Patients ‘literally froze to death’ Negligence lethal
CHARNÉ KEMP
A damning investigation report revealed that patients at the Northern Cape Mental Health Hospital “literally froze to death” due to systemic failures and infrastructure breakdown.
The investigation, conducted with the Human Rights Commission (HRC), examined patient deaths and treatment conditions at the Mental Health Hospital and the Robert Mangaliso Sobukwe Hospital (RMSH), both in Kimberley.
In the report released last week by Prof. Taole Mokoena, Health Ombudsman, “shocking” examples of clinical negligence were pointed out.
Two patients died under alarming circumstances, directly linked to exposure to extreme cold and inadequate medical care. The root cause of these deaths was the Mental Health Hospital’s prolonged electricity outage caused by cable theft and vandalism of the electrical substation. This also affected adjacent facilities including Mediclinic Gariep, who fixed the problem within days, as well as the Careline Clinic private psychiatric hospital, Mokoena said.
During the power outage nurses worked in complete darkness, using their mobile phone torches to navigate the facility. Electromagnetic door locks became non-functional, creating major security breaches as staff improvised by keeping doors open with heavy objects.
The telephone system was inoperable.
Essential medical equipment was not functioning. Emergency resuscitation equipment had flat batteries and could not be charged, preventing life-saving interventions.
Profound management and clinical care failures at both institutions were revealed. The Mental Health Hospital operates at only 53% of its commissioned capacity due to acute staff shortages. Some shifts operate without any professional nurses, with enrolled nurses and assistants inappropriately placed in charge of high-acuity units.
Poor record-keeping practices were found, with patient records incomplete, lacking proper identifiers and often featuring illegible handwriting without proper signatures. Clinical notes remained vague. Medical staff consistently violated guidelines of the Health Professions Council of South Africa (HPCSA) and the South African Nursing Council (SANC) through documentation failures.
It was found that the hospital purchased golf carts, inappropriate hospital beds, and superfluous furniture while lacking essential medical equipment. Poor-quality pyjamas and garments were supplied, failing to provide warmth for patients in winter. The mental hospital suffers from poor infrastructure maintenance such as broken windows, blocked sewage systems, leaking roofs and sagging ceilings.
Mokoena’s report includes extensive
recommendations. In his response, Maruping Lekwene, Northern Cape Health MEC, has committed to implementing it. At a media briefing on 31 July he said a task team had been appointed to fast-track improvements and initiate forensic investigations.
“The key actions include monitoring progress and reviewing monthly updates from hospital leadership, ensuring compliance with financial and healthcare regulations, disciplining implicated staff with referrals to professional bodies, investigating suspect procurement and inflated costs, and overhauling clinical governance and standard operating procedures across provincial hospitals.
The department emphasises system-wide accountability and learning from failures to improve patient care.
“We are committed to rooting out mismanagement and the misuse of public resources. A top priority is the accelerated review of contracts that do not serve the department’s interests. In consultation with the Provincial Treasury, we are initiating the appointment of a forensic investigation team to scrutinise all procurement processes at the affected hospitals.
“The Ombudsman’s report underscores that financial austerity, alongside mismanagement and malfeasance, has contributed to the current state of our facilities. These findings will guide our corrective actions,” Lekwene said. Continues on p.2.
Motor tref diep gat

Improvement project comes to a standstill
HELENA BARNARD
An infrastructure improvement project at the Anderson Primary School in Griekwastad “has been hanging for more than a year now”.
Work on a double-storey building at the school has not been completed yet, while it was supposed to be handed over in March.
“Currently, the teachers and learners have to make use of the premises in the state it is,” says a person involved at the school.
The incomplete work, building rubble and equipment not only is a sight for sore eyes, but also poses a safety risk for the more than 900 learners at the school. It also has an influence on performance.
“Local enterprises and other local labourers have been waiting for their payments for more than three months now.
