Dr. Harold Bafitis is a double board certified surgeon with board certifications in Plastic Surgery and General Surgery. After completing a Master’s degree in Public Health from the University of Texas School of Public Health, he began medical school at the University of North Texas, Texas College of Osteopathic Medicine. He graduated Cum Laude and then went on to complete a general surgery residency at Grandview Medical Center, in Dayton, Ohio. After his residency, he furthered his expertise by completing a two year fellowship in plastic and reconstructive surgery at the Plastic Surgery Institute and affiliated hospitals in Des Moines, Iowa. Dr. Bafitis has achieved the coveted title of Fellow (FACOS) of the College of Osteopathic Surgeons. He was the first Osteopathic plastic surgeon to earn the title of FACS and was honored by the American Osteopathic Board of Surgery with a seat on their Board of Plastic Surgery.

Dr. Bafitis is a world-renowned academic and key opinion leader in plastic surgery and other surgical procedures. He conducts teaching seminars for plastic surgeons, interns and residents at major institutions across the globe. He has been featured on numerous national news networks, including ABC, NBC, CBS and FOX news. Dr. Bafitis presently sits on the Residency Evaluation and Standards Committee of the American College of Osteopathic Surgeons in Washington, D.C. He hosts scientific programs in West Palm Beach, operates at his state-of-the-art certified surgery center and is currently studying REGENCare Treatments and growth factors to continue to give his patients the most advanced, safest and up-to-date treatments. >>> THE HIGHS AND LOWS of football players are well known. When you are young, strong and focused, the game has no limits. But the body can only take so many hits. There is a huge price to pay for years of playing hard on the field. Rob “The Gronk” Gronkowski, one of the NFL’s all-time greatest tight ends, knows this all too well.

In 2019 Rob announced his retirement from the New England Patriots, needing a break to focus on his health and well-being. At 30 years old he had an extraordinary run, making his mark in football history. Fans were devastated and Rob was hurting. Today, after a multitude of various regenerative treatments throughout this last year, Rob’s new nickname is “Mr. Recovery.” His body and mind are healed, and he has a fresh new deal with the Tampa Bay Buccaneers. The best part is he’s going to share his most amazing discovery. In fact, he is ready to shout it out to all who will listen.

That’s good news, because when it comes to overcoming pain, he’s someone we should listen to carefully. Not only is he an amazing athlete; he is also a successful businessman and philanthropist. The Gronk was always well-loved by fans and teammates, and many thought his career was over. But he was not the type to give up, and he decided that pain was not going to be the end of his story. How he eliminated it and got back on the field is an inspiring story for all of us.

In this interview, Rob shares how he found his fire and returned to the NFL through regenerative care treatments. Here is the incredible story of his journey back to health and wellness in his own words.

Q: I am here with the greatest tight end in NFL history who now calls himself “Mr. Recovery.” After a litany of injuries, you are coming out of retirement this year and making your comeback with the Tampa Bay Buccaneers. How did this all happen?

A: Thank you! I am super excited, super pumped up. I feel so good, you know. I let my body heal and recover. I’ve been playing football for fifteen plus years, plus other sports, and with all this action I’ve been getting into, my body just needed a little rest. I had to investigate and find connections to heal. Dr. Rick turned everything around for me. It’s a full time job as an athlete. Athletes don’t just show

up. We have to take care of our bodies and find solutions that work. The recovery is cake with Dr. Rick.

Your journey back to health is unprecedented. Few NFL players return to action after retiring like you did.

REGENCare Treatments or allografts seem to be the new buzz words for lasting recovery. Is this the type of therapy and injections you received from Dr. Rick at REGENCare?

Yes, I had REGENCare Treatments in October, and I just had REGENCare Treatments today, and they’re fantastic.

To tell you the truth, in your early twenties you really don’t get into the treatment. You just do the basics and the bare minimum.

But as you get older, you have to treat your body more, you need to find new ways, and that’s what I’ve been doing over the last few years. It is my favorite treatment because I get a nap and I am getting rejuvenated at the same time by all the oxygen. It’s great. We also do a couple other treatments to help with recovery, stress management, improving sleep and diet.

