Cardiographic Technician (230 Hours) Starting Spring 2014

The Cardiographic Technician program at Polaris Career Center is designed to prepare students to perform non-invasive cardiac testing procedures such as electrocardiograms, Holter and telemetry monitoring, and stress testing. Cardiographic technicians work in hospitals and cardiology offices to assist physicians in the diagnosis and monitoring of heart disease. Students will receive 130 hours of classroom and lab time. In addition, students will have the opportunity to gain practical experience through a 100-hour unpaid externship in a local clinical setting. Note: Externship hours must be completed during daytime working hours, Monday through Friday. Healthcare providers require immunizations for students participating in externships. The cost of the immunizations will be the student’s responsibility. Prior to the externship students will be required to have a background check. Upon completion, students will be eligible to take the Cardiographic Technician (CCT) through the registration or Cardiovascular Credentialing International. Prerequisite: Medical Terminology course or concurrent enrollment in a Medical Terminology course. Proof of registration or successful course completion (transcript or certificate of completion) is required. (See Medical Terminology class information, pg. 6)

Cosmetology Academy (1500 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify Let the Polaris Career Center Cosmetology Academy Starting Fall 2014 prepare you for a challenging career in this exciting, cutting-edge field. Gain hands-on experience in the Polaris Career Center New Beginnings Salon while obtaining the industry knowledge necessary for your Cosmetology license. Acquire skills commonly used in cosmetology. Career options include (State license required): Cosmetologist, Color technician, Perm specialist, Hair cutting specialist, Esthetician, Nail technician, Platform artist, and Equipment sales. Dental Assisting (900 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify This course will teach the skills needed to enter Starting Fall 2014 the Dental Assisting field. Skills taught include: introduction to dentistry/oral health and nutrition, infection control, head and neck anatomy/physiology, patient care/dental charting/psychology, restorative materials/advanced chairside, laboratory materials, radiology, practice management, computer technology, and dental specialties. Dental Assisting students will have the opportunity to gain practical experience through a 250-hour, unpaid externship (additional days may be required for the externship). Early Child Care Education/Child Starting Fall 2014 Development Associate (930 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify Polaris Career Center CDA program is designed to prepare students to successfully complete the requirements for the nationally recognized CDA credential. The CDA credential is based on a core set of competency standards, which guide early care professionals as they work toward becoming qualified teachers of young children. CDA students will learn how to nurture the emotional, physical, intellectual, and social development of children. Students will receive 450 hours of classroom training and have the opportunity to gain practical experience in a 480 hour unpaid field experience at an area child care center. Students will be required to have a physical exam prior to the field experience, show proof of immunizations/TB test and complete a background check. The cost for the physical exam, immunizations/TB test and background check will the responsibility of the student. Graphic & Web Design Certification (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify Graphic Design – 300 Hours Starting Fall 2014

This 300-hour training is designed to prepare students to perform specific functions in the graphic design/production field. This will be achieved by studying the basics of design, typography, color theory and basic layout; then applying them in the computer disciplines: Adobe InDesign CS6 (page layout); PhotoShop CS6 (photo correction, retouching, etc.) and Illustrator CS6 (creating and modifying vector images).

Web Design – 300 Hours This 300-hour training is designed to prepare students for the web design field. To achieve this, students will utilize Dreamweaver to learn the basics of HTML and Cascading Style Sheets (CSS). Students will also be introduced to Content Management Systems, Social Media Networks, branding, expert freelancing techniques, and more. Design skills used from the Graphic Design section are essential to have prior to beginning the Web Design section. The HVAC/R program at Polaris Career Center will Starting Spring 2014 help you access this growing field. This program and Fall 2014 takes place in a comprehensive classroom and practical learning environment. The program includes: Introduction to Construction and Maintenance (Core Curriculum), HVAC/R safety related issues, Heating, Ventilating, Air Conditioning & Refrigeration (HVAC/R), Heating & air conditioning installation procedures, Understanding schematics, Airside systems, Proper venting, CFC Refrigerant Recovery/ Recycling Certification, ICE Certification, Duct work, Electrical code & electrical controls, Mechanical maintenance, Air properties & balancing, Heating & air conditioning diagnostic techniques, Refrigeration troubleshooting, and Customer service. All instructional materials, as well as the CFC Refrigerant Recovery/Recycling Certification and ICE test fees, are included. All students are given the opportunity to earn National Center for Construction Education & Research (NCCER) Certification in Core Curriculum. Students are required to purchase safety glasses.