“Security personnel that are still on site only received payment for May, yet the Development Bank of Southern Africa (DBSA) keeps telling us about their processes," says an informed person.
“The main contractor was terminated three months ago, yet no clear answer was given as to what the way forward is,” the person says.
“And what about all those labourers who worked without a contract on site?
“Also, expenses for the ramp for children with a disability and school furniture are being cut because of other expenses. How many materials are going to waste because of their mismanagement?”
Geoffrey van der Merwe, spokesperson for the Northern Cape Department of Education, says the project is being implemented through the DBSA, which was appointed as the implementing agent by the Department of Basic Education as part of the Accelerated School Infrastructure Delivery Initiative (Asidi) programme.
“The Northern Cape Department of Education is aware of the delays
currently affecting the project and is in regular communication with the DBSA to monitor progress and mitigate risks.
“According to the DBSA, the original contractor was formally terminated due to non-performance, following due legal process.
“All payments due to the terminated contractor were processed, and as such, any outstanding payments to subcontractors and enterprises contracted under the main contractor, fall under that contractual agreement,” he says.
The DBSA is working with its legal department to assess payment requests to avoid instances of potential double payment.
Also, the DBSA has submitted additional documentation for legal review and is awaiting internal approval to release payments where possible.
Apparently, the procurement process to appoint a replacement contractor has already commenced.
A site briefing was held last week, and work is expected to resume in September, pending the appointment of the new contractor.
“The department acknowledges the impact these delays may have had on local workers.
“We continue to work closely with the DBSA to expedite the next phase of the project and to ensure that the Anderson Primary School receives the necessary infrastructure improvements as soon as possible.”
REMEDIES MANDATED
Continuing from p.1:
The report of the Health Ombudsman mandates emergency infrastructure repairs and medical equipment procurement within three to six months at both hospitals.
Hospitals must immediately end the practice of allowing unsupervised junior nurses to manage entire wards without professional nurse oversight, where they were left to make life-threatening decisions. Emergency response teams and comprehensive safety protocols must be established urgently, whilst disciplinary action proceeds against all implicated staff members.
Additionally, the investigation demands comprehensive nursing retraining focusing on risk assessment and care planning competencies. Healthcare workers must complete Advanced Psychiatric Nursing courses to properly manage high-risk patient populations.
All medical professionals require basic life support training from accredited bodies, alongside mandatory training in proper documentation and handover procedures to ensure continuity of care.
Within six to nine months, hospitals must appoint a permanent quality assurance manager and develop comprehensive standard operating procedures with mandatory staff training.
A proper supply chain management system must be established with qualified permanent personnel to prevent future procurement failures. Regular clinical audits and systematic performance monitoring will ensure ongoing compliance with national healthcare standards.
The investigation concluded that both hospitals showed gross negligence and systemic failure, violating patients’ constitutional rights to dignity and healthcare. ■

Hierdie neste is net buite Groblershoop afgeneem. Foto: Helena Barnard

“Alles gaan oor atmosfeer.”
Goed of sleg. Positief of negatief. Dit is verseker só. ’n Vraag wat nou daarby aansluit, is: Watter atmosfeer skep jy of help jy skep?
Voel jou kinders by jou tuis? Is jou huis warm?
Wil mense by jou wees? Of eerder nie? Alles gaan oor atmosfeer.
Kameeldoring
Reik baie letterlik uit
CHARNÉ KEMP
“We need four hugs a day for survival. We need eight hugs a day for maintenance. We need 12 hugs a day for growth.” Virginia Satir
Omhels jou geliefdes langer. Aanraking is nie ’n luukse nie, dit is noodsaaklik.
Meaningful projects bring joy
After visiting the Helen Bishop Home in Kimberley earlier the year, two matriculants of the Diamantveld High School realised how big the need is at this orthopedic aftercare facility.
