Your ID Card Explained
Review your ID card to ensure:
• All requested coverages are listed
• Your name, spouse, and eligible dependents are correct
• You refer to the ID Card Examples for guidance
Need changes?
• Contact your employer or call Nippon Life Benefits at 1-800-374-1835
A new ID card will be issued when:
• You become newly eligible
• You add/remove a dependent
• You change your benefits
• Your plan details change
• Legal/vendor updates require it (e.g., 2022 deductible info update)


Group #: The Employer’s Account Number
Nippon Life Benefits: Your insurance carrier. 3. EFF Your original Effective Date with Nippon Life Benefits.
4. ID #: Your member ID number is used instead of your Social Security number.
5. Benefit PH #: Your ID card is not a guarantee of coverage. Your provider should call this number for verification of coverage.
6. CVS Caremark: Your Pharmacy Network. To locate a participating pharmacy, go to: www.caremark.com.
7. Provider Network: Your Medical, Dental, and/or Vision networks. If your plan doesn’t use a network, these sections will be left blank.
Deductible and Out-of-Pocket Amounts: Deductible and outof-pocket amounts for both in-network and out-of-network services are displayed. If the member has family coverage, those amounts will also be shown
8. Quest Select: This is an optional benefit designed to save you money on outpatient laboratory needs covered by your medical plan. Please note this is not available on all plans
9. Claims Address: In most cases, your provider will submit claims for you. However, you may mail it to the address on your ID card if you need to submit a claim.
