Dear Community Stakeholder,

In Fiscal Year 2023, we provided over 150,000 services to 12,972 unique lives. We were able to do this by improving access to care through various programs and services. This past year came with its challenges and opportunities. We compassionately responded to tragedy in our community and engaged with our community by being present and intentional about our service to those in need. Our staff have been instrumental in living up to our mission and vision through their passion and dedication to our clients and embracing change though process and program improvements.

Diversus Health, with collaboration across multiple teams, sought to reduce barriers to accessing care through addressing the challenges of our intake process. Through the implementation of Same Day Access, what was then a several weeks’ long wait for an intake appointment is now down to minutes, allowing people to access the care they need when they need it.

Additionally, we have reduced barriers to quality, culturally appropriate care through providing all staff with LGBTQ+ trainings. For the work we have done creating safer spaces for our LGBTQ+ clients, Diversus Health received the 2023 Envision: You Lean on Me Award. These trainings proved to be invaluable in the wake of the tragic Club Q shooting. Our staff restored hope through demonstrating compassion and care for those impacted by being present and offering services at no charge. We are continuing to serve this vibrant, hurting community through the development of the Club Q Impact Team in collaboration with community partners.

Diversus Health increased our presence and support in over 80 community events this last year and through this engagement we developed and enhanced our connection to the community. By being present in the community, we had the fortunate opportunity to hear from individuals and families that have experienced our services and we have heard the depth of the impact Diversus Health has on their lives and the lives of their loved ones. Hearing these stories reinforces our mission and vision and the “WHY” behind the impactful work we do.

We take pride in backing this dedicated team of providers and staff, as well as celebrating the organization’s growth and progress observed in the past year. We are excited for the future opportunities for Diversus Health in the next fiscal year as we continue to improve services and provide exceptional behavioral health care to our community one person at a time.

OUR SERVICES

ADDICTION

Addiction services are offered for both adults and teens. Our programs provide trauma informed comprehensive services to individuals and families through a personalized treatment approach. We match settings, interventions, and services with each patient’s needs. Additionally, our medication assisted treatment provides a whole-patient approach to sustain recovery.

COUNSELING

We provide patient-focused, goal-driven, and evidence-based treatments to achieve immediate results and long-term recovery. Our counseling services include individual, group, and family therapy as well as many specialized modalities. Diversus Health’s Counseling Services are provided to children, teens, and adults of all ages and are provided in outpatient and virtual settings.

CRISIS

Our 24-hour Walk-in Center is staffed 365 days a year. Individuals experiencing a mental health crisis can walk-in and be seen immediately, providing a less expensive and more behavioral health centric option than an Emergency Department. We also offer a 16-patient Acute Treatment Unit that is a short-term facility for patients and mobile crisis evaluations.

PSYCHIATRIC

We have the unique ability to offer counseling services in conjunction with a psychiatrist and/or nurse prescriber, if medications are needed. Our therapists and prescribers provide cohesive, collaborative care in a supportive team environment to assist patient recovery and optimum outcomes.

STRATEGIC PRIORITIES

SERVICE & PROGRAM DEVELOPMENT

The work within the Service and Program Development focus area for Fiscal Year 2023 was directed at aligning Diversus Health’s delivery model with its vision, goals, and overall strategy to best meet community needs. Strategic initiatives focused on continuing to increase access to communities we serve by implementing Same Day Access intake process and Just in Time Prescriber Scheduling, and continuing to improve the services we offer.

QUALITY OUTCOMES & VALUES

A focal point for Diversus Health over the past Fiscal Year has been fostering a data-driven and value-based measurement culture. We continued to expand access to safe and accessible care in a hybrid model that meets the needs of staff and clients alike.

OUR PEOPLE

The services that Diversus Health provides to our community would not be possible without the people who provide them. In Fiscal Year 2023, we focused on building a workforce that exercises the highest standards of corporate citizenship, reflects the growing needs of Diversus Health, and is measured by improved employee engagement, satisfaction, retention, and community involvement.

I personally really like this place and every crisis counselor has helped immensely. I went in a few times when I did not have a therapist and they helped with resources and their compassion. Sometimes you just need a listening ear.

HIGHLIGHTS & ACHIEVEMENTS

ZERO SUICIDE

Diversus Health is invested in the Zero Suicide mindset and is an active partner in the local Suicide Prevention Collaborative. We value the importance of the role we play in our community in addressing suicide risk and train our staff to implement the pillars of Zero Suicide. All staff are trained in Envision Zero and we also offer Question, Persuade, Refer (QPR), Mental Health First Aid (MHFA), and Applied Suicide Intervention Skills Training (ASIST). Clinical staff are trained in Risk Assessment and Safety Planning, Counseling Access to Lethal Means (CALM), and offer additional evidence-based training, including CAMS and DBT. Our Outreach Team focuses on doing community training as well, to include QPR, MHFA, and ASIST.

OUTREACH TEAM

The Outreach Team was active in the community throughout Fiscal Year 2023, providing evidence-based trainings including MHFA, Youth MHFA, ASIST, and QPR suicide prevention trainings. Moving into Fiscal Year 2024, the team will continue to be present in the communities we serve, providing trainings and presentations, as well as representing Diversus Health at events.

