Absences during the Health & Benefits Fair must be pre-approved and the reason must be clearly documented (i.e., an advanced scheduled

vacation day). Any absence requests received after October 20th will only be reviewed on a case-by-case basis; with documented approval from both an immediate Supervisor and the HR Manager. Calling in sick may NOT qualify as a valid reason. Employees who are not able to attend the Health Fair for pre-approved reasons, will be eligible to complete a makeup HRA with the OHN. HR will contact you to set-up your appointment.

WHAT IS A HEALTH RISK ASSESSMENT? Medical Staff from CIGNA will be on site during the 2014 Health & Benefits Fair to conduct Health

FIRST RESPONDER NARCAN AVAILABLITY

Risk Assessments (HRA) for all employees. The HRA will give you feedback on how to focus on your specific health and wellness needs.

WHY PARTICIPATE? • It’s FREE • It is mandatory in order to prevent a significant increase in your health premiums WHAT TO EXPECT The staff from CIGNA will perform biometric screenings on-site, at the Health Fair. Immediately after the biometrics are performed, you will receive counseling from your appointed medical professional. You will then fill out the Health Risk Assessment questionnaire on-line, using the biometric results you received in your one-on-one session. This can be done at the Health Fair or after the event. But it must be completed no later than Friday, November 7th.

The questionnaire is compiled of approximately 40 questions regarding your overall health and wellness and should take approximately 5-10 minutes to complete.

A team from CIGNA (no Medic employees will be administering HRA tests) will be set up in the privacy of MED-1 to administer the HRA process.

Biometric screenings will be conducted as part of the Health Risk Assessment. Height and Weight, BP, Lipid Profile (finger stick) and Glucose test (finger stick)

*Fasting is not required for the biometric screening. Be prepared to take your shoes off for height and weight metrics.

RETIRE STRONG 2014 HEALTH AND WELLNESS FAIR

We want to ensure everyone is aware that we have added Narcan to our first responder scope of practice. The medication is in the process of being distributed with CFD tentatively set to go live by October 1st.

All other county first responders will be equipped for use by October 15th. Please remember to record the medication usage accordingly in your patient care documentation. Also, as you encounter a patient that has received Narcan from a first responder, it should be replaced to the first responder who administered it on a one-for-one basis.
