In the event that you’ve had a prior screening this year, you must still complete the entire HRA and biometric process during the Health Fair. The aggregate data is used to formulate wellness programs based on our employee population. You will also continue to receive discounted premiums by completing all necessary items during the event DO I NEED A FLU SHOT? Yes. Per policy 2.15, the Agency stresses that we are committed to providing a safe and healthy

environment for our employees and community; therefore, flu shots are mandatory barring any medical or religious exemptions. OPEN ENROLLMENT Open enrollment elections are passive for the upcoming year. It is not mandatory to elect coverages unless you wish to make changes. If you wish to enroll into FSA, DCA or HA=SA, you MUST log in to complete enrollments for these plans. Enrollment for all benefits will be online! You may complete through the payroll portal at any time from October 1st through November 2nd. Computers will be available in the Medic computer lab at the Health Fair to complete this process if needed. WHAT’S SAME FROM LAST YEAR? a. Employees must complete their biometrics, HRA on-line questionnaire and flu shot (proof of) or declination to received discounted premiums. b. Biometrics will continue to be administered by CIGNA with immediate counseling after results are produced. c. Flu shots will be covered by Medic if the employee does not have insurance. If you do have insurance through another provider, please bring your ID card. d. Stickers will be given for flu shots. Please remove your sticker from 2017 if possible prior to receiving your 2018 shot and sticker. e. Vendors will be available for employees and their dependents to discuss benefits. f. Dependents may get flu shots if they are 18+ and have their CIGNA ID card. e. Completing the vendor BINGO game cards makes you eligible for raffle giveaways. f. HR is responsible for assisting with payroll input, benefit processing, recruitment and on-boarding, personnel necessities, performance management, strategic planning and policy/procedure review. MEET THE TEAM JACKIE MANLEY, HR MANAGER Jackie oversees day-to-day activities of the department. These include benefit processes, compensation policies and review, performance management, employee health and well-being, risk and safety administration, personnel matters, policy administration, recruitment and on-boarding, professional development, record request processing and overall employee advocacy. SHANE ANAYA, TRAINING AND ON-BOARDING GENERALIST Shane is responsible for overseeing the successful on-boarding of new hires from start date through release to their respective department. This includes scheduling orientation activities for all incoming positions. He specifically devotes time to ensuring field operations new hires have the tools and resources needed to obtain competency through FTO time, Scope of Practice and Oral Board testing. He is the Explorer Advisor for the newly implemented EMS Post 100 program and is responsible for building course content, mentoring Explorers and working with Adult Committee Leaders to provide an effective and positive program for program participants. Shane is a credentialed paramedic and continues to provide support to field operations. CHELSEA CLINE, HR GENERALIST Chelsea’s role centers around employee benefits, compensation questions, personnel matters, general HR knowledge, process planning and policy review. Chelsea happily assists employees with updating all personal file information to include direct deposits, tax exemptions, retirement accounts, benefit changes, and all other needs pertaining to
employment. Chelsea also administers the tuition reimbursement program, all FSA and DCA reimbursements and consistently assists to provide support to all employees across the Agency. Chelsea is a credentialed EMT-Basic and continues to provide support to field operations. LAURIE FEOLA, RECORDS PROCESSOR AND RECEPTIONIST Laurie is the first friendly face that you see when walking into our front lobby. She is responsible for maintaining the multi-line switchboard for incoming calls to the main intake, directing those calls to the correct department and ensuring the best front-line customer service. Laurie greets and monitors the access of all visitors. She receives and sorts all incoming and interoffice mail assuring delivery to the appropriate personnel. Laurie is responsible for processing all patient record requests whether initiated by the patient, their attorney or their properly identified representative. LINDA HENDLEY, SENIOR HR GENERALIST Linda’s responsibilities revolve around maintenance of compensation ranges, research of market compensation data, review of job classification assignments and descriptions, and participation in salary surveys. In addition, Linda is the main point person for personnel matters and investigations, responds to complaints, and provides advice and guidance regarding discipline and grievances. She also leads the planning, development, implementation and monitoring of Agency health and well-being initiatives, serving as the Chair of the Wellness Committee. KATHY SIMPKINS, EXECUTIVE ASSISTANT Kathy is responsible for organizing all
managerial Agency meetings (ABOC, AMC, QMC, MCB), including the agenda and scheduling process, coordination of service needs, and distributing all pertinent information to involved parties. She is an integral part to producing and distributing reports for Service Inquiry, Destination Reports, Monthly Compliance, KPI and QMC charts for both internal and external entities. Kathy provides support for the Director’s Team particularly by attending weekly meetings, recording decisions and providing summary reports. Her administrative duties include office supply maintenance, catering, travel requests, expense reporting and being the liaison for the Executive Director, his team and both Dr. Swanson and Dr. Godfrey. ZEKE SMITH, RECRUITMENT SPECIALIST Zeke’s primary role as the recruitment specialist is to ensure all Agency staffing needs are met. This encompasses working with hiring managers for internal and external job postings, facilitating all assessment processes for potential candidates, providing interview preparation for internal positions and seeing all new hire candidates through the on-boarding process. He is also versed on general Agency HR information and serves as a resource for employee questions related to benefits, policies and practices. He continues to work on initiatives to assist with making it easier for candidates relocating from out of town/state to provide successful integration into our system. ANNUAL INFLUENZA VACCINATION 2018 – REFERENCE POLICY 2.15 As with previous years, we will be offering the flu vaccine to all employees. As a reminder, the agency requires every employee to do one of the following: 1. Receive a flu shot at the health fair 2. Receive a flu shot outside of the health fair but provide proof to the nurse for verification 3. Decline the flu shot 2018 requirements: 1. The deadline for all current employees to provide proof of their flu shot is Friday, November 2nd, 2018. 2. Any employee who declines the flu shot, does not provide proof of their flu shot, or does not have their sticker on their badge showing they have received their flu shot as of November 2nd, 2018, will be required to wear their N95 at all medical facilities and during all patient encounters during the announced flu season. Masks will be required until the end of the flu season is announced. 3. After the deadline, no stickers will be provided to employees, except for new hires. 4. Every newly hire employee following the health fair dates will be required to provide proof of their flu shot before their FTO time. The influenza vaccine will be offered at the Health Fair free of charge to all employees.

