • Disability-related needs - Support provided under the CSDPA 1970 • There may also be additional needs which can be supported by Early Help services • Support may be commissioned or funded • through Direct Payments • Child / Young Person supported at a carer level of 1:1 or less • Deprivation of Liberty authorised • There may be a number of professionals involved to coordinate support • Support does not require a safeguarding response The referral pathway currently is through The Children with Disabilities Team (CwD).