Medical Assisting (946 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify There is no better time than the present to Starting Spring 2014 enter the healthcare field. The Medical Assisting and Fall 2014 program is a great way to gain access to this exciting profession. The Medical Assisting curriculum prepares you to perform administrative, clinical and laboratory procedures for employment in a physician’s office, clinic, or hospital. This course will cover, but is not limited to, the following: Medical terminology, anatomy and physiology, Entry-level clinical skills, Phlebotomy certification, Scheduling appointments, Records management/Electronic medical records, Medical law and ethics, Entry-level ICD-9/CPT coding, Health insurance claim forms, AED/adult/child/infant CPR, First aid/medical emergencies, Computer skills, Psychology of human behavior, Employability skills, OSHA/universal precautions, HIPAA/standards and guidelines. Medical Assisting students will have the opportunity to gain practical experience through a 160 hour unpaid externship. Students will be required to have a routine physical, and criminal background check prior to externship. Some healthcare systems and physician practices may require proof of current immunizations for students participating in externships. Students must also complete the Hepatitis B series prior to participation in their externships. The cost of the physical, immunizations and uniforms will be the responsibility of the student. The Polaris Career Center Medical Assisting Program is accredited by the Commission on Accreditation of Allied Health Education Programs (www.caahep.org) upon the recommendation of the Medical Assisting Education Review Board. Medical Billing & Coding (900 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify This 900-hour program is designed for those Starting Spring 2014 who wish to gain employment in medical billing and Fall 2014 and health information management in physician practices, outpatient clinics, billing companies as well as a variety of other medical facilities. Students will develop a thorough knowledge of medical terminology, anatomy and physiology, electronic medical records, practice management software, Microsoft Office, medical office administration procedures, medical procedural coding, medical diagnostic coding, hospital billing, and reimbursement procedures. Graduates will be prepared for entry-level positions as medical records technicians, billing and coding specialists, electronic health records specialist, medical administrative assistant, or patient access representative. Students will be required to complete a 180-hour unpaid externship during daytime working hours at the end of the program. Students will be eligible to take the National HealthCareer Association Certified Medical Billing and Coding Specialist exam (CBCS), Certified Medical Administrative Assistant exam (CMAA), and the Certified Electronic Health Records Specialist exam (CEHRS) exam. Healthcare providers may require immunizations for students participating in externships. The cost of the immunizations will be the responsibility of the student.

Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R) (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify

Natural Resources/Ranger/Basic Peace Starting Fall 2014 Officer Academy (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify This 600-hour course is conducted under the auspices of the Ohio Peace Officer Training Commission. The instruction will be provided by those who have experience and expertise in the field of law enforcement, natural resources, and parks and recreation. College credit can be received via the Ohio Board of Regents Career Technical Transfer Guarantee System (CT2). Detailed information is available at https://transfercredit.ohio.gov/ap:1:8840875032079. Applicants must be 21 years old by the completion of the course, have a valid driver’s license, a background check and a physical examination. All state and federal guidelines dictating the carrying of a firearm and minimum physical fitness standards set by the Ohio Peace Officer Training Commission will be followed. Adult Education Job Training Programs

Nurse Assistant (90 Hours)

Starting Spring 2014

State Tested Nurse Assistants gain employment and Fall 2014 caring for diverse patient populations including the elderly in long-term care settings, assisted living facilities, home care environments and hospitals. This program is state-approved and consists of mandatory attendance for classroom/clinical hours. The STNA exam is included in the fee and is offered to students after successful completion of the program. Medicare approved Home Health Aide Certification and American Heart Association Basic Life Support (CPR) certification is also included. The 2-step Mantoux TB test is required. The first step of the Mantoux TB test must be completed prior to the start of class. Immunizations may be required and the cost of the immunizations are the responsibility of the student. Students will be required to have a criminal background check prior to participating in clinicals. Uniforms are required and the cost will be the responsibility of the student. Pharmacy Technician (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify The Pharmacy Technician program prepares Starting Spring 2014 students for pharmacy career opportunities in retail, and Fall 2014 hospital, and long-term care settings. Pharmacy Technicians assist licensed pharmacists to provide medication and other health care products. Pharmacy Technician students will have the opportunity to gain practical experience through an unpaid externship. The program prepares students to take the Pharmacy Technician Certification Exam. The course will include, but is not limited to, the following: Medical terminology, Anatomy and physiology, Pharmacology, Pharmacy practice, Pharmacy technology, Pharmaceutical terminology, Pharmaceutical calculations, Medication dispensing, Drug storage, Office skills, Recordkeeping, Pharmaceutical techniques Pharmacy law and ethics, Communication skills, Customer service, Computer technology, OSHA, HIPAA, Inventory control, Billing and reimbursement, Health insurance claim forms, Employability skills, Sterile admixtures preparation, Extemporaneous compounding, and Ambulatory pharmacy externship. Prior to externship participation, students will be required to complete a criminal background check, mandatory drug screening, and maintain 90% classroom attendance. Some healthcare systems, physician practices, and/or pharmacies may require immunizations for students participating in externships. The cost of the immunizations and drug screening are the student’s responsibility. Students will also be required to purchase their own uniforms.