Do REGENCare Therapy treatments make a difference in how fast you recover and heal?

I definitely feel a big difference. Back in the day, I would just let my body heal naturally, and when I say naturally I would just do the bare minimum. I would be running around on it. I would be partying on the injury while

it was trying to heal. But I was so young then, and I could get away with it. But now that I want to continue my career, and I’m older, I learned that I needed to start adapting. You gotta adapt to the change, you gotta start adapting to your body. And you know, finding that adaptation includes a whole spectrum of healing treatments from REGENCare Treatments, to IVs, and finally, to proper nutrition.

How does your focused mindset play a role in your healing? Is it an important part of your treatment process?

Yeah, definitely! Some people can be telling you to do this, and you just gotta stick with your gut. You gotta stick with your heart, knowing what is best for your body. To know that, you have to go through a lot of experiences. I’ve tried many, many things, and some do work for me. I focus on all the natural treatments that you could do for yourself. I find the best ones that suit me, and enjoy ‘em while doing ‘em.

What would you tell your friends and teammates who are suffering from chronic pain and injuries about these regenerative treatments?

You know, just get started on it now! The longer you wait, the harder it is gonna be to heal that injury and nagging pain. The longer you wait, it gets more settled into your brain that you have that nagging pain. So hop on it ASAP, get moving, get cruising on it, find some good workouts, strengthen your core tight, and start small. That’s where it begins.

You don’t gotta go big at all. Just start small and make little changes.

So what’s next for you? You have an exciting season coming up with Tampa Bay, what are some of the things that you’re doing to prepare?

You know, I’m just hanging out! Throwing footballs, just doing football workouts, doing band workouts, doing some strength workouts, just doing it all at high speed.

It’s a lot of expectations for sure. Just have to go in and be a consistent player and you’re just ready to go. I’m just excited to get back out into the field and play some football and help out the team.

If you were to leave a legacy behind, what would it be?

Just working hard, being the best teammate that I can be and doing the best that I can in my position to help out the team. Now that Dr. Rick has helped me to recover, I can achieve this and more.

Q: Fans would love to hear about your journey in coming to Dr. Rick and how he has helped you to heal naturally.

A: Well first of all, Dr. Rick has always been a good friend and we were introduced through another mutual friend. All of our brothers take care of our health and our bodies first. We were always looking for a place where we could go and heal our bodies naturally. We were introduced to Dr. Rick, and had an opportunity to get to know him and his practice. We have been going back to him several times a year ever since. It was exactly what we were looking for, which was a natural way to heal the body. It’s exactly what we were looking for, the natural way to heal the body. And with all the different options he has at his facility, it makes it a one stop shop for us. It just makes it so convenient to go down there and to get work done, whether we just want

hydration, some type of REGENCare Treatments activation, recovery and more. There are so many different options we can do when we go down there. I feel like I’m at a recovery party.

Which one of the treatments is your favorite, and why?

The IV treatments are fantastic especially because of all the vitamin options they have. It really helps with recovery, especially when I go down to Atlanta, I'll probably go out the night before and have some fun. I know I can really get hydrated when I go to his facility the next day. For me personally, since I’ve had a shoulder issue, the REGENCare Treatments in my shoulders have helped me to recover. Now I have no pain there at all, and that’s what’s worked best for me. For everyone it’s gonna be different. I came down to get REGENCare Treatments in my back as well. I’ve had back surgery before and I’ve had a couple major issues there.

This is something that’s been going on through my 20’s and all the way through my 30’s now. I think every situation is different for every person. How would you say that your lifestyle has changed with the REGENCare Therapy injections?

baseball career, which is unfortunate because I wish I had done it sooner.

biggest thing that I got out of it is that you don’t have to change completely. It’s not saying to not eat meat at all, it’s just saying don’t eat as much meat, don’t eat as much fish, eat more fruits, eat more veggies.

You’re gonna see changes in your body, have a lot more energy, and be more clear-minded. I wish I knew that before.

How did this affect your energy?