Helen Bishop cares for approximately 60 children and young adults with physical disabilities. After their first visit, Conduard Katzke and Wiandie Botha, finalists in the school’s Mr and Miss Diamantveld competition, engaged in several projects to improve the children’s quality of life.
One of their biggest projects was equipping the volunteers’ accommodation facility with a fridge, microwave and kettle.
A specially designed walking frame was donated, which supports children with disabilities to walk. This promotes their mobility, encourages independence, and boosts self-confidence. This was made possible through funds raised by the entry fees of a competition, as well as a donation from an anonymous individual.
A second project undertaken by Conduard and Wiandie was the creation of a play area. Although toys were regularly
made a donation.
As soon as the balls were handed over, the children immediately started playing. The project addressed a practical need, but also brought joy to these children.
During one of the visits to the home, the staff mentioned that five of the children attend school, but do not have the necessary school supplies. This led to a third project.
Matric learners were approached to donate school bags, and the community generously contributed. Enough donations were received to compile a complete “back to school” package for each child. Each package included a school bag, water bottle, book bags, portfolio files and a pencil case.

At a recent workshop on Kimberley’s history, Robert Hart showcased his favourite photographs from the collection that documents Kimberley and Southern Africa’s mining heritage. Hart is the curator of the photographic collection at the Duggan-Cronin Photographic Gallery, which is a satellite museum of the McGregor Museum.
The workshop, “Mining lives and relations: Multi-species histories of mineral extraction in Southern Africa,” brought together historians, geologists, and archaeologists to share knowledge and discuss the region’s complex past.
“Photographs can tell us so much, but at the same time, they raise many questions,” Hart said.
“My chosen selection is somewhat random, but I have chosen images that I find particularly striking for their content and aesthetic qualities.”
When assisting researchers, Hart poses basic questions about each photograph: What does it show? Where and when was it taken? Who took the image? What was their purpose? Does it have a caption?
“How do you, as an individual viewer, respond to the photograph?” he asked.
“If an image is accompanied by a caption, it will affect the way that the viewer responds. So depending on the context, it is sometimes useful to display an image without
an accompanying caption, to let the image speak for itself.”
Hart used artificial intelligence (AI) to describe one famous image of the Big Hole, a large excavation showing mounds of earth at different levels and ropeways leading from the edge of the pit.
The AI described it as “a picture of a destroyed city.”
“The caption states the picture is about working on the Kimberley mine, but to me it is a startling image,” Hart said.
“It says so much about the scramble to get diamonds out of the earth. Colesburg Kopje was demolished by hand. The diggers in the picture are just tiny specks. To me, the scene is nightmarish.”
Anthony Trollope, in his 1878 book South Africa, wrote that places where men are attracted by the desire of gain “seem to be so repulsive that no gain can compensate for the miseries incidental to such a habitation”.
Kimberley in the early years was indeed a rough and ready place.
A 1917 picture taken at Riverton shows the Vaal River in flood.
“So we also have a record of climatic conditions. The strange colours make it aesthetically pleasing. The light effects create a certain mood. Some historic pictures are therefore also artistic.”
Water was a constant challenge in early Kimberley (as it is today). There was no piped water during the diamond rush, and water was brought in by the barrel. Kimberley only got a constant water supply in 1883, when the pipeline from Riverton was completed.
Hart particularly favours a street scene portraying well-to-do people with horse-drawn vehicles and corrugated iron buildings in the background.
“It is obviously carefully staged; nothing suggests the man is a digger. Look closer and you will notice
water barrels on the carts, and a dog underneath one of them.”
Another compelling image shows a young boy in a widebrimmed hat and frilled white shirt, seated at a garden bench reading a book. He was Jack Drummond Rudd, son of Henry Percy Rudd and grandson of Charles Rudd. The boy later became a director of De Beers.
“To me, the image conjures up a whole epoch about the classes that lived in Kimberley, from lower to upper,” Hart said.