Phlebotomy (252 Hours)

Starting Spring 2014

Students will be trained to perform a variety of blood collection methods on adults using proper techniques and precautions including: vacuum collection devices, syringes, capillary skin puncture, and butterfly needles. Both classroom instruction and clinical experience during a 100-hour externship at a healthcare facility will provide an opportunity for the student to gain the skill and knowledge necessary to take the Certified Phlebotomy Technician (CPT) exam through the National Healthcareer Association (exam fee included). Note: Externship hours will be scheduled during the day, and students will be placed in clinical assignments as space is available. Students will be required to purchase their own uniforms and prior to the externship have a criminal background check. Note: Healthcare providers and labs require immunizations and may require blood titres (at the student’s expense). Students will be required to complete the three-part Hepatitis B vaccination series (at the student’s expense) prior to starting their externship. Students must have the first of three Hepatitis B vaccinations prior to the first day of class. Drug testing and a flu shot may be required (at student’s expense). Prerequisite: Medical Terminology course or concurrent enrollment in a Medical Terminology course. Proof of registration or successful course completion (transcript or certificate of completion) is required. (See Medical Terminology class information on pg. 6)

Precision/CNC Machine Technology (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify Journeyman machinists, die makers, mold makers and Starting Fall 2014 CNC programmers/operators enjoy stable employment in the dynamic environment of manufacturing. Individuals considering careers in all aspects of manufacturing will benefit. All participants will be given the opportunity to earn Level 1 credentials from the National Institute for Metalworking Skills, Inc. (NIMS). Welding (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM – for those who qualify This program takes place in a comprehensive Starting Fall 2014 classroom and state-of-the-art welding lab. It is designed to teach the student practical applications and theory related to the welding trade, while preparing the student for the American Welding Society (AWS) certifications and exams. The National Center for Construction Education & Research (NCCER) credentials may also be achieved. Instructional materials and welding helmets are included. Transfer of Students from Other Institutions Polaris Career Center does not grant advanced standing to students transferring from other institutions.

Transfer of Students Between Programs Prior to the program start date, registered students may not transfer to another program. Students must withdraw from their current program and re-register for another program. The $100 cancellation fee policy will be in effect and applied.

In addition, the following are requirements for specific programs: Cosmetology Academy – proof of residency (utility bill, tax bill etc.) Phlebotomy – proof of registration or successful completion of a Medical Terminology course Cardiographic Technician – proof of registration or successful completion of a Medical Terminology course Natural Resources/Ranger/Basic Peace Officer Academy – students must be 21 years of age by the completion of the program, have a valid driver’s license, a background check and physical exam. Paramedical Examiner  (10 hrs.) This course is designed specifically for the workplace healthcare professionals. Paramedical Independent Contractors are trained medical personnel with current licensure, certification or medical training (RN, LPN, MedTech, Medical Assistant and Paramedics) as required by state for phlebotomy services. Examiners conduct mobile exams, which include taking basic vitals, medical histories, venipuncture, urine collection, and EKG. The American Institute of Professional Bookkeepers (AIPB) has established a professional certification, the Certified Bookkeeper (CB) credential. Basic Rider Course - Beginning Rider (BRC) (Approximately 16 hrs.) This class is your first step in learning to ride a motorcycle. It is intended for the true beginner who has never ridden a motorcycle and for the experienced rider looking to refine their skills and obtain a motorcycle endorsement. If successful, you will receive a completion card which you will take to the driver’s license bureau, and after paying the necessary fee, a motorcycle endorsement will be added to your driver’s license. You do not have to take another riding test at the driver’s license bureau. Classes consist of 4 hours of classroom training, followed by approximately 12 hours of actual riding on a controlled course. Courses are offered on a variety of days and times, so make sure you know your class dates and times when registering. For the first class, bring your driver’s license along with either your motorcycle endorsement or a valid temporary riding permit, which you obtain by passing the written test for the permit at the driver’s license bureau. Otherwise, you will not be admitted into the class and your tuition will not be refunded. Also, bring an ink pen, and dress casually. Proper riding attire will be discussed on the first night of class. NOTE: All classes start promptly! Senior Citizens Discount Senior citizens will receive a 10 percent discount on the published tuition fee (discount does not apply to material fees) except where indicated. Note: Senior Citizens include persons over the age of 65 who can produce proof of age and identity.

School Closing Closing of the daytime classes at Polaris Career Center DOES NOT necessarily mean that the evening classes will be cancelled. The decision to cancel evening classes will be made by the Adult Education administration by 3:00 PM. Students should listen to the radio or TV or call the school at 440-891-7600 for further information.

Berea and Strongsville School Closing When the Berea and Strongsville City School District’s classes are canceled or dismissed due to inclement weather, evening classes will not be held regardless of weather changes. Classes missed will be added at the end of the session.

Withdrawals and Refunds • Full refunds will be issued when a class is canceled by the Adult Education Department, or fees may be applied to another class. • No refunds will be issued after the first class meeting. The Motorcycle Basic Rider, Basic Rider-RR Course, and Experienced Rider Course tuition fee is non-refundable. • A $10 cancellation fee will be withheld from all refunds except for Job Training Programs which have a $100 non-refundable fee. • Books and supplies are non-refundable.