I’ll just put it this way. I always needed a nap during the day. I’m 36 years old, I’m super fit and I grew up around fitness. My father has owned G&G Fittness for 30 years, and we are one of the top three fitness companies in the United States. I don’t take naps during the day anymore.

My energy is just consistent. I’m never too high or too low. I have lost a lot of my pain thanks to the REGENCare allograft injections and good, healthy food is my medicine.

Major League Baseball legend Ryan Klesko became a household name due to his aptitude for hitting home runs. From leading the Atlanta Braves to the 1995 World Series championship, to making the National League All-Star Team with the San Diego Padres in 2001, Klesko has been a winner. Despite his incomparable success, Klesko has experienced some major physical setbacks in the form of neck and shoulder injuries. After announcing his retirement from a sixteenyear major league career in 2008, Ryan Klesko sought out multiple forms of medical treatment. It wasn’t until nearly a decade later that he discovered that the solution to his pain could be found in an increasingly common yet unconventional approach: REGENCare Treatments. We had the opportunity to sit down with Klesko and discuss his journey from chronic pain to newfound relief.

Was there a particular injury that led you to seek treatment from Dr. Rick Ambrozic?

A: I tore the meniscus in my knee. I had heard about Dr. Rick from a couple of my friends who had received treatment from him. I thought to myself, “What’s it gonna hurt?” and ended up getting a REGENCare allograft injection from him. I went on a hiking trip a month later and it’s never bothered me again. I was sold on REGENCare Treatments from that point on.

How many REGENCare treatments did you receive?

I received one injection in my knee, as well as additional injections in other areas of my body. I recently had surgery on my neck from an old baseball injury, and received REGENCare allograft injections to help accelerate the healing process. It was one of those things where I said, “Man, I feel great!” and I started researching more about REGENCare Treatments and spending more time in Atlanta.

With REGENCare treatment, what are you able to do now that you weren’t able to do before?

I was coaching my eleven-year-old son’s All-Star travel baseball team, so I was throwing every day. My neck was bothering me, as well as the pain from past surgeries. Now I’m feeling better than ever! I’m lifting weights, running, and doing batting practice with my son.

I wish I had known back then about REGENCare allograft treatment. Obviously it’s not a cure-all for every injury.

Ultimately you want to stay away from surgery as long as you can, and explore all options. REGENCare allograft injections work in conjunction with surgery to help you heal faster.

You had a torn meniscus and anticipated getting surgery for it.

Now that you’ve received allograft injections, do you think you’re still going to need surgery?

I don’t think so, I’ve been feeling great. I went on a hundred-mile hike about three weeks after receiving the injections. I’m hoping that I don’t have to get surgery, but I’ve read that it can cause ligaments and tendons to heal themselves. I have to go back and get an MRI, which I haven’t done yet because my knee hasn’t bothered me. I wouldn’t be surprised to get back in there and see that it has reattached itself or that it has somewhat healed. Right now I’m back to my normal activities. At my age I’m not running sprints, but I’m working out, and going up and down mountains.

What other treatment programs do you participate in at REGENCare?

In addition to the REGENCare allograft injections, my other treatments include the Prism Light Pod bed. I’ve also had my own hyperbaric oxygen chamber since the time I’ve played for the Padres. It’s really good for every day recovery. This was especially critical when I was playing 200 games a year in major-league baseball, which included spring training, the regular season, and the playoffs. It’s obviously worth the money. I was at REGENCare yesterday with Rob Gronkowski, who also flew in for a full day of treatment.

What does the hyperbaric oxygen chamber do for you today? Does it make you feel more rested?

When I’m tired from all the travelling, I use it and it makes me feel like I’ve gotten a good night’s rest. When you get to your thirties, it’s just rough. You travel all the time, you don’t get many days off. If you were to lie there for just an hour, you’d feel like you had received a whole night of sleep. I always come out feeling fresh, clear in my mind and in my thoughts. It just makes me feel a whole lot better in general.

You mentioned earlier that you had friends who’ve received Regenerative Therapy treatment. Can you tell us more about that?

One friend had surgery on his rotator cuff and REGENCare allograft treatment helped him recover faster. Another friend was on crutches with plantar fasciitis, and was considering having surgery since he had been on crutches for quite a while.