He also highlighted a picture of corrugated buildings on Old Main Street under a heavy snow blanket, from 1876. These buildings, like the James Farrelly Criterion Bakery, were constructed of prefabricated material from England, shipped to South Africa and transported by wagon until the railway arrived.
The railway line from Cape Town opened on 28 November 1885, a significant occasion. Hart recalled reading about the ceremony in local newspapers: “The day was blazing hot. The crowd endured two extra hours in increasing heat as the train was delayed. By the time the governor gave his speech, the marquee was like an oven. The fire brigade was called to hose down the tent, soaking many of the exquisitely dressed VIPs in the process.”
“You can just imagine,” Hart chuckled, “all these very important people getting soaked.
“The newspapers describe everything in detail, so they really are useful if you can pinpoint a date and find the report on events.”

The sound of clinking cutlery and warm conversation filled the Siege Shellhole in Cassandra, Kimberley, on a chilly July afternoon as members of the Memorable Order of Tin Hats (Moths) gathered for their annual Christmas-in-July charity lunch.
Amid the fellowship, one table setting remained untouched, a candle flickering beside an empty plate in memory of soldiers who never came home.
“That is very sacred to us,” said Herman Schwikkard, the Old Bill (leader) of the Siege Shellhole.
“We always do a little salute. It is to say we will remember all the comrades who have passed. Our ideals are true comradeship, mutual help for charity, and sound memory to remember the fallen.”
The Sunday roast was more than a meal. It was part of an ongoing charity drive supporting elderly residents in

Working under the pseudonym “EVO” for Durban’s Mercury newspaper, Evenden established the Order based on principles of concord and harmony, operating independently of race, religion, or politics.
old age homes, people with disabilities, and children in care facilities across the region.
Founded in Durban in May 1927 by Charles Alfred Evenden, the Moths have been helping fellow veterans and community members in need for 97 years. Evenden, a London-born cartoonist who served with Australian forces at Gallipoli during World War I, envisioned an organisation that would preserve the front-line camaraderie
Today, the spirit of service continues through practical support that extends far beyond the veteran community. The organisation's charitable work includes funding prosthetics for soldiers who lost limbs in the South African Border War and paying for eye operations for veterans facing blindness. Their “jar of hope”, filled with a dried soup mixture sufficient to feed four people, provides emergency food assistance, while donations from local farmers allow them to distribute fresh vegetables.
“We take food donations to rest havens like old age homes, give presents to children, and help ex-servicemen or women in need,” explained Melvin Gouws, the Wee Bill (deputy leader).
“Because we are a non-profit organisation, we need community support.”
Plan lei maatskaplike sektor
Bursary fuels passion of uplifting young ones
Driven by her passion for nurturing young minds, Masego Eutricia Tlhageng, originally from Kimberley, graduated with a Bachelor of Education in Foundation Phase from the University of the Free State this year.
A beneficiary of the Matla a Bokone Scholarship Programme, Tlhageng’s dedication to education, especially Early Childhood Development (ECD), stems from her desire to help build strong foundations for young learners.
Established five years ago, the scholarship programme supports and impacts students within a 50km radius of the
The group organises monthly fundraising events throughout the year.
December marks a special milestone the Siege Shellhole’s 70th anniversary.
Beyond charity work, the Moths maintain a collection at their shellhole, preserving artifacts from various conflicts to ensure military history remains accessible in future.
“We are not just a military organisation,” Gouws emphasised.
“We remember the contributions of men and women in different wars. Imagine how our world would look if Germany had not been defeated in the Second World War.”
As the original ex-servicemen aged, the organisation has adapted to ensure its mission continues. A few years ago, membership opened to women, and non-veterans can join as “shellhole supporters”, taking on roles such as adjutant or Pay Bill.
“Our shellhole was the first to allow women, and we formed the Diamond Raiders,” Schwikkard noted.
The Raiders visit other shellholes in friendly “raids”, fostering interaction
and joint events.
