Dear Parent/ Guardian,
I hope this finds you well.
The Health Centre medical records indicates that your daughter is currently prescribed an Auto adrenaline injector (AAI). Following current guidance, Roedean school will now keep spare AAIs to be used in an emergency if your daughter’s fails.
Your daughter will still need to carry 2 AAIs at all times.
The spare AAIs will be Jext/Epipen and following the recent Emerade recall (0.3mg + 0.5mg AAT, 9 May 2023), dosage will be:
All students aged 11-18 years: 300mcg/0.3 milligram
Please complete this consent form to indicate that you consent for a spare AAI (s) to be used in an emergency.