What is the latest news that you can share with them today?

I have been involved in Wellness Medicine in terms of biologically identical hormones and growth factors for almost 28 years. I am a double boarded surgeon with board certification in Plastic Surgery and board certification in General Surgery. The wellness aspect of my practice is something that evolved over time and is a significant adjunct for the surgical interventions that I endeavor with. The regenerative medicine field where allografts take precedence kind of came out of the wellness aspect of my practice. I have had the opportunity to work with several large medical companies that create biological mesh that is used in surgical procedures. These provide significant support for the surgical intervention 18

and over the course of six months to a year dissolve, leaving behind a stronger, more resilient living tissue. I have had the opportunity to become a key opinion leader with these companies and lecture extensively all over the United States to plastic surgery departments as well as at international meetings in Greece and Egypt concerning these topics. What emerged from my expertise in biologic mesh was the next level of healing which is the use of allografts. These allografts are derived from donated placenta and umbilical cords that have been screened and tested to rule out any disease organisms. I would say that the next level of wound healing would be these amniotic products, allografts, which include REGENCare Treatments.

I would say that I have been to almost every major tissue bank in the United States with Dr. Ambrozic. Presently, I am personally involved with an innovative tissue bank company called New Life in Tampa and we are doing a joint study under the University of South Florida with the use of stem cell treatment products for erectile dysfunction. These products are injected with microneedles into the corpora part of the penis and have significantly improved this inevitable disease for so many men.

Part of the study involves patients who are resistant to the phosphodiesterase inhibitors which are drugs: Cialis, Levitra, and Viagra. Some people that are involved in the study are not resistant, they are actually just allergic to these products. The allografts (REGENCare Treatments) are injected into the penis and the magic of this is that they create new blood supply with new channels that will permanently improve erectile dysfunction. This erectile dysfunction, is as stated something that is inevitable for most men as we get older and is likened to the hardening of the arteries

everywhere in the body. These arteries also get smaller and harder even in the penis and can cause performance problems. It happens to everybody and is part of aging and I am sure even to Greek people that age like myself! It is phenomenal that we have this ability to be on a cutting edge with this type of technology. Of course, big pharma would not have anything to do with this because you would not buy all their products, if this was improved by the allografts.

There is a lot of confusion out there about REGENCare Treatments. What treatments are patients receiving? I have heard of the treatments that do not work.

To start off, you want to work with a good tissue lab. Only physicians should order the allografts; not a manager in an office or a chiropractor who orders it then has a physician’s assistant or midlevel nurse be the injector. That is not a good scenario in my opinion. The physician does the consult with the patient and then using their expertise and their guidance often with an ultrasound device for the injection.

You truly want a physician to manage the treatment plan from the beginning to the end. Some companies sell these allografts (stem cell treatments) and do not really care who they sell them to. The difference is for us, we use Wharton’s Jelly, a superior antiinflammatory and a healing product from the umbilical area of the placenta.

It is like getting a major cortisone shot without all the side effects. It literally takes away pain and inflammation. I have injected knees, backs, necks, hips, shoulders, elbows, fingers, and now even the penis. The truth is everyone is benefiting from it. It is just up to us as researchers to see how long the results last and what exactly is the most minimal dosage to effect these results. is benefiting. By the way, there are a few companies that have stem cell treatments that are not from Wharton’s Jelly. I have no idea why, but in those cases you would like to at least look at their whole global view of what they have available for healing. Some companies do have insurance products that are weaker forms of these allografts and in fact I do use them in the hospital when I am doing cases after getting insurance verification. This is a daunting task as the Wharton’s Jelly products that we use in the office are yet to be approved by insurance and my feeling is that it will take decades before big pharm let this happen.

The bottom line is, all of this works together. It’s available and it’s all out there. It’s just a matter of disseminating all the information to make people aware.

What are some of the most common

conditions that are treated with REGENCare Treatments therapy today?

