 Enrollment forms do not need to be submitted to NLB; however, please keep a copy for your records.

 A completed Nippon Life Benefits Beneficiary Form is required when submitting a Life claim.

• Always have members sign their forms on or before their effective date for contributory coverages.

• When using the portal to manage eligibility, be sure to select the appropriate unit under Company list.

• Be sure enrollment forms are clearly marked with the plan or network choice.

• The open enrollment period is the month prior to the policy anniversary.

EMPLOYEE INFORMATION CHANGE

 Eligibility Date: Subject to policy provisions (e.g., first of the following month or on the date)

 Timing of Request: Within 31 days after the date eligible

 Always list last date actually worked (in labeled field) for terminations.

 Report terminations within 31 days. (Timeframe may vary by state law.)

• Waivers are valid from the date of receipt, not the date of signature, if received after 31 days.

 Provide salary updates as they occur.

 Use the Employer Change Form to update salary information.

 Alternatively, email (admin-info@nipponlifebenefits.com) or fax (913-387-5920) a copy of your premium statement with updated salaries noted.

 Always report date of Full-Time employment, not initial Part-Time date

 Waiting period begins with date of Full-Time employment

• Access statements under Premium Statement History through the Employer/Client Portal.

• Only available online- invoices are not mailed.

• Review each month to ensure accuracy.

• No. We do not pro-rate premium.

 Standard credit is limited to two months for late notifications.

 Promptly report all terminations and changes.

 Submit changes by the 15th to guarantee inclusion on the next month’s invoice

• Typically available online by the 24th or 25th of each month.

 Premiums are due on the first of the month, with a 31-day grace period.

 Pay online using the Online Payment Portal found under ‘My Client Links’

 Pay as billed- do not exclude employee changes.

 To mail payment, print and enclose the remitter, and mail to our lockbox at Nippon Life Benefits, 62348 Collections Center Drive, Chicago, IL 60693-0623.