Please be sure to bring your CIGNA ID card so a claim can be processed. If you have coverage under another health insurance carrier, you may bring that insurance card in and still receive your flu shot during the Health Fair. Flu shots are also provided at various pharmacies at a first come first serve basis. If you have or plan to receive your flu shot outside of the Health Fair event, be sure to bring documentation to Claudia Tetenbaum by November 2nd, 2018. In the event that you need proof of your flu shot, you must obtain a copy the same day you receive your flu shot. You will not be provided one after. You are required to bring your Medic ID badge in order to obtain a flu shot verification sticker. If you do not bring your badge, you will have until Friday November 2nd, 2018 to make arrangements with Nurse Claudia Tetenbaum, RN, BSN to have a sticker placed on your Medic ID badge. If you do not do so, you will be required to wear a mask through the flu season. Policy Medic is committed to providing a safe and healthy environment for all employees and patients. In pursuit of this endeavor, there will be times, such as the influenza season, when the Agency will require employees to take action with regard to protection against infectious diseases. Required employee action will include mandatory vaccination with exception for valid, documented medical or religious reasons. Employees with documented medical or religious exemptions will be required to use personal protective equipment (PPE) as directed. All employees will be required to comply with all Agency mandated protective measures within the timeframe specified by the Agency. Anyone failing to meet the required standards will2014 be SEPTEMBER subject to placement in a non-work status and disciplinary action up to and including termination. The required actions may change as directed by the Agency, and will be based upon a host of factors emanating from various local, state and federal authorities.

The latest Performance Pay Tracking Period began on July 1. We are striving to meet the Patient Satisfaction goal of a ≥ 50% rating (experience as “Excellent”). In order to create a comfortable margin of performance, while continuing to improve the patient experience, we would like to share some feedback from recent patient focus groups.