When we are looking at REGENCare Treatments that are approved and reimbursed for insurance purposes, we are looking at the injection of arthritic joints, chronic pain in joints, muscle tears, and non-healing wounds. Dr. Ambrozic is on the cutting edge of treatment, which eventually one day will be reimbursed by insurance carriers. Dr. Ambrozic has injected people with disc issues, like bulging discs, inoperable areas that have had multiple procedures. We have also reproduced his results in our office here in Palm Beach. Another amazing aspect of stem cell therapy and I personally have seen it, is when someone who has had multiple back surgeries cannot have any other surgery, suffers from chronic pain, all of a sudden with these allograft injections they have a complete 80% or greater turn-around with improvement in the pain; it is life changing.

What is one last piece of advice you have for your readers?

Sometimes pushing the envelope will drive people crazy and they will want to stop you, because you are doing things that they do not want you to do or comprehend. If you are not pushing the envelope then you are not living. This is something miraculous! Why the use of these allografts is not something that is more talked about makes me crazy, but over time this will be the cutting edge of medicine and it is important for the word to get out.

The purpose of this interview is to educate and help patients to better understand the scientific breakthrough of regenerative medicine, to learn what is possible, and how the Signature Biologics brand makes all the difference. To be sure, not all regenerative medicine products are created equal. DW: The Signature Biologics’ history first relies on understanding the individuals and the founding. Dr. Neil Riordan has spent roughly the last 20 years of his career working specifically with human umbilical cord derived mesenchymal stem cells (MSCs). Most notably and publicly, that work has been done at Medistem and the Stem Cell institute in Panama. While he has made great impacts globally, Dr. Riordan has great interest in helping people in the United States. For example, he has MSC clinical trials for two Duchenne’s muscular dystrophy patients under US FDA’s designation of Investigational New Drug (IND) for single patient compassionate use.

(IND 16026 DMD Single Patient) These trials are the first in the United States to use hUCT-MSCs. Translational Biosciences, a fully-owned subsidiary of Medistem Panama is currently conducting a phase I/II clinical trial for rheumatoid arthritis and has published phase I/II clinical results for autism and multiple sclerosis in Stem Cells Translational Medicine and the Journal of Translational Medicine respectively.

Further development in the US market requires the right technological and regulatory conditions.

In November 2017 FDA’s CBER group (Center for Biologics Evaluation and Research) published guidance creating two different product classifications: a 361 biologic and a 351 biologic. One pathway, 361 products, are minimally manipulated for homologous use (like for like tissue use). These products are permitted to market without a traditional pre-market approval. The second pathway, the 351 products, are bio pharmaceuticals. These require pre-market approval through an IND and then a BLA (Biologics License Application), so on and so forth.

Dr. Neil Riordan has a never-ending thirst for innovation, but he also pursues science in a way that is very cognizant and respectful of the regulatory frameworks. With the pathways defined by the guidance from FDA, Dr. Riordan established Signature

Biologics in January of 2018. He appointed his long time scientific protege, Dr. Jonathan Hernandez to head-up the Research & Development.

Dr. Hernandez is a PhD graduate of University of Wisconsin-Madison, with two post-docs in the use of mesenchymal stem cells in BMV and in familial cardiac diseases.

This newly defined team then created two products utilizing the source tissue that they know best (humanderived placental tissue). We have these products today: Signature Cord and Signature Matrix – world-class minimally manipulated structural tissue allografts.

What is unique across the space as we look at these products is that: 1) Dr. Riordan is one of the few global leaders in regenerative medicine to develop products in the USA, 2) these products were among the first developed using the FDA roadmap for 361 products. There were many companies with products prior to the roadmap, but they had to shoehorn their products to fit the criteria after it was released. Add that to the Signature Biologics’ team’s 20 years of experience outside the U.S. and it is easy to see the difference in our products. This is continually bolstered by our attention to professionalism in the production laboratory which uses cGMP (Current

Good Manufacturing Practices) and cGTP (Current Good Tissue Practices).

Out of the gate, we implemented major quality standards that even a pharmaceutical company would aspire to. We did this because we know it is possible and it is the right thing to do to ensure quality and safety. This is above the standards we see across the biologics marketplace today.

So, in other words, your team has gone the extra mile to ensure quality, safety and outcomes for practitioners and their patients.