The hierarchy within the Moths reflects military tradition the Old Bill leads each shellhole, supported by Bills and Blokes, along with specialised roles like adjutant, sergeant major, and Pay Bill. The heart of the organisation remains unchanged from Evenden’s original vision: Ordinary people doing extraordinary things for their community.
As the last plates were cleared from the July charity lunch and the memorial candle was respectfully extinguished, the Moths’ commitment to service, both to fallen comrades and those still fighting life’s battles, burned as brightly as ever.
New members, both veterans and civilians, are welcome to join this organisation where remembrance meets action, and where a Sunday roast becomes a powerful statement of community care.
The Siege Shellhole meets monthly and welcomes new members.

Volgens schoolboyrugby.co.za is Diamantveld in 2018 en 2024 as rugbykampioene gekroon. HOCKEY TOURNAMENT WITH
One of Kimberley’s most cherished sporting traditions is back as the William Pescod Hockey Club (WPHC) gears up to host the William Pescod Memorial Tournament.
This takes place from 8 to 10 August at the Sol Plaatje University’s (SPU) astro turf and multi-purpose hall.
Now in its 43rd year, the WPHC has long been a beacon of both athletic and academic excellence.
Founded in 1982, the club is deeply rooted in community spirit and is especially proud of its many scholar-athletes who balance their educational commitments with their passion for the game.
Konoha Martial Arts, with teams in Kuruman and Kathu, has delivered an outstanding performance and improved records by receiving 147 medals at the recent Free State Provincials and Northern Cape Trials.
Held in Bloemfontein at the end of July, this Northern Cape-based academy once again proved its dominance and dedication to excellence.
This achievement of taking home 70 gold, 51 silver, and 26 bronze medals placed the academy firmly among the top martial arts institutions in the country.
From dynamic displays in karate and kickboxing to dominant performances in combat sports, boxing and grappling, the Konoha teams left no doubt about their skill and preparation.
Konoha also secured first place in the junior boys and girls U16 divisions, earning the top trophies in these categories. Adding to the glory, the seniors covered second place overall.
The academy extended its heartfelt thanks to the leadership team of Sensei Armand Grobler, Janko
Janse van Vuuren, and Andrea Daries for their dedication.
For Konoha, this achievement goes beyond podium finishes. It represents unity, perseverance, and the embodiment of martial arts values: Respect, honour, and determination.
Shihan Rainer Jacobs, chief instructor, says with this incredible milestone behind them, Konoha Martial Arts looks ahead to future competitions - stronger, prouder, and more determined than ever.
FOCUS ON YOUTH, EDUCATION, BETTERMENT MAKES ITS RETURN
The annual tournament is more than a competitive showcase. It is also a heartfelt tribute to former WPHC players who have passed on, honouring the lives and legacies of Patrick Moses, Alistair Fredericks, Peter Adams, Micheline Tricam, Tyra Hannie, Clayton Frederick and Finley Brooker − figures who helped shape the identity of the club and inspired generations of players.
This year’s event promises an exciting lineup of youth and senior teams from Kimberley, Johannesburg and Bloemfontein coming together in a weekend that emphasises sportsmanship, development and community engagement.
“As a community-based club with a strong focus on youth and education, this tournament is a cornerstone of what we stand for,” said a WPHC spokesperson.
“It is not just about hockey − it is about celebrating a legacy, bringing people together, and investing in the future of our young athletes.”
To make the event a success, the WPHC is seeking sponsorship and financial assistance.
The club is appealing to local businesses, institutions, and individuals to support this impactful event through sponsorships, donations or in-kind contributions.
Sponsors will receive high-visibility exposure through branding opportunities at the venue, social media mentions, and public acknowledgements during the tournament.
“This is more than just an investment in sport,” added the spokesperson.
“It is a commitment to education, community development, and the legacy of excellence we strive to uphold.”
