Welcome to the Wandsworth Care Coordination Service
The Wandsworth Care Coordination Service is run by Royal Trinity Hospice and works closely with professionals involved in your care, such as your GP, District Nurse and other organisations.
We arrange the services that you need while you are being cared for at home. The service is available to anyone aged eighteen or over who is living in Wandsworth or who is registered with a Wandsworth GP.
Working Together for Better Health and Care
The Wandsworth Care Coordination Service is funded by the Wandsworth GP Federation and we work closely with them to coordinate local care needs.
How we can help you
The Wandsworth Care Coordination Service is a one-stop-shop for all your care needs. We are here so you and your family don’t need to contact different care providers separately.
We have been running the service since 2015. Our team of Care Coordinators, supported by experienced clinicians, can help support you with: arranging and coordinating care agencies, community nurses, therapists and volunteer befrienders. We will regularly review your care plan to meet your needs.
sourcing equipment such as electric beds, commodes and hoists, depending on your needs. We will also arrange delivery and collection at home of equipment.
providing information and advice about the services that are available to you and answering any queries and concerns you may have.
supporting you when a problem arises.
This support can help you remain in your home, if that is what you wish, rather than being admitted to hospital.
Support in your home
Depending on your needs and condition, you may be eligible to receive temporary support in your own home from the Royal Trinity Carers. This dedicated team can provide support with things like washing, dressing, loading the washing machine or preparing a simple meal. Their involvement in your care is intended to be short term, until a care agency can be arranged.
Support for carers
From 10pm to 7am, Marie Curie Nurses and Healthcare Assistants are available so that the person caring for you can take a break while you are being looked after. Our Care Coordinators can request this for you.
