у двух пациентов с олигоанурической стадией почечной недостаточности отмечалось значительное улучшение функции почек (креатинин крови уменьшился от 273 до 195 ммоль/литр, нормализовалась протеинурия), что позволило избежать проведения запланированного гемодиализа. В исследовании С. Melander и соавт. у 6 из 9 больных с прогрессирующей ХПН удалось достичь стойкой ремиссии [45]. Проведенное в Клинике НИИР РАМН пилотное изучение терапии РТМ 56 больных СКВ, продемонстрировало высокую эффективность у больных с люпус-нефритом (78%), поражением ЦНС, кожным васкулитом и тромбоцитопенией. В пилотных и небольших рандомизированных исследованиях РТМ оказывается высокоэффективнм методом лечения ANCA ассоциированных васкулитов и БШ. Таким образом, интенсифицированная терапия аутоиммунных заболеваний является многокомпонентной, патогенетически направленной и является терапией выбора не только у больных с критическим течением, но и в случаях неэффективности стандартной консервативной терапии, а также при наличии предикторов неблагоприятного отдаленного жизненного прогноза.