DW: Well, good quality management is a continual pursuit. We have a tremendous professional by the name of Angelica Rivas to implement that process. She also had prior experience working with Dr. Riordan in Panama and a Master’s Degree relevant to her expertise. Angelica is very experienced, with both Bachelor’s and Master’s Degrees.

She established and continually runs the quality management team here at Signature Biologics. For someone like myself coming with 15 years of experience in world class medical device companies, this is something I very much appreciate, because that Is the foundation that is required to build great products and change lives. I do not have to second guess the product.

Tell us about Dr. Riordan’s family lineage relating to science Dr. Neil Riordan is from Wichita, Kansas.

His father, Hugh D. Riordan, M.D. founded the Riordan Clinic. He and his son Neil pioneered several modalities and therapies, most notably, the use of high-dose Vitamin C as a therapeutic for cancer patients. I am not as able to speak credibly about that, but it is a patented therapy that continues to be used today. His whole family is pretty darn impressive. One of his brothers founded Gilead Sciences, the company that makes the one a day HIV treatment, the cure for hepatitis C, as well as the drug Remdesivir which is currently being utilized to treat COVID-19.

What types of stem cells are on

the market today? And of those products, which ones do you produce?

There are so many products out there that the layperson would call ‘stem cells’, to the point that Signature Biologics has designed a patient and physician education program that we call “Beyond Stem Cells” to better inform the public. In the zeitgeist of ‘John Q Populace’, the term “stem cell” resonates, but when we get to the clinical and scientific side, that which is being called “stem cell” often is not.

So, you can see there are many types of “stem cells”. You and I and every human being have them circulating in our body. When we get down to it in the USA, Signature Biologics agrees with the FDA’s very proactive public service announcement that in the United States to date there is, to my knowledge, only one approved stem cell therapy. That is hematopoietic stem cell therapy. These are used in a very specific use case.

Stem Cell Isolates: For products that are a cellular isolate and/or have a primary reliance of the metabolic activity of the stem cells, FDA correctly labels those products a “stem cell therapy” and classifies them as a drug.

Our company is actively pursuing multiple initial new drug discovery (IND) pathways for that type of product.

Currently, we do not produce or sell a stem cell therapy product.

What we have an opportunity to talk about today is that you can have structural tissue allografts, which is what we produce with a primary purpose of the product that is structural in nature. Secondarily, they may have active mesenchymal stem cells present but the products are much more than a cellular isolate.

That is what we produce, which is the highest quality and grade today.

Autologous Treatments: In the public sphere of “stem cell” there are also several different autologous avenues which many practitioners can market as stem cells. These autologous treatments are harvested from the patient and then given back to the patient. One is bone marrow

aspirate, called BMAC. There is also adipose tissue, where clinicians use fat derived stem cells from the patient.

To us there is a little bit of an issue with autologous sources of tissue. One is, you have a patient who needs to get better, and to use an autologous treatment the physician must subject them to a moderately invasive procedure to harvest tissue to get them better. If that is the only option, then so be it, but there are other options that do not subject that patient to an invasive procedure.

The second issue that comes with autologous treatments, is the cell profiles of cells harvested from sick or older patients. I recognize there are many sides in the peer reviewed literature on this topic. However, from what I have seen, most of the literature shows that the mesenchymal stem cells harvested autologously share that genetic profile and genetic age that the patient has.

It seems like using cells of the exact genetic age and profile of the patient would be defeating the purpose of regenerative treatment, is that true?

It can be depending on the intent of the treatment. We know scientifically that cell’s activity and presence decline

with age, as one would expect. There can also be a third consideration -- we talked about the quality of the laboratory and the quality of the personnel that we employ. If you are going to harvest that material and you are going to spin it and process this material in a clinic you want those in charge of that process to be well trained. I am sure those people are very well educated to work with that machinery, but at the same time, I would venture that it’s not their only job to operate that machine inside the clinic. At Signature Biologics, our technicians are trained to have one job which is focused on our products.

Are you referring to the process autologous treatments like PRP? Yes, the PRP, the BMAC, adipose cells.

They all must be processed in the clinic.