LEAN ON ME AWARD

On May 20, 2023, Diversus Health was awarded the Lean on Me Award from Envision: You. We received the award for being an “exemplary organization that promotes the mental health of its employees, stakeholders, and community and harnesses the power of its staff to improve the well-being of the people they serve, including those that identify as LGBTQ+”.

We have worked diligently to educate all our staff on challenges that are unique to the LGBTQ+ community to create a safer space for all clients who walk through our doors.

2nd Annual

HEROES IN MENTAL HEALTH

G LF Tournament

The Second Annual Heroes in Mental Health Golf Tournament was held on May 15, 2023. Our beneficiaries were Ascending to Health Respite Care and Inside Out Youth Services.

Golfers had a great time connecting with each other, getting out in nature on the course, and golfing for a great cause.

DISASTER RESPONSE TEAM & CLUB Q

The Disaster Response Team was deployed for the Club Q shooting and available to support the community in the weeks and months following. In collaboration with key community stakeholders, we have continued to support those impacted by providing ongoing needs assessments and developing a team to outreach and provide behavioral health services to survivors and community members at large, in the aftermath.

ASCENDING TO HEALTH RESPITE CARE helps break the cycle of homelessness through quality healthcare, sheltering, and case management.

INSIDE OUT YOUTH SERVICES builds access, equity, and power with LGBTQIA2+ young people, through leadership, advocacy, community-building, education, and peer support.

HIGHLIGHTS & ACHIEVEMENTS

SAME DAY ACCESS & JUST IN TIME PRESCRIBER SCHEDULING

In Fiscal Year 2023, we implemented two unique transformational service delivery initiatives that have helped us achieve our vision.

These initiatives have reduced the wait time to treatment by 86% and improved client show rates for existing appointments to 90%.

SAME DAY ACCESS

Same Day Access provides clients with a full Patient-Centered Trauma-Informed Behavioral Health Assessment and Diagnostic with an Assessment Specialist and Counselor on the same day that it is requested, reducing wait time for treatment to an average eight days.

JUST IN TIME Prescriber Scheduling

Just in Time Prescriber Scheduling allows us to move clients from Patient-Centered Trauma-Informed Behavioral Health Assessment and Diagnostic to a Psychiatric Evaluation within three to five business days.

The intake experience was both pleasant and fast. I was set up with a therapist the same day and you don’t see that swift care anywhere else. I recommend everyone to try Diversus [Health] with any of your mental health needs.

SCHOOL-BASED SERVICES

The Child and Family School-Based team is pleased to report the success of the Second Step® SocialEmotional Learning (SEL) programs in two elementary schools (District 2 and District 11) during the 20222023 school year. The programs were implemented in seven separate groups with 36 youth ages seven to twelve years old and grades ranging from first to fifth.

Many participants reported that they felt more confident and capable of managing their emotions after participating in the programs. Parents and teachers also reported seeing positive changes in the participants' behavior, such as increased cooperation and decreased aggression. of participants had an average score increase.

FINANCIALS

Fiscal Year 2023 was a year of opportunities and challenges. We received key grant funding, continued to increase in-person services, and our team responded to challenges and tragedy in the community.

We were awarded new grants and continued operating under multi-year grants. The key grants in Fiscal Year 2023 were:

SAMHSA EMERGENCY RESPONSE GRANT (SERG):

Diversus Health was awarded grant funding from the Colorado Department of Public Health and Environment (CDPHE) to provide ongoing behavioral health response to the mass shooting at Club Q in Colorado Springs on November 19, 2022. SERG provides funding for clinical staff and peer outreach specialists to support survivors of the tragedy. Diversus Health staff work in collaboration with other organizations in the community to provide support to the community that was negatively impacted and who may be experiencing trauma from the shooting.

SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES ADMINISTRATION (SAMHSA):

Year Two funding of a Community Mental Health Centers (CMHC) grant from SAMHSA was awarded to fund services for COVID Recovery and Resilience. Services under this grant include reinstating the ClubhouseUnity House program, adding additional RN care managers, establishing a more robust care coordination program, which includes peer support services, supporting Mobile Crisis response to local area schools,

establishing high-quality telehealth services, and supporting our Diversity, Equity, Inclusion, and Belonging efforts across the organization. We also received approval to use SAMHSA CMHC grant carryover funds from Year 1 to fund additional services during this fiscal year. The funding was used to launch the new Same Day Access (SDA) initiative by hiring dedicated SDA counselors and intake assessment specialists.

AMERICAN

RESCUE PLAN ACT (ARPA):

Year two funding from El Paso County’s ARPA Program was received to support Diversus Health’s Suicide Prevention Community Outreach Program. Funding is used to provide staffing for Mental Health First Aid, Applied Suicide Intervention Skills Training, Question Persuade Refer and other suicide prevention programming.

COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT (CDPHE):

Diversus Health was awarded funds by CDPHE for Community Disaster Behavioral Health (DBH). This funding supports integration of behavioral health within disaster preparedness and response efforts, as well as develops DBH preparedness, response teams and coordination capacity. Moving forward, we will continue to focus on long-term sustainability and improving community access to quality behavioral healthcare.