I contrast that with Signature Biologics.

At Signature Biologics, we require our technicians to have a minimum of a Bachelor of Science degree in Biology, they are trained for 90 days before they ever are responsible for production grade materials or final products.

Through that process we have validated, we continually certify through our quality management system that tracks their work every day in doing one thing:

Processing placenta into placental derived allografts. Our technicians are extremely accurate, and we also have the quality procedures in place where variations are picked up and disqualified from the manufacturing process.

Cord Blood: We also see products like umbilical cord blood in the US market.

There is a lot of confusion in the marketplace. We have seen cord blood marketed as a stem cell product. The FDA is very clear on this: cord blood may only be used by a first-generation or second-generation blood relative.

If there is an umbilical cord harvested by mother A and child B and you have child C from the same mother, you could use that cord blood on child C. That would be permissible. The healthcare provider or manufacturer needs to track that chain of custody to one blood relative of the donor.

What has been relatively problematic is companies selling unrelated umbilical cord blood for clinical use in the US. For example, if I had banked an umbilical cord from one of my children and we are not related, I would be unable to give you the cord blood because we are unrelated.

Amnion: Another component that is heard of quite often is amnion.

property as a barrier or cover. It also contains mesenchymal stem cells. If processed correctly to maintain that structural integrity, it may be used under the aforementioned 361 product category, although the vast majority of amnion products that we see in the marketplace tend to be either sterilized scaffolds or cellular isolates without structural purpose.

Umbilical Cord: There are also the umbilical cord products, which as we know from published literature and medical texts, are one of the richest single tissue structures in Mesenchymal Stem Cells (MSC’s) in the human anatomy. The umbilical cord also has a structural purpose, so if processed to preserve that structural benefit, it can be again classified as 361.

There must be awareness though.

One of the adulterations we see widely in the marketplace is that many companies will sell an amnion or umbilical cord homogenate. In other words, they will sell a blended down cellular isolation. Some products are literally just kind of made into a soup Those isolate products would fail FDA’s structural or singleness of purpose test for minimal manipulation, and it would make the sole purpose of that product reliant upon metabolic activity, which would fail another one of the tests.

Exosomes: Finally, one of the last product categories out there are

exosomes. They are exciting to see studied in the scientific and clinical space, but also relatively concerning.

FDA has gotten out in front of this in late 2019 to be very clear that all exosome products are considered 351 bio pharmaceuticals and require a premarket approval prior to commercialization.

They are acellular (not cellular) and are a byproduct of cells. Exosomes or microvesicles are used by cells to communicate. We agree and respect FDA’s proactive nature in controlling this exciting area of study.

So that is the rough lay of the land.

Also, one key thing to the public that I want to distinguish is that everything we’ve talked about here today is based on what we would call adult mesenchymal stem cells.

Can you define the definition of an adult stem cell?

I think in the public understanding of stem cells, what was first put into the public sphere, is what we call embryonic stem cells. These are pluripotent, which means they can ‘stem’ or change into different cell types. These are present at the very beginning of the embryonic life, and so those would need to be harvested from either a deceased embryo or a live embryo that is terminated. To be clear, that is not what we are talking about in any of the products mentioned prior.

When we talk about adult mesenchymal stem cells, well, ‘mesenchyme’ is a type of cell, a description for connective tissue.

For example, before we talked about hematopoietic and hematopoietic would be of the blood. Mesenchymal would be, as we think of it, of the larger organ structure -- the skin or connective cells. Mesenchymal stem cells or medicinal signaling cells, as they are often called, are adult stromal cells belonging to that connective tissue family.

What are some of the ethical dilemmas in using the placental derived products? The biggest one is making sure that the donating party, that is the consenting mother, fully understands the possible outcomes and use of the tissue.

Signature Biologics is cognizant of that education process and we work with Certified Tissue Recovery Organizations (we call them TRO’s), to ensure that the donor provides well understood consent before donating. It doesn’t stop there, the TRO and Signature Biologics also want to be sure that the advising clinicians and physicians that are with that donating party can advise the mother and the family about any questions they might have. We also rely on them to provide accurate records of both social activity and the health records which allow us to streamline tissue acceptance to make sure that the tissue that is being accepted into processing has the best chance of clearing the relevant communicable disease testing that we do at the end of the processing procedures.

Are there any risks of communicable diseases such as infection, COVID, HIV or Hepatitis C with doing allogeneic therapy? And what measures are taken to ensure the safety and efficacy of their products?

The CDC sets out guidelines that we follow called “relevant communicable diseases,” and the reason it’s relevant is it’s really based on the overall cohort and populous of the United States.

Things like Hepatitis-C and HIV are relevant communicable diseases.

Signature Biologics controls for these from two perspectives: A.) The donating party provides a blood test which is processed with third-party laboratories that are CLIA certified under CMS. [The Clinical Laboratory Improvement Amendments (CLIA) regulate laboratory testing and require clinical laboratories to be certified by the Center for Medicare and Medicaid Services (CMS) before they can accept human samples for diagnostic testing.]

B.) We also look at the social history of the donor, which the donor consents and provides. What we’re looking for in the social history is things like tattoos, promiscuity, IV drug use, things that would lead toward activities that could present, for example the opportunity of Hepatitis-C. Signature Biologics reviews

for those activities as exclusion and inclusion criteria that we evaluate under our chief medical officer. We then look at the entirety of the documentation and sign the compendium of the tissue chain of custody with each produced lot. That compendium includes data from many internal controls in our manufacturing process and at the conclusion of the manufacturing process.

So before, during and after?

Correct. Before we even receive the tissue, we have the data on file. When we receive it through when we’re processing it, we have data on file. At the very end, we have both data, and we select a portion of the manufactured lot at random for third-party testing.

Are you using exclusively allogeneic tissue, these are other people’s, from the actual umbilical cord?

That is correct. We are using donated placental tissue. One thing that we also do that is above and beyond, is that we opt only to receive placenta from full-term, live births, delivered

by Cesarean section. From an ethics perspective, the first two are very important; a full-term, live birth. From a clinical and disease perspective, the C-section birth is very important because it limits the amount of bloodborn pathogen exposure that this source tissue comes into contact with.

That is something our company opts to do, because we know it to be best-in-class. Further, the guideline is that that tissue must be processed within 72 hours of its collection. Our company aims for 48 hours, because we know that there is a reduced percentage of bio burden to do it faster. We take every measure possible to do that and we are continually improving our processes.

Signature Biologics also has a twoweek minimum quarantine period for all our finished goods. This allows us time to collect all the data to ensure we have a good product for release into saleable inventory. So for example, say there is a Cesarean birth delivery on a Monday, that means that by Wednesday we have completed the entire processing and

cryopreservation process, and that a fortnight from that Wednesday, or two weeks past it, would be the earliest that we would clear that tissue from our voluntary quarantine. It is all very detailed and very impressive.

During that period, the randomized selected samples from that lot have been sent off to third-party laboratories.

Our clinicians, their working surfaces and their gowning is all tested as well.

That is sent out as a batch ‘Lot’ so it’s all tied to the exact same source material. All operations are under 24 /7 environmental monitoring which monitors for bio contaminants or even the possibility of particles that could harbor bio contaminants. That log is recorded to that specific production lot.

So really the two week quarantine often turns into three or four weeks, because as you can imagine, it is a monumental task to both accurately compile that production record, including all that data. Only once it’s been completed and once it’s been reviewed by our director for quality management, as well as our chief medical officer, is that one lot of product released into our finished goods inventory. It is a complex but respectable process.

Are there any other manufacturers out there who use the tissue from the umbilical cord?

Yes. There are a number. To date, in a flowable form (a liquid or a needle flowable form), we have not seen a single other company in the market who makes a structural tissue product like our products. Many of them are cellular isolates or a homogenate, which we do not believe to be compliant with the current guidelines.

We recognize that is not our call to make; that’s FDA’s call.

What has been your experience in dealing with Dr Ambrozic and REGENCare?

Injury I've always played sports such as basketball, tennis, and golf. One day, I was playing golf in the drizzling rain and I happened to slip on a wet log and tore my ACL.
